Because of instability in eastern Afghanistan, new refugees crossed into the federally administered tribal areas of northwestern Pakistan in 2002. In 2003, we investigated an epidemic of Plasmodium falciparum malaria in 1 of the camps. Incidence was 100.4 cases/1,000 person-years; in other nearby camps it was only 2.1/1,000 person-years. Anopheline mosquitoes were found despite an earlier spray campaign. Documented clinical failures at the basic health unit prompted a drug resistance survey of locally manufactured sulfadoxine-pyrimethamine used for routine treatment. The in vivo failure rate w

SETTING: The prevalence of substandard anti-tuberculosis drugs is unknown. To maximize the effectiveness of tuberculosis (TB) control efforts, simple, inexpensive drug quality screening methods are needed.
DESIGN: Isoniazid (INH) and rifampin (RMP) single- and fixed-dose combination (FDC) formulations were collected from selected TB programs and pharmacies in Colombia, Estonia, India, Latvia, Russia and Vietnam. Samples were screened using a recently developed thin-layer chromatography (TLC) kit. All abnormal samples and a 40% random sample of normal formulations were further analyzed using c

OBJECTIVE: Hypertension results in more deaths than any other risk factor and has been on the rise in sub-Saharan Africa over the past few decades. Generic drugs have helped improve accessibility and affordability of antihypertensive therapy in developing countries. However, assessment of quality standards of these products is important. We performed a quality assessment of five commonly used antihypertensive generic drugs in 10 sub-Saharan African countries and studied the impact of price on quality.
METHODS: Drug samples were prospectively collected using standardized methods between 2012 a

OBJECTIVES: The issue of quality of medicine is a worldwide phenomenon and counterfeit, substandard, spurious, and adulterated (CSSA) drugs are a substantial threat to public health. This issue is rampant in the context of low-middle-income countries such as Pakistan. The current study involved a phenomenology-based qualitative approach to explore these drugs' perception, knowledge, practice, and issues in combating this menace.
METHODS: A semi-structured interview guide was developed. Eleven drug law experts were interviewed through a purposive sampling technique. All interviews were audio r

Substandard and falsified medical products may cause harm to patients and fail to treat the diseases or conditions for which they were intended. It is therefore required to have analytical methods available to assess medical product quality. Benchtop NMR spectroscopy provides a generic, inherently quantitative, analytical method capable of separating specific signals from those of a matrix. We have developed an analytical method for the analysis of active ingredients in pharmaceutical products and illegal drugs, based on benchtop NMR spectroscopy. Within its resolution limits, benchtop NMR spe

The aim of this review is to present an overview of the current state of substandard and falsified (SF) drugs and the use of vibrational spectroscopies for their detection. A particular emphasis is on portable and handheld devices and chemical imaging techniques such as hyperspectral and multispectral imaging. The first section is an overview of SF drugs including their definition, global impact, distribution, supply chain issues, and analytical methods for identification and control. The paper highlights the growing importance of non-destructive, low-cost, and rapid methods for the analysis o

Counterfeit and substandard drug products pose significant public health challenges, affecting millions of people worldwide in both developed and developing countries. These products compromise healthcare systems, reduce treatment efficacy, and endanger patient safety. Therefore, this study assessed strategies to reduce the circulation of counterfeit and substandard drugs, identify potential factors leading to their circulation and evaluate the role of community pharmacy professionals in combating counterfeit and substandard drugs in Ethiopia. A cross-sectional multi-center survey of community

Over the past decade, artemisinin (ART)-combination therapies (ACTs) have shown declining efficacy within Southeast Asia (SEA). These resistance-like phenomena manifest as a delayed clearance phenotype (DCP) in some patients treated with ACTs. ACTs are currently the recommended treatment for P. falciparum infections by the World Health Organization (WHO), and they are our last line of defense to effectively treat all strains of malaria. Acceleration of antimicrobial resistance (AMR) is often theorized to be exacerbated by the use of subtherapeutic dosages of drugs ("substandard" drug), which f

BACKGROUND: India is an increasingly influential player in the global pharmaceutical market. Key parts of the drug regulatory system are controlled by the states, each of which applies its own standards for enforcement, not always consistent with others. A pilot study was conducted in two major cities in India, Delhi and Chennai, to explore the question/hypothesis/extent of substandard and counterfeit drugs available in the market and to discuss how the Indian state and federal governments could improve drug regulation and more importantly regulatory enforcement to combat these drugs.

