[Treatment of thymic diseases by bilateral video thoracoscopes].

OBJECTIVE: To explore the therapeutic efficacy of thymic diseases by bilateral video-assisted thoracoscopes. METHODS: A total of 46 cases with thymic diseases, including benign or malignant thymoma and myasthenia gravis (MG), were operated by bilateral video-assisted thoracoscopes. It was compared with 33 cases undergoing traditional thymic operations. RESULTS: The resection rate was 100%. There was no perioperative mortality. All cases were followed up for 3 years. The 1, 3-year survival rates were 95.7% (44/46) and 91.3% (42/46) versus 93.9% (31/33) and 90.9% (30/33) for a traditional thym

[Thoracoscopic surgery].

Thoracoscopic apparatus is essential in present general thoracic surgery. Historically, the first description of diagnostic use of thoracoscopy traces back to 1910 when Jacobaeus inspected thoracic cavity with cystoscope. In Japan, Kimoto treated lung tuberculosis by thoracic ablation with thoracoscope in 1944. Thereafter thoracoscopic procedure did not spread until 1990's when laparoscopic cholecystectomy had rapidly popularized in Japan. After 1992 many reports about thoracoscopic surgery or video-assisted thoracic surgery (VATS) were published. In the early years VATS was done only for spon

Thoracoscopic sympathectomy.

The objective was to evaluate the safety and effectiveness of endoscopic thoracic sympathectomy (ETS) for treatment of a variety of sympathetic disorders, including hyperhidrosis, splanchnic pain, reflex sympathetic dystrophy, and Raynaud upper extremity ischemia. Sixty-three ETS procedures were performed in 34 patients at the University of Maryland Medical System between March 1992 and August 1999 (14 male patients, 20 female patients; mean age 22 years). The indications for surgery were hyperhidrosis in 26 patients, upper extremity ischemia in 3 patients, splanchnic pain and reflex sympathet

Thoracoscopic sympathectomy.

Surgical sympathectomy has traditionally been achieved by 'open' surgical techniques. The transaxillary, cervical, or dorsal approaches have not been without morbidity and cosmetically have been found to be less than ideal. The main indication for sympathectomy in most units is palmar and axillary hyperhidrosis refractory to medical treatment, although it has been used with some success in troublesome causalgia. Use of sympathectomy in Raynaud's disease remains disappointing. In our unit thoracoscopic sympathectomy has been performed since 1980. A CO2 pneumothorax is initially created in the u

Thoracoscopic sympathicotomy.

Thoracoscopy - Looking into the thoracic cavity was first described in 1910 by the Swedish physician, Jacobeus. He used a cystoscope intrapleuraly in order to diagnose pleural diseases. He also used his method for cutting adhesions in order to achieve collapse of the lung in patients with tuberculosis of the lung. Thoracic sympathicotomy was first performed by Kotzareff in 1920. The operation was found to be effective for treatment of palmar hyperhidrosis. Different open techniques for sympathicotomy have since been described, the most common being the dorsal approach by Cloward in 1969. Sympa

Thoracoscopic lingulectomy.

Minimally invasive pulmonary segmentectomy makes adequate oncologic treatment possible in selected cases while preserving lung parenchyma and minimizing perioperative morbidity and length of hospital stay. Most lung segments may be resected as segmentectomies or as part of bisegmentectomies (as is the case for the lingula). Although several variations of minimally invasive pulmonary segmentectomy have been described, I favor a fully thoracoscopic multiport approach that offers direct access to the segmental structures and is straightforward and versatile enough to allow adaptation in case of u

Thoracoscopic sympathectomy.

With the advent of videotechnology, sympathectomy has assumed a more important role in the armamentarium of managing diseases of the autonomic system. Currently it is used primarily for hyperhydrosis, although sympathectomy for reflex sympathetic dystrophy (RSD), Raynaud disease and other diseases still are performed, but less frequently. Most of this article will refer primarily to hyperhydrosis patients.

Thoracoscopic fundoplication.

Thoracoscopic fundoplication and hiatoplasty were performed by a videoassisted technique in a patient with longstanding pathologic gastro-oesophageal reflux, reflux oesophagitis and hiatal hernia. Ten months later the patient was asymptomatic and had normal findings by oesophageal endoscopy and 24-hour pH-monitoring. Thoracoscopic fundoplication may be useful in patients with intra-abdominal adhesions due to previous abdominal surgery. Also obese patients with large hiatal hernias and perioesophageal adhesions may benefit from the thoracoscopic approach.

[Thoracoscopic interventions].

