The molecular biology of thyrotroph pituitary neuroendocrine tumors.

Pituitary neuroendocrine tumors (PitNETs), which were formerly known as pituitary adenomas, are classified in 5th Edition of the WHO Classification of Endocrine and Neuroendocrine Tumors. Since thyrotroph PitNETs are rare PitNETs, most previous studies about former thyroid stimulating hormone (TSH)-secreting pituitary adenoma have focused on a small number of cases. However, the diagnostic rate of thyrotroph PitNET has increased because of increased sensitivity of serum TSH measurement and widespread recognition that thyrotroph PitNETs are the cause of syndrome of inappropriate secretion of TS

Refractory Pit1 plurihormonal tumours and thyrotroph adenomas.

Pit-1 tumours are derived from neoplastic cells of either somatotroph, lactotroph or thyrotroph cell lineages, but there are also distinct mixed tumours and plurihormonal tumours within this category as described within the 2022 edition of the WHO classification of pituitary tumours. Plurihormonal tumours and thyrotroph adenomas are transcriptionally similar and grouped together to discuss in this review, although it is clear an immature type of plurihormonal tumour exists which are more commonly associated with refractory disease. Management of residual or recurrent disease should follow that

Somatostatin receptor profile in pituitary thyrotroph adenomas.

OBJECTIVES: Thyrotroph adenomas are the most infrequent adenohypophysial tumors. Somatostatin (SST) inhibits hormone secretion and suppresses cell proliferation. SST receptors (sstr) belong to a family of 5 types of G-coupled membrane proteins, which show high binding affinity to SST. Currently, SST analogs used to treat pituitary adenomas, have a preferential binding activity to sstr2 and sstr5. The aim of this study was to evaluate the status of all active sstrs on cell membrane of thyrotroph adenomas. PATIENTS: Nine cases of thyrotroph adenomas were studied for all types of sstrs. All pati

[Hyperthyroidism in the inappropriate thyrotrophic hormone secretion syndrome].

The authors draw attention to the possible presence of hyperthyroidism associated with the syndrome of so-called partial (hypophyseal) resistance to thyroid hormones in the absence of a pituitary tumour, based on the experience assembled during the follow-up of a 35-year-old female patient. The syndrome is characterized by elevated T4, T3 and also TSH levels. The syndrome of partial resistance to thyroid hormones differs from that of generalized resistance by a non-familial incidence and the presence of mild hyperthyroidism; from a pituitary tumour with overproduction of TSH it differs by a po

Pituitary thyrotroph hyperplasia mimicking prolactin-secreting adenoma.

A 32-year-old woman with persistent postpartum galactorrhea, hyperprolactinemia and a sellar configuration compatible with pituitary adenoma underwent transsphenoidal surgery. Mild hyperthyroidism had been suspected clinically preoperatively and thyroxine plus cortisol therapy was given. Morphological features of pituitary thyrotroph hyperplasia were present as shown by light microscopy, immunoperoxidase staining and electron microscopy. The thyrotroph hyperplasia was secondary to primary hypothyroidism as demonstrated by the TSH response to TRH stimulation and subsequent suppression with thyr

Thyrotropin-releasing hormone stimulates NR4A3 expression in the pituitary thyrotrophs of proestrus rats.

The secretion of several hypothalamic peptide hormones is activated during the preovulatory period. Hypothalamic thyrotropin-releasing hormone (TRH) is one such hormone with reproductive and/or metabolic significance. However, it remains unclear whether thyroid-stimulating hormone (TSH)-producing thyrotrophs are produced during the preovulatory period. We previously found a transient increase in the expression of the nuclear receptor NR4A3, a well-known immediate early gene, in the proestrus afternoon in the anterior pituitary glands of rats. To investigate the relationship between TRH secreti

Thyrotrophic status in patients with pituitary stalk interruption syndrome.

Pituitary stalk interruption syndrome (PSIS) is associated with simultaneous or subsequent pituitary hormone deficiencies (PHDs). Although the clinical features of multiple PHDs are well known, the status of the thyrotrophic axis in PSIS has not been thoroughly investigated. The clinical data of 89 PSIS patients and 34 Sheehan syndrome (SS) patients were retrospectively analyzed. The prevalence of central hypothyroidism in the PSIS patients and the SS patients was 79.8% and 70.6%, respectively. The thyroid-stimulating hormone (TSH) levels in the PSIS patients were significantly higher in compari

PITX2 AND PITX1 regulate thyrotroph function and response to hypothyroidism.

Pitx2 is a homeodomain transcription factor required in a dose-dependent manner for the development of multiple organs. Pitx2-null homozygotes (Pitx2(-/-)) have severe pituitary hypoplasia, whereas mice with reduced-function alleles (Pitx2(neo/neo)) exhibit modest hypoplasia and reduction in the developing gonadotroph and Pou1f1 lineages. PITX2 is expressed broadly in Rathke's pouch and the fetal pituitary gland. It predominates in adult thyrotrophs and gonadotrophs, although it is not necessary for gonadotroph function. To test the role of PITX2 in thyrotroph function, we developed thyrotroph

Ultrastructural immunocytochemical characterization of the thyrotroph in rat and human pituitaries.

