[Occult papillary thyroid cancer].

The clinical and morphological aspects of occult sclerosing carcinoma of the thyroid are presented on the basis of a ten-year review of the surgically removed thyroids in Geneva and from the study of thyroid glands obtained from 133 consecutive autopsies. The 15 cases found in the surgical material represented 7.7% of all malignant thyroid tumours and one fifth of the papillary carcinomas. Occult tumours are found at a mean age of 46.6 years and predominate in women. Clinically, two thirds presented as nodular goiters, two cases as primary hyperplasia and one as primary hyperparathyroidism. Th

Overdiagnosis of Papillary Thyroid Cancer.

IMPORTANCE: The magnitude of thyroid cancer overdiagnosis in the US, accounting for the true increase in underlying disease, has not been quantified. The implications of reducing thyroid ultrasonography use for these rates has not been calculated. OBJECTIVE: To estimate US population-level rates of papillary thyroid cancer (PTC) overdiagnosis, and to identify the implications of reducing thyroid ultrasonography use for thyroid cancer incidence and overall mortality. DESIGN, SETTING, AND PARTICIPANTS: This study used a validated simulation model (Papillary Thyroid Carcinoma Microsimulation Mo

[Papillary and follicular cancers of the thyroid].

UNLABELLED: MANAGEMENT STRATEGIES: Management of papillary and follicular cancer of the thyroid varies somewhat between centers because of the generally good prognosis and the absence of well-controlled therapeutic trials. The internationally recognized TNM system is widely used to modulate treatment and follow-up to the individual situation. PRIMARY TREATMENT: Surgery is indicated in well-differentiated thyroid cancer. Total thyroidectomy is required for clinically patent tumors (> or = 1 cm) and small tumors (< 1 cm) recognized prior to surgery. For small tumors found at histology examinati

Determinants of papillary cancer of the thyroid.

Determinants of papillary thyroid cancer were evaluated in a questionnaire-based case-control study from southeastern Sweden. A total of 104 cases, diagnosed from 1977 to 1987, and 387 randomly selected controls were included in the analyses. Female subjects with papillary cancer reported a work history as dentists/dental assistants, telephone operators, teachers, and day nursery personnel, and an occupational contact with chemicals and video display terminals more often than did controls. The 11 male cases more often reported working as mechanics and metal workers and having occupational cont

Coexistence of papillary thyroid cancer with Hashimoto thyroiditis.

AIMS: Conflicting data have been reported with regard to Hashimoto thyroiditis (HT) and risk of malignancy. The aim of this study was to evaluate coexistence of papillary thyroid cancer (PTC) with HT. PATIENTS AND METHODS: This is a retrospective cohort study in which HT was diagnosed in 452 (F/M ratio = 405:47, median age 53.5 ± 12.1 years) of 7,545 patients qualified for thyroidectomy throughout the years 2002 to 2010. Pathological reports were reviewed to identify prevalence of PTC in HT vs. non-HT patients. RESULTS: PTC was diagnosed in 106 of 452 (23.5 %) HT patients vs. 530 of 7,093 (7

Papillary Thyroid Cancer and Coexisting Autoimmune Thyroiditis.

Histological findings often display an association between papillary thyroid carcinomas (PTC) and autoimmune thyroiditis (AIT) and so differ significantly from follicular thyroid carcinomas (FTC). The aim of this interdisciplinary, retrospective study was to evaluate the association of AIT in patients with PTC and FTC and a control group of benign nodular goiters. One hundred thyroidectomies with histologically confirmed differentiated thyroid carcinomas, 67 with PTC and 33 with FTC, were submitted for examination. The two control groups consisted of 60 patients with euthyroid nodular goiter,

Adequate surgery for papillary thyroid cancer.

Papillary thyroid cancer (PTC) is the most common type of thyroid cancer, occurring in about 80% of cases. Treatment consists ofsurgery, selective adjuvant radioiodine ablation, thyroid stimulating hormone suppression and surveillance. The extent of thyroidectomy and the extent of lymphadenectomy are controversial. Total or near-total thyroidectomy is recommended for the treatment of PTC, except those with papillary microcarcinoma (PTC < 1 cm) found incidentally after a thyroid lobectomy. This allows for treatment of possible multifocality (up to 8o% of cases), facilitates the use of radioiodi

Percutaneous Cryoablation of Papillary Thyroid Cancer.

PURPOSE: To investigate the safety and effectiveness of cryoablation in the treatment of papillary thyroid cancer (PTC). MATERIALS AND METHODS: During a period of 7 years, 71 PTCs in 65 patients treated with cryoablation were retrospectively reviewed. On ultrasound (US), the mean volume of the tumors was 6.9 mL; 35 tumors were T1, 29 were T2, and 7 were T3. Hydrodissection was performed medially and laterally in 36 patients, whereas in 29, a single or no hydrodissection was performed. After cryoablations, patients were followed up with 6 months intervals, and US findings were statistically co

Management of pediatric papillary thyroid cancer.

