[Vagus nerve tonus].

The majority of investigators claim that the cause of vagotomic tachycardia is switching off the vagus nerve inhibitory tone, according to other authors the sympathetic nervous system stimulation is responsible for it. The second hypothesis is confirmed by the fact that vagotomic tachycardia is arrested by surgical desympathization in animals. Our data confirm this statement. We have ascertained that vagotomic tachycardia may be decreased by a surgical intervention without desympathization in animals. Artificial lung ventilation exerts the same effect on vagotomic tachycardia. Vagus tone depen

Vagus Nerve and Vagus Nerve Stimulation, a Comprehensive Review: Part II.

The development of vagus nerve stimulation (VNS) began in the 19th century. Although it did not work well initially, it introduced the idea that led to many VNS-related animal studies for seizure control. In the 1990s, with the success of several early clinical trials, VNS was approved for the treatment of refractory epilepsy, and later for the refractory depression. To date, several novel electrical stimulating devices are being developed. New invasive devices are designed to automate the seizure control and for use in heart failure. Non-invasive transcutaneous devices, which stimulate auricu

Vagus Nerve and Vagus Nerve Stimulation, a Comprehensive Review: Part III.

Vagus nerve stimulation (VNS) is currently undergoing multiple trials to explore its potential for various clinical disorders. To date, VNS has been approved for the treatment of refractory epilepsy and depression. It exerts antiepileptic or antiepileptogenic effect possibly through neuromodulation of certain monoamine pathways. Beyond epilepsy, VNS is also under investigation for the treatment of inflammation, asthma, and pain. VNS influences the production of inflammatory cytokines to dampen the inflammatory response. It triggers the systemic release of catecholamines that alleviates the ast

Vagus Nerve and Vagus Nerve Stimulation, a Comprehensive Review: Part I.

The vagus nerve (VN), the "great wondering protector" of the body, comprises an intricate neuro-endocrine-immune network that maintains homeostasis. With reciprocal neural connections to multiple brain regions, the VN serves as a control center that integrates interoceptive information and responds with appropriate adaptive modulatory feedbacks. While most VN fibers are unmyelinated C-fibers from the visceral organs, myelinated A- and B-fiber play an important role in somatic sensory, motor, and parasympathetic innervation. VN fibers are primarily cholinergic but other noncholinergic nonadrene

Intrathoracic vagus nerve tumours.

Strickland, B. and Wolverson, M. K. (1974).Thorax, 29, 215-222. Intrathoracic vagus nerve tumours. Three new cases of intrathoracic vagus nerve sheath tumour are presented. Two were neurofibromas associated with generalized neurofibromatosis, the third a neurilemmoma. All three presented as tumours of the superior middle mediastinum on the left in close association with the aortic arch. Previously reported cases are reviewed with special reference to the anatomical position and mode of clinical presentation. The tumours most often occur on the proximal part of the nerve close to the aortic arc

Morphology of the human cervical vagus nerve: implications for vagus nerve stimulation treatment.

OBJECTIVES: The vagus nerve has gained a role in the treatment of certain diseases by the use of vagus nerve stimulation (VNS). This study provides detailed morphological information regarding the human cervical vagus nerve at the level of electrode implant. RESULTS: Eleven pairs of cervical vagus nerves and four pairs of intracranial vagus nerves were analysed by the use of computer software. It was found that the right cervical vagus nerve has an 1.5 times larger effective surface area on average than the left nerve [1,089,492 ± 98,337 vs 753,915 ± 102,490 μm(2), respectively, (P < 0.05)] a

First-in-human microelectrode recordings from the vagus nerve during clinical vagus nerve stimulation.

INTRODUCTION: Vagus nerve stimulation (VNS) is an effective treatment for people with drug-resistant epilepsy. However, its mechanisms of action are poorly understood, including which nerve fibers are activated in humans during VNS in typical clinical settings and which are required for clinical efficacy. In particular, there have been no intraneural recordings of vagus nerve fiber activation in awake humans undergoing chronic VNS. In this study, for the first time, we report recordings from the vagus nerve in this setting. METHODS: The recordings were performed using a sterile tungsten micro

Anti-inflammatory properties of the vagus nerve: potential therapeutic implications of vagus nerve stimulation.

