[Tooth erosion - a multidisciplinary approach].

During the last decades, an increasingly greater interest in dental erosion has been observed in clinical dental practice, in dental public health and in dental research because prevalence of erosive tooth wear is still increasing especially in young age group of population. Erosive tooth wear is a multifactorial etiology process characterized by progressive loss of hard dental tissue. It is defined as the exogenous and/or endogenous acids dissolution of the dental tissue, without bacterial involvement. In the development of dental erosive wear, interactions are required which include chemical

Medical conditions and erosive tooth wear.

There are many reasons why it is vital that dental professionals identify signs and symptoms that suggest that an individual's erosive tooth wear (ETW) may be linked to a broader medical problem than just poor diet. Primarily, spotting an underlying medical cause for ETW increases the likelihood that further deterioration in the individual's oral health will be prevented. However, perhaps more importantly, many of the medical conditions which are commonly related to ETW can have serious and even possibly fatal consequences if left untreated or not diagnosed. This paper reviews the nature of th

Is erosive tooth wear an oral disease?

Erosive tissue loss is part of the physiological wear of teeth. Clinical features are an initial loss of tooth shine or luster followed by flattening of convex structures; with continuing acid exposure, concavities form on smooth surfaces, or grooving and cupping occur on incisal/occlusal surfaces. Dental erosion must be distinguished from other forms of wear, but can also contribute to general tissue loss by surface softening, thus modifying physical wear processes. The determination of dental erosion as a condition or pathology is relatively easy in the case of pain or endodontic complicatio

Dental erosion and erosive tooth wear in childhood and adolescence: from diagnosis to prevention.

BACKGROUND: Dental erosion and erosive tooth wear (ETW) are increasingly significant non-carious defects in industrialized countries. Reasons for an increased prevalence are mainly modern dietary habits, but an improved diagnostic awareness also contributes to an apparent increase in cases. Unlike caries, erosion involves acid-induced demineralization without bacterial involvement. When this is combined with mechanical wear, it results in ETW. Diagnosis commonly employs the Basic Erosive Wear Examination (BEWE), a standardized clinical index assessing lesion severity across sextants. ETIOLOGY

The use of fluoride for the prevention of dental erosion and erosive tooth wear in children and adolescents.

BACKGROUND: Erosive tooth wear (ETW) has gained increasing clinical relevance. It is estimated that worldwide 30-50% of deciduous and 20-45% of permanent teeth are affected. One of the most important nutritional factors causing ETW is the overconsumption of soft drinks, but also patient-related factors like reflux or eating disorders can lead to erosive lesions. Whether acids lead to erosive demineralisation depends on their degree of saturation with respect to tooth mineral at their actual pH. REVIEW: Fluoride compounds like sodium or amine fluoride seem to be of limited efficacy against ero

BEWE 2.0: Basic erosive tooth wear examination revisited: Introducing an additional level for more severe erosive tooth wear.

The Basic Erosive Wear Examination (BEWE) was proposed in 2008 as a screening tool for tooth wear. The original aim was to create a simple, convenient screening tool to prompt a clinician to record tooth wear. It was not intended to be accurate and was matched to the basic periodontal examination (BPE). Since that time it has been incorporated into dental practice software and has been adopted by Dentists. But the original screening tool created a ceiling effect, particularly at grade 3 which marked a significant progression from slight wear over the surface to almost complete obliteration of

Gastric reflux is a significant causative factor of tooth erosion.

Dental erosion is caused by dietary or gastric acid. This study aimed to examine the location and severity of tooth erosion with respect to causative factors, and to determine whether the clinical pattern of erosion reflected the dominant etiological factor. The study involved 249 Icelandic individuals and included: a detailed medical history; clinical oral examination; salivary sampling, and analysis for flow rate, pH, and buffering capacity. Reflux was assessed in 91 individuals by gastroscopy, esophageal manometry, and 24-hour esophageal-pH monitoring. Reflux symptoms were reported by 36.5%

Longitudinal study of gastroesophageal reflux and erosive tooth wear.

