Transitional care of biliary atresia.

Worldwide native liver survival (NLS) for young adults (>20 years) with biliary atresia varies between 14% and 44% with the majority of patients developing complications in adulthood. Cholangitis and portal hypertension with variceal bleeding are the most common complications and development of these during adolescence associated with the need for liver transplantation during adulthood. Adult listing criteria, typically developed on the background of adult liver disease might not be applicable to this patient population and leads to longer waiting time and risk of deterioration of their medica

Transitional care in pediatric urology.

The transition from childhood to adolescence and into adulthood occurs as a natural component of human development. As children progress through school and gain independence, health care practitioners must facilitate a parallel transition from pediatric to adult providers. Modern medicine has succeeded in extending the life expectancy for many children with complex conditions, and adult providers are participating in their medical care through adulthood. Transitioning pediatric urology care to adult urology care is unique to every individual and his or her underlying condition, while the trans

Multidisciplinary dentistry for transitional care patients.

A growing patient population is adolescents and young adults who have had one or more serious medical problems and are aging into adulthood. This group of patients has unique medical needs, which has resulted in the development of a specialized area of medicine: transitional care medicine. The case reviews of two of these patients are described. Patient 1 was a 23-year-old man with hereditary pancreatitis. His genetic condition resulted in the need for pancreatic splenectomy and removal of part of his small bowel, resulting in insulin-dependent diabetes and malnutrition. These complex clinical

[Neonatal transitional care unit (author's transl)].

Statistical analysis between a two year period with a five year interval (1972-1975) in a neonatal transitional care unit is evaluated. Number of resucitations doubled and neonatal mortality in the first seven days of life as well as in the first twenty four hours decreased significantly, being newborns under 1,000 gr. of weight and malformations the first two causes of death in both periods. Factors related to hypoxia which accounted for other 25% of deaths should be prevented. Importance of this type of units in huge maternities is emphasized considering their role in immediate neonatal care

Heart Failure Transitional Care and Ambulatory Monitoring.

Chronic heart failure is characterized by periods of stability punctuated by clinical decline often requiring hospitalization. Following hospitalization, patients remain at an increased risk of morbidity and mortality, often requiring rehospitalization due to recurrent deterioration. Transitional care interventions aim to improve the movement from the inpatient to ambulatory setting. Promising interventions, such as patient education, novel uses of loop diuretics, and invasive hemodynamic monitoring, have been developed over the past 20 years all with varying degrees of success.

Telehealth-based transitional care management programme to improve access to care.

BACKGROUND: The transition from hospital to home is a vulnerable time for patients and families that can be improved through care coordination and structured discharge planning. LOCAL PROBLEM: Our organisation aimed to develop and expand a programme that could improve 30-day readmission rates on overall and disease-specific populations by assessing the impact of a telehealth outreach by a registered nurse (RN) after discharge from an acute care setting on 30-day hospital readmission. METHODS: This is a prospective observational design conducted from May 2021 to December 2022 with an urban, n

[Transitional care in urology : from pediatrics to adulthood].

Advances in pediatric care have improved survival in chronic urological disorders, necessitating structured transitions to adult services. These programs aim to preserve renal and bladder function, continence, fertility, and quality of life, while promoting autonomy. Optimal timing remains uncertain, differing between guidelines and patient preferences. Conditions often requiring transition include spina bifida, bladder exstrophy, posterior urethral valves, and congenital anomalies of the kidney and urinary tract. Major challenges are loss to follow-up, parental dependence, patient anxiety, an

Transitional Care for Adult Patients with Congenital Heart Disease.

The prognosis of congenital heart disease (CHD) has improved, and most patients now reach adulthood. Owing to residual disease and comorbidities, it is recommended that adult CHD (ACHD) patients transition to adult care for lifelong monitoring and treatment. However, this transition period can be challenging for CHD patients owing to obstacles such as independence from their parents and establishing self-management. To achieve a seamless shift from pediatric to adult care and ensure continuity, it is important to educate and motivate patients appropriately, and an established transition system

Improving Patient-Centered Transitional Care after Complex Abdominal Surgery.

BACKGROUND: Poor-quality transitions of care from hospital to home contribute to high rates of readmission after complex abdominal surgery. The Coordinated Transitional Care (C-TraC) program improved readmission rates in medical patients, but evidence-based surgical transitional care protocols are lacking. This pilot study evaluated the feasibility and preliminary effectiveness of an adapted surgical C-TraC protocol. STUDY DESIGN: The intervention includes in-person enrollment of patients. Follow-up protocolized phone calls by specially trained surgical C-TraC nurses addressed medication mana

Transitional care into adulthood: Management of adolescent idiopathic scoliosis.

Adolescent idiopathic scoliosis (AIS) represents a frequently encountered pathology in pediatric neurosurgical clinics that involve complex surgical counselling and intervention into adulthood. Transition of care strategies to monitor these patients over time and determine optimal care frameworks for surgical, medical, and psychosocial management, however, has not been uniformly integrated into adult healthcare systems. Further, limited studies discuss models of transitional care for pediatric spinal pathologies that discuss management trees for postoperative pediatric patients or those newly

Progressive mobility as a team effort in transitional care.

With changing health care, progressive care nurses are working in diverse practice settings to meet patient care needs. Progressive care is practiced along the continuum from the intensive care unit to home. The benefits of early progressive mobility are examined with a focus on the interdisciplinary collaboration for care in a transitional care program of a skilled nursing facility. The program's goals are improved functional status, self-care management, and home discharge with reduced risk for hospital readmission. The core culture of the program is interdisciplinary collaboration and team
