[Velopharyngeal insufficiency in children].

Velopharyngeal insufficiency (VPI) represents an incomplete closure between the soft palate and the posterior pharyngeal wall. Its etiology can be anatomical (cleft palate), neurologic, or iatrogenic (after adenoidectomy). The evaluation of a VPI begins with a through speech and language assessment and can be complemented by instrumental investigations. VPI treatment relies on its early identification, followed by a specific speech therapy management. Surgery is performed in case of no improvement with speech therapy or in case of an anatomical defect not allowing the child to improve. IVP man

Surgery for velopharyngeal insufficiency.

Velopharyngeal inadequacy may be learned, neurologic, or anatomic in origin. Velopharyngeal insufficiency specifically refers to an anatomic deficiency, which subsequently impairs resonant control of speech and intraoral pressure for orally directed speech sounds. Preoperative speech therapy is useful, and in some children may provide definitive treatment. However, surgery is the foundation of effective treatment in most patients with anatomic defects and, when used appropriately, has been shown to result in resolution of velopharyngeal insufficiency in 62-98% of cases. In this chapter, the au

Velopharyngeal insufficiency following adenoidectomy.

Velopharyngeal insufficiency (VPI) is a well recognized but rare complication of adenoidectomy. Twenty children with this condition were seen and assessed at Great Ormond Street Hospital between 1993 and 2000. The commonest aetiology was occult submucous cleft palate (n = 5) but there was a wide range of other causes. Two children with severe behavioural disorders and normal palates developed mild symptoms, an aetiology not previously reported. Only two children had a classical submucous cleft palate. Nine children required surgical intervention and three revision procedures. Of the 15 treated

Surgical management of velopharyngeal insufficiency.

The primary goal of cleft palate repair is to create an anatomically and functionally intact palate while minimally affecting maxillary growth and development. In order to produce normal speech, a child must have velopharyngeal competence, defined as the ability to completely close the velopharyngeal sphincter that separates the oropharynx and nasopharynx. The absence of this ability, termed velopharyngeal insufficiency (VPI), is seen in a wide range of patients following primary cleft palate repair. This article discusses patient assessment, treatment options, and the surgical management of V

Adult pharyngoplasty for velopharyngeal insufficiency.

Nine adults underwent superiorly based pharyngeal flaps for the treatment of severe velopharyngeal insufficiency (VPI). The etiology of the VPI was failed pediatric cleft palate repair in eight and myasthenia gravis in the ninth. All patients were evaluated by a speech therapist pre-operatively. There were no significant early or late postsurgical complications. After extensive speech therapy all patients were noted subjectively and objectively to have a marked reduction in hypernasality and nasal air escape and improved speech intelligibility. The healed fibrotic flaps appeared to be adynamic

Management of noncleft velopharyngeal insufficiency.

PURPOSE OF REVIEW: Velopharyngeal insufficiency in the absence of an overt cleft-palate is a less common and often missed cause of a resonance disorder. The purpose of this manuscript is to provide the reader with an overview of the clinical assessment. Highlight the need for multidisciplinary involvement. Discuss the process of decision-making related to a repair and finally comment on the preoperative, intra-operative, and postoperative considerations. RECENT FINDINGS: With the advent of small calibre videonasendoscopes, evaluation of the size, location, and closure pattern of the velophary

Velopharyngeal insufficiency: diagnosis and management.

PURPOSE OF REVIEW: Journal articles relevant to the diagnosis and treatment of velopharyngeal insufficiency were reviewed. All studies ascertained by PubMed search were included. RECENT FINDINGS: Studies reported on the application of magnetic resonance scanning, reliability tests of the International Working Group diagnostic protocol, the use of nasometry, and techniques designed to assess the function of the velopharyngeal mechanism. Treatment studies focused on outcomes in small samples of cases and complication rates from pharyngeal flap. One study discussed ineffective speech therapy pro

Velopharyngeal insufficiency following palatine tonsillectomy.

