Application of neurally adjusted ventilatory assist in ventilator weaning of infants ventilator weaning.

BACKGROUND: To analyze the application of neurally adjusted ventilatory assist in ventilator weaning of infants. METHODS: A total of 25 infants (15 boys and 10 girls) who were mechanically ventilated by PICU in Hubei Maternal and Child Health Hospital were selected as the study subjects. After the improvement of the basic disease, regular spontaneous breathing, and the withdrawal of the ventilator, all the children obtained the electrical activity of the diaphragm (EAdi) signal. Then, each child was given CPAP and NAVA mode mechanical ventilation 1 h before the withdrawal of the ventilator. E

Cardiopulmonary interactions during ventilator weaning.

Weaning a critically-ill patient from the ventilator is a crucial step in global management. This manuscript details physiological changes induced by altered heart-lung interactions during the weaning process, illustrates the main mechanisms which could lead to weaning failure of cardiac origin, and discuss a tailored management based on the monitoring of changes in central hemodynamics during weaning. The transition from positive-pressure ventilation to spontaneous breathing results in abrupt hemodynamic and metabolic changes secondary to rapidly modified heart-lung interactions, sudden chang

Novel mechanical ventilator weaning predictive model.

Mechanical ventilation (MV) is a common life support system in intensive care units. Accurate identification of patients who are capable of being extubated can shorten the MV duration and potentially reduce MV-related complications. Therefore, prediction of patients who can successfully be weaned from the mechanical ventilator is an important issue. The electronic medical record system (EMRs) has been applied and developed in respiratory therapy in recent years. It can increase the quality of critical care. However, there is no perfect index available that can be used to determine successful M

Evidence-based ventilator weaning and discontinuation.

Ventilator management of a patient who is recovering from acute respiratory failure must balance competing objectives. Discontinuing mechanical ventilation and removing the artificial airway as soon as possible reduces the risk of ventilator-induced lung injury, nosocomial pneumonia, airway trauma from the endotracheal tube, and unnecessary sedation, but premature ventilator-discontinuation or extubation can cause ventilatory muscle fatigue, gas exchange failure, and loss of airway protection. In 1999 the McMaster University Outcomes Research Unit conducted a comprehensive evidence-based revie

[Weaning from mechanical ventilation. Weaning categories and weaning concepts].

The international classification of three weaning categories (simple weaning, difficult weaning, prolonged weaning) has been modified in the German weaning guidelines: the group of prolonged weaning has been subclassified into weaning without noninvasive ventilation (NIV), weaning with NIV, if necessary with continuing NIV in the form of home mechanical ventilation, and weaning failure.Strategies to prevent prolonged weaning comprise daily interruption of sedation, daily screening of capability of spontaneous breathing by a spontaneous breathing trial (SBT) and early implementation of NIV inst

[Effectiveness of intermittent self-ventilation after ventilator weaning].

The essential cause for long-term mechanical ventilation with unweanability from respirator is chronic failure of the inspiratory muscles. Principally two different causes exist for chronic respiratory failure: Primary pulmonary diseases with overload or load imbalance of primarily uncompromised respiratory muscles, and neuromuscular diseases with a significant decrease in respiratory muscle capacity. Intermittent nocturnal ventilation (INV) leads to recovery by unloading the respiratory pump. In the present retrospective study we examined the value of INV in the "post-weaning-phase" for previ

To Wean or Not to Wean: A Practical Patient Focused Guide to Ventilator Weaning.

Since the inception of critical care medicine and artificial ventilation, literature and research on weaning has transformed daily patient care in intensive care units (ICU). As our knowledge of mechanical ventilation (MV) improved, so did the need to study patient-ventilator interactions and weaning predictors. Randomized trials have evaluated the use of protocol-based weaning (vs. usual care) to study the duration of MV in ICUs, different techniques to conduct spontaneous breathing trials (SBT), and strategies to eventually extubate a patient whose initial SBT failed. Despite considerable mi

Ventilator-Weaning Pathway Associated With Decreased Ventilator Days in Pediatric Acute Respiratory Distress Syndrome.

OBJECTIVES: There is limited evidence on the impact of protocolized ventilator weaning in pediatric acute respiratory distress syndrome, despite utilization in clinical trials and clinical care. We aimed to determine whether protocolized ventilator weaning shortens mechanical ventilation duration and PICU length of stay in pediatric acute respiratory distress syndrome survivors. DESIGN: Secondary analysis of a prospective pediatric acute respiratory distress syndrome (Berlin definition) cohort from July 2011 to June 2019 analyzed using interrupted time series analysis pre- and postimplementat

Ventilator weaning outcomes in chronic respiratory failure in children.

The purpose of this study was to describe mechanical ventilation weaning outcomes for children with chronic respiratory failure discharged from one of six post-acute rehabilitation facilities. Demographic, clinical and outcome data were collected from the medical record. Forty-four children were included in this prospective series; 20 (45%) were weaned off the ventilator at discharge. Children required significantly lower levels of ventilatory support at discharge than admission. Hourly use on the ventilator decreased from admission to discharge for the full cohort and for the subgroup who req

Criteria for using a nurse-led ventilator-weaning protocol.

