Nonvital tooth bleaching: guidelines for the clinician.

The combination of intracoronal and extracoronal bleaching represents a conservative treatment to restore esthetics to darkened or stained nonvital teeth. Notwithstanding the eventual risks and difficulties accompanying such technique, nonvital teeth often can be successfully lightened. This article presents a few guidelines for bleaching nonvital teeth as well as a clinical case to illustrate the indications and clinical protocol for the technique.

Modified Technique for Nonvital Tooth Bleaching: A Case Report.

The aim of this study is to report a case of a nonvital, discolored, maxillary central incisor bleached by 35% hydrogen peroxide gel with the use of glass ionomer cement as a mechanical barrier in an attempt to minimize the undesirable side effects of intracoronal bleaching. The patient was a 13-year-old boy complaining of a discolored nonvital upper-right central incisor and was selected for this study from the pedodontic clinic in the Shibin Elkom teaching hospital in June 2013. After successful endodontic treatment, the tooth was bleached by 35% hydrogen peroxide gel (Opalescence Xtra), act

Revitalizing the Dull: A Classic Approach to Nonvital Tooth Bleaching.

Nonvital tooth bleaching is a treatment designed to enhance the cosmetic appearance of discolored teeth that have lost vitality. This procedure has become increasingly popular in contemporary dentistry due to its effectiveness. Among the various chemical agents used for this purpose, sodium perborate, when combined with hydrogen peroxide, has proven to be one of the most efficient solutions for achieving noticeable whitening outcomes. Sodium perborate is a stable oxidizing agent, frequently mixed with hydrogen peroxide to form a bleaching paste. This paste is then placed directly into the pulp

Nightguard bleaching to lighten a restored, nonvital discolored tooth.

Traditional techniques to lighten discolored endodontically treated teeth have used bleaching materials that had to be applied and directly monitored by dentists. These bleaching techniques have been associated with cervical resorption in nonvital teeth, and the treatment outcome is often difficult to predict. This article describes the use of nightguard bleaching to obtain an esthetic result for a restored nonvital tooth that had been treated with limited success with traditional office-based bleaching procedures.

Nonvital tooth bleaching: a review of the literature and clinical procedures.

Tooth discoloration varies in etiology, appearance, localization, severity, and adhesion to tooth structure. It can be defined as being extrinsic or intrinsic on the basis of localization and etiology. In this review of the literature, various causes of tooth discoloration, different bleaching materials, and their applications to endodontically treated teeth have been described. In the walking bleach technique the root filling should be completed first, and a cervical seal must be established. The bleaching agent should be changed every 3-7 days. The thermocatalytic technique involves placemen

Bleaching of severely darkened nonvital tooth case report-48 months clinical control.

OBJECTIVE: To record in detail, the long-term success of a bleaching treatment employing hydrogen peroxide in a severely darkened tooth. CLINICAL CONSIDERATIONS: Tooth discoloration is an important aesthetic complain of patients. Because it is a relatively common condition, different materials and techniques capable of restoring the tooth color in a minimally invasive, conservative, and longstanding manner were developed. In this case, it was used the association between mediate (walking bleach) and immediate (internal/external) bleaching techniques using hydrogen peroxide as the main agent.

Regeneration of Pulp and Radicular Tissues in a Nonvital Avulsed Tooth with Open Apex: A Case Report.

AIM AND BACKGROUND: The management of avulsed immature teeth poses a significant challenge to clinicians, as prolonged dry time before reimplantation may lead to replacement resorption or ankylosis. CASE DESCRIPTION: Recent studies have shown promising results with the use of an intracanal proprietary pharmaceutical preparation, followed by sealing of the orifice with mineral trioxide aggregate (MTA). Angiography suggested the presence of vascularized tissue after 1 year, indicating successful regeneration of pulpal tissue in the radicular area. CONCLUSION: Despite an unfavorable crown-root

Nonsurgical management of a nonvital tooth with orthodontically induced external root resorption and extensive periapical pathology.

The purpose of this article is to report a rare case of orthodontically induced external root resorption of a maxillary left central incisor with a large periapical radiolucency; it was treated successfully with a conservative approach that included the use of calcium hydroxide. The tooth was nonvital and had grade III mobility. It was treated endodontically for 2 years with an interim filling of calcium hydroxide mixed with propylene glycol. After 2 years, the postoperative intraoral periapical radiograph showed good periapical healing with complete resolution of periapical radiolucency. Ther

Allogeneic Mesenchymal Stromal Cells Transplantation Induces Arrest of External Apical Inflammatory Resorption in a Mature Nonvital Traumatized Tooth.

INTRODUCTION: Mesenchymal stromal cells (MSCs) have been proposed for tissue regeneration based on their paracrine and immunomodulatory effects. Based on these features, MSCs may regulate inflammation and tissue repair. This case report assessed whether allogeneic bone marrow MSCs (BM-MSCs) can arrest external apical inflammatory resorption (EAIR) in a mature, nonvital traumatized tooth. METHODS: A healthy 14-year-old male with intermittent edema and a sinus tract associated with tooth #9 was referred for endodontic evaluation. Radiographically, tooth #9 showed EAIR with a vertical/diagonal m

Does the Use of a "Walking Bleaching" Technique Increase Bone Resorption Markers?

OBJECTIVE: This randomized clinical trial evaluated the effect of 35% hydrogen peroxide in comparison with 37% carbamide peroxide in a nonvital bleaching technique of "walking bleaching" (four sessions of treatment) on periodontal markers: nuclear factor kappa B-ligand (RANK-L-process of root resorption marker) and interleukin 1β (IL-1β-inflammatory response marker). METHODS AND MATERIALS: Fifty volunteers presenting with discoloration of nonvital teeth and endodontic treatment in good condition participated. Fifty teeth were randomly divided into two study groups according to bleaching gel:

Odontogenic keratocyst looks can be deceptive, causing endodontic misdiagnosis.

