Molecularly Defined Thoracic Neoplasms.

Molecularly defined neoplasms are increasingly recognized, given the broader application and performance of molecular studies. These studies allow us to better characterize these neoplasms and learn about their pathogenesis. In the thorax, molecularly defined neoplasms include tumors such as NUT carcinoma, SMARCA4-deficient undifferentiated tumor (DUT), primary pulmonary myxoid sarcoma with EWSR1::CREB1 fusion, hyalinizing clear cell carcinoma, and SMARCB1-deficient neoplasms. Overall, these tumors are rare but are now more often recognized given more widely available immunostains such as NUT

DNA methylation in thoracic neoplasms.

Thoracic neoplasms, which include lung cancers, esophageal carcinoma, and thymic epithelial tumors, are the leading causes of tumor-related death and a major health concern worldwide. The development of neoplasms is a multistep process involving both genetic and epigenetic alterations. A growing body of research provides evidence that aberrant DNA methylation, including DNA hypermethylation in promoter regions, global DNA hypomethylation and the overexpression of DNA methyltransferases, plays an important role in tumorigenesis. In this review, we summarize published observations of methylation

Radiologic Evaluation of Thoracic Neuroendocrine Neoplasms.

Neuroendocrine neoplasms (NENs) of the thorax comprise a spectrum of pulmonary and mediastinal tumors ranging from well-differentiated, indolent lesions to highly aggressive carcinomas. Imaging, especially computed tomography and positron emission tomography, plays a central role in noninvasive tumor detection, characterization, staging, and treatment planning. Diagnosis of thoracic NENs prior to definitive histopathology is challenging, owing to their variable and often nonspecific imaging appearance, as well as the exceptional rarity of several subtypes. Because these tumors are often discov

Angiographic differentiation of thoracic aneurysms and neoplasms.

The distinction between nondissecting aneuryms of the thoracic aorta and thoracic neoplasms may be difficult. The aortographic findings associated with aneurysms may be subtle. However, when the aortogram is properly performed and interpreted and the findings correlated with the plain chest roentgenograms the distinction between aneurysms and neoplasms may be made consistently. The thoracic aortogram should be filmed in at least 2 projections and abdominal aortography and ultrasonography should be performed. With aneurysms the aortographic signs include widening (often slight) of the aortic lu

Imaging features of thoracic metastases from gynecologic neoplasms.

Gynecologic malignancies are a heterogeneous group of common neoplasms and represent the fourth most common malignancy in women. Thoracic metastases exhibit various imaging patterns and are usually associated with locally invasive primary neoplasms with intra-abdominal spread. However, thoracic involvement may also occur many months to years after initial diagnosis or as an isolated finding in patients without evidence of intra-abdominal neoplastic involvement. Thoracic metastases from endometrial carcinoma typically manifest as pulmonary nodules and lymphadenopathy. Thoracic metastases from o

Thoracic neoplasms: imaging requirements for diagnosis and staging.

This article reviews the pathophysiology of thoracic neoplasms and discusses current imaging recommendations for diagnosis and staging of these tumors. Particular emphasis is given to primary lung cancers, which comprise a variety of tumors of differing histologic type and behavior. The development of cost-effective diagnostic/staging sequences with continually changing imaging technology continues to be a challenging goal. Our recommendations are based on the TNM system and data from the current literature employing the experience from our institution.

[Reconstruction of the thoracic wall in neoplasms of the manubrium of sternum].

The case of a voluminous neoplasia originating at the manubrium sterni is described. Given the rapid growth of the mass and its dimensions, the mass was removed surgically together with the manubrium sterni and the sternal extremity of the clavicle and of the first two ribs. The defect in the anterior wall of the thorax was filled by a net of prolene partially covered with the mobilised pectoral muscles. Functional and aesthetic results were very good. Assisted respiration was not necessary in the immediate postoperative period and the net remained stable and firmly in place during respiration

SSTR-based therapies for gastroenteropancreatic and thoracic neuroendocrine neoplasms.

