[Histomorphology after transmyocardial laser revascularization].

From 11/1994 to 4/1997 we enrolled 140 patients with diffuse CAD refractory to maximum antianginal therapy who are not candidates for PTCA or CABG for transmyocardial laser revascularisation (TMLR). Of these patients aged 63.5 +/- 15 years, 98 had coronary 3-vessel disease, and the average left ventricular ejection fraction was 44%. Eleven out of these 140 patients died from different reasons (pneumonia, myocardial infarction, septicemia). Seven patients who died between the 1st and 20th postoperative day underwent a postmortem examination with histological analysis of the areas treated by TML

A review of the literature: transmyocardial laser revascularization.

BACKGROUND AND OBJECTIVE: Transmyocardial laser revascularization is an investigational procedure that improves chronic angina that is not amenable to other forms of therapy. The theorized mechanism is an increased supply of oxygenated blood to the myocardium via the creation of left ventricular transmural channels. The objective of this review is to facilitate an understanding of the current work published in the literature on the subject of transmyocardial laser revascularization. METHODS: Using Melvyl Medline, all pertinent literature associated with transmyocardial laser revascularization

Transmyocardial laser revascularization: is the enthusiasm justified?

BACKGROUND: Transmyocardial laser revascularization (TMLR) is advocated to offer relief of incapacitating angina for patients whose coronary vessels are poor targets for coronary artery bypass graft surgery (CABG) or balloon angioplasty and stenting. In spite of significant mortality and morbidity, the preliminary reports from centers performing the procedure were quite enthusiastic for a period of about 1 year following the procedure. HYPOTHESIS: The study aimed to determine mortality, morbidity, and long-term results of TMLR. METHODS: The study included 19 individuals with incapacitating a

[Transmyocardial laser revascularization--an innovative pathophysiologic concept].

In patients with coronary artery disease where standard revascularization procedures are not appropriate, transmyocardial laser revascularization (TMLR) represents an innovative technique which is currently validated worldwide. Initially, it has been assumed that myocardial perfusion of ischemic regions could be instantly improved by inducing TMLR channels, which, however, might not be confirmed in ongoing studies. Indeed, the gain in O2 diffusion surface obtained by 20 patent TMLR channels is only 6 cm2 which accounts for just 0.01% of the total capillary surface (47000 cm2) of the myocardium

Transmyocardial laser revascularization causes sustained VEGF secretion.

Transmyocardial laser revascularization (TMR) using a carbon dioxide (CO(2)) laser has been shown to relieve angina, increase vascular density, and improve myocardial contraction. A study of 28 patients receiving TMR was conducted to monitor vascular endothelial growth factor (VEGF) levels with the goal of clarifying the relationship between TMR, the amelioration of angina, and vascular density. Serum VEGF levels were measured during four periods (preoperative, postoperative, convalescence, and late) in these 28 patients who received sole therapy TMR for un-revascularizable ischemic angina and

Transmyocardial laser revascularization: historical background and future directions.

Transmyocardial laser revascularization (TMLR) was investigated as a possible treatment alternative for patients with refractory coronary artery disease. This paper is a summary of nearly four decades of research by the authors. Beginning in 1969 experimental studies were conducted on the beating heart. A prototype 450 W carbon dioxide laser was used to create channels in the ischemic myocardium. Initial clinical studies began in 1984. A protocol was developed for TMLR as an adjunct to CABG in those patient who had at least one vessel which could be bypassed and areas of ischemia which were no

Transmyocardial laser revascularization versus medical therapy for refractory angina.

BACKGROUND: This is an update of a review previously published in 2009. Chronic angina and advanced forms of coronary disease are increasingly more frequent. In spite of the improvement in the efficacy of available revascularization treatments, a subgroup of patients continue suffering from refractory angina. Transmyocardial laser revascularization (TMLR) has been proposed to improve the clinical situation of these patients. OBJECTIVES: To assess the effects (both benefits and harms) of TMLR versus optimal medical treatment in people with refractory angina who are not candidates for percutane

Treatment of refractory angina pectoris by transmyocardial laser revascularization.

Mounting evidence that Transmyocardial Laser Revascularization (TMR) is a safe and effective treatment for angina arrives as an increasing number of patients having angioplasties and coronary artery bypass grafts experience failure with time. Like angioplasty and coronary bypass, TMR is in fact a palliative rather than a cure for atherosclerotic heart disease.

[Transmyocardial laser revascularization in stable and unstable angina pectoris].

Endstage coronary artery disease still remains a therapeutic challenge. An increasing number of patients is no longer amenable for direct revascularization by PTCA or coronary bypass surgery and does also no longer respond to maximum medical therapy. This fact has directed the interest again towards surgical techniques of indirect revascularization, which had been introduced by Beck and other surgeons more than 60 years ago. Among these attempts we can also find transmyocardial needle punctures, firstly performed by Sen in Bombay. In the early eighties it was Mirhoseini, who used a laser for c

Quality of life and survival after transmyocardial laser revascularization with the holmium:YAG laser.

