IMPORTANCE: Despite evidence demonstrating that severely injured patients treated at trauma centers have improved outcomes relative to those treated at nontrauma centers, rates of undertriage (transportation of severely injured patients to nontrauma centers) remain high. While national expert panels have developed field trauma triage (FTT) guidelines to aid in the triage of injured patients, the impact of the implementation of these guidelines on existing triage patterns is unknown.
OBJECTIVE: To examine the association between the 2015 update to FTT guidelines and trends in trauma triage in

BACKGROUND: Over the past 25 years, numerous studies have evaluated triage system performance, but longitudinal data are limited. This study is the first to assess the Manchester Triage System (MTS) over a decade to determine the accuracy of initial patient assessments and system stability under external influences such as staff turnover, health policy changes, demographic shifts, and the COVID-19 pandemic.
METHODS: We conducted a large-scale, monocentric, retrospective observational study at a tertiary care hospital.
RESULTS: Between 2014 and 2023 a total of 355,135 emergency patients were

Triage, a medical term derived from the French word "trier", is the practical process of sorting casualties to rationally allocate limited resources. In combat settings with limited medical resources and long transportation times, triage is challenging since the objectives are to avoid overcrowding medical treatment facilities while saving a maximum of soldiers and to get as many of them back into action as possible. The new face of modern warfare, asymmetric and non-conventional, has led to the integrative evolution of triage into the theatre of operations. This article defines different tria

The use of limited ultrasound as a tool in obstetric triage has become increasingly popular. While most midwives only perform ultrasound in the third trimester, some institutions do include earlier sonographic testing as part of the midwifery management plan. This article addresses the professional issues related to limited ultrasound in obstetric triage protocol, the education and training needed to acquire the skill, the process for integrating the skill into clinical practice, and a discussion of obstetric triage procedures.

PURPOSE: This quality improvement project aimed to reduce administrative delays and workflow inefficiencies by optimizing the "Referral Triage" process using Lean methodology.
METHODS: A pilot study was performed on 3 disease sites at a large comprehensive Radiation Oncology Program: Head and Neck (H&N), Breast (Br), and Genitourinary (GU). In the revised workflow, RCs independently completed triage for pilot-eligible cases (defined by referral source, approval likelihood, and site-specific case complexity criteria, see section 1.5) without requiring RO approval to reduce the triage-related e

Rapidly identifying individuals who have received internal radiation exposure above action guidelines is crucial for mitigating health risks and addressing public concerns immediately following a radiological event involving the dispersal of radioactive materials. This study describes a novel triage method using a conventional Geiger-Mueller (GM) detector to select those individuals from the large group of persons who may have received an intake of radioactive material at levels corresponding to one Clinical Decision Guide (CDG). The triage method involves placing a portable GM detector agains

INTRODUCTION: A diphtheria outbreak occurred in 2017 in Jakarta, Indonesia, during which our hospital was appointed as a referral hospital where patients with upper respiratory tract symptoms were sent for confirmation of the diagnosis and medical intervention. In this study we review the implementation of the emergency department (ED) triage process and patient flow management during the diphtheria outbreak. No previous study in Indonesia has provided a detailed report on the triage process during infectious disease outbreaks.
METHOD: We modified our pre-existing hospital triage method accor

Implementation of population screening for colorectal cancer by direct colonoscopy or follow-up colonoscopy after a positive fecal blood test has challenged the overall capacity of bowel examinations. Certain countries are facing serious colonoscopy capacity constraints, which have led to waiting lists and long-time latency of follow-up examinations. Various options for improvement are considered, including increased cut-off values of the fecal blood tests. Results from major clinical studies of blood-based, cancer-associated biomarkers have led to focus, however, on a triage concept for impro

BACKGROUND: Emergency Department (ED) triage requires rapid assessment of acuity under time pressure with incomplete information. Existing AI triage systems require complete clinical data, including vital signs, before generating a prediction, and so cannot support the sequential, information-gathering reality of triage in which patients accumulate in queues before vital signs are collected. Pre-vital queue prioritisation, deciding which patients are seen next before vital signs exist, remains unaddressed.
OBJECTIVE: To design and evaluate ED-Triage-Agent (ETA), a multi-agent large language m