Somatostatin (SRIF) and its receptors (SSTR) are key players in the regulation of neuroendocrine neoplasms (NEN), with significant implications for diagnosis, prognosis, and treatment. The field of SSTR-targeted drugs/ligands (SRLs) is continuously evolving, with emerging novel therapeutic approaches aiming to address the numerous unmet needs and limitations of the standard-of-care (SOC) SRLs. In the present manuscript, we aim to provide a concise review of the field, focusing on the new and ongoing developments and potential novel approaches to be considered in the future. The paper (1) provi

[Inherited tumor syndromes of gastroenteropancreatic and thoracic neuroendocrine neoplasms].

About 5% of gastroenteropancreatic and thoracic neuroendocrine neoplasms (NENs) arise in the context of an inherited tumour syndrome. The two most frequent syndromes are: multiple endocrine neoplasia type 1 (MEN1), associated with a large spectrum of endocrine and non endocrine tumours, including duodenopancreatic, thymic and bronchial NENs, and the von Hippel-Lindau syndrome VHL, associated with pancreatic NENs. Two inherited syndromes have a low incidence of NENs: neurofibromatosis type 1 (NF1), associated with duodenal somatostatinomas, and tuberous sclerosis (TSC), associated with pancreat

Thoracic Manifestations of Genitourinary Neoplasms and Treatment-related Complications.

Genitourinary (GU) malignancies are a diverse group of common and uncommon neoplasms that may be associated with significant mortality. Metastases from GU neoplasms are frequently encountered in the chest, and virtually all thoracic structures can be involved. Although the most common imaging manifestations include hematogenous dissemination manifesting with peripheral predominant bilateral pulmonary nodules and lymphatic metastases manifesting with mediastinal and hilar lymphadenopathy, some GU malignancies exhibit unique features. We review the general patterns, pathways, and thoracic imagin

Soft Tissue Neoplasms Causing Apparent Venous Thoracic Outlet Syndrome.

Venous thoracic outlet syndrome (vTOS) usually results from compression of the subclavian vein classically as a result of narrowing of the costoclavicular space. We report 2 rare cases of soft tissue neoplasms resulting in apparent vTOS. The first case is a 46-year-old female with a 2-year history of intermittent unilateral shoulder pain, who was initially diagnosed with intervertebral disk herniation. Cervical fusion was performed; however, her symptoms progressed and she additionally developed paresthesias and venous congestion. Computed tomography (CT) angiogram demonstrated a 13-cm-encapsu

[Thoracic actinomycosis in the differential diagnosis of neoplasm: a propos of a case].

Actinomycosis is an infrequent infection caused by bacteria from Actinomyces genus that manifests as a chronic, suppurative and progressive disease. It's more common in men. Thoracic actinomycosis occurs in 15% of the cases, and infection of the chest wall is less frequent. The clinical presentation mimics tuberculosis or neoplastic processes. In this article we present the case of a 63 year-old man with no comorbidity, with pulmonary actinomycosis involving the chest wall mimicking a neoplastic process, basing the diagnosis on histopathologic findings.

Association between JAK2 mutations and thoracic aortic aneurysms in myeloproliferative neoplasms.

The Janus kinase 2 (JAK2) V617F mutation is recognized as a defining molecular lesion in Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs). This mutation was recognized in the context of clonal hematopoiesis of indeterminate potential, which has recently been linked with cardiovascular disease. Recent evidence has demonstrated a significant correlation between JAK2 mutations and thoracic aortic aneurysm (TAA) a condition frequently lacking identifiable risk factors. New insights into imaging of pulmonary metastases from extra-thoracic neoplasms.

The lung is one of the most common sites of metastases from extra-thoracic neoplasms. Lung metastases can show heterogeneous imaging appearance, thus mimicking a wide range of lung diseases, from benign lesions to primary lung cancer. The proper interpretation of pulmonary findings is crucial for prognostic assessment and treatment planning, even to avoid unnecessary procedures and patient anxiety. For this purpose, computed tomography (CT) is one of the most used imaging modalities. In the last decades, cancer patients' population has steadily increased and, due to the widespread application