Substandard and falsified medicines cause thousands of deaths and waste billions of dollars worldwide every year. There is an urgent need for low-cost and simple-to-use tools for identifying these "fake drugs." In this work we demonstrate "Disintegration Fingerprinting" (DF), a technique that identifies pills, tablets, caplets, and other solid-dosage drugs based on how the drug disintegrates and dissolves in liquid. The DF hardware consists of a water-filled transparent plastic cup atop a conventional magnetic stirrer. An inexpensive sensor mounted on the outside of the cup shines infrared lig

BACKGROUND: Substandard and falsified (SF) medicines are a global health problem. Their high prevalence is a threat to public health in low- and middle-income countries (LMICs). However, there are few street-level investigations of how this market works. This case study examines the supply and demand for SF medicines in Southern Ethiopia.
METHODS: A cross sectional qualitative design, using semi-structured interviews supplemented by participant observation, was adopted. Study participants were selected using purposive, convenience, and snowball sampling techniques. They included pharmacists,

The negative consequences of Substandard and falsified (SF) medicines are widely documented nowadays and there is still an urgent need to find them in more efficient ways. Several screening tools have been developed for this purpose recently. In this study, three screening tools were used on 292 samples of ciprofloxacin and metronidazole collected in Cameroon. Each sample was then analyzed by HPLC and disintegration tests. Seven additional samples from the nitro-imidazole (secnidazole, ornidazole, tinidazole) and the fluoroquinolone (levofloxacin, ofloxacin, norfloxacin, moxifloxacin) families

INTRODUCTION: Poor-quality antimalarial medications, including falsified and substandard formulations, pose significant public health risks, contributing to ineffective treatment and potential drug resistance. Our research conducted in 2013 on Bioko Island, Equatorial Guinea (EG), found 9.6% (n=61) of the artemisinin-containing antimalarials (ACAs) purchased were of poor quality. This study aimed to update the quality of all sold antimalarials and extend to include regions on the mainland.
METHODS: A total of 637 samples, 564 ACAs and 73 non-ACAs, were purchased from 424 outlets on Bioko Isla

Globally, it is estimated that about 10-30% of pharmaceuticals are of poor quality. Poor-quality drugs lead to long-term drug resistance, create morbidity, and strain the financial structure of the health system. The current technologies for substandard drug detection either are too expensive for low-resource regions or only provide qualitative results. To address the current limitations with point-of-care technologies, we have developed an affordable and robust assay to quantify the amount of active pharmaceutical ingredients (APIs) to test product quality. Our novel assay consists of two par

The prevalence of substandard and falsified (SF) drugs requires the development of effective identification and differentiation methods to distinguish genuine from SF medications. SF drugs may have harmful consequences, including increased morbidity and mortality. Near-infrared (NIR) spectroscopy is a well-established technique that has demonstrated its effectiveness in detecting poor-quality medicines. It provides a rapid, specific, and non-destructive method for identifying SF. NIR spectroscopy may be applied in hyperspectral imaging, multispectral imaging, or point spectrum recording. Spect

BACKGROUND: Over the past years, there has been a growing concern that a considerable amount of anti-malarial supply in the underdeveloped world particularly in the private sector, is of poor quality. The World Health Organization (WHO) has received about 1500 reports that mentions instances of substandard and falsified products since 2013. The majority of the reports concerned antibiotics and anti-malarials. The majority of reports (42%) originate from the WHO African region.
OBJECTIVE: This study intends to assess the quality of the most widely used anti-malarial medications [artemether-lum

The prevalence of substandard and falsified (SF) antimicrobial drugs is increasing around the globe. This poses a great concern for the healthcare system. The consumption of SF antimicrobial drugs has the potential to result in treatment failure, emergence and development of antimicrobial resistance, and ultimately a rise in mortality rate. The objective of this study was to assess the quality of four commonly used antimicrobials marketed in the cities of Dire Dawa and Jijiga and the town of Togo-Wuchale, which have high potential for illegal drug trade activities in Ethiopia because they are
