Tourniquets revisited.

BACKGROUND: Controversy swirls about optimal control of life-threatening hemorrhage from an injured extremity whether in combat in the Middle East or in trauma care at home. Left unanswered are four critical questions: (1) What is the simplest tourniquet available? (2) Can it be used below the elbow and the knee? (3) Is pain a factor? (4) What data support so called "Pressure Points?" METHODS: To address these questions, we measured the effects of three common tourniquets on arterial pulses (Doppler signals) at wrist and ankle of 10 healthy adult volunteers of either sex. We recorded ease of

[Pneumatic tourniquets in orthopedics].

Pneumatic tourniquets, often used to provide a bloodless operating field, carry a risk of adverse effects. Limb exsanguination by gravitation is less aggressive than by mechanical means. Skin, muscles, nerves and vessels suffer maximally under tourniquet because of mechanical pressure, with both a sagittal force, responsible for compression and an axial force responsible for stretchening. All parts of the limb are therefore affected by ischaemia. The restarting of circulation will also increase lesions at the microcirculatory level, responsible for the "no reflow" phenomena. Transient reperfus

Reduced bacterial contamination rates detected on silicone tourniquets compared to conventional tourniquets in clinical routine.

BACKGROUND: Tourniquets used for peripheral venous vascular access such as blood sampling are regularly contaminated in clinical routine. Although most contaminations are harmless, some pose a possible risk for infection. To improve peripheral venous access infection control standards, tourniquets with no or as few as possible bacterial burden should be used. Conventional tourniquets can be reprocessed by autoclaving or by incubating in disinfectants. However, both methods are time-consuming and not suitable for immediate use between patients. In contrast, silicone tourniquets can be quickly a

Assessing the Current Generation of Tourniquets.

INTRODUCTION: Tourniquet application is an urgent life-saving procedure. Previous studies demonstrated several drawbacks in tourniquet design and application methods that limit their efficacy; among them, loose application of the device before windlass twisting is a main pitfall. A new generation of modern combat tourniquets was developed to overcome these pitfalls. The objective of this study was to assess the effectiveness of three new tourniquet designs: the CAT Generation 7 (CAT7), the SAM Extremity Tourniquet (SAM-XT), and the SOF Tactical Tourniquet Wide (SOFTT-W) as well as its correlat

Do tourniquets for extremity injuries save lives?

Hemorrhage is the leading cause of preventable death in trauma, and tourniquets have become standard for extremity hemorrhage in both civilian settings and combat. Proponents argue that timely and properly applied tourniquets significantly improve survival in appropriately selected patients. Critics of frequent tourniquet use highlight the high rates of inappropriate application and associated complications, especially with prolonged use. Both sides agree that tourniquets can be lifesaving, but emphasize the need for proper training, appropriate use, and implementation of tourniquet conversion

Warning: Tourniquets Risk Frostbite in Cold Weather.

We sought to better understand the frostbite risk during first-aid tourniquet use by reviewing information relevant to an association between tourniquet use and frostbite. However, there is little information concerning this subject, which may be of increasing importance because future conflicts against near-peer competitors may involve extreme cold weather environments. Historically, clinical frostbite cases with tourniquet use occurred in low frequency but in high severity when leading to limb amputation. The physiologic response of vasoconstriction to cold exposure leads to limb cooling and

Use of Tourniquets in Limb Trauma Surgery.

Although tourniquets are commonly used in patients with limb trauma patients, both in the acute and elective settings, no set protocols exist for their indications, contraindications, or proper use. This article addresses the current literature on optimal pressure, timing, cuff design, and complications of tourniquets in trauma patients. General issues are discussed, followed by those specific to upper and lower extremities. Lastly, serious complications, such as pulmonary embolism, are described.

Tourniquets in Trauma Care: A Review of Application.

Traumatic hemorrhage has been identified as the leading cause of battlefield death in recent conflicts. Although injury patterns are not directly reproducible to the civilian world, treatment advancements can be used to provide care to patients worldwide. Long-standing dogma regarding the use of tourniquets has been disproved, and there is now recognition of the critical role that tourniquets play in trauma care. The history and evolution of tourniquets, including the identification of previous faults in application, will lead to an examination of the current devices in use along with evidence

Safety and Appropriateness of Tourniquets in 105 Civilians.

BACKGROUND: The United States military considers tourniquets to be effective for controlling bleeding from major limb trauma. The purpose of this study was to assess whether tourniquets are safely applied to the appropriate civilian patient with major limb trauma of any etiology. METHODS: Following IRB approval, patients arriving to a level-1 trauma center between October 2008 and May 2013 with a prehospital (PH) or emergency department (ED) tourniquet were reviewed. Cases were assigned the following designations: absolute indication (operation within 2 hours for limb injury, vascular injury

The Use of Tourniquets in the Russo-Ukrainian War.