Brain and viscera interplay within the autonomic nervous system where the vagus nerve (VN), containing approximately 80% afferent and 20% efferent fibres, plays multiple key roles in the homeostatic regulations of visceral functions. Recent data have suggested the anti-inflammatory role of the VN. This vagal function is mediated through several pathways, some of them still debated. The first one is the anti-inflammatory hypothalamic-pituitary-adrenal axis which is stimulated by vagal afferent fibres and leads to the release of cortisol by the adrenal glands. The second one, called the choliner

[Vagus Nerve Stimulation for Depression].

Vagus nerve stimulation (VNS) is used for treatment of refractory epilepsy and depression, particularly in western countries. In our country, VNS is covered by insurance for treatment of refractory epilepsy. In this study, we discuss the findings of previous studies that have reported VNS for depression, based on a history of VNS. Additionally, we have briefly described the mechanisms underlying the effects of treatment options used for depression.

Neurosarcoidosis affecting the vagus nerve.

OBJECTIVES: Our objective was to review patients who presented to our medical center with a diagnosis of neurosarcoidosis affecting the vagus nerve and to present symptoms, progression, treatments, and outcome. METHODS: We performed a chart review of patients who presented to our medical center in the past 10 years with a diagnosis of neurosarcoidosis specifically affecting cranial nerve X. RESULTS: A chart review of 53 patients revealed only 4 with findings suggestive of vagal neurosarcoidosis. All were male and had a mean age of 50 years (range, 42 to 57 years) at presentation of symptoms.

Intrathoracic schwannoma of vagus nerve.

Intrathoracic neurogenic tumors are relatively common mediastinal tumors generally located in the posterior mediastinum. The most common origin is the intercostal nerve or the sympathetic chain. These tumors rarely arise from vagus nerve in the anterior mediastinum. The English literature demonstrates a total of 29 cases, which are more often on the left than the right. Here, we present another case: a 41-year-old man with a history of chronic cough for 10 years, whose chest radiograph showed a homogenous mass in the right anterosuperior mediastinum. The chest CT scan revealed a well-defined m

Transcutaneous Auricular Vagus Nerve Stimulation.

Invasive vagus nerve stimulation (VNS) is an approved treatment for drug-resistant epilepsy. Besides recognized clinical efficacy in about 60% of patients, there are major drawbacks such as invasiveness and common side effects including hoarseness, sore throat, shortness of breath, and coughing. Invasive VNS applies electrical stimulation to the left cervical branch of the vagus nerve and excites thick-myelinated afferent nerve fibers. Peripheral vagus nerve afferent volley initiates brainstem activity in the nucleus of the solitary tract and provokes typical brainstem and cerebral activation

[Vagus nerve stimulation and depression].

Vagus nerve stimulation (VNS) is an old, yet new, option for treatment-resistant depression. Despite several clinical trials over the last 15 years showing a consistent benefit-risk balance of the technic, VNS still struggles to find its place in our therapeutic algorithms. This is especially true in France, where only a few surgeries have been performed nationwide, all in the last year. The reasons behind this lag are manifolds; (1) psychiatrists usually do not consider surgical treatments, even when they are minimally invasive and reversible, (2) early VNS trials stumbled on methodological d

Vagus nerve stimulation and cognition.

Vagus nerve stimulation (VNS) has been developed as an add-on treatment for patients with refractory epilepsy. Based on the clinical observation of improved cognition in many epilepsy patients who received VNS, we reviewed the recent literature for evidence concerning the cognitive effects of this treatment. From most of these studies it seems that, with currently used stimulation parameters, the effects on memory are only of theoretical importance. However, some animal studies suggest positive effects on specific modalities of memory function. In studies in epilepsy patients, there is no evid

Vagus nerve stimulation in depression.

Ever since the introduction of chemical and electrical convulsive treatment for psychiatric disorders in the 1930s and 1940s, biological techniques have been used extensively in the amelioration of a variety of psychiatric disorders. Techniques of recent vintage have included transcranial magnetic stimulation, deep brain stimulation and vagus nerve stimulation (VNS). Since VNS attenuates seizures in animal models, the treatment was initially developed and approved by the FDA for treatment of drug-resistant partial-onset epilepsy. Additional data, including the known neuroanatomy of the vagus n
