A new prism use for treatment of cyclo-deviation in trochlear nerve injury.

Visual problems are common in patients with severe TBI. Diplopia is among the most frustrating of visual disturbances for patients, due to its functional consequences. This is further compounded by often slow, and at times, incomplete or partial recovery over six months or longer. Ocular cranial nerve injuries (particularly trochlear nerve) occur in 0.2%-1.4% of severe TBI patients. This paper presents a new prism treatment application for treating rotational diplopia arising from Trochlear nerve injury.

Humeral trochlear hypoplasia secondary to epiphyseal injury as a cause of ulnar nerve palsy.

Humeral trochlear hypoplasia (HTH) is a rare condition that occasionally results in ulnar nerve palsy. A 41-year-old man developed HTH secondary to an epiphyseal injury of the trochlea incurred 33 years earlier. This may be the first report of a case of HTH caused by injury. The ulnar nerve appeared compressed by the malposition of the HTH against the medial head of the triceps brachii when the joint was flexed. The close proximity of the olecranon to the HTH also allowed the triceps brachii muscle to compress the nerve by displacing medially and overriding the bone. The condition was resolved

An endoscope-assisted craniometric cadaveric study for the brain stem and the cisternal segment of the trochlear nerve.

BACKGROUND AND PURPOSE: This study analyzed the relationship of trochlear nerve with neurovascular structures using craniometric measurements. The study was aimed to understand the course of trochlear nerve and minimize the risk of injury during surgical procedures. METHODS: Twenty trochlear nerves of 10 fresh cadavers were studied bilaterally using endoscopic assistance through the view afforded by the lateral infratentorial-supracerebellar, and the combined presigmoid-subtemporal transtentorial approaches. Trochlear nerves were exposed bilaterally taking seven parameters into consideration:

Trochlear nerve palsy following minor head trauma. A sign of structural disorder.

Trauma-induced superior oblique palsy usually results from contusion or avulsion of the trochlear nerve or from decompensation of a congenital trochlear nerve palsy. Severe craniocerebral trauma is often associated with the former mechanism, whereas more minor closed-head injuries can decompensate a congenital phoria. We report a patient who developed an isolated trochlear nerve palsy following minor head trauma. Investigation revealed an unsuspected tentorial vascular malformation that was compressing the trochlear nerve in its subarachnoid course. In the absence of other features (e.g., docu

Traumatic lesions of the optic, oculomotor, trochlear, and abducens nerves--computer tomographic findings.

Traumatic lesions of the oculomotor and optic nerves are usually associated with multiple injuries which pose difficulties for radiological examination. In 54 patients with functional disturbances of the optic, oculomotor, trochlear, or abducens nerve following severe multiple injuries, the initial computer tomographic examination was unable to document conclusively direct nerve damage. Indirect signs of nerve damage in the form of orbital and midface fractures were often found in the presence of optic and oculomotor nerve impairments, but seldom in the case of abducens nerve deficits. The cau

Localization of post-traumatic trochlear nerve palsy associated with hemorrhage at the subarachnoid space by magnetic resonance imaging.

PURPOSE: To report evaluation of traumatic trochlear nerve palsy using head magnetic resonance imaging. DESIGN: Observational case reports. METHODS: We examined two cases involving trochlear nerve palsy after closed head injury. RESULTS: Using a fluid attenuated inversion recovery pulse sequence, MRI showed a high-intensity lesion consistent with subarachnoid hemorrhage at the trochlear nerve area in the ambient cisterns. CONCLUSION: An impact force directed toward the tentorium can be a mechanism of injury in some post-traumatic trochlear nerve palsies. Fluid attenuated inversion recovery

Central trochlear palsy.

Historically, the trochlear (IV) nerve has been "neglected" by neurologists and ophthalmologists. However, the reported incidence of trochlear palsy in two large series has more than doubled in the past two decades, indicating increasing awareness of this nerve. Trauma is the most common cause of trochlear palsy, as the trochlear nerve is anatomically more vulnerable to trauma than the other ocular motor nerves. Trochlear palsy can also be caused by vascular and inflammatory diseases, congenital factors, toxic substances and tumors. Diplopia secondary to vertical and horizontal deviation is th

Segmental Imaging of the Trochlear Nerve: Anatomic and Pathologic Considerations.

BACKGROUND: The trochlear nerve (the fourth cranial nerve) is the only cranial nerve that arises from the dorsal aspect of the midbrain. The nerve has a lengthy course making it highly susceptible to injury. It is also the smallest cranial nerve and is often difficult to identify on neuroimaging. EVIDENCE ACQUISITION: High-resolution 3-dimensional skull base MRI allows for submillimeter isotropic acquisition and is optimal for cranial nerve evaluation. In this text, the detailed anatomy of the fourth cranial nerve applicable to imaging will be reviewed. RESULTS: Detailed anatomic knowledge o

The injury response of nerve fibres in the anterior medullary velum of the adult rat.

The injury response of myelinated central nervous system (CNS) axons was documented in the anterior medullary velum (AMV) of the adult rat. Study of silver-stained AMV whole-mounts revealed sprouting of injured axons as early as 14 h post-lesion (hpl), with a complex network of fibres formed by 48 hpl. Signs of fibre degeneration were also apparent from 48 hpl, increasing in extent until 15 days post-lesion (dpl). Fragmentation was largely confined to specific fibre bundles, constituted by the distal portions of severed axons. Although some degeneration of regenerated axons was evident from 15

The microsurgical anatomy of the cisternal segment of the trochlear nerve, as seen through different neurosurgical operative windows.

