BACKGROUND: The goal of this study was to examine the association between patient complications and admission to Level I trauma centers (TC) compared with nontrauma centers (NTC).
STUDY DESIGN: This was a retrospective cohort study of data derived from the National Study on the Costs and Outcomes of Trauma (NSCOT). Patients were recruited from 18 Level I TCs and 51 NTCs in 15 regions encompassing 14 states. Trained study nurses, using standardized forms, abstracted the medical records of the patients. The overall number of complications per patient was identified, as was the presence or absen

American journal of transplantation : official journal of the American Society of Transplantation and the American Society of Transplant Surgeons2025eng
Transplant centers hold the key to accelerating organ use and transplanting more patients more efficiently. However, not all transplant procedures are alike, as organ complexity and patient comorbidities can dramatically increase the risk of the procedures, demanding unique skills and more extensive resources to achieve good outcomes. What if transplant centers could emulate aspects of the trauma center model to ensure that necessary capabilities are consistently applied and uniformly available for all levels of organ risk and patient health? The proposed model recognizes that some transplant

INTRODUCTION: Residency training allows medical graduates to obtain the skills necessary to become safe and effective physicians. Analyzing data from two periods, before residents (BRs) and during residents (DRs), at our institution allowed us to examine the association between resident involvement and patient outcomes. We hypothesized that including residents in trauma resuscitation activities would not be associated with increased detrimental patient outcomes.
MATERIALS AND METHODS: We conducted a single-center retrospective review (January 1, 2018, to December 31, 2020) of patients that ar

INTRODUCTION: The practices of venous thromboembolism (VTE) prophylaxis are not standardized in pediatric trauma patients. We aimed to compare the rates of VTE prophylaxis and thromboembolic events in pediatric patients admitted to American College of Surgeons (ACS)-verified adult trauma centers (ATCs, nonpediatric verified) versus verified pediatric trauma centers (PTCs).
METHODS: We conducted a retrospective analysis of the ACS-Trauma Quality Improvement Program (2017-2020) including all pediatric (age <15 y) trauma patients with severe injuries (injury severity score ≥16). We excluded pati

In 1966, the National Academy of Sciences and National Research Council published 'Accidental Death and Disability: the Neglected Disease of Modern Society' which served as a national call to action to address the apparent public apathy towards the devastating and unnecessary toll that injury was taking on America. This white paper recommended the establishment of a National Trauma Association to drive public demand for injury prevention and mitigation. The American Association for the Surgery of Trauma heeding that call, founded the American Trauma Society (ATS) in 1968. Since its founding an

Severe trauma has the characteristics of complicated condition, multiple organs involved, limited auxiliary examinations, and difficulty in treatment. Most of the trauma patients were sent to primary hospitals to receive treatments. But the traditional mode of separate discipline management can easily lead to delayed treatment, missed or wrong diagnosis and high disability, which causes a high mortality in severe trauma patients. Therefore, if the primary hospitals, especially county-level hospitals (usually the top general hospital within the administrative region of a county), can establish

BACKGROUND: The development of a tiered trauma care system has lead to improved survival for the critically injured. The question as to whether the increased survival associated with the establishment of tiered levels of trauma care is paralleled by an improved functional outcome has not, however, been addressed.
METHODS: Multivariate logistic regression analysis of the National Trauma Data Bank from 1994 to 2001 was performed with functional independence measure (FIM) as the primary outcome. Trauma centers were dichotomized as Level II or above versus Level III or below. Blunt and penetratin

BACKGROUND: In rural state trauma systems, management of the obstetrical trauma patient often defaults to transfer to level I trauma centers. We evaluate the necessity of transferring obstetrical trauma patients without severe maternal injury.
MATERIALS AND METHODS: A retrospective 5-year review of obstetrical trauma patients admitted to a rural state-level I trauma center was conducted. Injury severity measures such as abdominal AIS, ISS, and GCS were correlated with outcomes. Furthermore, the impact of maternal and gestational age on uterine compromise, uterine irritability, and the need fo

BACKGROUND: The American College of Surgeons Committee on Trauma recommends universal alcohol screening be part of the evaluation of admitted trauma patients. Yet, suboptimal screening rates have been reported for admitted adult and adolescent trauma patients. This lack of screening, in turn, has limited the ability of trauma services to provide patients with brief interventions during their hospital admission and subsequent referrals to treatment after discharge. The primary aim of this study was to examine current rates of alcohol and other drug screening with admitted injured adolescents ac