The beta chain of thyroid stimulating hormone (TSH) was localized in normal rat and human pituitaries with the unlabeled antibody--peroxidase-antiperoxidase complex technique and antisera to rat or bovine TSH beta. Araldite embedded tissues, immunoreactivity of TSH was best preserved after fixation with 1% glutaraldehyde or picric acid formaldehyde. In the TSH cells, the immunocytochemical stain was located on granules. However, there was a variation in staining of individual granules, and among the population of granules. Rat TSH cells were ovoid or angular to stellate, and contained granules

Studies on thyroid and hypophysary thyrotrophic hormone (TSH) in Down syndrome.

Serum TSH was studied in 22 patients with Down syndrome, from 4 to 15 years old. In 6 of these patients radioidine uptake by thyroid gland after 2 and 24 hours of administration and clearance rates before and after TSH stimulus (10 mul-IM) were measured. Results show that serum TSH was normal in 17 patients and above normal limits in 5 patients. Thyroid uptake after 2 hours as well clearance rates, both below normal, had a response to TSH stimulus with normal or below values. These data along with previous reports, suggest, that in children with Down syndrome, there is a thyroid dysfunction in

Seasonal changes in the ultrastructure of the thyrotroph in the wookchuck (Marmota monax) adenohypophysis.

Throtrophs of the woodchuck anterior pituitary are described through the four seasons of the year. Evidence of an increase in number and activity in the spring is presented. Activity of the thyrotroph is seen to continue through the summer and decline in the autumn to quiescence during the hibernatory period of winter. The significance of these findings is discussed in relation to thyoid follicular cell morphology, serum T3 and T4 levels and the physiological activities of the animal. High levels of thyroid hormones in the serum may be consistent with the appearance of great activity seen in t

Cythion-induced histophysiological changes in thyroid and thyrotrophs of the teleost fish, Channa punctatus (Bloch).

The chronic effects of continuous exposure for 6 months, from January to June, to safe (2.0 ppm) and sublethal (4.0 ppm) concentrations of a commonly used organophosphorus pesticide, cythion, on the histophysiology of the thyroid and thyrotrophs in Channa punctatus are described in this investigation. The pharyngeal thyroid of fish of both treated groups exhibited hypertrophy and hyperplasia of the follicular epithelium and reduction of colloid content. Apart from this, in the fish exposed to 4.0 ppm, some of the follicles were exhausted and transformed into degenerated cystic masses. The pres

Thyrotroph Hyperplasia Caused by Severe Primary Hypothyroidism Leading to Adrenal Crisis.

Thyrotroph hyperplasia is commonly present but remains largely undiagnosed in primary hypothyroidism. It is easily reversible with thyroid replacement therapy. If imaging is performed prior to biochemical evaluation, then patients may undergo pituitary surgery unnecessarily. We present the case of a 30-year-old man with thyrotroph hyperplasia caused by profound primary hypothyroidism leading to hypopituitarism that resolved after levothyroxine replacement therapy. We will discuss the current literature regarding pituitary hyperplasia in primary hypothyroidism in adults.

Thyrotrophs in PIT1-lineage Pituitary Neuroendocrine Tumours Without Syndrome of Inappropriate Secretion of Thyrotropin Stimulating Hormone.

OBJECTIVE: Many patients with pituitary neuroendocrine tumours (PitNETs) containing beta (β) thyroid-stimulating hormone (TSH)-immunopositive cells, without syndrome of inappropriate secretion of TSH (SITSH), show a significant reduction in TSH levels following surgical removal of the tumour-like functioning thyrotroph PitNET. We evaluated the pathophysiological and clinical characteristics of TSH-positive cells in PIT1-lineage PitNETs in patients without the syndrome of inappropriate secretion of TSH. DESIGN: Retrospective cohort study. PATIENTS AND MESUREMENTS: Overall, 165 patients diagno

Calcium signaling properties of a thyrotroph cell line, mouse TαT1 cells.

TαT1 cells are mouse thyrotroph cell line frequently used for studies on thyroid-stimulating hormone beta subunit gene expression and other cellular functions. Here we have characterized calcium-signaling pathways in TαT1 cells, an issue not previously addressed in these cells and incompletely described in native thyrotrophs. TαT1 cells are excitable and fire action potentials spontaneously and in response to application of thyrotropin-releasing hormone (TRH), the native hypothalamic agonist for thyrotrophs. Spontaneous electrical activity is coupled to small amplitude fluctuations in intracel

Reversible thyrotroph hyperplasia with hyperprolactinemia: A rare presenting manifestation of primary hypothyroidism.

Pituitary thyrotroph hyperplasia with hyperprolactinemia has been described as a rare presentation of primary hypothyroidism. Premenopausal females with this disorder can present with features of hypothyroidism, menstrual disturbances, galactorrhea, and visual field defects because of enlarged pituitary. Here we describe a 32-year-old female presenting to her gynecologist primarily with galactorrhea and secondary amenorrhea. She was found to have raised serum prolactin, and MRI brain showed enlarged pituitary. She was referred for pituitary surgery when she came to us. Clinical examination and
