Meeting the health care needs of suburban youth: review of a clinical service.

The demographic and medical data from the first 1,000 patients registered at a suburban adolescent health service were reviewed and the findings compared with the results of an initial survey performed in the same community and other health services located in urban communities. Most of the patients were white (92%), girls (82%), and 16 to 18 years of age (63%) and had parents who graduated from high school (59%) or college (28%). They reported higher rates of participation in health-risk behaviors, including smoking (50%), drinking (60%), drug use (67%), and sexual intercourse (83%), than the

The characteristics and health service utilization of adolescents with low back pain in a suburban pediatric health care system: analysis of health records data.

BACKGROUND: Low back pain (LBP) is increasingly common among adolescents, yet little is known about the healthcare utilization in this population. We aimed to describe the characteristics and treatment patterns of adolescents with LBP presenting to a specialized comprehensive pediatric health system. METHODS: This retrospective single-arm cohort design analyzed de-identified data from a suburban healthcare system. Using the TriNetX analytics platform, we queried electronic health records for adolescents aged 12-18 years with a new diagnosis of LBP between 2018 through 2022 without serious pat

Expanding the reach of pre-exposure prophylaxis services in suburban and rural Ontario: an implementation science evaluation of a public health-led model.

BACKGROUND: Despite the proven efficacy of pre-exposure prophylaxis (PrEP) in preventing HIV, disparities among service delivery prevail in Canada, particularly among suburban and rural regions. This study evaluates the reach and effectiveness of a mixed-service delivery PrEP clinic embedded within a public health unit, located in Southeastern Ontario. METHODS: The PrEP clinic possesses an interdisciplinary team that delivers in-person and remote services. In consultation with PrEP clinic staff, we used the RE-AIM Framework to describe client characteristics (reach) and evaluate implementatio

Spatial Accessibility to Healthcare Services in Metropolitan Suburbs: The Case of Qingpu, Shanghai.

Spatial accessibility is an important factor for planning healthcare services to maintain a quality life for the metropolitan area. The metropolitan suburb is a special area for its location and rapidly changing population during urbanization. Taking Qingpu district, a suburb of Shanghai as a case, this study evaluated the spatial accessibility to healthcare services of 203 villages and neighborhoods based on the Two-Step Floating Catchment Area (2SFCA) method by ArcGIS software. The result shows that the spatial accessibility in the whole district is quite uneven under lower thresholds, and t

Socioeconomic variation in asthma hospitalization: excess utilization or greater need?

OBJECTIVE: To assess the hypothesis that higher incidence of severe acute asthma exacerbation, not lower severity threshold for admission, explains the difference between the asthma hospitalization rates of inner-city and suburban children. METHODS: All 2028 asthma hospitalizations between 1991 and 1995 for children (aged >1 month and <19 years) dwelling in Rochester, New York, were analyzed. ZIP codes defined residences as inner-city, other urban, or suburban. Based principally on the worst oxygen saturation (SaO2) during the first 24 hours of hospitalization, severity was examined by hospit

Methadone prescribing by addiction specialists likely to leave communities without available methadone treatment.

Methadone treatment for opioid use disorder is not available in most suburban and rural US communities. We examined 2 options to expand methadone availability: (1) addiction specialty physician or (2) all clinician prescribing. Using 2022 Health Resources and Services Administration data, we used mental health professional shortage areas to indicate the potential of addiction specialty physician prescribing and the location of federally qualified health centers (ie, federally certified primary care clinics) to indicate the potential of all clinician prescribing. We examined how many census tra

Defining Mental Disorder: Differences in Primary Care Providers' and Psychiatrists' Conceptions of Illness.

BACKGROUND: Increasingly primary care providers (PCPs) play a role in recognizing and diagnosing mental disorders. However, how PCPs conceptualize mental disorders within their patient population and how this conceptualization differs and aligns with a more psychiatric approach to mental disorder remains largely unknown. OBJECTIVE: To determine 1) how PCPs and psychiatrists describe their patients with suspected mental health conditions; 2) how PCPs and psychiatrists describe easy to treat and diagnose and challenging to treat and diagnose patients; and 3) how PCPs manage referrals to psychia

Oral Health in Low-Income Older Adults in Korea.

Oral diseases among older adults are prevalent and a major public health problem, but public attention regarding this matter is quite limited. Many older adults experience limited access to oral care services. The study aimed to describe characteristics of oral health conditions, perceived oral health status, and oral health practices and to examine factors related to living status and accessibility to dental health care among 9,660 low-income older adults living in a suburban city in Korea. Approximately 42% of low-income older adults lived alone; 68% perceived their oral health as either exc

Associations between injection risk and community disadvantage among suburban injection drug users in southwestern Connecticut, USA.

Increases in drug abuse, injection, and opioid overdoses in suburban communities led us to study injectors residing in suburban communities in southwestern Connecticut, US. We sought to understand the influence of residence on risk and injection-associated diseases. Injectors were recruited by respondent-driven sampling and interviewed about sociodemographics, somatic and mental health, injection risk, and interactions with healthcare, harm reduction, substance abuse treatment, and criminal justice systems. HIV, hepatitis B and C (HBV and HCV) serological testing was also conducted. Our sample

Presenting to the Emergency Department Versus Clinic-Based Sexually Transmitted Disease Care Locations for Testing for Chlamydia and Gonorrhea: A Spatial Exploration.

