Tirofiban.

Tirofiban is an antagonist of the gpIIb/IIIa receptor. It was developed as an intravenous agent for rapid, potent and reversible inhibition of platelet aggregation. It produces dose-related inhibition of aggregation. Its efficacy has been demonstrated in disease states associated with intravascular thrombus formation, such as occurs with unstable angina pectoris, non-Q-wave myocardial infarction and during percutaneous coronary interventions. This review describes the pharmacokinetics and pharmacodynamics of tirofiban in normal volunteers and in patients with coronary artery disease, including

Tirofiban-induced thrombocytopenia.

Tirofiban is a small non-peptide ligand-mimetic Glycoprotein (GP) IIb/IIIa inhibitor which can reversibly bind to the arginine-glycine-aspartic acid (RGD) recognition site of GP IIb/IIIa to prevent platelet aggregation. It reduces the incidence of thrombotic cardiovascular events in patients with non-ST-segment elevation acute coronary syndrome (NSTE-ACS). Although generally considered safe, tirofiban has been reported to be associated with thrombocytopenia in several case reports and clinical trials. The pathogenesis for this adverse reaction is not entirely understood, is thought to be due t

Tirofiban-associated acute thrombocytopenia.

Randomized clinical trials of glycoprotein IIb/IIIa inhibition during percutaneous coronary intervention have shown significant clinical efficacy and safety in reducing the morbidity and mortality. In Turkey, only tirofiban is available. Tirofiban is a safe and effective agent in combination with heparin and aspirin in the setting of an acute coronary syndrome. In previous studies involving tirofiban, the incidence of bleeding complications was more frequent than heparin alone but major bleeding complications were seen in 5.3% of patients receiving tirofiban and not significantly different fro

Tirofiban-induced diffuse alveolar haemorrhage.

A middle-aged man presented with inferior wall ST-elevation myocardial infarction and underwent primary percutaneous coronary intervention with tirofiban administered due to extensive thrombus. He developed sudden-onset dyspnoea, bilateral crepitations, haemoptysis, desaturation and hypotension an hour after starting tirofiban infusion. The tirofiban, antiplatelet medications and heparin were stopped immediately. Chest X-ray showed patchy opacities in the left upper, middle and lower zones. High-resolution CT showed confluent areas of consolidation with surrounding ground glass opacities and i

Tirofiban for the treatment of ischaemic stroke.

Tirofiban is one of three glycoprotein IIb/IIIa receptor antagonists approved by the US FDA, beside abciximab and eptifibatide. The approval of tirofiban covers conservative treatment of myocardial infarction and unstable angina, as well as percutaneous coronary intervention, for which treatment with tirofiban is recommended in moderate-to-high-risk patients. The efficacy of glycoprotein IIb/IIIa antagonists in myocardial infarction indicated that these agents may also be helpful in the treatment of acute ischaemic stroke. Although experimental data are lacking, observational studies are promi

Tirofiban-Associated Thrombocytopenia: A Case Report.

Tirofiban, a competitive inhibitor of glycoprotein IIb/IIIa receptors on platelet membranes that assist in aggregation, is widely used in high-risk acute coronary syndrome (ACS). We present a case of a 53-year-old man with acute coronary syndrome and triple vessel disease who developed thrombocytopenia after first exposure to tirofiban. He presented to our hospital after his left-sided chest pain had subsided, and he was started on tirofiban 0.15 µg/kg/min; baseline platelets were 264 × 103/µL. Within hours, repeat testing showed a profound fall to 10 × 103/µL, prompting immediate discontinuat

Subacute myocardial rupture following tirofiban treatment.

A 74-year-old male patient was admitted to our emergency department with post-MI angina. On account of the anginal complaint that continued for three days, a coronary artery angiography was undertaken. A percutaneous transluminal coronary angioplasty was performed, followed by the implantation of a coronary stent, and coronary perfusion (TIMI-3) was achieved in the left anterior descending artery. Medical treatment (with acetylsalicylic acid, clopidogrel, metoprolol, atorvastatin and enoxaparine) and tirofiban infusion were duly administered in the coronary care unit. After twenty-four hours,

Tirofiban-Induced Profound Thrombocytopenia: A Case Report.

Platelet glycoprotein IIB/IIIA antagonists are potent inhibitors of platelet aggregation. Although the incidence of thrombocytopenia following their administration is relatively low, it can sometimes lead to fatal consequences. We report a case of a 57-year-old gentleman presenting with non-ST elevation myocardial infarction complicated with tirofiban-induced profound thrombocytopenia that developed within 12 hours of administration. Given the high risk of acute-onset thrombocytopenia following treatment with glycoprotein IIb/IIIa inhibitors, platelet count monitoring is highly encouraged at f

Vertebrobasilar thrombolysis with intravenous tirofiban: case report.

