Malaria vectors of Timor-Leste.

BACKGROUND: The island of Timor lies at the south-eastern edge of Indonesia on the boundary of the Oriental and Australian faunal regions. The country of Timor-Leste, which occupies the eastern part of the island, is malarious, but anopheline faunal surveys and malaria vector incrimination date back to the 1960 s. Over the last decade the malaria vectors of south-east Asia and the south-west Pacific have been intensely studied using molecular techniques that can confirm identification within complexes of isomorphic species. The aim of this study is to accurately identify the Anopheles fauna of

Chromoblastomycosis in Timor-Leste: Highlighting an underdiagnosed condition.

Chromoblastomycosis is a chronic implantation mycosis caused by dematiaceous fungi, primarily affecting agricultural workers in impoverished (sub)tropical regions. It often causes disfiguring lesions on the lower limbs, leading to stigma, and reduced quality of life. Treatment is prolonged and challenging. We report the first documented case of chromoblastomycosis in Timor-Leste, highlighting the difficulties of managing this neglected disease in remote areas, including limited diagnostic capacity, restricted treatment options, low health literacy, and poor adherence.

Estimation of roaming dog populations in Timor Leste.

The continued spread of rabies through the eastern islands of Indonesia poses a risk of rabies introduction to Timor Leste. To prepare for such an incursion and to undertake surveillance activities, the size and distribution of the roaming dog population needs to be estimated. We present the results of the first such surveys ever undertaken in Timor Leste. Roaming dog surveys were undertaken in each capital of the 13 districts of Timor Leste, including the national capital, Dili. Within these locations, local urban areas (aldeias) were targeted and sight-re-sight counts were undertaken on cons

Mycetoma in Timor-Leste and first report of nocardiosis.

Mycetoma is a neglected tropical disease with an unknown global burden. Although considered endemic to South-east Asia, it has not previously been reported from Timor-Lest. We describe two cases in Timor-Leste, highlighting the challenges surrounding microbiological diagnosis and management shared by many low to middle-income countries. As characteristically described, both patients lived rurally and presented late with marked soft tissue involvement and multiple draining sinuses following a prolonged period of high morbidity. Nocardia brasiliensis, a beadedbranched, modified acid-fast, gram-p

Dengue in Timor-Leste during the COVID-19 phenomenon.

Dengue is a significant public health problem in mostly tropical countries, including Timor-Leste. Dengue continues to draw attention from the health sector during the COVID-19 phenomenon. Therefore, the goal of this study is to evaluate the dengue incidence rate in comparison with the COVID-19 cumulative number and associated dengue risk factors, including the fatality rate of dengue infection in each municipality during the COVID-19 phenomenon in Timor-Leste, by applying the data processing program in Geographic Information Systems (GIS). A descriptive study using GIS was performed to provid

Antimicrobial resistance in urine and skin isolates in Timor-Leste.

OBJECTIVES: High rates of antimicrobial resistance (AMR) are seen throughout Southeast Asia. However, limited AMR data exist for Timor-Leste, which is situated on the south-eastern portion of the Malay Archipelago. The purpose of this study was to identify AMR in bacteria isolated from urine and skin swabs from patients in Dili, the capital of Timor-Leste. METHODS: Urine and skin swabs were collected from symptomatic patients in Timor-Leste and were processed for bacterial culture. Isolates were processed in Australia using a VITEK®2 system for bacterial identification and to determine antimi

Stated and actual willingness to pay for spectacles in Timor-Leste.

PURPOSE: To conduct a survey of willingness to pay for ready-made spectacles in the low-resource country of Timor-Leste, and, uniquely, subsequent validation with actual payment information. METHODS: A systematic random sampling strategy was used to apply a binary with follow-up stated willingness to pay methodology. Findings were validated by comparing the amount declared willing to pay with the actual price paid at a subsequent visit. RESULTS: Of the 152 participants (96.2%; mean age 50.9 +/- 13.2 years; 50.0% female) agreeable to wearing spectacles if required, 84.9% were willing to pay f

A retrospective review of stillbirths at the national hospital in Timor-Leste.

BACKGROUND: Timor-Leste has high maternal and infant mortality rates. Estimates of stillbirths are unreliable and limited by poor collection of vital health statistics. Lack of accurate data impedes the development of interventions to address local determinants of stillbirth. AIMS: This study aimed to identify the rate, timing and causes of stillbirths at National Hospital Guido Valadares in Dili, Timor-Leste, between November 2009 and December 2010, during which data were available. METHODS: Hospital birth registry and maternal records were retrospectively reviewed to identify stillbirths d

Melioidosis in Timor-Leste: First Case Description and Phylogenetic Analysis.

Burkholderia pseudomallei, the causative agent of melioidosis, has not yet been reported in Timor-Leste, a sovereign state northwest of Australia. In the context of improved access to diagnostic resources and expanding clinical networks in the Australasian region, we report the first 3 cases of culture-confirmed melioidosis in Timor-Leste. These cases describe a broad range of typical presentations, including sepsis, pneumonia, multifocal abscesses, and cutaneous infection. Phylogenetic analysis revealed that the Timor-Leste isolates belong to the Australasian clade of B. pseudomallei, rather

Serology Confirms Near-Absence of Malaria Transmission in Timor-Leste.

Despite malaria remaining a major public health threat, several countries have made significant progress toward elimination. Timor-Leste has reduced the number of malaria cases from more than 200,000 to zero, with no indigenous cases reported since 2022, and has recently been certified as malaria-free by the WHO. At low transmission levels, serological surveys, which detect antibodies to malaria parasites, can augment conventional surveillance by revealing the absence of antibodies and, therefore, of exposure in previously endemic populations. Exposure to malaria, as determined using antibody

The first confirmed human case of rabies, Timor-Leste, 2024.

In March 2024, the first ever human case of rabies, following a dog bite, was detected in Timor-Leste. This paper briefly discusses the circumstances of transmission, clinical presentation, palliative care of the case and public health measures taken. Timor-Leste was previously considered rabies-free. Any person who is bitten or scratched by an animal that could potentially transmit rabies virus (especially dogs, bats, monkeys or cats) in Timor-Leste should be assessed for consideration of provision of rabies post-exposure prophylaxis.

Prevalence of scabies and impetigo in school-age children in Timor-Leste.

BACKGROUND: Scabies and impetigo are endemic in many tropical, low- and middle-income countries. Mass drug administration (MDA) with ivermectin has emerged as a control strategy for these conditions. In 2019, Timor-Leste Ministry of Health planned to implement MDA including ivermectin for the control of lymphatic filariasis, so we undertook a baseline assessment of scabies and impetigo to better understand local epidemiology and contribute to future surveys assessing the impact of MDA. METHODS: A cross-sectional school survey was conducted in April-May 2019 at six primary schools in a semi-ur
