Unmasking the dilemma: Public health, safety, and the challenges of anonymity in emergency management and homeland security.

Masks occupy a unique and dual role in society, functioning as both critical tools for public health protection and enablers of anonymity, which can be exploited for deviant or criminal purposes. Historically, masks have evolved from their origins in plague prevention to modern applications in medical, public health, and societal contexts. While essential during health crises such as the coronavirus disease 2019 pandemic, their use also reveals unintended consequences, including fostering riskier behavior through the Peltzman Effect and enabling anonymity-driven disassociation described by Dei

A Qualitative Review of Office of Inspector General Complaints Submitted by Individuals Held in ICE Detention.

Growing evidence highlights medical mismanagement and gross negligence at United States (U.S.) Immigration and Customs Enforcement (ICE) detention facilities. Accountability at these facilities is predominately limited to annual inspections to determine compliance with specific detention standards and responses to formal grievances and/or complaints. The grievance process is particularly important as it provides an opportunity for individuals directly impacted to document and formalize negative experiences. To gain a better understanding of the conditions and violations faced by individuals de

Implementation and modeling of a Regional Hub Reception Center during mass evacuation operations.

When developing response plans in the aftermath of a catastrophic incident, jurisdictions often fail to conduct the necessary interdisciplinary planning needed to fully address the needs across jurisdictional borders. The Purdue Homeland Security Institute (PHSI) was selected by the City of Chicago Office of Emergency Management and Communications (OEMC) in 2010 to lead an effort to address planning across jurisdictional borders during mass evacuations following a catastrophic incident. Specifically, PHSI was chosen to lead the effort in developing a planning and implementation guide for stand

Acceptance and utilisation of the Incident Command System in first response and allied disciplines: an Ohio study.

In response to the terrorist attacks of September 11th, 2001, an effort was made to establish a common and uniform command structure for use by the nation's first responder organisations, as well as those disciplines generally expected to assist first responders during a major incident or disaster. The result was the issuance of the National Incident Management System1 or NIMS by the US Department of Homeland Security in 2004. Included in the NIMS document was an embracing of the Incident Command System or ICS, long utilised in the fire service for the effective management of emergency respons

Regulatory Overview and Considerations for Medical Countermeasures Developed for Radiological/Nuclear Incident Responses.

After the terrorist attacks on September 11 and the anthrax attacks in 2001, the United States Government (USG) implemented several pieces of legislation to enable the federal government to prepare for future threats, including managing and funding product development of medical countermeasures (MCMs). The Project BioShield Act of 20041 and the Pandemic and All Hazards Preparedness Act (PAHPA) of 20062 (with reauthorizations in 20133 and 20194) described and fine-tuned the scope and mission of different agencies in this effort. Domestic understanding of the revised International Health Regulations.

OBJECTIVE: The International Health Regulations (IHR) (2005) require countries to report potential public health emergencies of international concern to the World Health Organization. Given that in the U.S. federal system, disease surveillance and reporting is a state, territory, or local-level responsibility, we wanted to discern the level of familiarity at the state and local level with the international obligations for disease reporting under IHR (2005). METHODS: We conducted a multisectoral survey of states, commonwealths/territories, and localities to assess basic knowledge of IHR (2005)

Cationic host defense peptides; novel antimicrobial therapeutics against Category A pathogens and emerging infections.

Cationic Host Defense Peptides (HDP, also known as antimicrobial peptides) are crucial components of the innate immune system and possess broad-spectrum antibacterial, antiviral, and immunomodulatory activities. They can contribute to the rapid clearance of biological agents through direct killing of the organisms, inhibition of pro-inflammatory mediators such as lipopolysaccharide, and by modulating the inflammatory response to infection. Category A biological agents and materials, as classified by the United States National Institutes for Health, the US Centers for Disease Control and Preven

GABAA receptor subtype selectivity of the proconvulsant rodenticide TETS.

The rodenticide tetramethylenedisulfotetramine (TETS) is a potent convulsant (lethal dose in humans 7-10 mg) that is listed as a possible threat agent by the United States Department of Homeland Security. TETS has previously been studied in vivo for toxicity and in vitro in binding assays, with the latter demonstrating it to be a non-competitive antagonist on GABAA receptors. To determine whether TETS exhibits subtype selectivity for a particular GABAA receptor combination, we used whole-cell patch-clamp to determine the potency of TETS on the major synaptic and extrasynaptic GABAA receptors a

Active Monitoring of Travelers Arriving from Ebola-Affected Countries - New York City, October 2014-April 2015.

