Transcranial magnetic stimulation.

PURPOSE OF REVIEW: To present state-of-the-art transcranial magnetic stimulation (TMS) therapy, especially when it is used in psychiatric disorders, on the basis of an exhaustive literature search from 2006 to date (June 2008) on TMS papers published in Medline and Embase. Other references and comments from our own experience started 8 years ago have also been taken into account. RECENT FINDINGS: The mechanism of action of TMS is now better understood. There is strong evidence of the safety and tolerability of TMS when standard protocols are used. The efficacy of the stimulation of the dorsol

[Transcranial magnetic stimulation.].

Transcranial Magnetic Stimulation (TMS) is a new non-invasive method to investigate the central nervous system. Initially it was used to assess the functional integrity of the pyramidal pathways but more recently various other aspects of brain function have been studied including cortical excitability. By localised interference with brain function, it is possible to use TMS to assess the relationship between various brain regions and cognitive functions. The therapeutic effect of TMS has been explored in the treatment of neurological diseases and psychiatric disorders such as epilepsy, cerebel

[Transcranial magnetic stimulation].

The technique of transcranial magnetic stimulation of the motor cortex is a new neurophysiological examination of the central motor pathways. In clinical application a reduced MEP/M wave amplitude ratio had a higher correlation with hyperreflexia than did a delayed MEP onset latency. The inhibitory period (IP) which is a transient suppression of muscle action potentials following motor evoked potentials (MEP) by magnetic cortical stimulation is caused primarily by inhibitory process in the motor cortex, because the duration of IP did not depend on the amplitude of MEPs, MEP distribution over t

Transcranial Magnetic Stimulation and Transcranial Electrical Stimulation in Horses.

Depending on the localization of the lesion, spinal cord ataxia is the most common type of ataxia in horses. Most prevalent diagnoses include cervical vertebral stenotic myelopathy (CVSM), equine protozoal myeloencephalitis (EPM), trauma and equine degenerative myeloencephalopathy (EDM). Other causes of ataxia and weakness are associated with infectious causes, trauma and neoplasia. A neurologic examination is indispensable to identify the type of ataxia. In addition, clinical neurophysiology offers tools to locate functional abnormalities in the central and peripheral nervous system. Clinical

Transcranial magnetic stimulation and epilepsy.

Epileptic conditions are characterized by an altered balance between excitatory and inhibitory influences at the cortical level. Transcranial magnetic stimulation (TMS) provides a noninvasive evaluation of separate excitatory and inhibitory functions of the cerebral cortex. In addition, repetitive TMS (rTMS) can modulate the excitability of cortical networks. We review the different ways that TMS has been used to investigate pathophysiological mechanisms and effects of antiepileptic drugs in patients with epilepsy and epileptic myoclonus. The safety of different TMS techniques is discussed too

Transcranial magnetic stimulation. Negative effects.

Myoclonus appears to be generated by a brief transient activation of a focal region of motor cortex. Activation of a focal region of motor cortex can also give rise to a transient depression of function. If a subject, or patient, is trying to produce movement, this depression would lead to a brief lapse in muscle activation that would be interpreted as either asterixis or negative myoclonus. Both positive events and negative events can occur either individually or together. The physiological substrate in the cortex appears to be different for the two phenomena, and the pharmacology may differ

Transcranial Magnetic Stimulation for Schizophrenia.

People with schizophrenia typically experience auditory hallucinations or delusions during acute episodes. Although effective drug treatments are available, many have intractable symptoms that do not recover between acute episodes. One proposed alternative to drug treatments is transcranial magnetic stimulation (TMS). To date, many research trials to assess effectiveness of TMS for people with symptoms of schizophrenia have been conducted worldwide. However, there is a lack of consensus on whether TMS should be recommended to be adopted in routine clinical practice. We conducted a systematic r

Transcranial magnetic stimulation in children.

Developmental disabilities (e.g. attention deficit disorder; cerebral palsy) are frequently associated with deviations of the typical pattern of motor skill maturation. Neurophysiologic tools, such as transcranial magnetic stimulation (TMS), which probe motor cortex function, can potentially provide insights into both typical neuromotor maturation and the mechanisms underlying the motor skill deficits in children with developmental disabilities. These insights may set the stage for finding effective interventions for these disorders. We review the literature pertaining to the use of TMS in ped

Myoclonus and transcranial magnetic stimulation.

