Improving recruitment to healthcare research studies: clinician judgements explored for opting mental health service users out of the time to change viewpoint survey.

BACKGROUND: There are significant challenges across the research pathway, including participant recruitment. This paper aims to explore the impact of clinician recruitment decision-making on sampling for a national mental health survey. METHOD: Clinical teams in 20 English mental healthcare provider organisations screened caseload lists, opting-out people whom, in their judgement, should not be approached to participate in a survey about stigma and discrimination. The reasons for each individual opted-out were requested. We assess these reasons against study recruitment criteria and investiga

Using a Tibetan Gong to Increase Staff Member Engagement During Time Outs.

Perioperative and procedure area nurses can encounter barriers during the time-out process. In March 2016, a mock regulatory agency surveyor identified a gap during a time out in our cardiac catheterization laboratory. We worked with our facility's holistic nursing group to identify a solution to gain the full attention of all procedure area team members during each time out. Historically, ceremonial leaders used a gong to begin events because they thought that the sound helped participants focus on the ceremony. Because we wanted staff members to participate in a mindful practice during time

The financial burden of cancer: A longitudinal analysis of out-of-pocket healthcare costs in Australia (2009-2021).

BACKGROUND: Cancer can impose substantial financial strain on patients and their families, particularly through high out-of-pocket healthcare costs. This study aims to compare out-of-pocket and catastrophic healthcare expenditures among individuals with cancer, those with non-cancer chronic conditions, and healthy individuals, and to identify socio-demographic and health-related factors associated with higher financial burden. METHODS: We analysed data from four waves of the Household, Income and Labour Dynamics in Australia (HILDA) survey. Two-part and random-effects logistic regression mode

Out-of-pocket expenses for maternity care in rural Bangladesh: a public-private comparison.

Out-of-pocket expenses incurred by women for availing maternal healthcare services at public and private health facilities in Bangladesh were examined using a baseline household survey evaluating the impact of demand side financing vouchers on utilization and service delivery for maternal healthcare. The survey was conducted in 2010 among 3,300 women who gave birth in the previous 12 months from the start of data collection. Information on costs incurred to receive antenatal, delivery, and postnatal care services was collected. Findings reveal that the majority of women reported paying out-of-

Ethnic differences in sudden cardiac arrest resuscitation.

OBJECTIVE: Ethnic differences in sudden cardiac arrest resuscitation have not been fully explored and studies have yielded inconsistent results. We examined the association of ethnicity with factors affecting sudden cardiac arrest outcomes. METHODS: Retrospective cohort study of 3551 white, 440 black and 297 Asian sudden cardiac arrest cases in Seattle and King County, Washington, USA. RESULTS: Compared with whites, blacks and Asians were younger, had lower socioeconomic status and were more likely to have diabetes, hypertension and end-stage renal disease (all p<0.001). Blacks and Asians we

Prioritising time for communication in healthcare-a normative analysis.

Modern healthcare faces a mismatch in terms of supply and demand which makes careful priority setting imperative. The debate on priority setting in healthcare has focussed on pharmaceuticals and other tangible medical interventions. This article instead looks at the time that healthcare professionals have at their disposal. How should healthcare professionals divide their time between tangible medical interventions and communication with patients? What communicative goals should be prioritised? This normative analysis focusses on situations which involve time consuming communication between pr

Time toxicity in cancer care: a systematic review of definitions and measurement methods.

PURPOSE: Time toxicity has emerged as an important concept in oncology, reflecting the burden of time patients spend receiving care. However there is no consensus on how time toxicity is defined or measured. We conducted a systematic review to characterise the definitions and measurement methods. METHODS: MEDLINE, EMBASE, and Cochrane Library were searched from inception to 10th March 2026. Studies were included if they involved cancer patients and reported a definable measure of time-related burden associated with care. Data were extracted on study characteristics, sample size, data sources,

Giving callers the option to bypass the telephone waiting line in out-of-hours services: a comparative intervention study.

OBJECTIVE: Acute out-of-hours (OOH) healthcare is challenged by potentially long waiting time for callers in acute need of medical aid. OOH callers must usually wait in line, even when contacting for highly urgent or life-threatening conditions. We tested an emergency access button (EAB), which allowed OOH callers to bypass the waiting line if they perceived their health problem as severe. We aimed to investigate EAB use and patient characteristics associated with this use. DESIGN: Comparative intervention study. SETTING: OOH services in two major Danish healthcare regions. INTERVENTION: Gi

Economic burden of food allergy in Canada: Estimating costs and identifying determinants.

