[Vascular causes of median and ulnar nerve compression syndromes].

From a group of more than 200 patients with median and ulnar nerve compression syndrome three cases of interest are demonstrated in detail. In these cases vascular complications led to acurate events accompanied by grave symptoms. In order to prevent irreversible damage surgical treatment of such cases is indicated as soon as possible.

Compression of the ulnar nerve at the elbow: cubital tunnel syndrome.

Although cubital tunnel syndrome has been described as the most common entrapment of the ulnar nerve, there is still considerable difficulty identifying the exact location of the pathologic compression of the nerve and deciding on the correct surgical or nonsurgical treatment. The most commonly recommended surgical techniques include simple (in situ) decompression, decompression with medial epicondylectomy, anterior subcutaneous transposition, and anterior submuscular transposition of the ulnar nerve at the elbow. It is important to understand the pitfalls and possible complications of these c

[Ulnar nerve compression syndrome of the elbow. Analysis of 50 operated cases].

This is a retrospective review of 50 elbows (44 patients) treated operatively by simple decompression (seven cases), decompression with epitrochlectomy (13 cases), and anterior transposition (30 cases) for compression neuropathy of the ulnar nerve at the elbow. We performed a superficial transposition in five cases, an intramuscular transposition in 13 cases, and a deep submuscular transposition in 12 cases. The mean follow-up was three years and two months. The overall results show cure or improvement in 78% of cases, no changes in 20% of cases, and one case with subjective worsening. The pos

The Association Between Concomitant Ulnar Nerve Compression at the Elbow and Carpal Tunnel Syndrome.

Background: Many patients treated for ulnar nerve compression at the elbow (UNE) are concomitantly treated for carpal tunnel syndrome (CTS). We sought to investigate the association between the conditions. Methods: The Statewide Planning and Research Cooperative System (SPARCS) database was used to determine the number of patients with UNE concomitantly treated for CTS in New York State from 2003 to 2014. We then retrospectively reviewed each patient who received surgical treatment for UNE (n = 222 patients) or CTS (n = 1063 patients) at our tertiary care institution in 2014 and 2015 to assess

Unusual complication of an opposition tendon transfer at the wrist: ulnar nerve compression syndrome.

Restoration of thumb opposition by tendon transfer may be necessary in cases of severe thenar atrophy caused by long-standing carpal tunnel syndrome. Routing the extensor indicis proprius transfer subcutaneously around the ulna to reanimate thumb opposition is an accepted procedure and is considered safe. Ulnar nerve compression leading to palsy is possible, however, as shown in the patient presented. Neurolysis failed to improve the palsy. Rerouting of the transfer deep to the ulnar nerve was necessary to treat the iatrogenic condition. Possible nerve compression should be kept in mind when p

[Heterotopic ossification and ulnar nerve compression syndrome of the elbow. A report of two cases].

Heterotopic ossification is a well known complication in patients after longterm coma following CNS trauma. The association of heterotopic ossification and a nerve compression syndrome situated near the calcifications is less common. The authors present 2 CNS traumatised patients who secondarily developed the association of heterotopic ossification of the elbow with a compressive syndrome of the ulnar nerve. Although heterotopic ossification does not appear to be the direct cause of the nerve compression, it certainly promotes compression it because of the flexion it causes.

Atypical carpal tunnel syndrome with compression of the ulnar and median nerves. Case report.

Clinical signs of ulnar nerve involvement at the wrist level were found in a 51-year-old man. The electrophysiological changes were indicative of a median nerve involvement in the carpal tunnel. At operation, the compressed ulnar nerve was found in the carpal tunnel, together with the median nerve. This localization of the ulnar has not been described previously.

Ulnar tunnel syndrome caused by two anomalous muscle bellies: a rare etiology of dynamic ulnar nerve compression.

Ulnar tunnel syndrome, or Guyon's canal syndrome, is an uncommon compressive neuropathy of the ulnar nerve at the wrist. While most cases result from trauma or ganglion cysts, anomalous muscles are rare. This case describes an ulnar tunnel syndrome caused by two anomalous muscle bellies crossing the ulnar neurovascular bundle in a 64-year-old man with recurrent ulnar neuropathy and intrinsic hand atrophy. Surgical excision and decompression resulted in durable symptom resolution at 5-year follow-up. This case highlights the importance of considering anatomical variations in persistent or recur

[Two anomalous muscles of a forearm revealed by ulnar nerve compressions, a Double Crush syndrome].