Although adenotonsillectomy is usually considered a minor operation, numerous uncommon but severe complications have been described. Even tonsillectomy alone can cause velopharyngeal insufficiency (VPI). We describe two cases in which severe VPI was noted after palatine tonsillectomy was performed because of recurrent peritonsillar abscesses. The patients underwent clinical examination, nasalance measurements, videonasopharyngoscopy and videofluoroscopy. Findings in both patients were consistent with lesions of branches of the vagus and glossopharyngeal nerves through lingual rami, while one o

Velopharyngeal insufficiency in hemifacial microsomia.

One-third of a group of 18 patients with hemifacial microsomia demonstrated velopharyngeal insufficiency of a significant degree. The ones with velopharyngeal insufficiency tended to have more severe soft tissue and skeletal deformities of the maxillary-malar complex, associated with a total unilateral palatal paralysis. Cinefluoroscopy was not helpful in diagnosing the velopharyngeal insufficiency. Fiberoptic nasopharyngoscopy, in contrast, proved to be an excellent diagnostic tool. We suggest that patients with hemifacial microsomia have a careful, periodic speech evaluation as an essential

Preoperative velopharyngeal morphology in older cleft palate patients with postoperative velopharyngeal closure versus velopharyngeal insufficiency.

The purpose of this study was to investigate the preoperative morphology of velopharyngeal structures in older patients with cleft palate and to evaluate which structure(s) could distinguish velopharyngeal function. The investigators implemented a retrospective study and 66 patients whose palate was repaired by 1 surgeon with the same technique were selected, including 38 boys and 28 girls, aged from 5 years and 1 month to 28 years and 6 months. All these patients were taken lateral preoperative cephalograms at rest and during phonation of /i/. Fifteen lengths, 3 ratios, and 1 angle variable w

Otologic manifestations in congenital velopharyngeal insufficiency.

A study of ear disease in children with congenital velopharyngeal insufficiency (VPI) was undertaken. A retrospective chart analysis of 80 cases of VPI formed the basis of this review. Thirty-seven percent of the children had evidence of middle ear disease, with middle ear effusion and recurrent otitis media being the most frequent findings. Thirty-one percent had abnormal audiograms, with 20% having a conductive or mixed hearing loss. Overall, 46% of the children had otologic manifestations, indicating the need for the pediatrician and the otolaryngologist together to be actively involved in

Pediatric otolaryngological relations of velopharyngeal insufficiency.

On the basis of his examinations, the author presents data and gives his opinion on the classification, etiopathogenesis, diagnostics, phonosurgery, and therapy of the otological complications of velopharyngeal insufficiency (VPI). VPI may be organic or functional, produced by congenital or acquired causes, due to paresis or local disorders, but most frequently occurs as a result of cleft palate. Usually, diagnosis is evident; however, the verification of the milder cases of anatomic disproportion and/or muscular dysfunction is sometimes difficult. For the examination of dubious cases the auth

Evaluation of sites of velopharyngeal structure augmentation in dogs for improvement of velopharyngeal insufficiency.

BACKGROUND: Velopharyngeal structure augmentation methods are used as alternatives to velopharyngeal plasty. Anatomic sites of implantation/injection vary widely due to a lack of standardized criteria. Here, we experimentally investigated optimal sites of velopharyngeal structure augmentation via saline injection in dogs as they naturally exhibit velopharyngeal insufficiency (VPI). METHODS: Velopharyngeal structure augmentation was performed on 10 beagles (age range: 20-24 months; weight range: 9-12 kg). Saline containing 1/80,000 epinephrine was injected intraorally in 1-mL increments into t

A Treatment Protocol for Velopharyngeal Insufficiency and the Outcome.

BACKGROUND: A simple algorithm is applied to treat velopharyngeal insufficiency. The purpose of this study was to assess its success rate and complications. METHODS: The diagnosis includes speech perceptual assessment and nasopharyngoscopy, focusing on velopharyngeal closure ratio. The treatment is composed of a double-opposing Z-plasty for marginal velopharyngeal insufficiency or a pharyngeal flap for moderate to severe velopharyngeal insufficiency. A retrospective chart review was conducted for 84 consecutive nonsyndromic postpalatoplasty patients undergoing velopharyngeal insufficiency sur