BACKGROUND: Limited data exist on the economic burden of food allergy (FA). OBJECTIVE: To assess FA-related direct (healthcare and out-of-pocket) and indirect (lost productivity) costs and their determinants in Canadian children and adults self-reporting FA. METHODS: FA-individuals self-reporting a convincing history or physician diagnosis were recruited through FA registries, an anaphylaxis registry, and advocacy associations, and electronically surveyed regarding FA-related healthcare use, out-of-pocket expenditures, and time lost from paid and unpaid labor. Direct and indirect costs (2020

Predictors of extracorporeal membrane oxygenation utilization and survival during cardiopulmonary resuscitation in out and in-hospital cardiac arrest.

INTRODUCTION: Extracorporeal membrane oxygenation during cardiopulmonary resuscitation (ECPR) has shown promise in managing both out-of-hospital cardiac arrest (OHCA) and in-hospital cardiac arrest (IHCA). METHODS: We analyzed hospital discharge records from the National Inpatient Sample of adult individuals who underwent ECPR between 2016 and 2020. Multivariable regression analyses were conducted to identify factors influencing ECPR utilization and survival. RESULTS: Among 1,585,960 patients (901,470 OHCA, 684,490 IHCA), ECPR utilization rates were 1 % for OHCA and 1.4 % for IHCA, with inpa

Priorities for 'out-of-hours' home-based palliative care for professionals, patients, and family caregivers: A qualitative interview study.

BACKGROUND: People with advanced illness at home, and their families, rely on 'out-of-hours' services provided by community, primary and specialist palliative care services. Home is commonly expressed as the preferred place to be cared for and die, and an increasing proportion of people are dying at home, but what constitutes 'good' care is poorly understood from the combined perspectives of healthcare professionals and patients and family caregivers. OBJECTIVE: To understand the convergence and divergence of the perspectives of healthcare professionals with those of patients and family careg

Understanding challenges in healthcare access: qualitative insights from healthcare providers and people living in socio-economically vulnerable circumstances.

INTRODUCTION: Equitable access to healthcare is fundamental to improving population health outcomes, yet persistent disparities leave many underserved, particularly those in socio-economically vulnerable circumstances. In Belgium, unmet healthcare needs are disproportionately concentrated among low-income groups, migrants and individuals with low educational attainment. These structural inequities are reflected in poorer health outcomes, including reduced life expectancy and limited access to essential services such as dental, mental, and specialist care. Existing research has primarily taken

Relation of people-centered public health and person-centered healthcare management: a case study to reduce burn-out.

Healthcare management is one practical tool for mediation and implementation of public health into clinical healthcare outcomes and is taken in our case study as an exemplar arena to demonstrate the vital importance of the person-centered approach. Healthcare personnel are frequently at risk for the 'burn-out' syndrome. However, modern measures of burn-out recognize burn-out only at a late stage when it is fully developed. There are no available methods to assess the risk for vulnerability to burnout in healthcare systems. Our aim was therefore to design a complex person-centered model for det

Factors affecting the use of home-based services and out-of-home respite care services: A survey of family caregivers for older persons with dementia in Northern Norway.

BACKGROUND: Family caregivers contribute substantially to the care for older home-dwelling people with dementia, although community healthcare services tend to be underutilized. In this study, we aimed to explore the use and predictors of use of home-based and out-of-home respite care services available to older home-dwelling persons with dementia, as reported by the family caregivers. METHOD: A cross-sectional survey was administered to family caregivers (n = 430) in Northern Norway during April to November 2016. The use of healthcare services was categorized into two types according to prin

How to Better Screen for Blood-Borne Viruses, Specifically HIV, in Hospital Emergency Departments: French and European Perspectives.

Hospital emergency departments (EDs) represent an acceptable and economically viable source of data on screening for blood-borne viruses (BBVs). In EDs in France, BBV screening is not based on a universal 'opt-out' policy (i.e. where all patients are informed that they will be screened unless they decline). Screening for BBVs is often not systematically offered when clinically appropriate and may have several challenges when it is offered. Nonetheless, opt-out BBV screening is useful in varied urban populations where BBV prevalence is high, although further evaluations of patient, caregiver an

How do middle-aged patients and their healthcare providers manage multimorbidity? Results of a qualitative study.

BACKGROUND: It is particularly difficult for healthcare providers to deliver optimal medical care to multimorbid middle-aged persons because patients' professional activities, family lives, and other everyday responsibilities hinder them from making necessary lifestyle changes. Our aim was to find out how patients and healthcare providers view and manage the problems of dealing with multimorbidity in middle age. METHODS AND FINDINGS: This qualitative study consisted of three steps. First, we conducted semi-structured in-depth interviews with 15 purposively sampled middle-aged persons living w
