Differentiating stress urinary incontinence from urge urinary incontinence.

Urinary incontinence (UI) is any involuntary leakage of urine and can be further defined according to the patient's symptoms or complaints. Stress urinary incontinence (SUI) is the complaint of involuntary leakage on effort or exertion, or on sneezing or coughing. Urge urinary incontinence (UUI) is characterized by the complaint of involuntary leakage accompanied by or immediately preceded by urgency. Mixed urinary incontinence (MUI) is the presence of both SUI and UUI symptoms. In order to effectively treat UI, an accurate diagnosis is necessary since treatment of SUI or UUI is very different

Flat Magnetic Stimulation for Urge Urinary Incontinence.

Background and Objectives: Strategies for overactive bladder syndrome (OAB) management involve, among others, strengthening the bladder outlet to suppress urgency and neuromodulating the sacral roots. Magnetic stimulation (MS) is a technology that involves an extracorporeal device that is able to provide an electromagnetic field specifically designed to interact with pelvic floor neuromuscular tissue. The resulting tissue electrical activity induces contraction of the pelvic muscle and neuromodulation of the S2-S4 sacral roots. Flat Magnetic Stimulation (FMS) is a relevant advancement involvin

Factors associated with urge urinary incontinence after surgery for stress urinary incontinence.

OBJECTIVE: : The objectives of the study were to estimate the severity of urge urinary incontinence (UUI) after surgery for stress incontinence and identify factors associated with symptom severity. METHODS: : Four hundred thirty-eight women who underwent surgery for stress incontinence over a 10-year period completed a mailed questionnaire protocol that included validated instruments to assess lower urinary tract function and mental and physical well-being. Bivariate analysis and generalized linear models were used to identify factors associated with severity of UUI as measured by the urge i

Tolterodine for the treatment of urge urinary incontinence.

INTRODUCTION: Overactive bladder (OAB) and its resultant urge urinary incontinence (UUI) are significant problems that medically, psychologically and financially affect people. The constellation of symptoms comprising OAB affects ∼ 16% of the adult population and its prevalence increases with aging. The typical class of medications used to treat OAB is antimuscarinics. AREAS COVERED: OAB medications, with a focus on tolterodine for the treatment of UUI are reviewed. A thorough review of English language literature using EMBASE/Medline and PubMed has been performed. EXPERT OPINION: Tolterodin

Management of coexistent stress and urge urinary incontinence.

Fifty-two patients with objective evidence of pressure equalization incontinence and detrusor instability were evaluated retrospectively to compare nonsurgical modes of therapy with retropubic surgery. Based on the patient's desire for surgery and her overall medical condition, 27 women were treated primarily with retropubic urethropexy (modified Burch procedure) and 25 with various combinations of oxybutynin, imipramine, and estrogen. Thirty-two percent of the patients treated medically were cured and 28% were markedly improved, whereas 59% of patients treated surgically were cured and 22% im

Pathophysiology of overactive bladder and urge urinary incontinence.

Storage symptoms such as urgency, frequency, and nocturia, with or without urge incontinence, are characterized as overactive bladder (OAB). OAB can lead to urge incontinence. Disturbances in nerves, smooth muscle, and urothelium can cause this condition. In some respects the division between peripheral and central causes of OAB is artificial, but it remains a useful paradigm for appreciating the interactions between different tissues. Models have been developed to mimic the OAB associated with bladder instability, lower urinary tract obstruction, neuropathic disorders, diabetes, and interstit

Urinary incontinence-assessment in women: stress, urge or both?

BACKGROUND: The aims of assessing urinary incontinence in women are to define the diagnosis, exclude other pathology and guide management. Treatment can be initiated when urinary incontinence is categorised as stress, urge or mixed incontinence. Once conservative measures have been exhausted, the management of stress incontinence is largely surgical, while that of urge urinary incontinence is largely medical. OBJECTIVE: This article discusses the clinical assessment of urinary incontinence in women with emphasis on the primary care assessment and indications for specialist referral. DISCUSSI

[Percutaneous posterior tibial nerve electrostimulation in urge urinary incontinence and faecal incontinence].

OBJECTIVE: The main aim of this study was to assess the efficacy of percutaneous posterior tibial nerve electrostimulation (P-PTNS) in urge urinary incontinence (UUI) and faecal incontinence (FI) refractory to first-line treatment. A secondary aim was to identify predictors of treatment response. To do this, we performed a 2-year analytical, longitudinal and prospective study in patients with UUI and FI treated with P-PTNS. MATERIAL AND METHODS: We included patients with UUI and FI who were treated with P-PTNS and who completed the bladder/faecal incontinence diary and severity and quality of

Actual treatment of overactive bladder and urge urinary incontinence.

Overactive bladder (OAB) is defined by its hallmark symptom, urgency. It can be associated with urge urinary incontinence (UUI), and dramatically impact the patients' quality of life. Etiologies of OAB are numerous, and under this common wording, virtually all the population is covered (men as well as women, patients with or without neurogenic disease, and all age categories). OAB and UUI management have been historically based on non-interventional therapies, antimuscarinics, and surgery. In the last decade, innovations in the treatment of this highly prevalent condition have been multiple, a

[Treatment of urinary urge incontinence with clenbuterol (author's transl)].

Ten female patients with motor and 16 with sensory urge incontinence were treated with clenbuterol, a beta 2-sympathicomimetic drug, over a period of at least 4 weeks with a dosage of 10 microgram t.i.d. Urodynamic volumes increased significantly in sensory urge incontinence during the first and maximal urinary urgency; the tonometric index clearly decreased. Autonomous detrusor contraction, characteristic of motor urge incontinence, could be decreased or suppressed by clenbuterol. Urodynamic pressure characteristics were equally reduced significantly at first and maximal urinary urgency. In c

A review of the quality-of-life aspects of urinary urge incontinence.

Urinary incontinence (UI) is prevalent and costly, occurring in 15 to 30% of the US population over the age of 60 years. Among people aged 15 to 64 years, UI occurs in 1.5 to 5% of men and 10 to 25% of women. Severe incontinence occurs in 6% of the general US population, and it is estimated that $US10 billion per year is spent in direct costs alone on care for these patients. This review presents a description of the various types of UI and describes the prevalence and costs of the condition. In addition, 3 approaches to assessing the impact of UI on quality of life are discussed, namely gener

Surgical treatment of mixed and urge urinary incontinence in women.

BACKGROUND/AIMS: The etiology of urge urinary incontinence is unknown. Pharmacological treatments are disappointing, since they are only slightly more effective than a placebo. In this study, we analyzed whether the surgical replacement of the uterosacral ligaments by an alloplastic tape can cure patients with mixed and urge urinary incontinence. METHODS: This study was a nonrandomized clinical trial. The study was performed in the Department of Gynecology of a general hospital and a university clinic. 135 women with urge or mixed urinary incontinence were operated on. The utero-sacral ligame

Predictors of outcomes in the treatment of urge urinary incontinence in women.

INTRODUCTION AND HYPOTHESIS: Women with urge predominant urinary incontinence received active intervention (drug therapy alone or combined with behavioral therapy) for 10 weeks, then stopped all therapy and were followed for 6 months more. In this planned secondary analysis, we aimed to identify predictors of therapeutic success at 10 weeks (> or =70% reduction in incontinence) and of ability to discontinue treatment and sustain improvements 6 months later. METHODS: Using data from 307 women, we performed logistic regression to identify predictors for outcomes described above. RESULTS: After