The Ebola virus disease (Ebola) outbreak in West Africa has claimed approximately 11,300 lives (1), and the magnitude and course of the epidemic prompted many nonaffected countries to prepare for Ebola cases imported from affected countries. In October 2014, CDC and the Department of Homeland Security (DHS) implemented enhanced entry risk assessment and management at five U.S. airports: John F. Kennedy (JFK) International Airport in New York City (NYC), O'Hare International Airport in Chicago, Newark Liberty International Airport in New Jersey, Hartsfield-Jackson International Airport in Atlan

Pediatric Outpatient Care After Operation Metro Surge in Minneapolis.

BACKGROUND: Operation Metro Surge, a joint initiative by the Department of Homeland Security and the United States Immigration and Customs Enforcement (ICE), significantly disrupted daily life in Minnesota. Despite numerous anecdotal reports, the impact and scale of this disruption to local health care systems had not been quantified. METHODS: An interrupted-time series analysis was used to assess the impact of ICE Metro Surge on missed appointments and reschedule requests in 2 hospital-based general pediatrics practices located on the Minneapolis and St Paul campuses of Minnesota's flagship

Persons Evaluated for 2019 Novel Coronavirus - United States, January 2020.

In December 2019, a cluster of cases of pneumonia emerged in Wuhan City in central China's Hubei Province. Genetic sequencing of isolates obtained from patients with pneumonia identified a novel coronavirus (2019-nCoV) as the etiology (1). As of February 4, 2020, approximately 20,000 confirmed cases had been identified in China and an additional 159 confirmed cases in 23 other countries, including 11 in the United States (2,3). On January 17, CDC and the U.S. Department of Homeland Security's Customs and Border Protection began health screenings at U.S. airports to identify ill travelers retur

Risk Assessment and Management of COVID-19 Among Travelers Arriving at Designated U.S. Airports, January 17-September 13, 2020.

In January 2020, with support from the U.S. Department of Homeland Security (DHS), CDC instituted an enhanced entry risk assessment and management (screening) program for air passengers arriving from certain countries with widespread, sustained transmission of SARS-CoV-2, the virus that causes coronavirus disease 2019 (COVID-19). The objectives of the screening program were to reduce the importation of COVID-19 cases into the United States and slow subsequent spread within states. Screening aimed to identify travelers with COVID-19-like illness or who had a known exposure to a person with COVI

Travel and Border Health Measures to Prevent the International Spread of Ebola.

During the 2014-2016 Ebola virus disease (Ebola) epidemic in West Africa, CDC implemented travel and border health measures to prevent international spread of the disease, educate and protect travelers and communities, and minimize disruption of international travel and trade. CDC staff provided in-country technical assistance for exit screening in countries in West Africa with Ebola outbreaks, implemented an enhanced entry risk assessment and management program for travelers at U.S. ports of entry, and disseminated information and guidance for specific groups of travelers and relevant organiz

A Comparative Analysis of the US Coast Guard Dental Emergency Visits.

INTRODUCTION: Identifying and analyzing dental emergencies among United States Coast Guard (USCG) personnel is critical, as the USCG is tasked with responding to natural disasters, narcotics interdiction, terrorism, and search-and-rescue missions. Dental Readiness Classifications (DRCs) are used by the USCG to monitor the dental health of personnel and assess force availability for USCG, Department of Homeland Security, and Department of Defense (DoD) missions. The goal of this study was to investigate the nature of dental emergencies in the USCG. MATERIALS AND METHODS: A retrospective review

Whole Genome Analysis of Injectional Anthrax Identifies Two Disease Clusters Spanning More Than 13 Years.

BACKGROUND: Anthrax is a rare disease in humans but elicits great public fear because of its past use as an agent of bioterrorism. Injectional anthrax has been occurring sporadically for more than ten years in heroin consumers across multiple European countries and this outbreak has been difficult to trace back to a source. METHODS: We took a molecular epidemiological approach in understanding this disease outbreak, including whole genome sequencing of Bacillus anthracis isolates from the anthrax victims. We also screened two large strain repositories for closely related strains to provide co

Second-Class Care: How Immigration Law Transforms Clinical Practice in the Safety Net.

This article examines how U.S. immigration law extends into the health care safety net, enacting medical legal violence that diminishes noncitizens' health chances and transforms clinical practices. Drawing on interviews with health care workers in three U.S. states from 2015 to 2020, I ask how federal citizenship-based exclusions within an already stratified health care system shape the clinical trajectories of noncitizens in safety-net institutions. Focusing specifically on cancer care, I find that increasingly anti-immigrant federal policies often reshape clinical practices toward noncitize
