[The complete dislocation of the talus or «missing talus».].

The complete dislocation of the talus or «missing talus» corresponds to a simultaneous dislocation of the tibiotalar, talocalcaneal and talonavicular joints, without associated talar fracture. This lesion occurs as part of high energy trauma and accounts for 3 to 5 % of talus injuries. It is most frequently anterolateral and in more than 75 % of cases, this dislocation is open. Among the soft tissue lesions that are often associated with the dislocation, vascular lesions leading to avascular necrosis of the talus are found in almost 100 % of cases. This necrosis is generally well tolerated by

Defining and Differentiating Congenital Vertical Talus and Congenital Oblique Talus: It's Not Primarily About the Talus.

BACKGROUND: Congenital vertical talus (CVT) and congenital oblique talus (COT) are rocker-bottom foot deformities that have similar names and no objective definitions. This has led to confusion for practitioners, as well as scientific challenges for researchers. Our goal was to provide objective radiographic criteria to define and differentiate CVT and COT. METHODS: We evaluated 62 pairs of maximum dorsiflexion and plantar flexion lateral radiographs of infant feet that had been clinically diagnosed with CVT. The dorsiflexion tibiotalar angle, the plantar flexion talus-first metatarsal angle,

[Vertical talus].

Authors differentiate four etiological groups of vertical talus. In their material the congenital and symptomatic rocker bottom foot was rarer than the paralytic one. Good result can be expected mainly from the operative treatment. From 69 cases good clinical and radiological result was reached in 58, whereas in 6 cases the treatment had no result. It was their experience that a reduction of the talus and of the navicular bone can be reached by tenotomies and capsulotomies and by these a normal form of the foot can be secured. In the conservation of the operative results the significance of th

[Post-traumatic ischemia of the talus. Is talus necrosis unavoidable?].

50 patients following talar fractures type Marti 2, 3 and 4 during the years 1972-1993, could be analysed retrospectively. Evaluated factors effecting posttraumatic avascular necrosis of the body of the talus are: type of fracture, age of the patient, additional fracture of the medial malleolus and time of non weight bearing. The Hawkins sign is reliable to show vitality in the body of the talus. We could see vascular impairment in 51% of all Marti 3 fractures and in 100% of all Marti 4 fractures. Patients with a concomitant fracture of the inner malleolus showed positive influence on the bloo

Chondroblastoma of the talus.

We report the case of a 13-year-old male child who presented with a painful left ankle. On imaging (radiography and computed tomography scan with 3-dimensional reconstruction views), an osteolytic lesion in the body of the talus was revealed. Open biopsy, curettage, and fibular bone grafting were done, and the specimen was sent for histopathologic examination. The histopathologic report confirmed the specimen to be chondroblastoma of the talus bone. Chondroblastoma is a rare benign cartilaginous neoplasm that accounts for approximately 1% of all bone tumors and characteristically arises in the

Imaging of the talus.

There are numerous options for imaging talar pathology. Of these, radiography, CT, scintigraphy, and MRI are used most commonly. Advantages and limitations of these techniques should be considered when formulating an imaging algorithm for evaluation of the patient's clinical problem.

Fractures of the talus.

A total of 35 patients treated in the years 1970 to 1977 for fractures of the talus were investigated for an analysis of the long-term clinical end results. Falling from height was the most common cause of injury (15 patients), the next most common being road traffic accidents (13 patients). The mean followup time was 8 years. At followup, 19 patients were free of any discomfort and none had pain at rest. The incidence of pain was correlated significantly (p less than 0.01) with the grade of dislocation both before and after reduction. In severely dislocated fractures open reduction and osteos

Morphomics of the Talus.

Previous studies of frontal crash databases reported that ankle fractures are among the most common lower extremity fractures. While not generally life threatening, these injuries can be debilitating. Laboratory research into the mechanisms of ankle fractures has linked dorsiflexion with an increased risk of tibia and fibula malleolus fractures. However, talus fractures were not produced in the laboratory tests and appear to be caused by more complex loading of the joint. In this study, an analysis of the National Automotive Sampling System - Crashworthiness Data System (NASS-CDS) for the year

Fractures of the talus.

