In ineffective esophageal motility, failed swallows are more functionally relevant than weak swallows.

BACKGROUND: Esophageal pressure topography (EPT) diagnosis of ineffective esophageal motility (IEM) can be non-specific with unclear clinical significance. AIMS: To determine whether peristaltic vigor or lower esophageal sphincter (LES) integrity is associated with poor clearance and acid reflux in IEM. METHODS: Bolus clearance on high-resolution impedance manometry (HRIM) and available reflux studies in patients with IEM were retrospectively reviewed. Bolus clearance was assessed using both line tracing and colored contour methods on HRIM. EPT parameters, bolus clearance, and acid reflux va

[When the intestine swallows the pounds].

The case of an 88-year-old male patient with progressive, unintentional weight loss is reported. The condition was subjectively asymptomatic and subsequently completely reversible. Following a comprehensive internal medicine examination, chronic mesenteric ischemia due to arteriosclerosis was identified as the cause and treated interventionally. Over time, the patient experienced significant weight gain, a comparatively rare phenomenon in very old geriatric patients. Broad and targeted investigation of the cause and interventional therapies are safe and feasible even in old age.

Solid bolus swallows during high-resolution manometry complement multiple rapid swallows in predicting symptoms following antireflux surgery.

BACKGROUND: Solid bolus swallows (SBS) assess esophageal peristalsis on high-resolution manometry (HRM). It remains unknown if increase in contraction vigor (contraction reserve) following SBS (SBS-CR) provides information similar to that observed following multiple rapid swallows (MRS-CR). METHODS: Clinical and HRM data from adult patients (n = 96, age 58.2 years, 61.5% female) undergoing esophageal testing prior to anti-reflux surgery (ARS) were analyzed if both MRS and SBS were available, and compared to similar data from healthy asymptomatic volunteers (n = 18, age 27.5 years, 55.6% femal

Advancing high-resolution manometry: evaluating the use of multiple rapid swallows versus apple viscous swallows in clinical practice.

BACKGROUND: High-Resolution Manometry (HRM) with provocative maneuvers, such as Multiple Rapid Swallows (MRS) and Apple Viscous Swallows (AVS), is commonly utilized to diagnose esophageal disorders. Increasing standardization in HRM protocol can help save time and reduce patient discomfort. This study assesses AVS and MRS to determine their respective benefits and limitations. METHODS: Retrospective reviews were performed on 100 patients to analyze their AVS and/or MRS results. Parameters included abnormal motility patterns, tolerance, and DCI. Diagnostic benefits from MRS and AVS were assess

Fragmented and failed swallows on esophageal high-resolution manometry associate with abnormal reflux burden better than weak swallows.

BACKGROUND: Association between proportions of hypomotile swallows on esophageal high-resolution manometry (HRM) and esophageal reflux burden remains incompletely understood. We investigated relationships between hypomotility, acid exposure time (AET), and mean nocturnal baseline impedance (MNBI) on ambulatory reflux monitoring. METHODS: Clinical data, HRM, and ambulatory pH-impedance studies (performed off acid suppression) from patients with persisting reflux symptoms were reviewed from five international centers. AET (abnormal > 6%) and MNBI (abnormal < 2292 ohms) were extracted from pH-im

VIDEOFLUOROSCOPIC EVALUATION OF SWALLOWS IN ANOREXIA NERVOSA.

BACKGROUND: There are some studies in the literature about the feeding behavior and masticatory process in patients with feeding disorders; however, it is not very well known if there are alterations in oral-pharyngeal swallowing dynamics in subjects with anorexia nervosa. OBJECTIVE: To evaluate the oral and pharyngeal bolus transit in patients with anorexia nervosa. METHODS: The study was conducted with 8 individuals clinically diagnosed and in treatment for restricting-type anorexia nervosa (seven women and one man), and 14 healthy individuals with no digestive or neurological symptoms (10

Are contrast swallows necessary immediately postlaparoscopic Heller cardiomyotomy?

Laparoscopic cardiomyotomy is an effective treatment for achalasia. Intraoperative leak tests are carried out to exclude mucosal perforations, additionally some surgeon perform postoperative contrast swallows. The aim of the study was to identify whether postoperative contrast swallows were necessary in all patients who undergo laparoscopic cardiomyotomy. All patients who underwent a laparoscopic cardiomyotomy at a single center between 2004 and 2011 were identified. Median age was 55 (18 to 79), median body mass index 26 (17 to 37), and median length of stay was 1 day (1 to 4). A total of 54%

Evaluation of manometric measures during tongue-hold swallows.

PURPOSE: Based on visual inspection, prior research documented increased movement of the posterior pharyngeal wall in healthy volunteers during tongue-hold swallows. This manometric study investigated the immediate effects of the tongue-hold maneuver on pharyngeal peak pressure generation, duration of pressure generation, and pressure slope measurements in healthy volunteers. METHOD: Pharyngeal pressures from 40 young, healthy individuals (mean age = 25.8 years, gender equally distributed) were recorded at 3 locations: oropharynx, hypopharynx, and upper esophageal sphincter (UES), during norm

Patterns of esophageal dysmotility elicited by multiple rapid swallows.

