Using tibiofemoral joints from older (age, 53-80 years) human cadavers with articular cartilage degeneration, contact pressures and contact areas were measured in the extended knee in four conditions: (1) neutral alignment; (2) 5 degrees varus (simulating single limb stance of gait); (3) 5 degrees valgus; and (4) after a 5 degrees proximal tibial closing wedge valgus osteotomy. In degenerated cartilage, contact pressures were reduced at the lesion sites and were high on the borders of the lesions. No statistically significant changes occurred in contact pressures and areas when values from neu

Knee motion involves intricate coordination among various anatomical structures. Effective treatment of knee pathologies requires precise identification of deformities and accurate surgical interventions, which often involve rapid tissue modification based on established knowledge. However, motion disorders are typically detected long after surgery. To address this, a simulation environment is proposed to plan and analyze surgical impacts on knee motion. Comprehensive knee joint modeling is crucial for a successful simulation. Clinically accepted movement procedures based on passive knee motio

OBJECTIVE: The aim of this study was to evaluate the influence of partial medial meniscectomy on tibiofemoral joint space width (JSW).
DESIGN: Thirty-seven patients (mean age: 51 years), suffering from medial meniscal lesions, (post-traumatic (n=22) or degenerative (n=15)), and requiring arthroscopic partial meniscectomy, were enrolled in a prospective, controlled study. Bilateral weight-bearing radiographs in extended and flexed position were performed in the same Radiology Unit just before and 2 days after meniscectomy. During arthroscopy, the extent of meniscectomy was evaluated in percent

Vitamin D is an important component in musculoskeletal development and function in children. The aim of our study was to investigate serum vitamin D levels and tibiofemoral joint orientation angles in children. We performed a retrospective review on a consecutive series of children presenting with lower limb complaints. The children underwent an assessment of serum vitamin D level and full-length standing lower limb anteroposterior radiographs, and were divided into normal and deficient vitamin D groups. Tibiofemoral angles (TFAs) [lateral distal femoral angle (LDFA); medial proximal tibial an

BACKGROUND: Joint arthroplasty, replacement of damaged articular surfaces with a prosthetic component, is an increasingly common orthopaedic procedure. Despite the overall success of arthroplasty, complications such as periprosthetic joint infection remain a persistent problem with severe consequences. With increasing clinical demands, the need for a reliable, reproducible model for testing therapeutic strategies for arthroplasty complications is fundamental.
RESULTS: In this study, a novel small animal model was designed and evaluated for downstream investigations. Two different implant desi

CASE: This case report aims to highlight a rare injury of a combined right tibiofemoral and patellofemoral joint dislocation. The patient was found to have a partial quadriceps tendon rupture, lateral and medial-sided soft-tissue injuries, and avulsion of the anterior and posterior cruciate ligaments. Treatment involved lateral and medial-sided repairs, as well as repair of his partial quadriceps tendon tear, allowing eventual return to daily activities following extensive rehabilitation.
CONCLUSION: Successful surgical repair and rehabilitation of a combined tibiofemoral and patellofemoral d

BACKGROUND: Proximal fibular partial excision (PFPE) is a novel treatment for medial compartment knee osteoarthritis (KOA), but its benefits, especially radiological benefits, are still controversial. This retrospective cohort study aims to investigate the osteoarthritic progression in medial tibiofemoral congruence and joint space width (JSW) following PFPE.
METHODS: The patients with medial compartment KOA who accepted PFPE or nonsurgical treatment were retrospectively enrolled. Preoperative/baseline, postoperative, and 5-years follow-up X-ray radiographs were collected. A novel self-design

BACKGROUND: The purpose was to investigate survival of cartilage repair in the knee joint by osteochondral autograft transfer stratified by location of the lesion; patellofemoral joint (N = 26) versus the medial or lateral femoral condyles (N = 58).
METHODS: For survival analyses, "failure" was defined as the event of a patient reporting a poor Lysholm score (below 65 points) or undergoing a knee replacement procedure.
RESULTS: The survival distribution was not significantly different between the patellofemoral joint and the tibiofemoral joint groups.
CONCLUSIONS: The current study suggest

BACKGROUND: Intraneural cysts involving the tibial nerve in the knee region (popliteal fossa) are rare. According to the articular (synovial) theory, which posits a joint origin for this pathology, these cysts originate from either the superior tibiofibular joint (STFJ) or the tibiofemoral (knee) joint. As tibial intraneural cysts arising from the tibiofemoral joint remain poorly understood, the authors present 2 illustrative cases and a review of the world's literature on all tibial intraneural ganglion cysts in the knee region.
OBSERVATIONS: Fourteen cases of tibial intraneural ganglion cys