Odontogenic keratocyst (OKC) is the cyst arising from the cell rests of dental lamina. It can occur anywhere in the jaw, but commonly seen in the posterior part of the mandible. Radiographically, most OKCs are unilocular when presented at the periapex and can be mistaken for radicular or lateral periodontal cyst. When the cyst is multilocular and located at the molar ramus area, it may be confused to ameloblastoma. Lots of cases have been reported in the literature where OKC is associated with the nonvital tooth. So trauma could be one of the reasons in inducing this cyst. In our case, it was

Bleaching of nonvital teeth. A clinically relevant literature review.

Today, the bleaching of nonvital, discolored teeth is a low-risk routine treatment for improving esthetics. This review article focuses on the etiology of tooth discolorations, different treatment techniques, and risks of bleaching procedures. Some tooth discolorations in endodontically treated teeth are caused by dental treatments. The three most popular techniques for nonvital tooth bleaching are the walking bleach technique, inside/outside bleaching, and in-office bleaching. The walking bleach technique is a relatively reliable, fairly simple technique for dentists and patients. Inside/outs

Interrelation between endodontic status, pathologic cemental granules and periodontal ligament adhering to the root of extracted teeth.

OBJECTIVES: To evaluate if the endodontic status has influence on the adherence of periodontal ligament (PDL) to extracted teeth and if the presence of "pathologic granules" is correlated to the endodontic status and/or the amount of adhering PDL. STUDY DESIGN: Extracted teeth with different endodontic status and exhibiting no marginal periodontitis were histologically examined. RESULTS: Observation of pathologic granules was related to a nonvital endodontium and to acute peroperative inflammation. Pathologic granules and denudation of the extracted roots were significantly related. Acute pe

Maxillary and mandibular unusually large radicular cyst: A rare case report.

Radicular cysts are the most common odontogenic cyst. It is an inflammatory cyst associated with the root apex of a nonvital tooth. Most radicular cysts are small but can reach a large size causing displacement of surrounding structures. Here, we present a rare case of huge radicular cyst in both maxilla and mandible in a 36-year-old patient.

Dentigerous cyst in a young child.

Dentigerous cyst is a type of odontogenic cysts and generally occurs in the ages of twenties or thirties. Dentigerous cyst always includes a tooth which cannot complete the eruption process and occurs around the crown by the fluid accumulation between the layers of enamel organ. In rare cases, dentigerous cyst occurs in the first decade of life and develops in an immature permanent tooth as a result of a chronic inflammation of overlying nonvital primary tooth. In this report, a case of dentigerous cyst in primary dentition in a 5-year-old child patient and its treatment were presented. The de

Bleaching of vital and nonvital teeth.

Although tooth bleaching has been known to the dental profession for over 100 years, new knowledge is continually surfacing. This review discusses the 1990 to 1991 literature on two nonvital bleaching techniques (thermocatalytic and walking) and three classes of vital bleaching techniques (in-office; dentist-prescribed, home-applied; and over-the-counter kits). The choice for nonvital bleaching is the walking bleach technique, with the use of sodium perborate alone having less potential for cervical resorption. The choices for vital bleaching techniques are either the dentist-prescribed, home-

Dentigerous cysts of inflammatory origin. A clinicopathologic study.

The exact histogenesis of dentigerous cysts remains unknown, but most authors favor a developmental origin from the tooth follicle. The aim of this article is to report a series of 15 dentigerous cysts that we believe to be of inflammatory origin. These inflammatory dentigerous cysts occurred in the first and early part of the second decades of life. Males were affected more frequently, and there did not appear to be any racial predilection. All of the cases involved permanent teeth: premolars in nine cases, canines in four cases, and second molars in two cases. The mandible was affected twice

Why teeth associated with inflammatory periapical lesions can have a vital response.

It is a general conception that teeth having an inflammatory periapical lesion are nonvital and the associated periapical lesions are not innervated. However, structurally intact and functioning nerve fibers have been shown to be present in the periapical lesion and/or the associated pulp. Therefore, when a patient presents with an endodontically involved tooth associated with a periapical lesion and experiences pain during canal instrumentation, the need for local anesthesia is apparent.

Orthodontic-endodontic treatment planning of traumatized teeth.

Occasionally, an orthodontic patient will accidentally traumatize a maxillary anterior tooth before or during orthodontic treatment. In some situations, the trauma will be substantial and avulse the tooth. In other accidents, the tooth may not avulse, but the pulp becomes nonvital. If the pulp is devitalized, and the root has not fully formed, the apex of the root canal may be wide. In this situation, the endodontist may recommend apexification procedures to help close the apex before conventional obturation of the root canal. If the patient is currently undergoing orthodontic movement of the

Diagnostic accuracy of Transmitted-light plethysmography for the assessment of pulpal circulation in traumatized young permanent incisors.

Transmitted light plethysmography (TLP) is a noninvasive, objective optical method used to assess dental pulp circulation and evaluate pulp vitality. This study aimed to develop a quantitative diagnostic approach for TLP and assess its accuracy through cross-correlation analysis of plethysmographic signals obtained from both the tooth and the finger. A total of 131 maxillary incisors from 97 pediatric patients (aged 6-17 years) were categorized into three groups: nontraumatized teeth, traumatized vital teeth, and traumatized nonvital teeth. Standard diagnostic procedures, including assessments