BACKGROUND: High-resolution manometry (HRM) is a gastrointestinal motility diagnostic system that measures intraluminal pressures using closely aligned sensors. Multiple rapid swallows (MRS) are used in conjunction with HRM to assess esophageal physiology prior to anti-reflux and hiatal hernia procedures. METHODS: A retrospective, qualitative study was conducted on 90 patients who underwent HRM with MRS in a single community clinic. 80 patients met the inclusion criteria. MRS testing consisted of rapid 2 mL swallows in 2-3 s intervals with patients in a seated, upright position. Clinical info

Costs and benefits of late nesting in cliff swallows.

Many organisms of temperate latitudes exhibit declines in reproductive success as the breeding season advances. Experiments can delay the onset of reproduction for early breeders to investigate the consequences of late nesting, but it is rarely possible to observe a distinct second round of nesting in species that normally nest only once. The colonial cliff swallow (Petrochelidon pyrrhonota) is a migratory songbird that has a relatively short breeding season in the western Great Plains, USA, with birds rarely nesting late in the summer. Previous work suggested that ectoparasitism is a primary

Are contrast swallows a relevant investigation for paediatric stridor?

The contrast swallow has traditionally been used in the investigation of paediatric stridor with the aim of identifying pathology such as vascular rings and confirming the presence of gastro-oesophageal reflux. Currently, a laryngotracheobronchoscopy (LTB) is the gold standard investigation for stridor and frequently, a child is commenced on antireflux medications based on clinical history. We seek to identify whether a contrast swallow has a valuable and cost effective role in the investigation of paediatric stridor. This was a retrospective audit study. All the children who underwent a contr

Elevated frequency of abnormalities in barn swallows from Chernobyl.

Ever since the Chernobyl accident in 1986, that contaminated vast areas in surrounding countries with radiation, abnormalities and birth defects have been reported in human populations. Recently, several studies suggested that the elevated frequency of such abnormalities can be attributed to poverty and stress in affected human populations. Here, we present long-term results for a free-living population of barn swallows, Hirundo rustica, demonstrating the presence of 11 morphological abnormalities in populations around Chernobyl, but much less frequently in an uncontaminated Ukrainian control

Reproductive Drivers of Temporal Telomere Dynamics in Female Tree Swallows.

Understanding trade-offs between life-history traits, particularly between reproductive investment and somatic maintenance, remains a major challenge in evolutionary ecology. Telomeres progressively shorten with age and have been suggested as good predictors of survival and longevity. Both intrinsic factors and external stressors, including high reproductive effort, accelerate telomere attrition. Thus, the dynamic nature of telomere length (TL) offers a promising avenue for assessing individual variation in the cost of reproduction. Here, we quantified telomere length (TL) in breeding female T

Hyolaryngeal Movement During Normal and Effortful Swallows Determined During Ultrasonography.

PURPOSE: This study investigated (a) the effects of the effortful swallow under two different instructions (tongue emphasis vs. pharyngeal squeezing) on hyoid displacement and hyoid-larynx approximation, (b) the association between tongue pressure and hyolaryngeal movement during normal swallowing and the effortful swallow produced with tongue emphasis, and (c) age-related differences in hyolaryngeal movement during normal and effortful swallows (tongue emphasis vs. pharyngeal squeezing) in healthy individuals. METHOD: Forty healthy adults (20 younger and 20 older) swallowed their saliva as t

Increased oxidative stress in barn swallows from the Chernobyl region.

The Chernobyl nuclear accident produced the largest unintended release of radionuclides in history, with dramatic consequences for humans and other organisms. Exposure to ionizing radiation is known to reduce circulating and stored levels of specific antioxidants in birds and humans, thus potentially increasing oxidative stress. However, overall effects of radioactive exposure on oxidative status have never been investigated in any free ranging vertebrate. We measured plasma antioxidant capacity and concentration of reactive oxygen metabolites in adult barn swallows (Hirundo rustica) from colo

Albinism and phenotype of barn swallows (Hirundo rustica) from Chernobyl.

The effects of mutation on phenotypic expression are supposed to be mainly deleterious because mutations disrupt the expression of genes that function relatively well under current environmental conditions. Thus, mutations are assumed to give rise to deviant phenotypes that are generally selected against. Radioactive contamination in the Chernobyl region of Ukraine is associated with a significant increase by a factor two to 10 in mutation rate in microsatellite markers of the barn swallow, Hirundo rustica. Barn swallows from Chernobyl had a temporally constant, elevated frequency of partial a

Automated acoustic analysis in detection of spontaneous swallows in Parkinson's disease.

Acoustic monitoring of swallow frequency has become important as the frequency of spontaneous swallowing can be an index for dysphagia and related complications. In addition, it can be employed as an objective quantification of ingestive behavior. Commonly, swallowing complications are manually detected using videofluoroscopy recordings, which require expensive equipment and exposure to radiation. In this study, a noninvasive automated technique is proposed that uses breath and swallowing recordings obtained via a microphone located over the laryngopharynx. Nonlinear diffusion filters were use
