Entrapment neuropathies of the tibial (posterior tibial) nerve.

Entrapment neuropathies of the tibial nerve are relatively rare. They are often misdiagnosed largely because of the clinician's low index of suspicion. The clinical features, diagnostic studies, and treatment of these disorders are reviewed in detail in this article. Almost all of these disorders can now be confirmed through nerve conduction and other image studies.

Posterior tibial nerve--primary.

Tarsal tunnel is an uncommon disorder of the foot that presents as a burning or tingling pain that is located diffusely on the plantar aspect of the foot. Many investigators have considered tarsal tunnel syndrome to be analogous to carpal tunnel syndrome. As more becomes known about tarsal tunnel syndrome, it is evident that this is not purely an entrapment neuropathy. It differs from carpal tunnel syndrome in anatomy, etiology, clinical presentation, and response to nonoperative and operative treatment.

Schwannoma of the Posterior Tibial Nerve.

A 61-year-old woman was referred to physical therapy by a podiatrist who suspected a posterior tibialis degenerative tear. To further examine the irritable posterior tibial nerve, a musculoskeletal ultrasound examination was performed, showing a vascularized focal lesion suggestive of a nerve tumor. The patient was referred back to the referring podiatrist, who ordered magnetic resonance imaging, which confirmed the schwannoma of the posterior tibialis nerve. J Orthop Sports Phys Ther 2020;50(2):111. doi:10.2519/jospt.2020.9103.

Tibial nerve dynamics during ankle dorsiflexion: The relationship between stiffness and excursion of the tibial nerve.

Peripheral nerves extend with a gradual increase in stiffness and also with excursion, namely reduction of fiber bundle waviness, to adapt to joint movements. Although the close relationships between the tibial nerve (TN) excursion and stiffness during ankle dorsiflexion in cadaver studies, the precise in vivo their relationships remain unclear. We hypothesized that the excursion of the TN can be estimated from its stiffness in vivo using shear-wave elastography. This study aimed to analyze the relationships between the TN stiffness at the plantarflexion and dorsiflexion and TN excursion durin

Anomalous tibial nerve. A case report.

A rare anomaly of a nonterminating tibial nerve is reported. It was found to be associated with tarsal tunnel syndrome. The nerve failed to divide in the tarsal tunnel and in the talocalcaneal canal distally to the Knot of Henry. The branches of the medial and lateral plantar nerves as well as the medial calcaneal nerve, the nerve to the abductor digiti quinti, and motor branches to the abductor hallucis and to the joints, vessels, and skin took origin directly from the tibial nerve. The presence of this nerve anomaly and its relevance to surgery in the posterior ankle region is discussed. Sur

Posterior tibial nerve neurotmesis complicating a closed tibial fracture. A case report.

A case of complete transection of the posterior tibial nerve complicating a closed mid-shaft fracture of the tibia is reported. Early surgical exploration and repair resulted in return of function within 6 months. Such injuries are not common, but the case described highlights the importance of vigilance in assessment of all limb fractures.

An anatomical study of the tibial nerve branches innervating the posterior tibial artery.

The arteries of the lower limbs are innervated by vascular branches (VBs) originating from the lumbar sympathetic trunk and branches of the spinal nerve. Although lumbar sympathectomy is used to treat nonreconstructive critical lower limb ischemia (CLLI), it has limited long-term effects. In addition, the anatomical structure of tibial nerve (TN) VBs remain incompletely understood. This study aimed to clarify their anatomy and better inform the surgical approach for nonreconstructive CLLI. Thirty-six adult cadavers were dissected under surgical microscopy to observe the patterns and origin poi

Ultrasonography of the tibial nerve in vasculitic neuropathy.