The use of thrombolytic agents in the setting of established cerebral infarction is limited by concerns for hemorrhagic transformation. Novel thrombolytic approaches, which have received minimal consideration, may be associated with lower risks of hemorrhage. We illustrate vertebrobasilar thrombolysis with intravenous tirofiban, a selective platelet glycoprotein IIb/IIIa receptor antagonist, and discuss the potential thrombolytic properties of this class of antithrombotics.

Eptifibatide and tirofiban: new preparations. Limited benefit.

(1) Two antiplatelet drugs, eptifibatide and tirofiban, are licensed for the prevention of early myocardial infarction in patients with unstable angina or non Q-wave myocardial infarction. (2) These drugs appear to have a mechanism similar to that of abciximab. Abciximab reduced the incidence of myocardial infarction during the month following administration to patients with unstable angina who were scheduled for a percutaneous coronary procedure. (3) The clinical assessment files on eptifibatide and tirofiban are both centred on a large double-blind, placebo-controlled trial. (4) In patients

Safety of downstream high-dose tirofiban bolus among 1578 patients undergoing percutaneous coronary intervention: the Sant'ANna TIrofiban Safety study.

BACKGROUND: The Sant'ANna TIrofiban Safety study (SANTISS) is an open-label investigator-initiated single-centre registry launched to assess the combination of bleeding and access site in-hospital complications (primary end-point) in patients undergoing percutaneous coronary intervention (PCI) by femoral approach only. METHODS: We compared patients who were on oral single antiaggregating drug (AAD) and received, just prior to PCI, high-dose tirofiban and a second oral antiplatelet agent (triple AAD: group 1, n = 970) with those who were already on an oral double AAD regimen and did not receiv

Severe tirofiban-induced thrombocytopenia following percutaneous coronary intervention.

UNLABELLED: Tirofiban is an intravenous glycoprotein IIb/IIIa inhibitor (GPI) that can be used as a bailout strategy in patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI) with high thrombus burden. A rare complication of this agent is severe thrombocytopenia, with an incidence ranging from 0.1 % to 0.5 %. We present a case of a patient who presented with ACS, underwent PCI, and subsequently developed severe thrombocytopenia within 24 h of receiving tirofiban. Tirofiban-induced thrombocytopenia is a rare immune-mediated condition that significantly h

Glycoprotein IIb/IIIa inhibitors: The resurgence of tirofiban.

Glycoprotein (GP) IIb/IIIa inhibitors block platelet aggregation, reducing thrombotic events in acute coronary syndrome. They are most often utilized in patients who likely have an intracoronary thrombus. Tirofiban, eptifibatide, and abciximab are the three GP IIb/IIIa inhibitors approved for use in the United States. Each agent has unique pharmacological properties. They all have a rapid onset and are most often utilized in conjunction with heparin. Tirofiban, in particular, fell out of favor due to inferior dosing with its original Food and Drug Administration (FDA) approved indication, but

The role of tirofiban in the management of coronary artery disease.

Glycoprotein IIb/IIIa receptor antagonists are potent antiplatelet agents by inhibiting the final common pathway of platelet aggregation. Tirofiban binds specifically to the glycoprotein IIb/IIIa receptor resulting in immediate and extensive inhibition of platelets. Studies have shown that intravenous administration of tirofiban in combination with aspirin and heparin reduces major adverse cardiac events in patients undergoing percutaneous coronary intervention and in those patients with acute coronary syndromes. Large randomised trials using tirofiban demonstrate early clinical and long-term

Abciximab-associated thrombocytopenia after previous tirofiban-related thrombocytopenia.

A 62-year-old man with a history of coronary artery disease and coronary artery bypass graft, chronic heart failure, and peripheral vascular disease required percutaneous coronary intervention (PCI) after progression of shortness of breath and fatigue over 2 years. Four hours after the procedure, the patient developed hematemesis and was found to be thrombocytopenic. The thrombocytopenia was presumed to be due to the abciximab infusion the patient received during and shortly after the PCI. Further review of the patient's medical history revealed that a similar episode had occurred 11 years ear

Tirofiban: an investigational platelet glycoprotein IIb/IIIa receptor antagonist.

The deposition of a platelet rich thrombus on an atherosclerotic plaque is a critical step in the development of unstable coronary syndromes. Currently available therapeutic agents such as aspirin and ticlopidine are relatively weak inhibitors of platelet aggregation. Recently, antagonists to platelet glycoprotein IIb/IIIa (GPIIb/IIIa), a platelet surface integrin whose activation and subsequent binding to fibrinogen is the final common step in the formation of platelet aggregates, have been utilised to treat unstable angina and myocardial infarction. Tirofiban is a novel, specific, low molecu
