Assessing treatment effect heterogeneity (TEH) in clinical trials is crucial, as it provides insights into the variability of treatment responses among patients, influencing key decisions related to drug development. Furthermore, it can lead to personalized medicine by tailoring treatments to individual patient characteristics. This paper introduces novel methodologies for assessing treatment effects using the individualized treatment effect as a basis. To estimate this effect, we use a doubly robust (DR) learner to infer a pseudo-outcome that reflects the causal contrast. This pseudo-outcome

Replication studies are crucial for evaluating treatment efficacy and exploring the conditions that moderate treatment effects. Understanding this treatment effect heterogeneity across studies is essential for building robust theories. However, identifying the causes of this heterogeneity is challenging, as multiple study characteristics often vary simultaneously, even in replication studies in which many aspects are deliberately held constant across studies. Traditional metaregression typically lacks a clear causal framework, obscuring the causal interpretation of moderating effects. Although

The recent meta-analysis by Matsushita et al on "Heterogeneity in treatment response for patent ductus arteriosus" confirms prior results showing that medical treatment of PDA does not reduce but increases mortality. The discussion implies that there is heterogeneity in effects of treatment on mortality, but this is not consistent with the data. The implication of their analysis is that early medical treatment of PDA in preterm infants should be abandoned, as recommended in the recent Clinical Report from the American Academy of Pediatrics.

BACKGROUND: Direct oral anticoagulants (DOACs) are recommended over vitamin K antagonists (VKAs) for stroke prevention in atrial fibrillation (AF). However, limited accessibility in resource-constrained settings makes it important to identify patient subgroups in whom VKAs may achieve comparable net clinical outcomes (NCOs).
OBJECTIVES: The purpose of this study was to identify AF patient subgroups and explore potential predictors of differential treatment benefit to inform anticoagulant selection where DOAC access is limited.
METHODS: Individual patient data from the COMBINE-AF data set poo

Individuals differ in how they respond to a given treatment. In an effort to predict the treatment response and analyze the heterogeneity of treatment effect, we propose a general modeling framework by identifying treatment-covariate interactions honoring a hierarchical condition. We construct a single-step l1 norm penalty procedure that maintains the hierarchical structure of interactions in the sense that a treatment-covariate interaction term is included in the model only when either the covariate or both the covariate and treatment have nonzero main effects. We developed a constrained Lass

ARDS is a clinically heterogeneous syndrome, rather than a distinct disease. This heterogeneity at least partially explains the difficulty in studying treatments for these patients and contributes to the numerous trials of therapies for the syndrome that have not shown benefit. Recent studies have identified different subphenotypes within the heterogeneous patient population. These different subphenotypes likely have variable clinical responses to specific therapies, a concept known as heterogeneity of treatment effect. Recognizing different subphenotypes and heterogeneity of treatment effect

BACKGROUND AND OBJECTIVES: Pancreatic cancer is one of the most lethal malignancies. Even though many substantial improvements in the survival rates for other major cancer forms were made, pancreatic cancer survival rates have remained relatively unchanged since the 1960s. Even more, no standard classification system for pancreatic cancer is based on cellular biomarkers. This review will discuss and provide updates about the role of stem cells in the progression of PC, the genetic changes associated with it, and the promising biomarkers for diagnosis.
MATERIALS AND METHODS: The search process

There is growing interest in tailoring treatment decisions to individual patient characteristics, but few studies have examined the implementation and performance of individualized treatment rules (ITRs) for count data. Our objective was to compare ITR methods in randomized trials with count outcomes and explore the impact of sample size and distribution of heterogeneity of treatment effect (HTE) on the validity of treatment recommendations. We conducted a simulation study where patients were randomized to receive one of two treatments and created five responder strata to reflect different HTE

BACKGROUND: It is currently unknown if disease severity modifies response to therapy in pulmonary arterial hypertension (PAH). We aimed to explore if disease severity, as defined by established risk-prediction algorithms, modified response to therapy in randomised clinical trials in PAH.
METHODS: We performed a meta-analysis using individual participant data from 18 randomised clinical trials of therapy for PAH submitted to the United States Food and Drug Administration to determine if predicted risk of 1-year mortality at randomisation modified the treatment effect on three outcomes: change