AIM: The objective of the study was to evaluate the use of tourniquets in the Russo-Ukrainian war. METHODS: The type, number, and duration of tourniquets per limb, the clinical course of limb injuries, and the functional status of the injured limbs during the 24 hours post-injury were evaluated in military hospital facilities for the period of 2014-2022. Statistical frequencies and variances were analyzed. RESULTS: During active hostilities, the medical units of the Southern Operational Command received 2,496 patients with limb injuries that required the application of tourniquets. Lower ext

Extremity tourniquets raise blood pressure and maintain heart rate.

BACKGROUND: Tourniquets have been modified and used for centuries to occlude blood flow to control hemorrhage. More recently, the occlusion of peripheral vessels has been linked to resultant increases in blood pressure, which may provide additional therapeutic potential, particularly during states of low cardiac output. OBJECTIVE: The objective of this study was to investigate a causal relationship between tourniquet application and blood pressure in healthy adults. METHODS: Healthy adult volunteers were recruited to participate in this IRB-approved study. Each participant met inclusion crit

Hair toe tourniquets: a review of two case studies.

Digit hair tourniquets are relatively uncommon. There are reports in the literature of hair tourniquets involving other appendages, such as the penis and uvula, however the phenomenon is not widely recognised and is often overlooked by healthcare professionals. This article discusses two case studies in which hair was responsible for creating a tourniquet around a digit. The article explores possible causes and management options for patients, with reference to the case studies. Midwives and health visitors are central to minimising the risk of injury to children as they can educate expectant

[Tourniquets, a drama in the infant: about 3 cases].

The time limit for the removal of a tourniquet is short; any delay in tourniquet deflation, especially if it exceeds the 3 hour limit, exposes to amputation hazards. Our objective was to report three cases of ischemic limb gangrene, caused by having forgotten to take a tourniquet off after a blood sampling, to inform healthcare professionals about the risk associated with that negligence. We encountered 3 cases of infants (2 three-month-old infants and 1 five-month-old infant), hospitalized in intensive care unit of Yalgado Ouédraogo University Hospital for upper-left limb swelling. Their medi

Another civilian life saved by law enforcement-applied tourniquets.

Increasing data and anecdotal operational reports are supporting the early, aggressive, prehospital application of tourniquets in potentially life-threatening extremity trauma. Especially in the civilian urban setting where transport times are short, the benefit in terms of lives saved far outweighs the potential risk to the extremity. The popular press has reported frequently on law enforcement-applied tourniquets, but to date, no group has published a scientific review of any of these cases. This case report suggests that law enforcement personnel can be trained to safely identify indication

Smart, automated junctional tourniquets leveraging AI-driven ultrasound guidance.

Tourniquets are commonly used devices for hemorrhage control; however, their effectiveness is reduced in anatomical junctions such as the neck and inguinal region. Junctional tourniquets specifically require precise placement to be effective. This precision can be enabled with ultrasound technology to help locate and occlude the major vessels in the junctional regions properly. However, interpretation of ultrasound requires highly skilled personnel, who may not necessarily be available in emergency situations. To overcome this hurdle, we have developed two ultrasound-enabled, AI-driven junctio

BACTERIAL COLONIZATION IN ORTHOPEDIC SURGICAL TOURNIQUETS: A MULTICENTER STUDY.

OBJECTIVE: To evaluate the prevalence and microbiological profile of contamination in reusable tourniquets in Brazilian hospitals. METHODS: A multicenter study conducted in six hospitals. Swabs were collected from 54 tourniquets immediately after surgical use and before disinfection, covering an estimated area of 10 cm². The samples were cultured and identified using automated methods. The microbial load was described by median and interquartile range (IQR), and comparisons between public and private hospitals were performed using Fisher's exact test and Mann-Whitney test. RESULTS: The preva

The penile tourniquet.

Tourniquets are commonly used in penile surgery to achieve a bloodless operating field or produce artificial erections intraoperatively. Several techniques have been described, but there is a paucity of data and a lack of guidelines to direct their safe use. In penile surgery, it is the local rather than systemic effects of tourniquet use that are the main concern. Tourniquet time should be kept to a minimum, as the limited data available suggests that reperfusion injury can occur even after short periods of ischaemia. High risk groups such as diabetics and arteriopaths are at particular risk.
