OBJECTIVES: To report our experience of using freeze-dried irradiated fascia lata allografts for suburethral sling procedures.
PATIENTS AND METHODS: Between December 1996 and September 1998, 35 patients (mean age 60.25 years, range 37-79) underwent suburethral sling placement with fascia allograft. These patients were reviewed, with the findings at the time of any surgical re-exploration. Eleven (31%) had undergone prior surgery for genuine stress incontinence and 32 (91%) had a preoperative diagnosis of intrinsic sphincter deficiency.
RESULTS: On re-operation for persistent or recurrent str

Since the introduction of original tension-free vaginal tape in 1995, a number of different suburethral slings have been marketed. Much of the published literature on midurethral sling procedures relates to the original TVT. Although there has been a considerable number of publications comparing different routes of tape insertion, there is relatively little published identifying the efficacy of different retropubic slings. This study was designed to identify the success rate and complications in a case series of Advantage slings. A total of 70 women were assessed 6 months after insertion of an

BACKGROUND: The suburethral sling is an effective surgical procedure for curing genuine stress incontinence. However, a well-known complication is urinary retention. This report describes a method of modifying sling placement 6 weeks after the initial surgery to ameliorate the complication of urinary retention.
CASE: A 68-year-old woman had undergone suburethral sling placement as part of her pelvic reconstructive surgery. Preoperatively, she had objective, urodynamic evidence of severe genuine stress incontinence, consistent with intrinsic sphincter deficiency. She was unable to void for 6 w

OBJECTIVE: Autologous rectus fascia is commonly used to construct suburethral slings for the treatment of genuine stress incontinence. This fascia performs well and has not been associated with clinical problems related to its choice as a sling material. However, the histologic appearance of such slings after implantation has not been documented.
STUDY DESIGN: At the time of revision of autologous rectus fascia suburethral slings in 5 patients, biopsy specimens of the slings were obtained and submitted for histologic examination. A specimen of rectus fascia before implantation was also obtain

INTRODUCTION AND HYPOTHESIS: We aimed to evaluate the success of suburethral slings in women ≥70 years of age.
METHODS: This was a retrospective cohort study of women who underwent suburethral sling placement. Subjects were separated into three groups: ≤50 years of age (group 1), 51 to 69 years of age (group 2), and ≥70 years of age (group 3). The primary aim was to evaluate success as defined by ≥ improved on a validated patient improvement satisfaction score and a negative postoperative standardized stress test.
RESULTS: There were 1,464 subjects. Mean age was 44.51 ± 4.25 (n = 296) for gr

AIM: Recently a miniature version of the Monarc suburethral sling has been introduced. This paper aims to evaluate the postoperative recovery, effectiveness and complications of these two types of suburethral sling.
METHODS: This was a prospective randomized series of 50 female patients with stress incontinence who underwent either a Monarc or Miniarc suburethral sling.
RESULTS: The only significant intraoperative difference was a shorter operation time for the Miniarc (18.8 vs 22.4 min). The success rates were similar at 6 weeks and 6 months.
CONCLUSION: There were no significant differenc

Considering the poor long term results with the bladder neck suspensions, the poor results with the retropubic approach in obese or intrinsic sphincter deficiency and the complexity of the classical retropubic sling procedure, a simple vaginal sling urethropexy approach was developed. The vaginal mucosa is opened at 12 o'clock under the urethra. Dissection of the endopelvic fascia is undertaken. Polypropylene sutures are placed at the 4 corners of a free 2 x 4 cm rectus abdominis muscle flap which is suspended under the bladder neck transvaginally. Polypropylene sutures are tied at the rectus

Pubovaginal slings are being used more frequently for complicated stress urinary incontinence (SUI) secondary to impairment of the proximal sphincteric mechanism. The excellent long-term results of the sling surgery expands its role in genuine stress incontinence, especially high-risk groups. The modification described herein simplifies the surgery and avoids many of its potential complications, meanwhile maintains a high success rate.

OBJECTIVE: The purpose of this study was to examine the histopathologic changes of HMDI (Hexamethylene di-isocyanate) cross-linked porcine dermis grafts used for suburethral sling surgery.
STUDY DESIGN: Twelve patients underwent reoperation with graft removal for urinary retention or recurrent stress urinary incontinence after transvaginal sling surgery. Tissue specimens were available for pathologic evaluation in 7 patients. Graft specimens underwent histologic preparation including hematoxylin and eosin staining. A single pathologist reviewed the slides blinded to clinical outcomes.

OBJECTIVES: To ascertain whether a phenomenon of sling migration exists after suburethral sling placement, whether this might be responsible for suboptimal sling location and persistent incontinence, and whether a link exists between sling dislocation or migration and risk factors, such as obesity or age.
METHODS: The present prospective cohort study was carried out in a group of 244 patients who underwent retropubic sling implantation. Sling location was determined by means of pelvic floor ultrasound, and calculated relative to the individual patient's urethral length measured before the pro

The study objective was to review the existing literature regarding complications of anti-incontinence sling procedures. PubMed listings using keywords related to slings and associated complications with no date or language restrictions through May 2007 and the Manufacturer and User Facility Device Experience Database were searched for specific device- and procedure-related complications. Where no information was available, published abstracts were cited. Published reports of complications for all types of anti-incontinence sling procedures are analyzed and reported. INTRODUCTION: Impact on sexual function has received little attention in the medical literature for a long time. Because of the site of insertion of permanent tension free vaginal tape (TVT) the G spot might be affected or the tape might interfere with arousal and sensory stimulation. Recent studies have reported varying degrees of sexual impairment after TVT insertion ranging from 0% to 15% including dyspareunia.
AIM: The aim of this study was to evaluate sexual function before and after suburethral sling removal due to postoperative female de novo dyspareunia. As a secondary outcome, genera