BACKGROUND: This project has investigated the role of the Bacillus Calmette-Guérin (BCG) vaccine as a potential treatment against Alzheimer's disease (AD) and related dementias (ADRD).
OBJECTIVE: To further establish that BCG treatment results in lower risk of ADRD through novel machine learning methods and to analyze the heterogeneity of treatment effects.
METHODS: This retrospective cohort study was conducted from May 28, 1987 to May 6, 2021, in patients who were 50 years or older and were diagnosed with non-muscle-invasive bladder cancer (NMIBC). Follow-up duration was 15-years. Machine l

BACKGROUND: Exploration and modelling of heterogeneous treatment effects as a function of baseline covariates is an important aspect of precision medicine in randomised controlled trials (RCTs). Randomisation generally guarantees the internal validity of an RCT, but heterogeneity in treatment effect can reduce external validity. Estimation of heterogeneous treatment effects is usually done via a predictive model for individual outcomes, where one searches for interactions between treatment allocation and important patient baseline covariates. However, such models are prone to overfitting and m

Discontinuation of denosumab is associated with a rebound increase in osteoporotic fracture (OF) risk, and bisphosphonates (BPs) are commonly recommended as sequential therapy to mitigate this risk. However, their real-world effectiveness-and whether certain patients benefit more than others-remains unclear. To address this question, we employed a causal machine learning approach capable of estimating heterogeneous treatment effects. We conducted a nationwide retrospective cohort study using Korean health insurance claims data and applied the causal survival forest model to evaluate the effect

BACKGROUND: Randomized trials have shown equivocal outcomes between a direct aspiration as first-pass technique (ADAPT) and stent retriever (SR) stroke thrombectomy, yet whether specific patient subgroups benefit differentially from one technique over the other remains unknown.
METHODS: In this retrospective multicenter cohort study, patients with large vessel occlusion were included, and those who underwent ADAPT versus SR first-pass thrombectomy (either as standalone or combined with aspiration) were compared. The primary outcome was a composite of first-pass effect (FPE, modified Thromboly

BACKGROUND AND OBJECTIVES: Endovascular thrombectomy (EVT) is standard of care for large vessel occlusion (LVO) stroke with moderate-to-severe deficits, but its role in minor stroke (NIHSS <6) remains uncertain. We characterized EVT's association with outcomes, examined complications as potential mediators of that association, and explored heterogeneity of treatment effect (HTE).
METHODS: Retrospective cohort study using the Nationwide Readmissions Database (2016-2022) in adults with non-elective LVO stroke and NIHSS <6. Primary outcomes were routine discharge and in-hospital mortality; secon

BACKGROUND: The average treatment effect (ATE) reported by most randomised clinical trials provides estimates of treatment effects for the theoretical, non-existent average patient. However, ATE may not accurately reflect the outcomes for all subsets of the trial population; some individuals may benefit from the intervention, while others experience worse outcomes or no effect at all. Heterogeneity of treatment effect (HTE) is the non-random and explainable variation in the magnitude or direction of a treatment effect among individuals within a population. Predictive approaches to HTE seek to

Treatment effects are often anticipated to vary across groups of patients with different baseline risk. The Predictive Approaches to Treatment Effect Heterogeneity (PATH) statement focused on baseline risk as a robust predictor of treatment effect and provided guidance on risk-based assessment of treatment effect heterogeneity in a randomized controlled trial. The aim of this study is to extend this approach to the observational setting using a standardized scalable framework. The proposed framework consists of five steps: (1) definition of the research aim, i.e., the population, the treatment

RATIONALE: Despite numerous randomized controlled trials (RCTs) in acute respiratory distress syndrome (ARDS), few have identified effective therapies. One reason may be heterogeneity of treatment effect (HTE), in which distinct subgroups respond differently to the same intervention, potentially reflective of underlying biological endotypes.
OBJECTIVES: We conducted a systematic review to evaluate effect modification by molecular endotype across randomized ARDS trials and assess credibility of reported subgroup effects. In doing so, we aim to expose current knowledge gaps in how subgroups are

OBJECTIVES: Inherited defects of the neuromuscular junction (NMJ) comprise an increasingly diverse range of diseases. Several recently identified genes highlight the overlap between peripheral neuropathies and congenital myasthenic syndromes (CMS). The beta-2 adrenergic receptor agonist salbutamol has been shown to provide symptomatic benefit in CMS, while improving structural defects at the NMJ. Based on these findings, we identified cases of motor neuropathy with NMJ dysfunction and assessed the effect of salbutamol on motor function.
METHODS: Cases of motor neuropathy with significant NMJ