BACKGROUND AND PURPOSE: Clinical triage scales for prehospital recognition of large vessel occlusion (LVO) are limited by low specificity when applied by paramedics. We created the 3-step ambulance clinical triage for acute stroke treatment (ACT-FAST) as the first algorithmic LVO identification tool, designed to improve specificity by recognizing only severe clinical syndromes and optimizing paramedic usability and reliability.
METHODS: The ACT-FAST algorithm consists of (1) unilateral arm drift to stretcher <10 seconds, (2) severe language deficit (if right arm is weak) or gaze deviation/hem

An audit of triage was conducted between June and September 2003 in a district general hospital genitourinary medicine department to evaluate the appropriateness of triage criteria to identify those who require urgent intervention. We selected gonorrhoea (B1), chlamydia (C4a), gential herpes (C10a/b) and epidemiological treatment of contacts of B1 or C4a (C4e/B4) as conditions requiring urgent intervention. Eighty-eight percent met one or more of the criteria and were offered urgent appointments and 69% who did not meet the criteria were offered routine appointments. The incidences of B1, C4a,

We examined the outcome of active duty psychiatric inpatients (N = 101) 2 years after admission. Most patients were young with minor diagnoses (adjustment disorders), yet only 20% were still on active duty 2 years later. This compared poorly with national retention rates for all lengths of time in service (p < 0.0001). Longer time in service and low severity of diagnosis predicted a better outcome. We discuss implications of these findings.

Many types of ocular trauma can be diagnosed and treated in the primary care office, particularly if a slit lamp is available. Treatment for corneal abrasions consists of applying a cycloplegic medication, antibiotic ointment, and a patch (unless a corneal ulcer is suspected). Iritis can be treated with cycloplegics and topical corticosteroids; the prescribing physician should be familiar with the potential ocular side effects and complications. Hyphemas are treated with bed rest, topical atropine sulfate drops and topical corticosteroids, as well as measures to prevent rebleeding. A slit lamp

Patients evaluated by paramedics but not transported to the hospital account for 50% to 90% of emergency medical services lawsuits. We reviewed 2,698 consecutive paramedic run reports to examine documentation in these cases. Documentation criteria for prehospital patient release were history, physical examination, vital signs, mental status, lack of significant mental impairment, and for patients refusing care, that risks of refusing were understood. Criteria for appropriate release were met in 65.2% of cases. Criteria omitted in inappropriate releases were risks of refusing care in 481 (51.3%

BACKGROUND: Emergency department (ED) triage is performed to prioritize care for patients with critical and time-sensitive illness. Triage errors create opportunity for increased morbidity and mortality. Here, we sought to measure the frequency of under- and over-triage of patients by nurses using the Emergency Severity Index (ESI) in Brazil and to identify factors independently associated with each.
METHODS: This was a single-center retrospective cohort study. The accuracy of initial ESI score assignment was determined by comparison with a score entered at the close of each ED encounter by t

BACKGROUND: Alerts embedded in electronic health records (EHRs) are designed to improve processes at the point of care.
OBJECTIVE: The aim of this study was to determine if an EHR alert-at emergency department (ED) triage-increases the ED triage nurse's utilization of triage protocols.
METHODS: ED triage nurses were randomized to receive either a passive EHR alert or no intervention for patients with the following complaints: minor trauma with extremity injuries or female patients with abdominal pain. The EHR alert notified the nurse that the patient was eligible for diagnostic testing: radi

This article discusses basic head and intracranial CNS anatomy, cerebral physiology, and classifications of head injury. Management principles and the practice of head-injury evaluation are reviewed and supplemented by the presentation of selected head-injury scenarios. The optimal evaluation and treatment of the head-injured patient is predicated on initially following established ATLS principles and preventing secondary injury. Once specific injuries have been identified, maintenance of cerebral perfusion and oxygenation is the key to maximizing patient outcomes, and this often requires meas
