Addressing Health-Related Social Needs: Value-Based Care or Values-Based Care?

Health-related social needs, such as food insecurity, housing instability, and lack of transportation, are associated with worse health outcomes, and are increasingly the focus of health-related social needs interventions within healthcare. Adoption of health-related social needs interventions is often justified by the potential to reduce healthcare costs. However, this can present a conundrum to clinicians. Physicians are often more accustomed to justifying clinical innovation based on improvements in health, in accord with the fundamental values of the medical profession, which include using

Value-Based Health Care in Undergraduate Medical Education.

PROBLEM: Value-based health care (VBHC) is an innovative framework for redesigning care delivery to achieve better outcomes for patients and reduce cost; however, providing students with the skills to understand and engage with these topics is a challenge to medical educators. APPROACH: Here, the authors present a novel, VBHC curriculum integrated into a required course for post-core clerkship students-launched in 2018 at Harvard Medical School and taught in conjunction with Harvard Business School faculty-that highlights key principles of VBHC most relevant to undergraduate medical education

Cutting the Stone: Health Defined in the Era of Value-based Care.

The immune system contributes to the maintenance of health by preventing and limiting the clinical consequences of infections by pathogenic microorganisms. During the evolution of Homo sapiens, those with the fittest immune system survived. The immune system of Homo sapiens was further improved and adapted by admixture with Neanderthal genes. Nowadays, the human immune system provides adequate protection against the majority of infections. For some 20 infectious diseases, the immune system needs to be improved by vaccination. Vaccination is the number one value-based healthcare intervention an

Evidence-Based Behavioral Health in Value-Based Primary Care: Aligning Incentives to Accelerate Adoption of Collaborative Care.

Behavioral health (BH) is a critical component of value-based primary care. However, access to BH services is a challenge, with a limited, geographically concentrated supply of providers. The Collaborative Care Model (CoCM) is an evidence-based, cost-effective way for value-based primary care leaders to deliver high-quality BH services. Despite a robust clinical and implementation science evidence base, real-world adoption of CoCM remains limited. A key barrier to CoCM adoption is a one-size-fits-all fee-for-service reimbursement methodology, which fails to adequately capture the value of CoCM

A Value-Based Health Care Maternity Program and Perinatal Outcomes in Brazil.

The Brazilian private health sector is characterized by cesarean delivery rates approaching 88% and fragmented maternity care models, contributing to preventable perinatal complications and high health care costs. Value-based health care (VBHC) frameworks offer a structured approach to aligning care delivery with outcomes that matter to patients. The authors conducted a propensity score-matched cohort study using data from 5477 consecutive pregnancies delivered between January 2022 and March 2026 at Hospital and Maternity São Joaquim, Franca, São Paulo, Brazil. The matched analysis was restric

IMPORTANCE: Medicaid patients with mental illness comprise one of the most high-need and complex patient populations. Value-based reforms aim to improve care, but their efficacy in the Medicaid program is unclear. OBJECTIVE: To investigate if New York state's Medicaid value-based payment reform was associated with improved utilization patterns for patients with mental illness. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used a difference-in-differences analysis to compare changes in utilization between Medicaid beneficiaries whose outpatient practices participated in v

Prehabilitation: A Catalyst for Transforming Toward Value-Based, Personalized Perioperative Health and Care.

The growing strain on global healthcare systems, driven by aging populations, rising prevalence of chronic diseases, and workforce shortages, has increased interest in strategies that enhance perioperative outcomes and healthcare sustainability. From this perspective, prehabilitation-a proactive, multimodal approach to enhance patients' functional, nutritional, and psychological status prior to surgery-has gained attention as a potential contributor to value-based, personalized care. This study aims to synthesize mechanistic rationale, clinical evidence, and system-level considerations for pre

Risk Stratification in Orthopaedic Surgery: An Important Adjustment for Value-Based Health Care and Quality Measurement.

➢ Risk stratification in orthopaedic surgery is complex and depends on the outcome of interest and multiple interdependent factors. Effective risk stratification has uses for limiting and predicting adverse events in patients undergoing discretionary surgery, avoiding the penalization of surgeons for operating on candidates whose health is situated in more difficult circumstances, and ensuring that inordinate attention is not placed on discrete musculoskeletal pathophysiology when there are other pressing health priorities.➢ For individual patient decision-making, no comprehensive risk-stratif

Mobilizing the U.S. Military's TRICARE Program for Value-Based Care: A Report From the Defense Health Board.

The U.S. Military Health System spends about $50 billion annually to provide care to 9.6 million active duty service members, retirees, and their families through its TRICARE health plans. TRICARE follows the predominant payment model in the USA-fee-for-service-although the Department of Defense (DoD) and Congress encourage and mandate a move toward alternative payment models-mainly, fee-for-value. For the next TRICARE contracts which will begin in 2023, the DoD asked its health-focused federal advisory committee, the Defense Health Board (DHB), to recommend how best to assess and prioritize l

Revisiting the Eight Basic Payment Methods in Health Care: A Contemporary Framework for Value-Based Policy and Practice in the United States.

BACKGROUND: Payment reform is a central policy lever for influencing cost, quality, and access in the United States health care system. Despite decades of reform activity, confusion persists regarding how modern reimbursement models differ, overlap, and redistribute financial risk, particularly as hybrid arrangements proliferate across payers. This update addresses structural payment layering, AI-enabled coding intensity, and the dominance of Medicare Advantage, developments not present at the time of Quinn's original framework. OBJECTIVE: To update Quinn's Eight Basic Payment Methods to refl

Association Between Federal Value-Based Incentive Programs and Health Care-Associated Infection Rates in Safety-Net and Non-Safety-Net Hospitals.

IMPORTANCE: In the US, federal value-based incentive programs are more likely to penalize safety-net institutions than non-safety-net institutions. Whether these programs differentially change the rates of targeted health care-associated infections in safety-net vs non-safety-net hospitals is unknown. OBJECTIVE: To assess the association of Hospital-Acquired Condition Reduction Program (HACRP) and Hospital Value-Based Purchasing (HVBP) implementation with changes in rates of targeted health care-associated infections and disparities in rates among safety-net and non-safety-net hospitals.
