The term "therapeutic nondisclosure" refers to a clinician's decision to withhold diagnostic or prognostic information from a patient to protect him or her from perceived harm. We present a vignette in which the son of a 75-year-old Italian born immigrant asks her physician to withhold her new diagnosis of advanced myeloma. In the U.S., trends over the last 30 years have been toward more complete disclosure of cancer-related information. This can be attributed to the evolution of research subject protections, the "war on cancer," the civil rights and patient rights movements, and the rise of h

PURPOSE: The purpose of this study was to describe whether adolescent and young adult patients truthfully disclose sexual activity to providers during a sexual history and explore associations between disclosure and receipt of recommended services.
METHODS: Data from the 2018 National Survey of Sexual Health and Behavior were used to describe self-reported disclsoure of sexually active 14- to 24-year-olds who had a health care visit in the previous year where a sexual history was taken (n = 196). We examined bivariate associations between disclosure and age, race/ethnicity, sex, sexual identi

The New Jersey Supreme Court held in New Jersey v. J.L.G. (2018) that experts can no longer explain to juries why sexually abused children might deny abuse. The court was influenced by expert testimony that "methodologically superior" studies find lower rates of denial. Examining the studies in detail, we argue that the expert testimony was flawed due to three problems with using child disclosure studies to estimate the likelihood that abused children are reluctant to disclose abuse: the ground truth problem, disclosure suspicion bias, and disclosure substantiation bias. Research identifying g

This study examined the utility of two interview instructions designed to overcome children's reluctance to disclose transgressions: eliciting a promise from children to tell the truth and the putative confession (telling children that a suspect "told me everything that happened and wants you to tell the truth"). The key questions were whether the instructions increased disclosure in response to recall questions and in response to recognition questions that were less or more explicit about transgressions and whether instructions were differentially effective with age. A total sample of 217 4-

BACKGROUND: Children are often hesitant to disclose transgressions, particularly when they feel implicated, and frequently remain reluctant until confronted with direct questions. Given the risks associated with direct questions, an important issue is how interviewers can encourage honesty through recall questions.
OBJECTIVE: The present study examined the use of three truth induction strategies for increasing the accuracy and productivity of children's reports about a transgression.
PARTICIPANTS: A total of 285 4-to-9-year-old maltreated and nonmaltreated children.
METHODS: Each child took

Potential difficulties experienced by nursing students diagnosed with dyslexia can be minimised with the introduction of appropriate policies and guidance around reasonable adjustment and support. In order to access all relevant services, however, a student must actively decide to disclose their dyslexic identity to relevant faculty personnel. Disclosure of such personal information is a complex matter and, critically, requires a receptive environment where diversity and disability are embraced in a positive and meaningful way. The act of disclosure for the most part has previously been descri

The management of patients and families affected by Huntington disease (HD) is complicated by several factors, both practical and ethical. It can be difficult to determine the onset of clinically manifest HD (mHD). In addition, it can be challenging to decide when to disclose the diagnosis to the affected individual. Firstly, the features of HD, an incurable, inherited, neurocognitive disorder that often manifests in young adulthood, influence how the person presents and accepts a diagnosis. Secondly, a positive genetic test for HD may result in a genetic diagnosis, sometimes years before the

BACKGROUND: Recantation after child sexual abuse (CSA) disclosure refers to the complete denial of prior reports by a victim. In substantiated cases of CSA, recantation has negative effects on criminal proceedings and the protection of current and future victims. Past studies have identified the socio-family factors and CSA characteristics that are associated with recantation. However, the systematization of these risk factors remains limited.
OBJECTIVE: This study aimed to identify the predictors of recantation using a sample of criminal cases of CSA.

OBJECTIVE: Children who experience sexual abuse (CSA) often delay disclosure, thus impeding early interventions. This study explores predictors of disclosure latency in a sample of forensically substantiated cases.
METHODS: Court files were analyzed for a total of 124 cases of CSA. The ground truth of the alleged victims' statements had been substantiated by at least one inclusion criterion-an evaluation of the alleged victims' statement as credible by psychological experts applying Statement Validity Assessment (SVA), or the conviction of the suspected offender by the court. To face the traumatic shock after the announcement of a severe and fatal disease, patients build psychological defences that physician must identify and respect as long as they don't endanger the patient's psychological fate and his medical care. ALS patient's do not have any curative perspective thus they will try "to escape" using different strategies the traumatic reality, that they cannot integrate and control. The physician has to prove his understanding and empathy facing these psychological reactions that could become of pathological appearance. The physician has to progressively bring

Breaking the news of amyotrophic lateral sclerosis (ALS) is considered as a daunting task in most cases and is not a standardizable procedure. However, proven techniques exist to reduce the trauma to the patient. Announcing ALS falls upen the neurologist who must respect the ethical principle of the patient's independence. After the diagnosis is firmly established, the patient should be informed that he or she has a progressive disease of the motor nerves, for which no curative therapy is available. The name of the disease must be stated and explained. If the family history is negative, it is

BACKGROUND: Prognostic disclosure enables shared decision-making by keeping patients informed about their health and involved in treatment choices. Prognostic uncertainty is prominent in metastatic breast cancer (mBC) and is a known barrier to prognostic disclosure, yet knowledge about prognostic disclosure in mBC has not been systematically mapped.
OBJECTIVE: This scoping review aimed to explore the literature on prognostic disclosure in mBC and highlight research gaps.
METHODS: We followed Arksey and O'Malley's expanded framework. Electronic databases MEDLINE (via PubMED), Scopus, PsychINF

OBJECTIVES: Truth-telling in pediatric oncology remains challenging, and school-aged children's perspectives are seldom directly examined. This study explored diagnostic disclosure experiences and perceived communication preferences among school-aged children with acute lymphoblastic leukemia (ALL).
METHODS: This study employed a descriptive phenomenological design. Data were collected between August 2019 and May 2020 through semi-structured individual interviews with 15 school-aged children diagnosed with ALL. Interviews were analyzed using Colaizzi's method to identify emerging themes.

Discussing serious news is a fundamental communication skill, and many clinicians have been taught to ask their patients how much detail they want to hear before sharing difficult information. Over the past decade, we have taught hundreds of medical students how to discuss serious news and reviewed hundreds of their recorded conversations. We've found that asking how much detail a patient wants to hear often results in confusion and is not an effective way to understand their communication preferences. Instead of asking how much detail your patient wants to hear, we propose an alternative way