Ultrasonography is a new imaging method for visualizing peripheral nerves. In vasculitic neuropathy, pain or axonopathy often can prevent the lesion from being localized during electrophysiological examinations, but the ability of ultrasonography to evaluate it morphologically is unknown. Our aim was to determine whether ultrasonography could be used to detect abnormalities in tibial vasculitic neuropathy at the medial ankle. We evaluated 11 tibial nerves in 8 patients with tibial vasculitic neuropathy, and 35 tibial nerves in 35 control subjects. In the controls, the tibial nerve was successf

Tibial nerve branching in the tarsal tunnel.

Tarsal tunnel syndrome is an infrequent and probably underdiagnosed clinical condition. Diagnosis and treatment depend on understanding the tibial branching pattern within the tarsal tunnel. A total of 68 foot dissections were performed. Bifurcation into the medial and lateral plantar nerves occurred within the tunnel in 93% and proximal in 7%. Proximal bifurcation may predispose to tarsal tunnel syndrome, and its infrequent occurrence correlates with the infrequent clinical diagnosis. Nine different calcaneal branching patterns were noted; they provide an anatomical explanation for heel spari

Intraneural ganglion cyst of the tibial nerve.

Intraneural ganglion cyst of the tibial nerve is very rare. To date, only 5 cases of this entity in the popliteal fossa have been reported. We report a new case and review the previously reported cases. A 40-year-old man experienced a mild vague pain in the medial half of his right foot for 3 years. Magnetic resonance imaging scan demonstrated a soft-tissue mass along the right tibial nerve. At surgery, an intraneural ganglion cyst was evacuated. After 12 months, the patient was pain-free with no signs of recurrence. Trauma might be a contributing factor to the development of intraneural gangl

Branches of the tibial nerve: anatomic variations.

Anatomic variations in tibial nerve branches may help explain discrepancies between clinical examination and electrophysiologic tests as to the location of neuronal lesions. Dissection of 20 cadaveric feet (10 pair) along the course of the tibial nerve and its branches confirmed that it bifurcates within 2 cm of the medio-malleolar-calcaneal axis in 90% (18/20) and that it gives off frequent small branches with its accompanying vascular structures. Unlike other studies, however, we found that 60% had multiple calcaneal branches off the tibial nerve and that 20% evidenced previously undescribed

Tibial nerve branching in the tarsal tunnel.

To provide an anatomical basis for diagnosis and treatment of the tarsal tunnel syndrome, the relationship of the tibial nerve to the tarsal tunnel was investigated in 31 feet of 20 cadavers. The bifurcation into medial and lateral plantar nerves occurred within 1 cm of the malleolar-calcaneal axis in 90% of the feet. Seven of 11 bilateral specimens were bilaterally symmetrical in the bifurcation location; three varied within 1 cm between sides; and in the fourth cadaver, one side bifurcated at 3 cm and the other at 5 cm proximal to the axis. The calcaneal nerve showed great variability; in se

Tibial Nerve Stimulation: Recent Progress and Challenges.

PURPOSE OF REVIEW: This review examines the evolution, mechanisms of action, clinical efficacy, and future directions of tibial nerve stimulation (TNS) in the management of overactive bladder (OAB). We sought to clarify how percutaneous (PTNS), transcutaneous (TTNS), and implantable (ITNS) modalities compare in efficacy, safety, durability, and practical application relative to other advanced therapies. RECENT FINDINGS: Contemporary randomized trials, cohort studies, and meta-analyses demonstrate pooled response rates of approximately 60–80% across TNS modalities with meaningful improvements i

Tibial Nerve Palsy After Lateralizing Calcaneal Osteotomy.

Background: Lateralizing calcaneal osteotomy (LCO) is a common procedure used to correct hindfoot varus. Several complications have been described in the literature, but only a few articles describe tibial nerve palsy after this procedure. Our hypothesis was that tibial nerve palsy is a common complication after LCO. Methods: A retrospective study of patients undergoing LCO for hindfoot varus between 2007 and 2013 was performed. A total of 15 patients (18 feet) were included in the study. The patients were examined for tibial nerve deficit, and all the patients were examined with a computed to
