Plasma exchange conditioning for ABO-incompatible renal transplantation.

The supply of deceased donor kidneys is inadequate to meet demand. To expand the pool of potential donors, ABO-incompatible transplants from living donors have been performed. We present the Mayo Clinic experience with such transplants. Enrollment was open to patients when the only available potential living kidney donor was ABO-incompatible. Conditioning consisted of plasma exchanges followed by intravenous immunoglobulin. Splenectomy was performed at the time of transplant surgery. Post-transplant immunosuppression consisted of anti-T lymphocyte antibody, tacrolimus, mycophenolate mofetil, a

Improved conditioning regimens for autologous transplantation using targeted radiotherapy.

While modern high efficacy disease modifying therapies have revolutionized the treatment of relapsing-remitting multiple sclerosis, they are less effective at controlling progressive forms of the disease. Meningeal inflammation is a recognized risk factor for cortical gray matter pathology which can result in disabling symptoms such as cognitive impairment and depression, but the mechanisms linking meningeal inflammation and gray matter pathology remain unclear. Here, we performed magnetic resonance imaging (MRI)-guided spatial transcriptomics in a mouse model of autoimmune meningeal inflammat

Microbiota injury during conditioning for allogeneic hematopoietic stem cell transplantation.

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a potentially curative treatment for hematological malignancies, but it is associated with substantial gut microbial dysbiosis linked to graft-versus-host disease, infections, and transplant-related mortality. The conditioning regimen represents the first major iatrogenic insult delivered within the transplant pathway, but its specific contribution to microbiota injury, independent of subsequent aplasia, antibiotic exposure, and mucositis, remains incompletely characterized. We conducted a prospective, single-center, observation

Reduced-intensity conditioning for allogeneic transplantation in classical Hodgkin lymphoma.

INTRODUCTION: Hodgkin lymphoma (HL) is a curable disease; however, 10-20% of patients experience relapsed/refractory disease. While autologous hematopoietic cell transplantation (auto-HCT) remains standard, a substantial proportion relapse, necessitating alternative strategies. Allogeneic HCT (allo-HCT) remains a potentially curative option and here we emphasize the role of reduced-intensity conditioning (RIC) in HL. AREAS COVERED: In this review, we compare the feasibility, efficacy, and safety of myeloablative conditioning (MAC) and RIC allo-HCT in relapsed/refractory HL. Additionally, we d

Host conditioning and rejection monitoring in hepatocyte transplantation in humans.

BACKGROUND & AIMS: Hepatocyte transplantation partially corrects genetic disorders and has been associated anecdotally with reversal of acute liver failure. Monitoring for graft function and rejection has been difficult, and has contributed to limited graft survival. Here we aimed to use preparative liver-directed radiation therapy, and continuous monitoring for possible rejection in an attempt to overcome these limitations. METHODS: Preparative hepatic irradiation was examined in non-human primates as a strategy to improve engraftment of donor hepatocytes, and was then applied in human subje

Allogeneic hematopoietic cell transplantation following conditioning with 90Y-ibritumomab-tiuxetan.

Radioimmunotherapy (RIT) of relapsed lymphoma is gaining increasing importance. Especially the commercially available anti-CD20 antibody 90Y-ibritumomab tiuxetan is currently under investigation in various trials including dose escalation and autologous hematopoietic progenitor cell support. It is not clear, however, whether the implementation of this radiolabeled antibody into another treatment option for relapsed or poor risk lymphoma patients-allogeneic hematopoietic cell transplantation-interferes with or delays successful engraftment. This study reports encouraging results with 2 relapsed

Stem cell transplantation for congenital immunodeficiencies using reduced-intensity conditioning.

The optimal preparation for stem cell transplantation (SCT) in children with congenital immunodeficiencies is currently unknown. In all, 81 children with immunodeficiency underwent 82 SCTs using reduced-intensity conditioning (RIC). The incidence of significant GVHD was low; viral reactivation was prominent with an unexpected increase in EBV reactivation; immune reconstitution was similar between different donor groups and comparable to conventional SCT. Overall, 68/81 (84%) survive with no significant difference between donor types or between severe combined immunodeficiency (SCID) and non-SC

Umbilical Cord Blood Transplantation in Children with Acute Leukemia: Impact of Conditioning on Transplantation Outcomes.

