Trapezoid bone fracture.

Fractures of the carpal bones involve only a single bone or complex bones with or without ligament rupture. However, fractures of the trapezoid are rarely seen. Because the trapezoid is fastened to the trapezium, capitate, and scaphoid by strong ligaments, fracture or dislocation is limited by this rigid fixation. The authors present a single bone fracture of the trapezoid in a 40-year-old man. A tomogram of the carpal bone was useful in diagnosing the trapezoid fracture. The mechanism for development of fracture of the trapezoid alone is unknown. However, fracture of the trapezoid seemed to o

Avascular necrosis of the trapezoid bone following carpometacarpal arthroplasty.

A 58-year-old female developed avascular necrosis of her trapezoid approximately 3 months after undergoing carpometacarpal arthroplasty. The patient was treated conservatively with immobilization and had complete resolution of her clinical symptoms during her year of follow-up. Additionally, radiographic examination showed complete restoration of the height of her trapezoid approximately 1 year after the index procedure. Avascular necrosis of the trapezoid is extremely rare with very few cases described in the literature. This is the first description of avascular necrosis following carpometac

Solid Variant Aneurysmal Bone Cyst of the Trapezoid Bone: A Case Report.

An aneurysmal bone cyst (ABC) is classified as a benign osteoclastic giant cell-rich tumour. We report a case of a 48-year-old male patient with a solid variant ABC involving the trapezoid bone. We chose to fill the defect with bone cement after curettage. At the 12-month follow-up, the patient remained recurrence-free with satisfactory hand function. Carpal bone tumours are rare and typically benign, with malignant and metastatic lesions being extremely uncommon. We were unable to find any other reports of a solid variant ABC in the trapezoid bone. This case highlights the importance of histo

[Dislocation due to injury with trajectory in the scaphoid-trapezoid-trapezoid periscaphoid-perisemilunar bones. Case report and literature review].

INTRODUCTION: dislocations of carpal bones without associated fractures are considered a rare injury, the most common mechanism of injury being axial loading with wrist in extension plus ulnar deviation. The literature reports a wide variety of complex carpal injuries, even so, it is possible to identify previously undescribed injuries. OBJECTIVE: to present an atypical case of a patient with an injury to the midcarpal stabilizing mechanism and the stabilizing mechanism of the proximal row of the wrist following trauma to the hand that required carpectomy as definitive surgical treatment. Trapezoid rotational bone graft osteotomy for metacarpal and phalangeal fracture malunion.

Metacarpal and phalangeal fracture malunions with significant angulation deformity are associated with bone shortening, prominence of the metacarpal head in the palm or pseudoclaw deformity and may be symptomatic. If so, they may need corrective osteotomy procedures. Conventional methods of closing, or opening, wedge osteotomy do not restore the length of the bone exactly. Simultaneous correction of the angular deformity and restoration of bone length can be addressed by a trapezoid rotational bone graft osteotomy. A double osteotomy is done and the segment of bone is rotated and re-inserted a

[The combined luxation of the trapezoid and 2nd metacarpal bones. Apropos of a case].

The authors report a case of posterior dislocation of the trapezoid bone together with the second metacarpal. Among the 25 cases of trapezoid dislocations reported in the literature, no similar case has been found. The patient was seen 4 months after the injury and there was also a scaphoid instability. Open reduction and a scapho-trapezio-trapezoid arthrodesis were performed. The diagnosis, the mechanism and the treatment of this very unusual injury are discussed.

[A case of open carpo-metacarpal dislocation complicated by dislocation of the trapezoid bone].

A case of open carpo-metacarpal dislocation complicated by dislocation of the os trapesium is reported in this paper. The patient's wrist healed with good functional results, despite a partial mistake in diagnosis. The authors draw attention to the fact, that X-ray films taken at admission must be analysed carefully and intraoperative X-rays films must be taken. By demonstration their own mistake in diagnosis and treatment, the authors hope that a similar mistake will be avoided. Following early open or closed reduction, a temporary wire fixation of the metacarpal bones to the carpal bones is

Intra-Articular Osteoid Osteoma of the Trapezoid Bone: A Rare Presentation Mimicking Wrist Synovitis.

Osteoid osteomas predominantly occur in the cortices of long bones, with the femur and tibia being the most commonly affected sites. However, they can occasionally present in atypical locations, such as the carpus, which can lead to diagnostic confusion with other conditions. This case report details an intraarticular osteoid osteoma in the trapezoid bone. Initial evaluations, including standard radiographs, joint ultrasound, and wrist MRI performed twice, initially pointed toward a diagnosis of wrist synovitis. This case underscores the diagnostic challenges posed by atypical presentations of

A comparison of scaphoid-trapezium-trapezoid fusion and four-bone tendon weave for scapholunate dissociation.