The technique of minimal invasive surgery has successfully been introduced into the area of thoracic surgery. Surgery of the intrathoracic organs may now be performed without compromise of respiratory mechanics, thus allowing the operative treatment of an ever widening range of pathological conditions. An analysis of our experience (287 thoracoscopic interventions in the last three years) reveals that the diverse procedures performed on the pleura (pleurectomy and pleurodesis), on the lung (wedge resection, fistula closure and ligature of parenchymatous leaks) and on other defined anatomical s

Thoracoscopic Esophagojejunostomy in the Upper Mediastinum After Thoracoscopic Esophagectomy with Total Gastrectomy.

BACKGROUND: Esophagectomy and esophageal reconstruction with organs other than the gastric tube are complicated and difficult surgical procedures. We developed a new method of thoracoscopic esophagectomy with intrathoracic esophagojejunostomy in the upper mediastinum when the gastric tube cannot be used as an esophageal substitute for reconstruction. MATERIALS AND METHODS: Total gastrectomy, preparation of pedicled jejunal conduit, and transhiatal lower mediastinal dissection were done under laparotomy. Upper and middle mediastinal dissection was performed thoracoscopically. After esophageal

Thoracoscopic aortopexy for tracheomalacia.

BACKGROUND: Tracheomalacia is a condition that may result in acute life-threatening events. Aortopexy has become a generally accepted mode of treatment, although the impact of a thoracotomy is considerable. With the advent of minimal invasive surgery the impact of such a procedure may be greatly reduced. This study evaluates the outcome of thoracoscopic aortopexy for tracheomalacia. METHODS: Retrospective analysis of a database containing all patients with thoracoscopic aortopexy. Based on symptomatology, on indication 24 h pH study and/or rigid tracheo-bronchoscopy were carried out to confir

[Thoracoscopic sympathectomy for hyperhidrosis].

BACKGROUND: Primary hyperhidrosis affects between 1 and 3% of the population. It is a problem of sympathetic deregulation which is characterized by profuse sweating on the palmar surface of the hands, armpits, groin and feet. The therapeutic options for the management of hyperhidrosis have traditionally been non-operative. However, there are several studies demonstrating that primary hyperhidrosis is best treated by endoscopic thoracic sympathectomy. METHODS: This article presents a review of existing approaches and techniques of thoracoscopic sympathectomy for treatment of hyperhidrosis as w

Thoracoscopic esophagectomy in children.

Minimal access surgery is an alternative to open surgery in esophageal resections. Thoracoscopic surgery has been utilized for both benign and malignant esophageal lesions and is a complex and challenging surgical procedure that can provide some benefits in comparison with open thoracotomy. Many studies have described laparoscopic or thoracoscopic-assisted esophagectomy in adults. So far, to the best of our knowledge, there is no study about thoracoscopic esophagectomy in children. This report describes 2 pediatric cases of benign esophageal lesions: one stricture after accidental ingestion of

[Thoracoscopic truncal vagotomy].

Abdominal re-operation in patients with recurrent peptic ulcer disease is associated with a high morbidity rate and a mortality rate of 5%. As an alternative procedure, therefore, transthoracic truncal vagotomy was early recommended as a less invasive intervention, and good results can be achieved with it. With the development of minimal invasive surgery, this procedure can now be performed via thoracoscopy and patient stress thus reduced even further. Via a left-sided thoracoscopy, the parietal pleura is incised and a 3-5 cm long segment of the distal oesophagus mobilised and dissected free.

Thoracoscopic plication of the diaphragm.

Plication of the diaphragm in symptomatic patients with phrenic nerve paralysis provides excellent relief of excertional dyspnea and significantly increases arterial oxygen tension, and all lung volumes except residual volume. We report diaphragmatic plication using the minimally invasive technique of VATS. This procedure provides excellent relief of symptoms with minimal morbidity and short hospitalization.

[Techniques in thoracoscopic surgery].

The presently known methods of thoracoscopic surgery are a symbiosis of traditional thoracic surgery and of new endoscopic techniques. The traditional methods determine the proceeding on the organ, the innovative ones the measure to reach it. The challenge is to carry out traditional procedures on the lung by means of the new technique without loss of quality. The accomplishments of simple thoracoscopic operations on lung, pleura, and mediastinum are mostly determined. In cases of major thoracic surgical interventions new resection techniques, not yet proven on lung vessels and bronchi are inc

Thoracoscopic ganglionectomy for hyperhidrosis.

Thoracoscopic sympathectomy for the treatment of hyperhidrosis has been carried out with techniques that involve either monopolar coagulation or laser injury to the T2 ganglion. Although this has the advantage of being minimally invasive, it has not been established whether these techniques are superior to complete ganglion excision, as carried out during open surgery. A new technique of complete T2 ganglion excision for palmar hyperhidrosis (with T3 ganglionectomy for axillary sweating) was developed using thoracoscopic techniques. Sixteen patients were treated with thoracoscopic T2 ganglion