OBJECTIVE: To describe the anatomy of the cisternal segment of the trochlear nerve as seen through different neurosurgical approaches. METHODS: The cisternal course of ten trochlear nerves was observed in five cadaveric embalmed heads, through the view afforded by the median infratentorial-supracerebellar, the extreme-lateral infratentorial-supracerebellar, and the combined presigmoid-subtemporal transtentorial approaches. The relationships of the trochlear nerve with the surrounding neuro-vascular structures were analyzed. RESULTS: We identified 3 segments of the cisternal trochlear nerve:

Regeneration of nerve fibres in the anterior medullary velum of neonatal and weanling rats.

The responses to injury of myelinated central axons in neonatal (5-day) and weanling (30-day) rats were compared, using the trochlear nerve fibres in the anterior medullary velum (AMV) as an experimental system. In intact animals, fibres of the trochlear nerve decussate completely in the AMV, to enter the contralateral nerve. Sprouting from proximal axon stumps was seen within 36 h after injury in silver-stained, whole mounts of mid-sagittally lesioned AMVs from weanlings. Subsequent growth produced a tangled axon mass which persisted until 15-20 days post-lesion (d.p.l.) but then began to deg

Nerve injury associated with orthognathic surgery. Part 1: UK practice and motor nerve injuries.

The head and neck is anatomically complex, and several nerves are at risk during orthognathic operations. Some injuries to nerves are reported more commonly than others. To find out what consultant surgeons tell their patients about the prevalence of common nerve injuries before orthognathic operations, we did a postal survey of fellows of the British Association of Oral and Maxillofacial Surgeons (BAOMS). We also reviewed published papers to find out the reported incidence of injuries to cranial motor nerves during orthognathic operations. Only injuries to the facial nerve were commonly repor

Bifurcation of the trochlear nerve: cadaveric case report and literature review.

BACKGROUND: The trochlear nerve [the fourth cranial nerve (IV CN)], one of the extraocular motor cranial nerves, exits the midbrain at its dorsal side, runs inside the lateral wall of the cavernous sinus (CS), and innervates the superior oblique muscle. In the wall of the CS, the IV CN travels in the vicinity of the oculomotor and ophthalmic nerves. These nerves border the infratrochlear and supratrochlear triangles (ST), which are part of a parasellar group of triangles of the CS, and provide safe entry zones to the inside of the CS. CASE REPORT: During standard anatomical dissection, the un

Trochlear nerve palsy after repeated percutaneous balloon compression for recurrent trigeminal neuralgia: case report and pathogenic considerations.

OBJECTIVE AND IMPORTANCE: Repeated percutaneous balloon compression for the treatment of idiopathic trigeminal neuralgia is infrequent. When a second procedure is performed, the outcome is unknown. A patient developed an isolated trochlear nerve palsy after undergoing percutaneous trigeminal ganglion balloon compression for a second time. The mechanism of diplopia and the complications associated with this technique were studied. CLINICAL PRESENTATION: The patient was a 67-year-old woman with a history of medically refractory idiopathic trigeminal neuralgia involving all three divisions of th

Relationships of the cisternal segment of the trochlear nerve.

OBJECT: The cisternal portion of the trochlear nerve (fourth cranial nerve) can easily be injured during intracranial surgical operations. To help minimize the chance of such injury by promoting a thorough understanding of the anatomy of this nerve and its relationships to surrounding structures, the authors present this anatomical study. METHODS: In this study, in which 12 cadaveric heads (24 sides) were used, the authors describe exact distances between the trochlear nerve and various surrounding structures. Also described are relatively safe areas in which to manipulate or enter the tentor

Return of function after intracranial suture of the trochlear nerve. Case report.

An excellent functional result was obtained after microsuture of a trochlear nerve which was inadvertently divided during the course of a craniotomy. Successful intracranial suture of this nerve has not previously been reported. Ophthalmoscopic detection of excyclotorsion of the eye on the side of the trochlear nerve palsy is discussed, along with its value in the differential diagnosis of acquired vertical diplopia.

Anatomy of the tentorial segment of the trochlear nerve in reference to its preservation during surgery for skull base lesions.

BACKGROUND: In traditional descriptions of the intracranial course of the trochlear nerve, the tentorial segment of this nerve has not been described. This segment assumes importance as it is at risk of injury during tentorial sectioning in skull base surgery. In the present study, the tentorial segment of the trochlear nerve was studied. METHODS: 30 cadaver sides were studied and the following parameters were measured: the parts and lengths of the tentorial segments of the fourth nerve; the distances between the point where the third nerve came into contact with the tentorial edge and (a) th

[Decompensated strabismus surso-adductorius].

Strabismus surso-adductorius is a frequent unilateral or bilateral eye movement disorder. Its clinical features include eye elevation with concomitant vertical deviation in adduction, an abnormal head posture from which the patient is unaware (head turned and tilted towards the healthy side), a moderate subjective excyclotorsion, and a positive Bielschowsky head tilt test. Despite its anglo-saxon denomination as "congenital fourth nerve palsy", it is not a paretic disorder. Strabismus surso-adductorius differs from fourth nerve palsy both by etiology and by symptoms. A proper diagnosis is impo

Fourth and sixth cranial nerve injury after halo traction in children: a report of two cases.

BACKGROUND: Spinal traction is the application of a longitudinal force to the spinal column as a means of stabilizing a damaged or abnormal spine. Although not well documented in the ophthalmic literature, complications include cranial nerve palsies, with the sixth nerve being most commonly affected. Fourth nerve palsies have not previously been reported to our knowledge. We present 2 cases of combined fourth and sixth palsies after cervical traction. METHODS: Retrospectively, we reviewed the ophthalmic findings in 2 children with diplopia after spinal traction. RESULTS: Case 1 suffered a tr
