Prenatal superficial musculoaponeurotic system anlage.

INTRODUCTION: The superficial musculoaponeurotic system (SMAS) is an acronym describing a facial subcutaneous anchored three-dimensional (3D) fibro-adipose-muscular tissue network connected to mimic muscles. SMAS transfers, distributes and reinforces mimic facial muscles contractions to the skin determining mimic expression and facial fold formation. The aim of this study was the histomorphological analysis of prenatal SMAS (pre-SMAS) development in analogy to the adult SMAS typology. METHOD: Histological serial sections (n=7300 of 31 embryos and fetuses) of different staining obtained from t

The Extended Superficial Musculoaponeurotic System.

The extended superficial musculoaponeurotic system (SMAS) facelift targets the 2 cardinal signs of facial aging: (1) descent of the malar fat resulting in deepening of the nasolabial folds as well as accentuation of the palpebral malar groove and tear trough deformity; (2) formation of jowls, which also obscure the definition of the lower mandibular border. In this article the authors describe the concepts behind the extended SMAS operation, importantly the extent of skin dissection and SMAS elevation. They present a brief history of the operation and landmark papers and supplement with fresh

Nuances in Superficial Musculoaponeurotic System Rhytidectomy.

Rhytidectomy techniques have evolved since the early 1900s. As the understanding of facial anatomy and the aging process expanded, the superficial musculoaponeurotic system (SMAS) became a focal point in developing longer-lasting, natural results. Further evolution led to various approaches in repositioning the SMAS layer, including subperiosteal, composite, and deep plane rhytidectomies. This article describes the nuances of SMAS rhytidectomy, the biplanar SMAS imbrication technique, and adjuvant procedures used. This biplanar SMAS technique has been refined over more than 25 years and has pr

Extended Superficial Musculoaponeurotic System Flap Rhytidectomy.

The various rhytidectomy techniques share a common goal of safe repositioning of the facial soft tissues with a lasting effect. This article reviews rhytidectomy approaches and the current methods and practice patterns of the senior author. It includes a discussion of the extended sub-superficial musculoaponeurotic system rhytidectomy technique, which, in the opinion of the senior author, offers the best result with respect to neck rhytids, cervicomental angle and jawline definition, and improvement of jowling. With its ability to be readily coupled with a deep plane dissection, when indicated

Superficial Fascia in the Cheek and the Superficial Musculoaponeurotic System.

The origins and validity of the term "superficial musculoaponeurotic system" (SMAS) is reviewed. Gray stated the superficial fascia connects the skin with the deep or aponeurotic fascia and consists of fibro-areolar tissue. Hollinshead wrote superficial fascia exists throughout the body and contains a variable amount of fat. In the head and neck, it encloses voluntary muscles in its deep portion. Skoog found superficial fascia was fixed to the dense, deep fascia by fibrous adhesions in the temporal, preauricular, and parotid area. Mitz stated "There is a 'superficial muscular and aponeurotic s

The Extended Superficial Musculoaponeurotic System Deep Plane Facelift.

Over the past four decades, the senior author has evolved and perfected his preferred method for face and neck lifting, the extended superficial musculoaponeurotic system deep plane rhytidectomy, and submentoplasty. With this procedure, the superficial musculoaponeurotic system layer is addressed both in the face and neck, repositioning the tissues in a superior and posterior vector and creating a sling in the cervicomental area. Outcomes have proven to be both successful and long lasting. In this article, a detailed update and discussion of the specific techniques utilized are provided.

Superficial musculoaponeurotic system elevation and fat graft reconstruction after superficial parotidectomy.

OBJECTIVE/HYPOTHESIS: Elevation of the superficial musculoaponeurotic system (SMAS) with or without fat graft interposition during superficial parotidectomy prevents a concave facial deformity and Frey's syndrome. STUDY DESIGN: Retrospective, case-control study. METHODS: Charts for 248 patients who underwent superficial parotidectomy were reviewed for pathologic, radiographic, clinical, and operative data. Sixteen patients who underwent SMAS elevation and 34 patients who underwent SMAS elevation with fat graft interposition were included in two study groups. Nonreconstructed controls were ra

Prevention of Frey's syndrome with superficial musculoaponeurotic system interposition.