Mechanical ventilation is a process by which gases are moved into and out of the lungs by means of a ventilator, a machine that delivers a controlled flow of gas to a patient's airway. The reasons for mechanical ventilation include respiratory arrest, acute lung injury, critical illness and respiratory support following surgery.

Weaning from mechanical ventilation.

Invasive mechanical ventilation can be lifesaving for patients with acute respiratory failure, but numerous complications have been identified. Therefore, once clinical improvement has occurred, emphasis is placed on quickly weaning (ie, liberating) the patient from mechanical ventilation. Weaning can be subdivided into 2 components: readiness testing and progressive withdrawal. Traditionally, both clinical factors and weaning predictors have been used to assess readiness for spontaneous breathing trials, which can be carried out using a T-piece or a low level of ventilatory support. The role

Weaning from mechanical ventilation.

Mechanical ventilation is often required by very preterm infants with respiratory failure, even if invasive respiratory support is related to lung injury and adverse neurologic outcomes. The exposure to mechanical ventilation should be therefore limited. Optimal extubation however remains challenging, as approximately 30% of intubated preterm infants fails attempted extubation due to poor respiratory drive, atelectasis, residual pulmonary function abnormalities or intercurrent illness. This review outlines the advantages of different weaning strategies that should be considered by neonatologis

Intermittent mandatory ventilation and weaning.

Oxygen, PEEP, and mechanical ventilatory therapy should be administered to patients in varying amounts and should be removed gradually and independently. The method of determining optimal PEEP, oxygen, and ventilation is not unlike that recommended for many other therapies. Nine years of prospective evaluation have demonstrated the numerous clinical advantages of this technique, and relatively few complications have been associated with it. Reduced FIO2 may promote resistance to atelectasis and allow rapid discontinuation of mechanical ventilation and PEEP. Similarly, optimal levels of PEEP ma

[Application of dexmedetomidine in children with agitation during ventilator weaning].

OBJECTIVE: To study the clinical effect and safety of dexmedetomidine in children with agitation during ventilator weaning. METHODS: A prospective open observational study was performed for children who were admitted to the intensive care unit and experienced mechanical ventilation between March 2017 and August 2018. They were given dexmedetomidine due to the failure in the spontaneous breathing test (SBT) caused by agitation. A sedation-agitation scale score of ≥5 was defined as agitation. The children were observed in terms of the sedation state at 0.5, 1, 2, 6, and 12 hours after the admin

Protocolized ventilator weaning verses usual care: A randomized controlled trial.

BACKGROUND: Protocolized ventilator weaning (PW) strategies utilizing spontaneous breathing trials (SBTs) result in shorter intubation duration and intensive care unit (ICU) length of stay (LOS). We compared respiratory therapy (RT)-driven PW versus usual care (UC) as it pertains to physiologic respiratory parameters, intubation duration, extubation success/reintubation rates, and ICU LOS. METHODS: prospective, multicentric, randomized controlled trial was performed in closed medical and surgical ICUs with 24/7 in-house intensivist coverage at six academic medical centers in a resource-limite

Characteristics associated with successful weaning in ventilator-dependent preterm infants.

Eighteen ventilator-dependent preterm infants with hyaline membrane disease were studied for 24 hours before and after an attempt at extubation. All were treated with theophylline prior to weaning and achieved average levels of 8.9 +/- 1.7 micrograms/ml (49 +/- 9 mumol/liter) in 13 successfully weaned infants and 8.4 +/- 1.1 micrograms/ml (47 +/- 6 mumol/liter) in 5 infants not extubated, p > 0.05. Infants successfully weaned were significantly (p < 0.01) older, more mature (29 +/- 1 versus 26 +/- 2 weeks' gestational age) and heavier (1107 +/- 236 versus 1016 +/- 256 gm) than infants not succ

Helmet noninvasive ventilation for weaning from mechanical ventilation.

We saw a patient who presented with carbon dioxide narcosis and acute respiratory failure due to an exacerbation of chronic obstructive pulmonary disease. We intubated and 12 hours later he had recovered consciousness and could cooperate with noninvasive ventilation, at which point we extubated and used a helmet to provide noninvasive positive-pressure ventilation in assist/control mode, and then during the ventilator-weaning process, pressure support, and finally continuous positive airway pressure. The patient had no complications from the helmet, and he was discharged from intensive care 48

[Role of noninvasiveness ventilation in the weaning phase of mechanical ventilation].

During the last decade, noninvasive ventilation has proved to be a valuable alternative to endotracheal intubation in many patients with acute respiratory failure, allowing a reduction in morbidity, mortality and length of intensive care and hospital stay compared to conventional mechanical ventilation. Recent data from clinical studies suggest that these beneficial effects could also be extended to the weaning phase of mechanical ventilation, by reducing, in some patients at any rate, its duration and complication rate. The present paper focuses on some of these promising new aspects of nonin