BACKGROUND: Approximately 60% of patients presenting to dentists with erosive tooth wear have significant gastroesophageal reflux (GERD), despite minor reflux symptoms. No longitudinal studies of reflux-associated erosive tooth wear and of reflux characteristics have been reported to date. The aim of this study was to characterize the longitudinal course of GERD and of associated erosive tooth wear, as well as factors predictive of its progression, in a large group of patients. METHODS: Seventy-two patients presenting to dentists with clinically significant erosive tooth wear and increased es

Interaction of dental erosion and bruxism: the amplification of tooth wear.

Bruxism and erosion are important to identify during a dental examination. By understanding the etiologies of both processes, a management strategy can be implemented to decrease their effects. Management for bruxism, clenching, and parafunction can include medications, cognitive behavioral therapy, and dental appliances. Bruxism, clenching, and parafunction combined with dental erosion can cause dental wear to increase faster than any component alone.

Defining progression in erosive tooth wear: an interpretive challenge.

OBJECTIVES: To critically evaluate current approaches to longitudinal erosive tooth wear (ETW) monitoring and examine whether quantitative intraoral scanner (IOS)-based measurements and conventional clinical indices can be meaningfully compared in defining progression. MATERIALS AND METHODS: A systematic review identified longitudinal studies evaluating ETW using IOS-based quantitative approaches alongside conventional clinical assessment methods. MEDLINE, Embase, Scopus, and Web of Science were searched from database inception to January 2026, with grey literature sources also searched. The

Factors Related to Erosive Tooth Wear throughout a Lifetime.

Factors related to the development of erosive tooth wear (ETW) over a lifetime can largely be divided into dietary, environmental and/or patient related. As the properties of the teeth change over time, these factors will be involved at different ages, and the susceptibility to ETW will vary according to age. Based on the available literature, dietary acid is the most important determinant of ETW. Various eating/drinking habits and oral hygiene routines are considered moderate factors in children and minor factors in adults. Medications and use of illicit drugs in adolescents and adults are po

Tooth Enamel Demineralization: Caries and Erosion from the Viewpoint of Chemistry.

The demineralization of tooth enamel is the primary consequence of dental caries, leading to cavities and finally tooth loss. Erosive tooth wear from acidic beverages and food is another factor that degrades enamel. In both cases, an acidic environment leads to etching and the final dissolution of tooth mineral, i.e., hydroxyapatite. Here, this process is discussed from a chemical perspective, taking into account the solubility of calcium phosphate and the presence of the pellicle (protein layer) and plaque (bacterial biofilms), which both affect the dissolution rate. While low pH is definitel

Digital and artificial intelligence-based screening for erosive tooth wear.

BACKGROUND: Accurate detection of Erosive Tooth Wear (ETW) is essential for early intervention and monitoring. The Basic Erosive Wear Examination (BEWE) is frequently used clinically, yet reproducibility is limited. Digital intraoral scans and Artificial Intelligence (AI) may offer increased sensitivity, reduced bias, and improved diagnostic reliability. METHODS: This cross-sectional clinical study (ISRCTN16797270) recruited 61 dentate adults with mild, moderate and severe ETW, with analysis of 1600 teeth (4797 surfaces). Participants received a clinical BEWE and dentine-exposure examination

Erosive tooth wear and caries experience in children and adolescents with obesity.

CLINICAL SIGNIFICANCE: A large consumption of fermentable carbohydrates, for instance the high intake of sweetened beverages, is an important risk factor for overweight and obesity. As lemonades and fruit juices present high sugar content and quite low pH-values, overweight and obese children might also have an increased risk for dental caries and erosive tooth wear. OBJECTIVES: The aim was to analyze the prevalence and severity of erosive tooth wear and caries experience in children and adolescents with overweight, obesity and extreme obesity compared to children with normal weight and to de