Frey's syndrome after parotidectomy has an incidence generally reported to be 50% to 60% and is thought to be due to aberrant reinnervation of sweat glands by postganglionic parasympathetic fibers normally supplying the parotid gland. One hundred twelve patients had interposition of a flap of the superficial musculoaponeurotic system between the parotid bed and the overlying skin. Only one patient developed symptomatic Frey's syndrome, and one other patient had positive results on starch iodine testing. In patients not having the interposition, the incidence of symptomatic Frey's syndrome and

The Phylogeny and Ontogeny of Facial Superficial Musculoaponeurotic System.

INTRODUCTION: The superficial musculoaponeurotic system (SMAS), a complex network of fibromuscular tissue, is a fundamental component of facial anatomy, playing a key role in both dynamic function and aesthetic appearance. Although the anatomy of the SMAS has been widely studied, several aspects remain unclear-particularly the presence of its fascial component, the variability in its attachment to deeper tissues, the possibility of distinct SMAS layers, and the association of facial nerve branches with first arch structures. In this study, we propose a hypothesis to explain the anatomical vari

U-SMAS: ultrasound findings of the superficial musculoaponeurotic system.

The superficial musculoaponeurotic system (SMAS) is a complex fibrous network connecting facial muscles to the dermis, with varying morphological characteristics across different facial regions. Recent studies have identified five distinct types of SMAS morphology, highlighting the need for region-specific interventions in facial rejuvenation. This pictorial essay explores ultrasound imaging of the SMAS using ultra-high frequency (24-33 MHz) probes, known as U-SMAS. Analysis of 186 full-face U-SMAS scans revealed consistent patterns in the facial and neck layers, with regional variations align

Superficial musculoaponeurotic system interpositional graft after temporomandibular joint discectomy.

Arthroplasty with discectomy is a proven treatment for internal derangement of the temporomandibular joint (TMJ), however there is no consensus on the type of interpositional graft that should be used after the disc is removed. While an ideal graft should be easy to obtain and provide an adequate buffer between the articular surfaces, the authors suggest that it should also minimize donor site morbidity. This Technical Note highlights the technique for harvesting and utilizing the superficial musculoaponeurotic system (SMAS) as an interpositional graft. Three patients were treated with a SMAS

Increasing Superficial Musculoaponeurotic System Durability and Retention in Facelift Surgery.

BACKGROUND: Facelift surgery remains the gold standard for facial rejuvenation, yet limitations such as suture pull-through failure, superficial musculoaponeurotic system (SMAS) laxity, and the need for secondary procedures persist. Although novel facelift techniques have emerged, none have effectively addressed the inherent biomechanical challenges. Photochemical tissue passivation (PTP) is a promising method to enhance tissue durability and improve facelift longevity by modifying the biomechanical properties of the SMAS itself. OBJECTIVES: In this study, the authors investigated whether PTP

Modification of Rhytidectomy: Stepped Lift of the Superficial Musculoaponeurotic System.

Rhytidectomy is a complicated surgery. Less experienced surgeons are often confronted with technical challenges. To simplify the procedure and achieve long-lasting effects, the authors propose a modified rhytidectomy method, reorder surgical sequences and suggest a stepped lift of the superficial musculoaponeurotic system.A total of 54 patients underwent this surgical procedure. Superficial musculoaponeurotic layers of the temple, face, and neck were dissected sequentially and suspended to lift the ptosis tissue. They scored the frontalis and excised the corrugator after raising the flap in th

Superficial musculoaponeurotic system flap for reconstruction of the intraoral defects.

Intraoral tumors are the main causes of the intraoral and maxillofacial defects. Skin grafts and several soft tissue flaps can be used to reconstruct the intraoral defects including local, regional, and free flaps. Here we present a case of intraoral adenocystic carcinoma treated with segmental maxillectomy, resection of the parotid gland, and the buccal mucosa leaving a full-thickness intraoral defect. The defect was covered with a superficial musculoaponeurotic system (SMAS) flap elevated using the preauricular incision. Reconstruction with SMAS flap for temporal defects and parotidectomy de

Application of the extended superficial musculoaponeurotic system flap to facial clefts.

We believe that the extended SMAS technique may be useful in selected facial cleft patients. In three Treacher Collins patients, this technique produced a modest improvement in the quantity and quality of the malar soft tissue that had not been achieved after orbitozygomatic or maxillomandibular surgery.