This article describes the concomitant presence of two anomalous muscles on a left forearm in a 40-year-old man. The anconeus epitrochlearis muscle was responsible for a cubital tunnel syndrome and the unusual origin of the flexor digiti minimi brevis muscle was responsible for a compartment syndrome with ulnar nerve compression at the level of Guyon's canal during effort diagnosed by MRI. Resection of these muscles relieved the symptoms and allowed the patient to return to work.

Handlebar palsy--a compression syndrome of the deep terminal (motor) branch of the ulnar nerve in biking.

We describe 3 patients who developed a severe palsy of the intrinsic ulnar supplied hand muscles after bicycle riding. Clinically and electrophysiologically all showed an isolated lesion of the deep terminal motor branch of the ulnar nerve leaving the hypothenar muscle and the distal sensory branch intact. This type of lesion at the canal of Guyon is quite unusual, caused in the majority of cases by chronic external pressure over the ulnar palm. In earlier reports describing this lesion in bicycle riders, most patients experienced this lesion after a long distance ride. Due to the change of ri

Acute ulnar nerve compression syndrome in a powerlifter with triceps tendon rupture--a case report.

We report on the case of a bodybuilder and powerlifter who suffered from triceps tendon rupture complicated by acute ulnar nerve compression syndrome. The diagnosis was made clinically, radiologically, and sonographically. Ultrasound was helpful to demonstrate a large hematoma at the site of the injury. Early surgical intervention confirmed the presence of the hematoma compressing the ulnar nerve and led to a complete restoration of ulnar nerve and triceps muscle function. Few reports on distal triceps rupture have been published but its complication by acute ulnar nerve compression has not be

[Post-traumatic ulnar nerve irritation and paralysis as a compression syndrome under the palmar carpal ligament].

The palmar carpal ligament is defined as the distal border of the fascia antebrachii. It is proposed that this name is not used for the region of the fascia antebrachii proximally to this structure or for parts of the retinaculum distal to this structure, i.e. in the hypothenar region. The palmar carpal ligament has been found to be a cause of posttraumatic irritation of the ulnar and median nerve at the wrist proximal to the carpal tunnel or Guyon's canal.

Progression From Steroid Injection to Surgery in Carpal Tunnel Syndrome Patients With Concurrent Ulnar Nerve Compression: A Retrospective Analysis.

BACKGROUND: Steroid injections are a common treatment option in the management of carpal tunnel syndrome (CTS). This study assesses various prognostic factors for progression to carpal tunnel release (CTR) after a first-time steroid injection for CTS with specific focus on concomitant ulnar nerve compression (UNC). METHODS: This is a retrospective study of 426 hands with CTS treated with a first-time steroid injection in the Long Island region of New York. The main predictor variable was UNC measured in two analytical models of positive UNC and location of UNC (wrist or elbow). Multivariate l

A 60-Year-Old Woman with Symptoms of Combined Carpal Tunnel Syndrome and Cubital Tunnel Syndrome Due to an Elastofibroma Causing Compression of the Median and Ulnar Nerves.

BACKGROUND Cubital tunnel syndrome results from pressure or stretching of the ulnar nerve, and carpal tunnel syndrome involves the median nerve. Elastofibroma is a rare, benign, slow-growing soft-tissue tumor that commonly occurs as a bilateral infrascapular tumor in elderly women. This report is of a 60-year-old woman who presented with combined carpal tunnel syndrome and cubital tunnel syndrome due to an elastofibroma causing compression of the median and ulnar nerves. CASE REPORT We report the case of a 66-year-old woman with left-hand numbness, tingling along the fingers, sleep disturbance

[Ulnar nerve compression in Guyon's canal by an accessory abductor digiti minimi brevis muscle].

Compression syndromes of the ulnar nerve are more often located proximally in the cubital tunnel than distally in Guyon's canal. In many cases anatomic structures can be found that induce compression. These may be ganglia, tumours, vascular abnormalities, crossing fibrous bands or accessory muscles. The ulnar nerve has therefore to be revised all along its passage through Guyon's canal. When other causes are excluded, it might be necessary to excise an accessory muscle to decompress the ulnar nerve sufficiently.
