Simplification of continuous intracoronary thermodilution.

BACKGROUND: Continuous intracoronary thermodilution with saline allows for the accurate measurement of volumetric blood flow (Q) and absolute microvascular resistance (Rμ). However, this requires repositioning of the temperature sensor by the operator to measure the entry temperature of the saline infusate, denoted as Ti. AIMS: We evaluated whether Ti could be predicted based on known parameters without compromising the accuracy of calculated Q. This would significantly simplify the technique and render it completely operator independent. METHODS: In a derivation cohort of 371 patients with

Transpulmonary thermodilution: advantages and limits.

BACKGROUND: For complex patients in the intensive care unit or in the operating room, many questions regarding their haemodynamic management cannot be answered with simple clinical examination. In particular, arterial pressure allows only a rough estimation of cardiac output. Transpulmonary thermodilution is a technique that provides a full haemodynamic assessment through cardiac output and other indices. MAIN BODY: Through the analysis of the thermodilution curve recorded at the tip of an arterial catheter after the injection of a cold bolus in the venous circulation, transpulmonary thermodi

Comparison of cardiac output measurements using transpulmonary thermodilution and conventional thermodilution techniques in anaesthetized dogs with fluid overload.

OBJECTIVE: To evaluate the agreement between cardiac output (CO) values obtained using a transpulmonary thermodilution technique (TPTDCO) and conventional thermodilution technique (TDCO) in anaesthetized dogs with fluid overload. STUDY DESIGN: Prospective experimental study. ANIMALS: Six healthy Beagle dogs aged 7-8 years. METHODS: Dogs were anaesthetized with sevoflurane in oxygen, and catheters were inserted for TPTDCO and TDCO measurement. After instrumentation, baseline CO was measured using each technique at a central venous pressure (CVP) of 3-7 mmHg. Dogs were subsequently administer

Cardiac Output During Exercise: Thermodilution Versus Direct Fick.

BACKGROUND: Accurate cardiac output (CO) measurements during exercise are essential for the diagnosis of exercise pulmonary hypertension (PH), exercise precapillary PH, and exercise postcapillary PH. The purpose of this study is to compare performance of thermodilution CO to gold-standard direct Fick CO (dfCO) at rest and exercise. METHODS: A single-center prospective cohort study of patients undergoing invasive cardiopulmonary exercise test over a 3-year period. For the primary outcome, we predicted CO at each stage of exercise and recovery using generalized additive modeling. In secondary a

Continuous Thermodilution Method to Assess Coronary Flow Reserve.

Coronary flow reserve (CFR) is a well-validated flow-based physiological parameter that has shown value in clinical risk stratification. CFR can be invasively assessed, classically by Doppler and, more recently, by thermodilution with saline boluses (CFRthermo-bolus). Alternatively, continuous thermodilution is a novel operator-independent, highly-reproducible technique to invasively quantify maximum absolute coronary flow (AF). This study aimed to assess the feasibility of this method to quantify resting AF and to determine CFR (CFRThermo-infusion) as compared with CFRthermo-bolus. Sixty-two

Thermodilution right ventricular ejection fraction. Catheter positioning effects.

Right ventricular (RV) ejection fractions have been difficult to estimate clinically. It has been demonstrated recently that RV ejection fractions can be calculated by thermodilution techniques using a rapid response thermistor and computer. This method critically depends on adequate mixing of the thermal bolus and sensing of the rapid response thermistor. This study examined the effects of the thermistor position within the pulmonary artery and injectate site within the right atrium on RV thermodilution ejection fraction measurements. Ten pigs were instrumented with a RV thermodilution cathet

Measurement of the cardiac output in small animals by thermodilution.

