stillbirth prevention: Raising public awareness of stillbirth in Australia.

Prevention of stillbirth remains one of the greatest challenges in modern maternity care. Despite this, public awareness is low and silence is common within families, the community and even healthcare professionals. Australian families and parent advocacy groups given a voice through the Senate Enquiry have made passionate and articulate calls for a national stillbirth awareness campaign. This fourth paper in the Stillbirth in Australia series outlines why stillbirth needs a national public awareness campaign; and provides an overview of good practice in the design, development and evaluation

Stillbirth epidemiology, risk factors, and opportunities for stillbirth prevention.

World-wide, there are between 3 and 4 million stillbirths delivered each year, 98% of these deaths occur in the developing world. Although there are very significant differences in the rate and causes of stillbirth in these settings, the study of stillbirth globally gives insight into the interaction between the fetal and maternal conditions in the setting of varying access to healthy food, access to medical care, treatment of infections, screening of congenital anomalies, education, and access to antepartum and intrapartum care.

Wisconsin stillbirth services program: a multifocal approach to stillbirth analysis.

Stillbirth accounts for about 26,000 deaths annually in the US. In most previous studies, discrete causes are identified in less than half of all stillbirths. In order to identify causes and non-causal but potentially contributing abnormalities, we analyzed 416 of the most recent (2004-2010) Wisconsin Stillbirth Service Program (WiSSP) cases from a multifocal approach. In 70% of cases a cause sufficient to independently explain the demise was identified including 40% placental, 21.5% fetal, and 12.7% maternal. Results for stillbirths and second trimester miscarriages did not differ significant

Prevention of Stillbirth.

Stillbirth prevention requires a multifaceted approach focused on identifying and managing placental insufficiency-the leading cause in high-resource settings. Traditional risk factors such as age and body mass index have limited predictive value, emphasizing the need into investigation using ultrasound, Doppler studies, and biomarkers for universal screening. Effective interventions include low-dose aspirin, which reduces placental-related complications, and optimal management of maternal conditions like hypertension and diabetes. Broader strategies such as education on fetal movement, smokin

Evaluation of Stillbirth.

Stillbirth remains a significant and poorly understood pregnancy complication in the United States. Despite medical advancements, the US stillbirth rate exceeds that of other high-income countries. A key challenge is the high proportion of unexplained cases, which hinders prevention efforts. However, systematic evaluation can significantly reduce unexplained stillbirths. Identifying causes is crucial for families and health care providers, aiding in grief processing, future pregnancy planning and stillbirth prevention efforts. Genomics of stillbirth.

Stillbirth, defined as fetal death at 20 weeks gestation or later, is a devastating pregnancy outcome affecting 1 in 175 pregnancies in the United States. Although efforts to understand the etiology of stillbirth have expanded, 25 % of cases remain unexplained and some cases previously thought to be explained may have additional unknown causative factors. Determining an etiology for stillbirth is important for clinical management and for grieving families to obtain closure, to find meaning, and to understand recurrence risks. However, the evaluation of stillbirth is not completed uniformly des

Predicting antepartum stillbirth.

Stillbirth affects approximately 1 in 200 pregnancies and is one of the most common serious complications of pregnancy. Stillbirth can occur as a consequence of diverse processes, including fetal abnormality, isoimmunization, infection, maternal preeclampsia, placental abruption, maternal medical conditions, complications of labor and delivery, growth restriction, and also often occurs in the absence of an apparent cause or predisposing factor. This review summarizes a number of factors which identify women at increased risk of stillbirth, including maternal characteristics, blood tests, and b

Genetics of stillbirth.

Approximately 25% of stillbirths have been attributed to genetic etiologies. The most common cytogenetic abnormalities are similar to those seen in liveborns and include 45X, trisomy 21, trisomy 18, and trisomy 13. Cytogenetic abnormalities are more common when fetal structural anomalies are identified. Mendelian and metabolic causes of stillbirth are less well understood although single gene disorders can result in stillbirth. With new cytogenetic and molecular technologies additional genetic causes of stillbirth are likely to be described and will provide additional information for appropria

Stillbirth and Infection.