BACKGROUND: Injury is the leading cause of hospitalization in children, and as many as 5% of hospitalized injured children require mechanical ventilation. Despite this, little is known about the complications associated with mechanical ventilation, including pneumonia.
METHODS: This is a retrospective analysis of trauma patients younger than 19 years from the National Trauma Data Bank from 2009 to 2011. Descriptive statistics were used to examine the patient population. Baseline characteristics were compared between subgroups using t tests and χ tests. Generalized linear models were used to i

PURPOSE: A large portion of patients admitted to trauma centers present with isolated or concomitant facial injuries. Multiple surgical specialties including oral and maxillofacial, plastic, and otolaryngology/head and neck surgeons are trained and involved in the management of oral and maxillofacial trauma. The purpose of this study is to evaluate the current distribution of different specialties that cover facial trauma among the leading trauma centers in the United States.

BACKGROUND: Injured children undergo radiologic studies as part of trauma evaluations. Children are more sensitive than adult patients to the effects of ionizing radiation. Few studies have described the radiation exposure to pediatric patients during trauma evaluations. We sought to describe the rate of use of radiology studies and to estimate the effective dose of radiation delivered to pediatric trauma patients presenting to trauma centers within the United States.
METHODS: We performed an analysis of pediatric patients younger than 19 years who presented to an American College of Surgery-

BACKGROUND: Traumatic breast injuries that require surgical intervention are rare and incompletely studied. The study objective was to define the incidence, mechanism/burden of injury, interventions, and outcomes after breast injuries requiring surgery nationally.
METHODS: All patients with breast trauma necessitating surgery were identified from the National Trauma Data Bank (NTDB) (2006-2017) using ICD-9 and -10 codes, without exclusions. Demographics, injury mechanism/severity, procedures, and outcomes (mortality, hospital length of stay [LOS, days], ICU LOS, and AIS >1 in >1 body regions,

IMPORTANCE: As trauma care-related demand continues to rise, the US trauma system's current utilization and ability to accommodate surges from mass casualty events or disasters remain uncertain. Understanding existing trauma bed occupancy and reserve capacity is essential for national preparedness.
OBJECTIVE: To assess the current occupancy and distribution of adult trauma-designated beds across US hospitals and evaluate the system's ability to absorb a sudden and sustained surge in trauma volume.
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study analyzed 121 weeks (January 2022-

BACKGROUND: A trauma system based on trauma centers was established in Poland only at the beginning of the second decade of the century. The aim of this study was to quantitatively and comparatively assess the functioning of the trauma system in Poland during the postpandemic period, with a particular emphasis on adult trauma center (TC) performance indicators.
METHODS: Since there is no national registry of multiple trauma in Poland, the analysis is based on official public-use datasets. Among the indicators related to trauma centers, the following are available: number of EMT missions to tr

OBJECTIVES: Injuries are the leading cause of morbidity and mortality in children ages 1 to 18 years. There are limited studies about pediatric thoracolumbar (TL) spinal injuries; the purpose of this study was to characterize TL spinal injuries among pediatric patients evaluated in US trauma centers.
METHODS: This was a retrospective cohort study of the National Trauma Data Bank. Patients aged 1 to 18 years with a thoracic or lumbar spinal injury sustained by blunt trauma during calendar years 2011 through 2016 were included. Cervical spinal injuries, death before arrival, or penetrating trau

PURPOSE: The utility of chest computed tomography (CCT) is not well established in the diagnostic algorithm of adolescent blunt trauma patients. Our study's aim was to review CCT usage in the initial evaluation of adolescent blunt trauma.
METHODS: We retrospectively reviewed adolescent blunt trauma patients treated at our urban level 1 adult trauma center from 2015 to 2019. Our primary outcome was the rate of positive CCT findings. Univariate and multivariate logistic regression analyses were performed.
RESULTS: There were 288 patients that met our inclusion criteria and 153 positive CCT and

BACKGROUND: Pediatric lower extremity vascular injury (PLEVI) is uncommon and the availability of granular data is sparse. This study evaluated the surgical management of PLEVIs between a Level I adult (ATC) vs pediatric (PTC) trauma center.
METHODS: We performed a retrospective review of PLEVIs (< 18 years) managed surgically between 01/2009-12/2022. Demographics and outcome data were obtained. Primary outcomes included amputation and fasciotomy rates. Secondary outcomes included type of vessel repair, mortality, and hospital length of stay.
RESULTS: Seventy-nine patients were identified, 4
