Safety evaluation of telbivudine.

IMPORTANCE OF THE FIELD: Successful treatment of chronic hepatitis B (CHB) often requires long-term oral nucleoside/nucleotide agents which can be associated with viral resistance, patient non-compliance and adverse effects. Telbivudine is one of the more potent options available, with a 6.5- to 6.6-log copies/ml hepatitis B DNA reduction at 12 weeks in an early viral kinetic study, a potency comparable to entecavir. It is also one of the few drugs in the treatment of CHB under FDA pregnancy Category B. AREAS COVERED IN THIS REVIEW: The efficacy and safety profile of telbivudine in compensate

Clinicopathological Features of Telbivudine-Associated Myopathy.

Telbivudine, a thymidine nucleoside analog, is a common therapeutic option for chronic hepatitis B infection. While raised serum creatine kinase is common, myopathy associated with telbivudine is rare. Reports on its myopathological features are few and immunohistochemical analyses of inflammatory cell infiltrates have not been previously described. We describe the clinical, myopathological and immunohistochemical features of four patients who developed myopathy after telbivudine therapy for chronic hepatitis B infection. All four patients presented with progressive proximal muscle weakness, e

Transplacental Transfer of Telbivudine in Hepatitis B Virus-Infected Pregnant Women Treated with Telbivudine during the Third Trimester.

Telbivudine is an FDA-approved pregnancy category B drug for the treatment of chronic hepatitis B virus (HBV) infection. It has been used in HBV-carrier women with high viremia to prevent mother-to-child transmission of HBV. However, placental transfer of telbivudine from mother to fetus has not been studied. Using a sensitive ultraperformance liquid chromatography-tandem mass spectrometry method, we measured the concentration of telbivudine in maternal blood collected at delivery from 29 pregnant women, who took telbivudine (daily 600 mg) from gestation week 28-32 to week 4 postpartum, with a

Telbivudine attenuates gentamicin-induced kidney injury in rats.

Nephrotoxicity has been associated with nucleos(t)ide analogues other than telbivudine (LdT). This study investigated the potential effects of LdT and lamivudine (LAM) on renal function in an experimental rat model of gentamicin-induced acute nephrotoxicity. A total of 28 healthy Wistar albino rats were randomly divided into four experimental groups: negative control; positive control (PC); LdT; and LAM. Nephrotoxicity was induced by gentamicin in the LdT, LAM and PC groups. LdT and LAM were administered to two groups for 6 weeks starting on the ninth day. Blood samples were collected weekly a

Telbivudine myopathy in a patient with chronic hepatitis B.

CASE: A 25-year-old man with hepatitis B virus (HBV) infection received antiviral treatment with telbivudine 600 mg daily. Six months after starting treatment, the patient developed progressive weakness and myalgia. Physical examination showed symmetrical proximal weakness. Blood tests at admission revealed positive hepatitis b surface antigen (HBs Ag), and, elevated creatine kinase (CK) level (1,614 U/L, normal range: 38-174 U/L). Aspartate aminotransferase was 64.7 U/L (normal range: 8-40 U/L), and LDH was 293 U/L (normal range: 80-285 U/L). Electrodiagnostic studies indicated myopathic chan

Myotoxicity of telbivudine in pre-existing muscle damage.

OBJECTIVES: It is unknown if telbivudine causes muscle damage only in patients with pre-existing muscle pathology. CASE REPORT: A 27 yo male of African origin received telbivudine for hepatitis B during 3 months. Three weeks after initiation of the drug he developed myalgia, and tiredness. Creatine-kinase increased from 278 U/l (n, <170 U/l) at baseline to 3243 U/l. Shortly after discontinuation of telbivudine muscle symptoms and hyper-CK-emia disappeared. The findings suggest that pre-existing muscle damage favored the myotoxic effect of telbivudine. CONCLUSIONS: Telbivudine appears to caus

Telbivudine: a new treatment for chronic hepatitis B.

Three hundred and fifty million people worldwide are estimated to be chronically infected with hepatitis B virus. 15%-40% of these subjects will develop cirrhosis, liver failure or hepatocellular carcinoma during their life. The treatment of chronic hepatitis B has improved dramatically over the last decade merits to the advent of nucleoside/nucleotide analogues and the use of pegylated interferons. Approved drugs for chronic hepatitis B treatment include: standard interferon-alpha 2b, pegylated interferon-alpha 2a, lamivudine, adefovir dipivoxil, and entecavir. Unfortunately, these agents are

Therapy of chronic hepatitis B: focus on telbivudine.

Hepatitis B virus (HBV)viral load is closely related to necroinflammation and the outcome of chronic hepatitis B. The available treatment options to reduce viral load, and hence improve outcome, are either based on IFN or on nucleoside/nucleotide analogue antiviral agents, which inhibit HBVDNA replication. Use of IFN alfa or pegylated IFN alfa-2a for periods longer than 48 weeks is limited by their side-effects. The antiviral agents have much more acceptable side-effect profiles, and lamivudine, the first antiviral to become available, was widely used until it became apparent that it carries a

Telbivudine: a new option for the treatment of chronic hepatitis B.

Chronic hepatitis B virus (HBV) infection affects > 350 million individuals worldwide. Chronic hepatitis B is associated with complications of end-stage liver disease, including cirrhosis and hepatocellular carcinoma. HBV replication is the best predictor of liver disease progression to cancer, and antiviral therapy may diminish or halt this unfavorable outcome. Six drugs have been approved for the treatment of chronic hepatitis B: interferon-alpha(2b), pegylated interferon-alpha(2a), lamivudine, adefovir, entecavir and telbivudine. Most agents designed to target hepatitis B are hindered by th

Telbivudine-Induced Myopathy: Clinical Features, Histopathological Characteristics, and Risk Factors.

BACKGROUND AND PURPOSE: Oral nucleos(t)ide analogs (NAs) are the mainstay treatment for chronic hepatitis B (CHB). Myotoxicity is an important extrahepatic effect related to NA treatment. Telbivudine is the NA for CHB that is frequently associated with muscle-related side effects. The risk factors for telbivudine-induced myopathy (TIM) are not yet clear. METHODS: This study characterized the clinical, magnetic resonance images (MRI), and pathological features of 12 TIM cases. A group of telbivudine-tolerant (TT) patients with CHB who received regular telbivudine treatment during the same peri

Insights into the mechanisms of telbivudine-induced myopathy associated with mitochondrial dysfunction.

As a nucleotide analogue (NA), telbivudine was widely used in the treatment for chronic hepatitis B (CHB) by interfering with reverse transcriptase of hepatitis B virus. However, the use of NAs for hepatitis B treatment has been accompanied by numerous reports highlighting the occurrence of neuromyopathy, particularly in the case of telbivudine. This study aimed to investigate the underlying mechanisms responsible for telbivudine-induced myopathy. We established animal and cell models of telbivudine-induced myopathy using C57BL/6 mice and C2C12 cells, respectively. Our findings revealed that t

Safety of telbivudine treatment for chronic hepatitis B for the entire pregnancy.

Orally administered nucleus(t)ide analogues (NA) have brought about a simple, safe and effective therapeutic approach for chronic hepatitis B (CHB). However, treatment duration is long and some female patients become pregnant during treatment. In recent years, there have been gradually increasing reports on the safety of telbivudine (LdT) treatment for chronic hepatitis B virus (HBV) infection in the third trimester of pregnancy to block mother-to-infant transmission (MTIT) of HBV; however, the safety of LdT treatment for chronic HBV infection for the entire pregnancy has not been reported.