BACKGROUND: Rates of sexually transmitted diseases (STDs) including chlamydia and gonorrhea are increasing in the United States while public health funding for STD services is decreasing. Individuals seek care in various locations including the emergency department (ED). The objective of this study is to investigate whether there are more physically proximal clinic-based STD care locations available to individuals who present to the ED in a major metropolitan area. METHODS: Addresses of EDs, clinics, and patients 13 years or older in St. Louis City or County given a nucleic acid amplification

Suburbanisation of oral cavity cancers: evidence from a geographically-explicit observational study of incidence trends in British Columbia, Canada, 1981-2010.

BACKGROUND: Recent studies have demonstrated an elevated risk of oral cavity cancers (OCC) among socioeconomically deprived populations, whose increasing presence in suburban neighbourhoods poses unique challenges for equitable health service delivery. The majority of studies to date have utilised aspatial methods to identify OCC. In this study, we use high-resolution geographical analyses to identify spatio-temporal trends in OCC incidence, emphasising the value of geospatial methods for public health research. METHODS: Using province-wide population incidence data from the British Columbia

Syringe Service Programs for Persons Who Inject Drugs in Urban, Suburban, and Rural Areas - United States, 2013.

Reducing human immunodeficiency virus (HIV) infection rates in persons who inject drugs (PWID) has been one of the major successes in HIV prevention in the United States. Estimated HIV incidence among PWID declined by approximately 80% during 1990-2006 (1). More recent data indicate that further reductions in HIV incidence are occurring in multiple areas (2). Research results for the effectiveness of risk reduction programs in preventing hepatitis C virus (HCV) infection among PWID (3) have not been as consistent as they have been for HIV; however, a marked decline in the incidence of HCV infe

The suburban-city divide: an evaluation of emergency department mental health presentations across two centres.

OBJECTIVE: To evaluate the characteristics of mental health presentations to the emergency department in two different hospital settings. METHODS: This was a retrospective cross-sectional study examining ED referrals to psychiatry in an inner-city and suburban centre. The authors collected data on gender, age, employment, housing, clinical presentation, time of assessment and admissions, over a 1-month period. RESULTS: The total number referred was 213: inner-city n = 109 and suburban n = 104. The inner-city saw a younger population; 47/109 (43%) were aged between 20 and 29 years, compared w

Comparing the Illness Perception, Help-Seeking Behaviors, and Healthcare Determinants Between the Rural and Suburban Indigenous People in Malaysia: A Socio-Biological Perspective.

Due to the health disparities among the Indigenous population, this theory-driven study aimed to compare the suburban and rural Indigenous populations in their illness perceptions, help-seeking behaviors, and determinants of healthcare utilization. Between 2022 and 2023, we conducted a community-based structured survey with the Orang Asli (OA, Indigenous people) residing in suburban (N = 70) and rural regions (N = 131) of Peninsular Malaysia. The orally administered survey consists of demographic and socio-economic info, indigenously developed measurements of illness expression, explanations f

The geography of risk: understanding disparities in nonmedical opioid mortality and the role of socio-built environments in New Jersey.

BACKGROUND: Disparities in nonmedical opioid (NMO) mortality reflect a shifting geography of risk that presents urgent public health challenges. This study uses a socio-built environment (SBE) framework to investigate how place-based conditions shape NMO-related risks across urban, suburban, and rural municipalities in New Jersey. METHODS: Six SBE domains with multiple indicators were analyzed. Generalized linear models with a negative binomial distribution examined associations with NMO mortality, estimating incidence rate ratios with 95% confidence intervals. Domain-level contributions were

Understanding disparities in access to naloxone among people who inject drugs in Southeast Michigan using respondent driven sampling.

BACKGROUND: Given the rising incidence of opioid overdose in the United States, naloxone access is critical for high-risk populations, such as persons who inject drugs (PWID). Yet not all PWID have access to this life-saving antidote. With PWID in Michigan recruited via respondent driven sampling in 2017, after the 2016 standing order expanding naloxone availability through local pharmacies, we explored possible access disparities. METHODS: With 46 seeds recruited from agencies serving local PWID communities, we obtained a sample of N = 410 PWID from Southeast Michigan (n = 285 form urban Det

Asthma care and management before an emergency department visit in children in western Michigan: how well does care adhere to guidelines?

OBJECTIVES: Asthma is one of the more common reasons for children's visits to the emergency departments (EDs). Many studies show that the level of asthma care and self-management in children before an ED visit for asthma is often inadequate; however, most of these studies have been conducted in the inner cities of large urban areas. Our objectives were to describe asthma care and management in children treated for asthma in 3 EDs located in an urban, suburban, or rural setting. METHODS: We studied a prospective patient cohort consisting of children aged 2 to 17 years who presented with an acu
