Clinical indications for thrombopoietin and thrombopoietin-receptor agonists.

Thrombocytopenia is a common hematologic disorder. Stimulation of thrombopoiesis may reduce the risk for thrombocytopenia-induced bleeding, prevent severe thrombocytopenia, and reduce the need for platelet transfusion. The key cytokine is thrombopoietin (TPO). It regulates proliferation and maturation of megakaryocytes as well as platelet production. TPO is synthesized in the liver. Development of TPO from the laboratory into a therapeutic tool has turned out to be an unexpected challenge. Clinical trials on first-generation thrombopoietic growth factors were stopped in 2001. At present, secon

Thrombopoietin-receptor agonists.

PURPOSE OF REVIEW: Thrombopoietin-receptor agonists (TPO-RAs) have been approved for use in immune thrombocytopenia (ITP) after showing safety and efficacy. There is increasing interest to expand the role of TPO-RAs, both in ITP as well as in other thrombocytopenic disorders. RECENT FINDINGS: In ITP, more studies are providing evidence of TPO-RA efficacy and safety, as well as their applicability to various patient groups, including children. Use of TPO-RAs in hepatitis C has shown early success in allowing treatments in patients who would otherwise be excluded due to thrombocytopenia. Use in

Thrombopoietin in the fetus and neonate.

C-mpl ligand or thrombopoietin (Tpo) is increasingly recognised as the major regulator of platelet homeostasis in humans. Relatively little is known about Tpo in the fetus and neonate but no evidence has yet been found to suggest any fundamental difference in Tpo structure, function and regulation in the fetus and neonate compared to older age groups. Tpo mRNA transcripts have been detected in the fetus as early as 6 weeks post conception and the liver appears to be the main site of Tpo production in both the fetus and neonate. The vast majority of healthy newborns have detectable levels of ci

A Narrative Review of the Use of Thrombopoietin and Thrombopoietin Analogues in Trauma and Critical Illness.

Introduction: Studies exploring the use of thrombopoietin (TPO) in trauma and hemorrhage-associated thrombocytopenia are limited. Transfusion of platelets, whether as part of whole blood or component therapy, is a cornerstone of survival in trauma patients. Adjuncts to transfusion may prove useful in the pre-hospital setting or in other settings where the logistics or risks of transfusion may not prove feasible. The purpose of this review is to evaluate the literature for the applications of TPO in critical illness and trauma. Methods: We conducted a review of relevant literature to provide a

Thrombopoietin in thrombocytopenias of childhood.

This review summarizes the biology of thrombopoietin (TPO) in childhood. Studies on TPO and its receptor (c-mpl) have improved the understanding of inherited and acquired thrombocytopenias in childhood. Data are presented in this review regarding the molecular biology of TPO, differences in cellular effects on megakaryopoiesis, the regulation of TPO production, and TPO concentrations in health and disease. For neonatal thrombocytopenia, the focus is on early-onset thrombocytopenia associated with maternal diabetes, pregnancy-induced hypertension, intrauterine growth retardation, hypoxia, and s

[Therapeutic potential of thrombopoietin].

Thrombocytopenia induced by disease and/or chemotherapy is one of the major complications in the treatment of patients with hemato-oncological malignancies. The discovery of Mpl ligand, a hematopoietic growth factor that stimulates megakaryopoiesis and thrombopoiesis lineage-specifically both in vitro and in preclinical models, is an important step in the development of new treatment concepts for patients with thrombocytopenia. Two recombinant forms of Mpl ligand, recombinant human thrombopoietin (rHuTPO) and pegylated recombinant human megakaryocyte growth and development factor (PEG-rHuMGDF)

Thrombopoietin and myelodysplastic syndromes.

Thrombopoietin (TPO), a major cytokine involved in megakaryocystopoiesis/thrombopoiesis, may be effective for the treatment of thrombocytopenia associated with myelodysplastic syndromes (MDS). We reviewed the available data relating to the therapeutic potential of TPO for MDS and found the following. The endogenous TPO level is elevated in MDS patients, especially in those with refractory anemia (RA). In RA patients, but not in patients with RA with excess blasts (RAEB) or RAEB in transformation (RAEB-t), both the platelet and megakaryocyte counts correlate inversely with the endogenous TPO le

Thrombopoietin in patients with hepatoblastoma.

Marked thrombocytosis (over 50 x 10(4)/microl) is frequently seen in patients with hepatoblastoma. Thrombopoietin (TPO), c-mpl ligand, has recently been purified as the major physiological regulator of the thrombopoiesis and is mainly produced in the liver. Since it is possible that TPO participates in thrombocytosis and the tumor growth of this particular hepatic tumor, serum TPO levels in addition to interleukin 1beta (IL-1beta) and IL-6 levels were assessed in seven untreated patients by using a sandwich enzyme-linked immunosorbent assay. High serum TPO levels were observed in all of the ex

C-Mannosylation of thrombopoietin receptor (c-Mpl) regulates thrombopoietin-dependent JAK-STAT signaling.