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) has been shown to alter kinematics, which may influence dynamic tibiofemoral joint congruency (a measure of how well the bone surfaces fit together). This may lead to abnormal loading of cartilage and joint degeneration. However, joint congruency after ACLR has never been investigated.
HYPOTHESES: The ACLR knee will be more congruent than the contralateral uninjured knee, and dynamic congruency will increase over time after ACLR. Side-to-side differences (SSD) in dynamic congruency will be related to cartilage contact location/area

Epidemiological studies show that tibial plateau fractures comprise about 10% of all below-knee injuries in car crashes. Studies from this laboratory document that impacts to the tibiofemoral (TF) joint at 50% of the energy producing gross fracture can generate cartilage damage and microcracks at the interface between calcified cartilage and underlying subchondral bone in the tibial plateau. These injuries are suggestive of the initiation for a long term chronic disease, such as osteoarthritis. The disease process may be further encouraged by acute damage to chondrocytes in the cartilage overl

BACKGROUND: The healing process is initiated by injurious stimuli in response to cellular damage. Upon recruiting proinflammatory biomarkers to the tissue site of injury, the release of additional biomarkers occurs, including the likes of cytokines, matrix molecules, macrophages, neutrophils, and others. This influx of immune system mediators can occur for chronic periods, and though its intention is for healing the original injurious stimuli, it is also suspected of causing long term cartilage impairment following internal structure damage. The objective of this narrative review is to identif

BACKGROUND: Lateral meniscal tears are often seen with acute anterior cruciate ligament (ACL) injury and may be left in situ, repaired, or treated with meniscectomy. Clinical studies have shown good outcomes with vertical tears left in situ and poor outcomes following meniscectomy. However, clinically relevant studies are needed to establish a biomechanical foundation for treatment of these tears, particularly regarding the effects of meniscectomy.
PURPOSE: To compare tibiofemoral joint mechanics following vertical lateral meniscal tears and meniscectomies. We hypothesized that a peripheral v

BACKGROUND: Studies have shown that medial subluxation of the tibia occurs after anterior cruciate ligament (ACL) rupture. However, it is unclear whether anterior cruciate ligament reconstruction (ACLR) can correct tibial coronal subluxation.
PURPOSE: To determine whether the tibia is medially subluxated after ACL rupture, and whether ACLR can correct medial subluxation of the tibia.
STUDY DESIGN: Case series; Level of evidence, 4, Retrospective clinical study.
METHODS: The distance of tibial coronal subluxation before and after ACLR surgery was measured in 48 patients with ACL rupture and

BACKGROUND: Knee osteoarthritis is a highly prevalent degenerative joint disorder characterized by joint tissue damage and pain. Knee joint distraction has been introduced as a joint preserving surgical procedure to postpone knee arthroplasty. An often used standard externally fixation device for distraction poses a burden to patients due to the absence of joint flexion during the 6weeks treatment. Therefore, a personalized articulating distraction device was developed. The aim of this study was to test technical feasibility of this device.
METHODS: Based on an often applied rigid device, usi

BACKGROUND: Incidence rates of pediatric anterior cruciate ligament (ACL) injuries and ACL reconstruction (ACLR) are increasing. In adult patients with ACLR, limb-level loading profiles are less dynamic compared with uninjured controls (ie, lesser peaks and minimal offloading during midstance) early post-ACLR, and less dynamic profiles are associated with deleterious knee tissue changes. However, joint-level loading magnitudes during gait in the pediatric ACLR population are unknown.
PURPOSE/HYPOTHESIS: The purpose of this study was to compare medial and lateral tibiofemoral joint contact for

The knee joint is a complex mechanical system, and computational modelling can provide vital information for the prediction of disease progression and of the potential for therapeutic interventions. This review provides an overview of the challenges involved in developing finite element models of the tibiofemoral joint, including the representation of appropriate geometry and material properties, loads and motions, and establishing pertinent outputs. The importance of validation for computational models in biomechanics has been highlighted by a number of papers, and finite element models of th

We have studied the long-term effect of comminuted fractures of the patella with particular reference to the development of degenerative changes in the tibiofemoral joint. Degenerative changes which could convincingly be ascribed to the original injury were found in only one case, but we stress that the numbers are small, and that carefull search sould be made at operation for damage to the femoral condyles.