The neural dysfunction at the origin of myoclonus may locate at various anatomical levels within the central nervous system, including the motor cortices. Transcranial magnetic stimulation (TMS) can be used to assess the balance between inhibitory and excitatory processes involved in the regulation of motor cortex activity and thereby, may be of value to determine the pathophysiological mechanisms of myoclonus. Using paired-pulse paradigms with various interstimulus intervals, TMS studies showed that intracortical inhibition (ICI) was reduced in progressive myoclonic epilepsy (PME). In contras

Transcranial magnetic stimulation in neurorehabilitation.

In neurorehabilitation, transcranial magnetic stimulation (TMS) offers information regarding prognosis and pathophysiology and could also be useful for therapeutic purposes. Numerous studies have indicated that, after stroke, the absence of motor evoked potentials is associated with a poor motor recovery. In contrast, MEPs obtained in the paretic muscle with low stimulus intensities suggest a good restitution of motor function. TMS studies have shown that the location of a brain lesion determines motor cortex excitability changes: Patients with central somatosensory lesions show a disinhibitio

Novel ways to modulate the vestibular system: Magnetic vestibular stimulation, deep brain stimulation and transcranial magnetic stimulation / transcranial direct current stimulation.

BACKGROUND: Advances in neurotechnologies are revolutionizing our understanding of complex neural circuits and enabling new treatments for disorders of the human brain. In the vestibular system, electromagnetic stimuli can now modulate vestibular reflexes and sensations of self-motion by artificially stimulating the labyrinth, cerebellum, cerebral cortex, and their connections. OBJECTIVE: In this narrative review, we describe evolving neuromodulatory techniques including magnetic vestibular stimulation (MVS), deep brain stimulation (DBS), transcranial magnetic stimulation (TMS), and transcran

Transcranial magnetic stimulation for chronic pain.

Current data suggest that transcranial magnetic stimulation (TMS) has the potential to be an effective and complimentary treatment modality for patients with chronic neuropathic pain syndromes. The success of TMS for pain relief depends on the parameters of the stimulation delivered, the location of neural target, and duration of treatment. TMS can be used to excite or inhibit underlying neural tissue that depends on long-term potentiation and long-term depression, respectively. Long-term randomized controlled studies are warranted to establish the efficacy of repetitive TMS in patients with v

[Transcranial magnetic stimulation: physiology and applications].

Transcranial magnetic stimulation (TMS) is a relatively new technique that allows painless activation of cortical motor neurons. In the clinical setting, TMS is primarily used for the investigation of the corticospinal tract in various neurological diseases, being especially useful in the detection of subclinical dysfunction. In addition to the motor cortex, TMS can be applied to examine other structures inaccessible to electrical stimulation, such as the canalicular portion of the facial nerve. In healthy individuals, TMS can be utilized to monitor excitability changes of the motor system in

Transcranial magnetic stimulation and Parkinson's disease.

While motor cortical areas are the main targets of the integrative activity of basal ganglia, their main output consists of the corticospinal system. Transcranial magnetic stimulation (TMS), a relatively new method to investigate corticospinal physiology, has been widely used to assess possible changes secondary to Parkinson's disease (PD). The use of single- and paired-pulse TMS, two varieties of the original technique, disclosed multiple functional alterations of the corticospinal pathway. For instance, when the latter was tested at 'rest', or in response to somesthetic afferents, it showed

[Panic disorder and transcranial magnetic stimulation].

Transcranial magnetic stimulation (TMS) has been tried in some Anxiety Disorders (Obsessive-compulsive disorder and Posttraumatic Stress Disorder) with different results. We present a pilot study including three Panic Disorder patients. The subjects who were enrolled had a history of the disease for at least 1 year and they had unsuccessfully followed psychotherapy and pharmacological treatment. The patients received 10 sessions during two weeks; each session lasted 30 trains of 60 seconds at a frequency of 1 Hz, on the right dorsolateral prefrontal cortex, at 110% of the motor threshold. [Transcranial magnetic stimulation for tinnitus treatment].

High prevalence of tinnitus, its negative influence on the quality of life and psychoemotional status of patients support need and extreme urgency of further research of various treatment methods. Rhythmic transcranial magnetic stimulation (rTMS) is an effective method of therapy in patients with chronic tinnitus. The optimal protocol for rhythmic stimulation is low-frequency (1 Hz) rTMS with low-intensity stimuli (below 110% of the motor threshold) with a limited number of stimuli per series. There is evidence that high-frequency rTMS in the area of relatively thin temporal bones can lead to