The thrombopoietin receptor, also known as c-Mpl, is a member of the cytokine superfamily, which regulates the differentiation of megakaryocytes and formation of platelets by binding to its ligand, thrombopoietin (TPO), through Janus kinase (JAK)-signal transducer and activator of transcription (STAT) signaling. The loss-of-function mutations of c-Mpl cause severe thrombocytopenia due to impaired megakaryocytopoiesis, and gain-of-function mutations cause thrombocythemia. c-Mpl contains two Trp-Ser-Xaa-Trp-Ser (Xaa represents any amino acids) sequences, which are characteristic sequences of typ

Notch1 regulates hepatic thrombopoietin production.

Notch signaling regulates cell-fate decisions in several developmental processes and cell functions. However, the role of Notch in hepatic thrombopoietin (TPO) production remains unclear. We noted thrombocytopenia in mice with hepatic Notch1 deficiency and so investigated TPO production and other features of platelets in these mice. We found that the liver ultrastructure and hepatocyte function were comparable between control and Notch1-deficient mice. However, the Notch1-deficient mice had significantly lower plasma TPO and hepatic TPO messenger RNA levels, concomitant with lower numbers of p

Thrombopoietin: tickling the HSC's fancy.

Thrombopoietin (THPO) has been well characterized as a key regulator of platelet production. THPO also plays an important role in the maintenance and regulation of hematopoietic stem cells (HSCs). In this issue of EMBO Molecular Medicine, Pecci et al (2018) describe a newly identified homozygous mutation in THPO causing congenital amegakaryocytic thrombocytopenia, a disease characterized by a significant impairment in platelet production with rapid onset of aplastic anemia within a few years. The paper nicely investigates the underlying pathogenic mechanisms of this disease. Importantly, this

Thrombopoietin: from theory to reality.

Much is now known about the physiology of platelets and their role in primary hemostasis; however, until recently far less was understood about their immediate precursors, marrow megakaryocytes (MKs). With the cloning and characterization of thrombopoietin (TPO), the principle regulator of the growth and development of MKs, research has been rapid and broad-based. In several laboratories TPO was cloned based on it's binding to the product of the proto-oncogene c-mpl, at the time an orphan cytokine receptor, and was subsequently found to affect nearly all aspects of thrombopoiesis. Many of the

Hepatoblastoma, thrombocytosis, and increased thrombopoietin.

Two children with hepatoblastoma, marked thrombocytosis, and extremely elevated alpha fetoproteins are reported. It is not clear from review of the literature whether hepatoblastomas or hepatocarcinomas are associated with thrombocytosis. Elevated thrombopoietin levels were found in liver tumor extract from one patient.

A network map of thrombopoietin signaling.

Thrombopoietin (THPO), also known as megakaryocyte growth and development factor (MGDF), is a cytokine involved in the production of platelets. THPO is a glycoprotein produced by liver and kidney. It regulates the production of platelets by stimulating the differentiation and maturation of megakaryocyte progenitors. It acts as a ligand for MPL receptor, a member of the hematopoietic cytokine receptor superfamily and is essential for megakaryocyte maturation. THPO binding induces homodimerization of the receptor which results in activation of JAKSTAT and MAPK signaling cascades that subsequentl

Elevated thrombopoietin levels in patients with myelofibrosis may not be due to enhanced production of thrombopoietin by bone marrow.

Thrombopoietin (TPO) is recognized as the primary regulator of megakaryocyte and platelet production. Two alternative hypotheses for the mechanism of regulation have been proposed: (1) platelet and/or megakaryocyte mass regulate circulating TPO levels by binding to TPO through TPO receptors (c-MPL), with subsequent internalization and degradation of the protein; (2) TPO mRNA produced by bone marrow (BM) stromal cells or BM cells modulates blood TPO levels or platelet counts. In myeloproliferative disorders (MPD), including primary myelofibrosis (MF) and essential thrombocythemia (ET), elevated

Thrombopoietin in normal pregnancy and preeclampsia.

OBJECTIVE: In normal pregnancy and pregnancies complicated by preeclampsia it has been demonstrated that there is increased activation of platelets and the clotting and fibrinolytic system. We measured plasma levels of thrombopoietin, a major regulator of platelet production in these conditions. STUDY DESIGN: We compared the thrombopoietin plasma levels of healthy term pregnant patients (n = 21) with those of healthy nonpregnant controls (n = 17), as well as patients with severe preeclampsia (n = 8) and the hemolysis, elevated liver enzymes, low platelets syndrome (n = 6). RESULTS: Thrombopo

Thrombopoietin: biology and potential clinical applications.

After an almost 40-year search for a primary regulatory of platelet production, thrombopoietin has recently been purified and cloned. Thrombopoietin regulates all stages in the production of platelets by promoting both the proliferation of megakaryocyte progenitors and their maturation into platelet-producing megakaryocytes. In preclinical studies in normal mice and non-human primates, administration of thrombopoietin resulted in a rapid rise in platelet counts to levels previously unattainable with other thrombopoietic cytokines. In myelosuppressed animal models, use of thrombopoietin followi