Cardiac output (CO) measurements based on indicator dilution, microspheres, thermodilution and ultrasonic sensors are not suitable for small animals, because of limited blood volume, high heart rates and small caliber vessels that do not allow probe placement within the heart. We developed a modified thermodilution method to measure CO in awake animals weighing less than 100 g. Under anesthesia, the animal is instrumented with a jugular vein catheter placed proximal to the subclavian vein and a temperature probe in the carotid artery with the thermocouple positioned at the aortic arch. Two day

Thermodilution curves for diagnosis of ventricular septal defect in cattle.

An indicator dilution method developed for detection of intracardiac shunts and determination of cardiac output in man and domestic animals was evaluated in 4 calves and 1 cow with holosystolic cardiac murmurs. Thermodilution curves recorded from the pulmonary artery and the right ventricle confirmed a left-to-right intracardiac shunt and provided a means for its quantitation. Ventricular septal defect was confirmed in 2 cows, which were euthanatized. The data obtained by thermodilution were evaluated along with information obtained from other diagnostic procedures (auscultation, oximetry, ech

Minimal Microvascular Resistance: Agreement Between Continuous and Bolus Thermodilution.

BACKGROUND: Continuous thermodilution quantifies absolute microvascular resistance (Rμ, Wood units), a key metric of microvascular function. Rμ is minimal during hyperemia (Rμ,hyper) with increased Rμ,hyper suggestive of microvascular dysfunction. Bolus thermodilution measures the index of microcirculatory resistance (IMR), a dimensionless surrogate of Rμ,hyper. AIMS: We compared Rμ,hyper measured by continuous thermodilution (invasive Rμ,hyper) with the gold standard [15O]H2O positron emission tomography (PET Rμ,hyper), and assessed the correlation between invasive Rμ,hyper and IMR. Thermodilution Assessment of Cardiac Index in Patients With Tricuspid Regurgitation.

BACKGROUND: Disparities between thermodilution (TD) and Fick measurements of cardiac index (CI) are common in real-world clinical practice. Published studies about the effect of tricuspid regurgitation (TR) on TD are small and describe conflicting results. We tested the correlation between TD and Fick across a wide range of TR severity, in a larger group of patients undergoing right heart catheterization (RHC). We aimed to determine if TD is an acceptable alternative to Fick in patients with TR in clinical practice. METHODS: We retrospectively evaluated patients undergoing RHC at a single cen

Accuracy and precision of transcardiopulmonary thermodilution in patients with cardiogenic shock.

Hemodynamic monitoring plays a crucial role in the supportive treatment of critically ill patients. In this setting, the use of the pulmonary artery catheter (PAC) is a standard procedure. In this study we prospectively compare the accuracy and precision of pulmonary thermodilution (PTD) by PAC and transcardiopulmonary thermodilution (TC-PTD) in patients with cardiogenic shock following an acute cardiac event. In this prospective study 77 hemodynamic measurements were taken in 11 patients presenting cardiogenic shock (CS) treated at the medical intensive care unit of our university hospital. H

Reproducibility of transpulmonary thermodilution measurements in patients with burn shock and hypothermia.

Thermal noise, including hypothermia and short-term variations in body temperature, has been reported to influence the accuracy and reproducibility of thermodilution measurements. This variation might theoretically limit the usefulness of this technique in patients with acute burns. We therefore sought to determine the reproducibility of hemodynamic parameters derived from arterial thermal dilution in patients with acute burns and hypothermia. This was a prospective, clinical study of 50 mechanically ventilated patients with burns involving more than 25% of the body surface area that were trea

Measurement of cardiac output in standing horses by Doppler echocardiography and thermodilution.

Measurement of cardiac output by Doppler echocardiography were compared to simultaneous measurements by thermodilution in 9 conscious horses. In the Doppler technique, mean blood flow velocities for estimation of cardiac output were recorded from the aorta and pulmonary artery. The flow area of each vessel was calculated from the vessel diameter, measured from a 2-dimensional ultrasound image. Differences in the site and method of measuring the vessel diameter altered the estimation of cardiac output by the Doppler method. Cardiac output was modified by the i.v. infusion of 4 micrograms/kg bwt
