Translational Medicine: Towards Gene Therapy of Marfan Syndrome.

Marfan syndrome (MFS) is one of the most common inherited disorders of connective tissue caused by mutations of the fibrillin-1 gene (FBN1). Vascular abnormalities, such as the enlargement of the aorta with the risk of life-threatening rupture are frequently observed. However, current treatment is limited and therapeutic options focus solely on symptomatic therapy. Gene therapy focuses on genetically modifying cells to produce a therapeutic effect and may be a promising treatment option for MFS. Here, we first provide an overview of the historical background and characterization of MFS. Subseq

Combined historical and electrocardiographic timing of acute anterior and inferior myocardial infarcts for prediction of reperfusion achievable size limitation.

The historical time of acute symptom onset is not always an accurate indication of the timing of onset of an acute myocardial infarction (AMI). Consideration of electrocardiographic (ECG) timing parameters could supplement historical timing alone as a clinical guide for decisions regarding the use of reperfusion therapy. Three hundred ninety-five patients from 4 trials of thrombolytic therapy conducted in the northwestern United States and western Canada are included in the present study. A total of 316 patients received either streptokinase or tissue plasminogen activator, and 79 received no

Amyloid in Primary Hip Arthroplasty Specimens: An Opportunity for Early Detection of Amyloidosis?

BACKGROUND: Recent advances in pharmacologic therapies to treat amyloidosis demand renewed focus on early identification of patients who will benefit from these therapies. Incidental detection of amyloid in orthopaedic specimens has been previously described, but the clinical significance of these findings remains unclear. Modern techniques for amyloid detection and subtyping may identify clinically relevant amyloid in routine orthopaedic specimens. The purpose of this study was to assess the prevalence of amyloid in a unique contemporary cohort of total hip arthroplasty (THA) specimens using

Oral mucosal hyperpigmentation secondary to antimalarial drug therapy.

A case of oral mucosal hyperpigmentation resulting from antimalarial drug therapy is presented. The patient reported a history of long-term quinacrine therapy and exhibited diffuse blue-gray pigmentation of the nail beds and the skin of the nasal ala. Microscopic examination of the involved mucosa showed macrophages, containing both melanin and ferric iron, scattered within the connective tissue adjacent to the epithelium. The clinical, historical, and microscopic features of antimalarial-induced pigmentation are discussed. Other causes of diffuse or multifocal oral pigmentation are also addre

Anatomy in dentistry: From the beginnings to contemporary reality.

The history of dentistry and medicine has a deep and ancient bond with the history of anatomy. From the ancient times to the present, the study of the human body represents the basis for research and clinical therapy. The historical contribution of anatomists to the development of dental medicine also helped in its characterization as an integrated but special and independent branch of medicine. From Galen's studies on animals to the discoveries of dental hard tissues characteristics using a microscope, this paper provides a brief literature review of how anatomy broadly contributed to the dev

Light-Induced Cancer Therapy.

Photodynamic therapy (PDT) and photothermal therapy (PTT) are promising light-induced approaches for cancer treatment. This article outlines the historical development of PDT and summarizes the underlying photophysical and photochemical mechanisms responsible for reactive oxygen species (ROS) generation and tumour destruction. Major classes of clinically approved and emerging photosensitizers (PSs) are discussed together with important design criteria and current limitations. In addition, the principles of PTT, including its oxygen-independent mechanism and major classes of photothermal agents

Phase II trial of high-dose conformal radiation therapy with concurrent hepatic artery floxuridine for unresectable intrahepatic malignancies.

PURPOSE: A phase II trial was conducted to determine if high-dose radiation with concurrent hepatic arterial floxuridine would improve survival in patients with unresectable intrahepatic malignancies. PATIENTS AND METHODS: Three-dimensional conformal high-dose radiation therapy was delivered concurrently with hepatic arterial floxuridine in 128 patients. The radiation dose was based on a normal-tissue complication probability model and subjected the patient to an estimated maximum risk of radiation-induced liver disease of 10% to 15%. The study design provided more than 80% power to detect a

Thromborepair: the tissue repair partner to thromboinflammation.