BACKGROUND: The purpose of this investigation was to assess postoperative survival and quality of life with transmyocardial laser revascularization (TMR) in high-risk patients. METHODS: During a 24-month period, 81 consecutive patients underwent either sole therapy TMR (n = 34) or TMR with coronary artery bypass grafting (n = 47) using a holmium:yttrium-aluminum-garnet (YAG) laser. Outcomes were assessed in three high-risk groups, including patients with left ventricular dysfunction (ejection fraction < or = 0.40) (n = 37), unstable angina (n = 30), and congestive heart failure (n = 33). Sustained angina relief 5 years after transmyocardial laser revascularization with a CO(2) laser.

BACKGROUND: Although transmyocardial laser revascularization (TMR) has provided symptomatic relief of angina over the short term, the long-term efficacy of the procedure is unknown. Angina symptoms as assessed independently by angina class and the Seattle Angina Questionnaire (SAQ) were prospectively collected up to 7 years after TMR. METHODS: Seventy-eight patients with severe angina not amenable to conventional revascularization were treated with a CO(2) laser. Their mean age was 61+/-10 years at the time of treatment. Preoperatively, 66% had unstable angina, 73% had had >/=1 myocardial inf

Alternative Therapy for Medically Refractory Angina: Enhanced External Counterpulsation and Transmyocardial Laser Revascularization.

Medically refractory angina pectoris (RAP) is defined by presence of severe angina with objective evidence of ischemia and failure to relieve symptoms with coronary revascularization. Medication and invasive revascularization are the most common approaches for treating coronary artery disease (CAD). Although symptoms are eliminated or alleviated by these invasive approaches, the disease and its causes are present after treatment. New treatment approaches are needed to prevent the disease from progressing and symptoms from recurring. External enhanced counterpulsation therapy provides a treatme

Continued symptomatic improvement three to five years after transmyocardial revascularization with CO(2) laser: a late clinical follow-up of the Norwegian Randomized trial with transmyocardial revascularization.

OBJECTIVES: The goals of this study were to assess late clinical outcome and left ventricular ejection fraction (LVEF) after transmyocardial revascularization with CO(2) laser (TMR). BACKGROUND: During the 1990s TMR emerged as a treatment option for patients with refractory angina not eligible for conventional revascularization. Few reports exist on clinical effects and LVEF >3 years after TMR. METHODS: One hundred patients with refractory angina not eligible for conventional revascularization were block-randomized 1:1 to receive continued medical treatment or medical treatment combined with

Acute changes in functional and metabolic parameters following transmyocardial laser revascularization: an experimental study.

Today there is no doubt that angina in patients with diffuse end-stage coronary artery disease can be treated effectively by transmyocardial laser revascularization (TMLR). But until now, the underlying mechanism of TMLR is a matter of debate. The aim of this study was to investigate the immediate changes in functional and metabolic parameters after the creation of TMLR channels. In 12 pigs the local myocardial function (pressure-wall thickness-loops), tissue oxygen partial pressure (PO(2)), and the left ventricular intramyocardial wall pressure (IMP) was investigated simultaneously. The left

An examination of potential mechanisms underlying transmyocardial laser revascularization induced increases in myocardial blood flow.

Within the past few years, transmyocardial laser revascularization (TMR) has attracted the attention of cardiologists and cardiac surgeons as a therapy for patients suffering from otherwise treatable coronary artery disease. Clinical studies have consistently shown symptomatic improvement that lasts at least 1 year in a majority of patients. The original hypothesis that prompted development of the technique was that direct myocardial perfusion from the chamber could be achieved through chronically patent channels, as is the case in reptilian hearts. Results of our early studies failed to suppo

[Surgical alternatives to direct repeat revascularization of coronary arteries--common mechanisms for reducing ventricular volume, laser transmyocardial revascularization and cardiomyoplasty].

There are a number of surgical alternatives to repeat artery bypass grafting, which is the primary treatment for recurrent severe myocardial ischemia. In patients with endstage coronary artery disease unsuitable for repeat bypass procedures, orthotopic heart transplantation is now well established. However, the increasing donor shortage limits this option to relatively few patients, a fact well documented by longer waiting lists despite less stringent donor criteria. Hence, other surgical therapies, which may at this time be underused, should be explored. In addition to mechanical circulatory

Impact of unstable angina on outcomes of transmyocardial laser revascularization combined with coronary artery bypass grafting.

BACKGROUND: For sole therapy transmyocardial laser revascularization (TMR), unstable angina has been demonstrated to be a significant independent predictor of operative mortality. The objective of this study was to investigate the preoperative risk profile of patients undergoing TMR plus coronary artery bypass graft surgery (CABG) and to determine the impact of unstable angina on outcomes. METHODS: Using The Society of Thoracic Surgeons National Cardiac Database from 1998 to 2003, 5,618 patients underwent TMR plus CABG. These patients were compared with 932,715 patients who underwent CABG onl