Papillary thyroid carcinoma (PTC) originates from the follicular cell of the thyroid gland. PTC is a rare cancer and usually develops in pre-existing thyroid nodules, which are not common in children. PTC is often multifocal and bilateral. Low-risk subtypes such as classic PTC and follicular variant account for the majority of PTC, while high-risk histologic subtypes such as tall cell variant, diffuse sclerosing variant and poorly differentiated thyroid cancer occur more rarely in children. It is worth noting that the size of the thyroid in children is smaller compared to that of adults. There

Papillary thyroid cancer in three children.

Thyroid cancers (TC) are rare diseases in the pediatric population and represents 0.5-3% of all malignant tumors in children. Differenti-ated thyroid cancer (DTC) is a major TC in children. Every patient under 18 years of age diagnosed with a thyroid nodule should under-go a detailed medical examination. The screening test for children with an increased risk for DTC is ultrasound examination of the neck. Both ultrasound and clinical images of a tissue lesion are more important than its size. It should also be emphasized that autoimmune thyroiditis (AIT), a comorbid condition for TC, is increas

Multicompartment metabolism in papillary thyroid cancer.

OBJECTIVES/HYPOTHESIS: In many cancers, varying regions within the tumor are often phenotypically heterogeneous, including their metabolic phenotype. Further, tumor regions can be metabolically compartmentalized, with metabolites transferred between compartments. When present, this metabolic coupling can promote aggressive behavior. Tumor metabolism in papillary thyroid cancer (PTC) is poorly characterized. STUDY DESIGN: Immunohistochemical staining of tissue samples. METHODS: Papillary thyroid cancer specimens from 46 patients with (n = 19) and without advanced disease (n = 27) were compare

Reoperative experience with papillary thyroid cancer.

BACKGROUND: Intense postoperative monitoring has resulted in increasing detection of patients with recurrent papillary thyroid cancer (PTC). Our goals included quantifying successful reoperation, and analyzing surgical complications and reasons for relapse. METHODS: From 1999 to 2008, a total of 410 patients underwent reoperation for PTC relapse. We analyzed post-reoperative disease outcomes, reasons for relapse, and complications. RESULTS: Bilateral reoperative thyroidectomy was performed in 13 (3 %) patients; lobectomy, 34 (8 %); central neck (VI) soft tissue local recurrence excision, 47

Thyroid Lobectomy Versus Total Thyroidectomy for Papillary Thyroid Cancer.

Classical papillary thyroid cancer (PTC) is the most common well-differentiated tumor of the thyroid gland and generally has a very favorable prognosis. Historically, standard-of-care for all PTCs was total thyroidectomy and radioactive iodine (RAI) therapy. Consensus guidelines in the past decade have de-escalated surgical therapy from total thyroidectomy to thyroid lobectomy without postoperative RAI in select low-risk PTCs. This shift in surgical practice may result in equivalent rates of recurrence and overall survival while reducing rates of recurrent laryngeal nerve injury, hypoparathyro

Simultaneous primary thyroid MALT lymphoma and papillary thyroid cancer.

The mucosa-associated lymphoid tissue (MALT) lymphoma subtype, specifically extranodal marginal zone B-cell lymphoma, is a rare variant. Within this subtype, primary thyroid MALT lymphoma is an uncommon occurrence. The literature provides limited documentation on thyroid MALT lymphomas, as their prevalence is comparatively lower than in other organ sites. The coexistence of papillary thyroid carcinoma (PTC) and thyroid MALT lymphomas is exceedingly rare. It presents a rare case of primary thyroid MALT lymphoma accompanied by PTC, thyroid lymphoma not being considered before surgery. A 64-year-

Hashimoto's Thyroiditis and the Risk of Papillary Thyroid Cancer in Children.

The association between Hashimoto's thyroiditis (HT) and pediatric thyroid cancer is controversial. Most studies examining this connection have been based on adults, and larger studies in children are lacking. We performed a retrospective study of all sequential pediatric patients who underwent a thyroidectomy for a neoplasm at our institution over a twenty-year period in order to explore the link between HT and pediatric thyroid cancer. A total of 153 patients, median age 16.5 (interquartile range [IQR] 14.2-18.3) years, underwent thyroid surgery for a neoplasm. Patients were mainly female (8

Papillary thyroid cancer case masked by subacute thyroiditis.

Subacute thyroiditis (SAT) association with thyroid carcinoma has been rarely reported in the literature. We present a patient with SAT and papillary thyroid cancer that was suspected by ultrasonographic evaluation (US) following SAT treatment. A fifty-four-year old female patient referred to our department due to tachycardia, jitteriness and pain in cervical region for the past one month. SAT diagnosis was established by physical examination, laboratory and ultrasonographic findings. After treatment, control thyroid US revealed regression of the hypoechogenic regions seen in both lobes, and a

Hashimoto's thyroiditis and papillary thyroid cancer: are they immunologically linked?

Hashimoto's thyroiditis (HT) is the most common autoimmune disease in humans frequently leading to hypothyroidism. HT is characterized by a cellular immune response with lymphatic infiltration of the thyroid gland by T and B cells, as well as by a humoral immune response leading to specific antibody production. The synchronous appearance of HT and papillary thyroid cancer (PTC) indicates an immunological link between the two entities. Three different pathomechanisms may be postulated, including preexisting autoimmunity leading to malignancy due to inflammation, immunity towards preexisiting tu