The Blood and Marrow Transplant Clinical Trials Network (BMT CTN 0501) randomized children with hematologic malignancies to transplantation with 1 or 2 cord blood units (UCB) between 2006 and 2012. While the trial concluded that survival was similar regardless of number of units infused, survival was better than previously reported. This prompted a comparison of survival of trial versus nontrial patients to determine the generalizability of trial results and whether survival was better because of the trial treatment regimen. During the trial period, 396 recipients of a single UCB unit met tria

Clinical Relevance of Transplant Conditioning Intensity Score-Based Refinement of Conditioning Intensity in Cord Blood Transplantation for Myelodysplastic Syndrome.

Cord blood transplantation (CBT) is an alternative donor strategy for patients with myelodysplastic syndrome (MDS). To clarify the impact of the transplant conditioning intensity (TCI) score, we examined 1127 patients with de novo MDS who underwent CBT between 2008 and 2022, using a nationwide registry database. Patients were stratified into three age groups: <50 years (Younger group, n = 256), 50 to 64 years (Middle group, n = 545), and 65 to 74 years (Older group, n = 326). In the Older group, the multivariate analysis revealed that TCI score of 4.5 to 5.0 correlated with higher overall mort

Influence of conditioning on the outcome of allogeneic bone marrow transplantation.

Allogeneic bone marrow transplantation is widely used for the treatment of various hematologic disorders. The results are quite reproducible from center to center with a mean disease-free survival of 50%, which varies from 10% in patients transplanted in relapse to 70% in young patients transplanted in first complete remission or in the chronic phase of chronic myeloid leukemia. Relapse is one of the main complications, and its frequency increases with disease status and the use of T cell depletion and the subsequent loss of the graft versus leukemia effect of transplanted allogeneic cells. Subsequent Cancers following Non-myeloablative Conditioning for Allogeneic Hematopoietic Cell Transplantation.

We examined the risk of subsequent malignant neoplasms (SMNs) in 1720 patients with hematologic cancers given allogeneic hematopoietic grafts from 03/1998 to 08/2023 after nonmyeloablative conditioning regimens. With a median follow-up of 12 years, the cumulative incidence of SMNs was 17% (95% CI, [15%, 19%]). Most SMNs (n = 543) were non-melanoma skin cancers seen in 208 patients; unfortunately, information on these cancers was not available in the Surveillance, Epidemiology, and End Results (SEER) database for comparison with such tumors in the general population. However, developing non-mel

Successful hematopoietic stem cell transplantation for osteopetrosis using reduced intensity conditioning.

BACKGROUND: Infantile malignant osteopetrosis (IMO) is an autosomal recessive condition characterized by defective osteoclast activity, with hematopoietic bone marrow transplant being the only available cure. Over the past several years, new conditioning regimes and donor options have emerged, thus extending the possibility of cure to a greater number of patients and improving the outcomes of bone marrow transplant. Here we detail the outcomes of bone marrow transplant in a cohort of 31 patients treated with a combination of fludarabine, treosulphan, thiotepa, and antithymocyte globulin. PROC

Reduced-intensity conditioning umbilical cord blood transplantation in Nijmegen breakage syndrome.

NBS is a rare autosomal recessive congenital disorder associated with chromosome instability caused by a mutation in the NBN gene (8q21). Clinical manifestations include microcephaly, growth retardation, combined immunodeficiency, and a strong predisposition to develop (mainly lymphatic) malignancies. There is no specific treatment for patients with NBS, and the prognosis is generally poor. The therapeutic option for some patients with NBS may be HSCT. We present a case of safe and successful non-myeloablative UCB transplantation in the 19th month of the life of a female child with NBS concomi

High-dose treosulfan in conditioning prior to hematopoietic stem cell transplantation.

IMPORTANCE OF THE FIELD: Despite the marked development in the field of preparative regimens prior to hematopoietic stem cell transplantation (HSCT) over the last decade, the search for a superior conditioning agent is still continuing. In view of the literature reports, treosulfan (TREO), a structural analog of busulfan (BU), appears to be a promising candidate in terms of myeloablative, immunosuppressive and antimalignancy effects as well as low organ toxicity. AREAS COVERED IN THIS REVIEW: The article focuses on pharmacological activity, pharmacokinetics and toxicity of TREO. Compressed de