Stillbirth is a common adverse pregnancy outcome and of the approximately 2 million stillbirths that occur each year, about 15% are estimated to be caused by an infectious agent. Bacteria, viruses, fungi, and parasites all have been shown to be causal for stillbirth. While the numerical relationship between most infections and stillbirth has been little investigated, world-wide, malaria and syphilis are the infections most associated with stillbirth.

Infection and stillbirth.

Infection may cause stillbirth by several mechanisms, including direct infection, placental damage, and severe maternal illness. Various organisms have been associated with stillbirth, including many bacteria, viruses, and protozoa. In developed countries, between 10% and 25% of stillbirths may be caused by an infection, whereas in developing countries, which have much higher stillbirth rates, the contribution of infection is much greater. In developed countries, ascending bacterial infection, both before and after membrane rupture, with organisms such as Escherichia coli, group B streptococci

Predicting antepartum stillbirth.

PURPOSE OF REVIEW: Rates of stillbirth in the developed world have been static or rising in recent years. Clinical prediction of stillbirth risk may allow interventional studies. RECENT FINDINGS: The most prevalent independent risk factors are nulliparity, advanced age and obesity. These are increasingly prevalent in the developed world. Obesity is particularly associated with stillbirth at term and after term. Pregestational diabetes is a major risk factor for stillbirth and these women are usually offered intensive surveillance during pregnancy. Despite this, a recent national study in the

Interpretations of stillbirth.

BACKGROUND: Continuity is a major concept in the father-son domain of the Han Chinese value system in Taiwan. Aspects of continuity may include structure, interactions and other facets of family; however, providing descendants is the keystone of women's reality in these families. In a culture in which death is seen as a taboo subject and the unborn child has not been recognized as a real baby, losing a long-expected child at the end of pregnancy becomes a great challenge to women who have experienced stillbirth. AIM: The aim of this paper is to report a study exploring Taiwanese mothers' inte

Stillbirth: a review.

Stillbirth occurs in nearly 1% of all births in the USA, and is one of the most common but least studied adverse pregnancy outcomes. The many risk factors for and causes of stillbirth are presented. Over the past several decades, the rate of stillbirth has been substantially reduced, with the reduction most apparent in those stillbirths previously occurring at term and/or in labor. Reductions have occurred because of reductions in risk factors (i.e. prevention of Rh disease and better control of diabetes), better antepartum monitoring of those with risk factors followed by early delivery for t

Stillbirth and the placenta.

Stillbirth affects a large proportion of pregnancies world-wide annually and continues to be a major public health concern. Several causes of stillbirth have been identified and include obstetrical complications, placental abnormalities, fetal malformations, infections, and medical complications in pregnancy. Placental abnormalities such as placental abruption, chorioangioma, vasa previa, and umbilical cord abnormalities have been identified as causes of death for a significant proportion of stillbirths. In the absence of placental abnormalities, the gross and histologic changes in the placent

Valuing Stillbirths.

Estimates of the burden of disease assess the mortality and morbidity that affect a population by producing summary measures of health such as quality-adjusted life years (QALYs) and disability-adjusted life years (DALYs). These measures typically do not include stillbirths (fetal deaths occurring during the later stages of pregnancy or during labor) among the negative health outcomes they count. Priority-setting decisions that rely on these measures are therefore likely to place little value on preventing the more than three million stillbirths that occur annually worldwide. In contrast, neon

Stillbirth, Inflammatory Markers, and Obesity: Results from the Stillbirth Collaborative Research Network.

BACKGROUND: Obesity is associated with increased risk of stillbirth, although the mechanisms are unknown. Obesity is also associated with inflammation. Serum ferritin, C-reactive protein, white blood cell count, and histologic chorioamnionitis are all markers of inflammation. OBJECTIVE: This article determines if inflammatory markers are associated with stillbirth and body mass index (BMI). Additionally, we determined whether inflammatory markers help to explain the known relationship between obesity and stillbirth. STUDY DESIGN: White blood cell count was assessed at admission to labor and