The contact characteristics of the radiocarpal joint (area, pressure, and ratios of scapholunate area, pressure and force transmitted) were determined with the use of pressure-sensitive film in seven normal wrists after ligament sectioning to create scapholunate instability, with reconstruction involving a scaphoid-trapezium-trapezoid fusion, and with reconstruction involving a four-bone tendon weave procedure. The results demonstrated that the scaphoid-trapezium-trapezoid fusion significantly altered the contact characteristics of the wrist, transferring load, pressure, and area to the radios

[Mid-term results of scapho-trapezio-trapezoid arthrodesis in aseptic semilunar bone necrosis stage IIIa/b].

The mid-term results of 20 cases of Kienböck's disease treated with scaphotrapezio-trapezoid arthrodesis with an average follow-up period of 25 months between 1993 and 1997 are presented. Clinical results were good with regard to pain, grip strength and range of motion; the results with regard to the DASH questionnaire were very good. It can be concluded from this study that STT arthrodesis is a useful procedure for the treatment of Kienböck's disease in stage III.

Treatment of scaphoid nonunion by radical curettage, trapezoidal iliac crest bone graft, and internal fixation with a Herbert screw.

Ten adult male patients with scaphoid nonunions were treated by radical curettage, trapezoidal iliac crest bone grafting, and internal fixation with a Herbert screw. The mean patient age was 24.7 years, and the mean duration of the nonunion before surgery was 37.3 months. Mean follow-up time was 30.4 months. Nine of the ten nonunions healed, although one patient required a second bone-grafting procedure. The mean postoperative grip strength was 45.0 kg, and the mean postoperative pinch strength was 11.5 kg. The mean postoperative range of motion was volar flexion, 76.1 degrees; dorsiflexion, 7

A rare combined injury of dorsal fracture-dislocation of four carpometacarpal joints and trapezium, trapezoid and distal radius bone fractures.

Incorrect or delayed diagnosis and treatment of the carpometacarpal fracture-dislocations is often associated with poor prognosis. We present a rare case of unusual pattern of injury, involving dorsal dislocation of four ulnar carpometacarpal joints, associated with fracture of the trapezium, a burst fracture of the trapezoid  bone and an extra-articular fracture of the third distal  of the radius. The first surgical intervention was followed by unsatisfactory results, confirmed by the CT scans. A second surgery followed and an open reduction and pinning with K wires performed. Post-operative

Six new examples of the bipartite trapezoid bone: morphology, significant population variation, and an examination of pre-existing criteria to identify bipartition of individual carpal bones.

Carpal bone bipartition is a developmental variant resulting in the division of a normally singular carpal into two distinct segments. Cases involving the scaphoid are best known, though many other carpals can be affected, including the trapezoid. Six new examples of bipartite trapezoids, identified in African and Asian anatomical and archeological samples, are reported here and compared with the eight previously known. While the site of bipartition is consistent, the resulting segments exhibit variability in their articulations with neighboring carpals. Five of the six affected trapezoids wer

Limb salvage for malignant bone tumours of distal tibia with dual ipsilateral vascularized autogenous fibular graft in a trapezoid-shaped array with ankle arthrodesis and preserving subtalar joint.

BACKGROUND: The treatment of malignant tumours of the distal tibia is a challenging surgical problem due to the scarce soft tissue coverage and the instability of the ankle joint that often occurs after resection. However, there is no consensus on the ideal treatment for malignant tumours of the distal tibia. METHODS: We report a new reconstruction for five patients with high-grade osteosarcoma of distal tibia, using dual ipsilateral vascularized autogenous fibular graft in a trapezoid-shaped array and external fixator, with ankle arthrodesis and preserving subtalar joints. The patients were

Isolated Trapezoid Fracture in a Boxer.

BACKGROUND Trapezoid fractures are very uncommon, accounting for less than 1% of all wrist fractures. Isolated fractures of this bone are even more rare, with just a few reports in the literature. The trapezoid bone has a very stable position within the wrist, forming a relatively immobile joint with the second metacarpal base distally. It is also connected by very strong ligaments to the trapezium, capitate and, scaphoid. Trapezoid fractures occur when a strong bending or axial force is applied to the second metacarpal base. CASE REPORT We present a clinical case of an isolated, non-displaced

Isolated trapezoid fractures: a case report with compilation of the literature.

Isolated fractures of the trapezoid bone have been rarely reported in the literature, the mechanism of injury being an axial or bending load transmitted through the second metacarpal. We report a case of an isolated, nondisplaced trapezoid fracture that was sustained by direct trauma and subsequently treated successfully in a short-arm cast. Diagnostic and treatment strategies for isolated fractures of the trapezoid bone are reviewed as well as the results of operative and nonoperative treatment.

Isolated Trapezoid Nonunion: Case Report of a Rare Injury and Review of Literature.

Trapezoid fractures are rare injuries with few descriptions of surgical treatment in the literature. We present a case of trapezoid nonunion in a 23-year-old male who injured his hand at work. Initial radiographs did not demonstrate a fracture. However, a bone scan and computed tomography (CT) scan revealed a nonunion 6 months after the initial injury. The patient had continued pain and therefore open reduction and internal fixation with distal radius bone grafting was performed. Postoperatively, he underwent immobilization with a short arm cast and used a bone stimulator. After 6 months of su