Platelets are small blood cells, historically viewed principally as mediators of hemostasis and thrombosis. However, it is now well established that platelets are pivotal modulators of the inflammatory response, establishing the concepts of thromboinflammation and immunothrombosis. Here, we propose the novel term "thromborepair" to describe how platelets promote tissue repair by facilitating leukocyte extravasation, resolving tissue inflammation, and coordinating fibrosis and angiogenesis. This review aims to discuss these processes and stimulate further exploration of platelet-based therapies

[Bacillary angiomatosis. A pseudoneoplastic infection in AIDS patients].

Bacillary angiomatosis is a newly recognized pseudoneoplastic vascular disease seen in the setting of human immunodeficiency virus (HIV) infection. The disease is characterized by a cutaneous infection with reddish papules or nodules that are similar to pyogenic granulomas or Kaposi's sarcoma in clinical appearance. It is caused by the mildly gram-negative bacillus Rochalimaea henselae, which can be identified in tissue sections by means of Warthin-Starry stain. The historical, clinical, histopathological, and microbiological aspects and the differential diagnosis and therapy of bacillary angi

Physiology of Mechanotransduction: How Do Muscle and Bone "Talk" to One Another?

The complexity of cell interactions with their microenvironment and their ability to communicate at the autocrine, paracrine, and endocrine levels has gradually but significantly evolved in the last three decades. The musculoskeletal system has been historically recognized to be governed by a relationship of proximity and function, chiefly dictated by mechanical forces and the work of gravity itself. In this review article, we first provide a historical overview of the biomechanical theory of bone- muscle interactions. Next, we expand to detail the significant evolution in our understanding of

Pulmonary manifestations of systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) can affect the lung in multiple ways. All components of the respiratory system, including the pleura, pulmonary parenchyma, airways, vessels, and respiratory muscles can be involved in various degrees at some time in the disease course and contribute to its morbidity and mortality. This article reviews the clinical symptoms, imaging techniques, histopathology, prognosis, and treatment of pulmonary manifestations of SLE and the related disorder mixed connective tissue disease (MCTD), from a historical perspective and with regard to new insights into pathogenes

[Erythema induratum Bazin. "Tuberculid" or tuberculosis?].

A 66-year-old woman presented with erythematous nodules on both legs and told of a history of lymph node tuberculosis that had been treated with surgery and tuberculostatic therapy years before. A biopsy taken from a nodule showed nodular vasculitis. PCR for Mycobacterium tuberculosis-specific DNA on the paraffin-embedded tissue was positive. Based on the experience with our patient, positive PCR results for Mycobacterium tuberculosis-specific DNA in lesions of nodular vasculitis are discussed critically. The historical background of "tuberculids", such as erythema induratum Bazin, is elucidat

The hypereosinophilic syndrome: dramatic response to therapeutic intervention.

The Hypereosinophilic Syndrome (HES) is a disease of unknown etiology and pathogenesis characterized by blood and bone marrow eosinophilia associated with infiltration of eosinophils into tissues and multi-system organ dysfunction. Patients with HES historically have very significant morbidity and a high mortality of 77% at 3 years. This study is a prospective (9 years) and retrospective (24 years) analysis of the therapy and prognosis of 26 patients with HES. Five patients (19%) showed no evidence of progressive organ system dysfunction and were given no therapy; all have done well. Sixteen p

The First Reported Case of Infant Soft Tissues Infection Caused by Legionella Maceachernii: A Case Report and Literature Review.

Legionella spp. predominantly Legionella pneumophila, are recognized respiratory pathogens, while soft tissue infections caused by non-pneumophila species remain exceptionally rare. We present the first documented case of L. maceachernii soft tissue infection in an infant worldwide. The patient presented with fever accompanied by occipital, anterior thoracic, and wrist masses. Diagnosis was confirmed through metagenomic next-generation sequencing (mNGS) of tissue samples with histopathological correlation. Initial empiric therapy with vancomycin and cefotaxime yielded no clinical improvement.

A new dawn for Parkinson's disease: commentary on the emergence of stem cell-based therapies.

The field of regenerative medicine for Parkinson's disease (PD) has reached a pivotal moment. After decades of preclinical research, recent first-in-human clinical trials demonstrated that cell replacement therapy using stem cell-derived dopaminergic neurons is not only feasible and safe but also shows promising signs of efficacy. Here we analyze three landmark 2025 studies, including the phase I/II trial of allogeneic induced pluripotent stem cell-derived dopaminergic progenitors, that mark a significant leap forward for PD therapy. We discuss principles underpinning the therapy, the historic