The talus is the most proximal bone of the hindfoot that couples the foot to the leg. It is the second most common fracture of the tarsal bones, second in frequency to the calcaneous. However, overall injuries to the talus are relatively rare, and most surgeons have little experience in managing them. This article discusses fractures of the talus, including injuries to the talar neck, body, head, and processes. Although subtalar dislocations and osteochondral injuries are important topics, they are not addressed in this article.

[Fractures of the talus].

Fractures of the neck of the talus are a relatively uncommon fracture of the foot but they have potentially serious complications. This article details the Hawkins classification, operative treatment and indications, and complications of fractures of the neck of the talus. It also discusses the treatment of fractures of the body of the talus and talar head. An English full text version of this article is available at SpringerLink as supplemental.

[Fractures of the talus].

Peripheral fractures of the talus, such as fractures of the posterior and lateral process, need no special therapy. Larger fragments with dislocation require open reduction and screw fixation. Small flake fractures with stability of the joint can be removed arthroscopically, but larger fractures should be repositioned with K-wires, small screws. Ethipin or fibrin sealant. Dislocated fractures of the head of the talus should be reduced and fixed by screws or K-wires. Fractures of the neck of the talus with anteromedial or lateral dislocation can be treated by closed reduction and external fixat

Total Talus Replacements.

BACKGROUND: Total talus replacements are a surgical treatment for talar avascular necrosis (AVN) replacing the entire talus. The potential for total talus replacements has increased with the advent of patient-specific implants using 3D printing based on computed tomographic scanning of the ipsilateral or contralateral talus. The primary aim of this review is to summarize the literature on total talus replacements, providing a historical survey, indications, controversies, complications, survival, and functional outcomes. METHODS: A systematic review was performed. Articles with survival of to

Congenital Vertical Talus.

Congenital vertical talus represents a congenital structural foot deformity characterized by the classical rocker bottom deformity. The main feature is dislocation of the talonavicular joint along with contractures of the dorsolateral tendons of the foot and tendo Achilles. In the past treatment consisted of 2- or single-stage more or less extensive soft tissue releases including reduction of the talonavicular joint following casting in the first phase. Nowadays all feet are treated by serial casting, closed or miniopen talonavicular joint reduction, and percutaneous achillotenotomy. Functiona

Talus fracture management.

Talar head and fracture injuries are not easily recognized and can create significant long-term disability when missed. Careful investigation of any injury about the ankle requires both clinical and radiographic examination. A computed tomography scan is extremely helpful in diagnosing and treating talus fractures. Displaced fractures require open reduction of the major joint surfaces and internal fixation. Prolonged non-weight bearing and immobilization is the norm. Despite aggressive management, complications involving avascular necrosis and posttraumatic arthritis to both the subtalar and t

[Congenital round talus].

The collective consists of 11 patients with a "ball and socket"-anomaly of the talus with a mean age of 13.7 years. All patients demonstrated typical symptoms of the FFU-syndrome with a leg length discrepancy of 4.3 cm (2-20). In 6 patients a close follow-up was done in the early development of the talus form. In the metacarpal region also synostosis of some bones were seen at a mean age of 4.8 years (3.2-5.9). No clinical complaints but a slight reduction of hind foot mobility could been registered. Ball and socket deformity can not been seen as congenital, but as a subsequent deformity after

Congenital vertical talus.

A classification of Vertical Talus deformities into three types is proposed. The three types, Teratogenic, Neurogenic and Acquired have different etiologies, clinical features and behavior characteristics. It is anticipated that this classification will permit easier recognition of these entities and development of more individualized treatments.

3D Printed Total Talus Replacement for Avascular Necrosis of the Talus.

BACKGROUND: Talus avascular necrosis (AVN) is a challenging entity to treat. Management options depend on disease severity and functional goals. Total talus replacement (TTR) is a treatment option that maintains joint range of motion. The literature on TTR is limited with variability in implant design and material. The purpose of this study was to evaluate outcomes following TTR with a custom 3D printed metal implant. METHODS: Patients who underwent TTR were retrospectively reviewed over a 3-year period. Basic demographic data and comorbidities were collected. Medical records were reviewed to
