Technology of flushing vascular access devices.

Maintenance of catheter lumen patency is an ongoing challenge. Catheter flushing is the primary nursing intervention used to prevent lumen occlusion from thrombotic and precipitate causes. The catheter and all devices attached to it must be regarded as a system in which each component directly affects the others. The technology of catheter flushing includes the flush solution itself, the source of these solutions, syringe design, mechanical pumps, needleless injection systems, and the design of the catheter. Effective catheter flushing is a combination of a technique and technology that requir

HeRO Vascular Access Device.

Chronic hemodialysis via a tunneled dialysis catheter (TDC) is associated with a high incidence of infectious complications and increased mortality and, therefore, should only be considered when all other options for vascular access are exhausted. The Hemoaccess Reliable Outflow (HeRO) Vascular Access Device (Hemosphere, Inc., Minneapolis, MN) is an alternative to the TDC. Early results suggests that the infectious complications are significantly less for the HeRO device when compared to the TDC, and the secondary patency for the device approximates that for prosthetic accesses. The device can

Autonomous Vascular Access Devices: Current Solutions and Future Challenges.

INTRODUCTION: Central venous cannulation is essential for life-saving interventions including resuscitation of critically ill patients, hemodynamic monitoring, endovascular intervention, pressor therapy, dialysis and apheresis, and extracorporeal life support. Cannulation of major arteries is required for cardiac catheterization, endovascular interventions such as arterial stenting or aneurysm repair, as well as emergency interventions such as intra-aortic balloon pumps and extracorporeal membrane oxygenation (ECMO). Both venous and arterial cannulation are commonly performed by specialized pr

Management of infection and occlusion associated with vascular access devices.

OBJECTIVE: To provide an overview of the etiology, assessment, diagnosis, and management of infections and occlusions that occur with venous access devices (VADs). CONCLUSIONS: The two major complications of VADs are infections and occlusions. Several strategies have been attempted to prevent and treat infections and occlusions; however, one specific method has yet to be determined. As a result, major controversies exist regarding the best method to manage these complications. Controlled randomized studies are needed to examine each type of VAD in relation to preventive and treatment strategi

Complications associated with an implantable vascular access device.

PURPOSE: Implantable vascular access devices (ports) are well accepted in the management of many pediatric conditions. Modifications have improved port function, patient satisfaction, and enhanced compatibility with imaging studies. We reviewed our experience with a port system and identified unique mechanical complications. METHODS: From 1998 to the present, 301 patients underwent 296 port insertions and 175 port removals. We assessed medical records, radiographs, and operative findings. The 6.6F MRI Low-Profile Implanted Port (Bard Access Systems, Salt Lake City, Utah) was used almost exclu

Vascular access complications after catheter ablation of ventricular arrhythmias: Impact of vascular closure devices.

BACKGROUND: Vascular access site complications are the most frequent complications of percutaneous catheter ablation (CA) of ventricular arrhythmias (VAs). Whether arterial/venous vascular closure devices (VCDs) prevent vascular complications is unknown. OBJECTIVE: We investigated the benefit of VCDs in patients undergoing CA of VAs. METHODS: Consecutive CA of VAs were included (2018-2022). Vascular accesses were obtained with ultrasound guidance. At the discretion of the operator, arterial and/or venous VCDs were used. Cases were divided into 3 groups: no use of VCDs for any arterial/venous

Development of a polyurethane percutaneous access device for long-term vascular access.

The percutaneous placement of intravascular devices creates a portal for microbial invasion that can result in local infections or septicemia. In nature, resistance to "exist site" infection in percutaneous organs, such as teeth, is prevented by a dense collagen/epithelial barrier. A new percutaneous access device has been developed that incorporates a porous polyurethane "button" at the subdermal level. This device promotes the development of a collagen/epithelial interface, thus inhibiting sinus formation. Twelve percutaneous access devices (PCADS) were implanted in calves; eight devices wer

Development of a Modular Tissue Phantom for Evaluating Vascular Access Devices.

Central vascular access (CVA) may be critical for trauma care and stabilizing the casualty. However, it requires skilled personnel, often unavailable during remote medical situations and combat casualty care scenarios. Automated CVA medical devices have the potential to make life-saving therapeutics available in these resource-limited scenarios, but they must be properly designed. Unfortunately, currently available tissue phantoms are inadequate for this use, resulting in delayed product development. Here, we present a tissue phantom that is modular in design, allowing for adjustable flow rate

Optiflow anastomotic device for hemodialysis vascular access creation.

The need to have consistent methods and consistent technique to optimize hemodialysis outcomes is behind the concept of the Optiflow™ device. This device was created to allow for consistency in size of the arterial anastomosis and consistency in angle of the vein to the artery at the anastomosis. Early data suggest that allowing these two technical entities can improve outcomes in regards to flow and maturity in arteriovenous fistula creation. This article is a summary of early data that demonstrate the impact the Optiflow device on brachial cephalic fistulas.

Experience with a needleless vascular access device (Hemasite) for hemodialysis.

Experience with 35 needleless implants (Hemasite) in 27 patients over the past 27 months has shown that this device can provide immediately available painless access in patients lacking adequate veins for a direct internal fistula for hemodialysis. The cumulative patency rate of 46% at two years is comparable to that of polytetrafluoroethylene grafts. The major complication was infection which contributed to graft loss in one third of patients. Obese female patients were particularly susceptible to infection and had a worse graft prognosis. Long-term results with a graftless model were poor.

The end stage of dialysis access: femoral graft or HeRO vascular access device.

BACKGROUND: Maintaining and establishing vascular access in end-stage renal disease (ESRD) patients is complicated when they are poor candidates for traditional upper extremity access. Our objective was to compare our experience with 2 alternative dialysis accesses, the femoral arteriovenous graft (fAVG) and the Hemodialysis Reliable Outflow (HeRO), in patients with limited remaining options. METHODS: A single institution, retrospective review of ESRD patients with fAVG or HeRO placed between May 2009 and February 2013 was performed. Adult patients were selected by reviewing all arteriovenous

Vascular access in neonates and infants--indications, routes, techniques and devices, complications.

Venous cannulation has been in regular use in neonates since the 1940s. This was at first through the umbilical vein, but the frequency of complications lead to other central and peripheral routes being used for infusion of fluid, nutrients and drugs. Today, peripheral venous access is preferred except for high volume fluid resuscitation, reliable infusion of irritant drugs and long-term parenteral nutrition. Intraosseous infusion provides a reliable alternative to peripheral veins for rapid infusion of fluid. Long, thin silastic catheters can be inserted through a peripheral venous cannulae f

Management of totally implantable vascular access devices in patients with cystic fibrosis.

Cystic fibrosis (CF) is a genetic disease associated with recurrent lung infections, that represent a major cause of mortality and morbidity. Cystic fibrosis requires frequent antibiotic treatments, sometimes by mouth or via aerosol but often via the intravenous route. Totally implanted venous access devices (ports) allow an easy and safe vascular access for unlimited periods of time, and they can be used in CF to administer antibiotics and other i.v. infusions; if compared to external central venous catheters, ports are better tolerated, since they permit almost unlimited physical activity an
