Poliomyelitis can affect patients with severe hypoxia and hypercapnia. Historically, most of these patients were supported by mechanical ventilation via tracheostomy. However, this procedure can lead to serious complications. Subsequently, non-invasive alternative procedures have been introduced that provide mechanical respiratory support; these include negative pressure ventilators and positive pressure ventilation via a nasal, oral, and oronasal interface. Although these ventilators are effective, various limitations have been noted. The strapless oronasal interface positive pressure ventila

Home mechanical ventilators (HMV) are positive pressure devices that allow patients with chronic respiratory failure to live in the comfort of their own homes. HMV are more advanced than standard respiratory assist devices, providing features including internal and external batteries for daytime use outside the house, unique modes such as volume-assured pressure support with auto expiratory positive airway pressure and mouthpiece ventilation, and more alarms to provide safety for patients with severe disease. The process of prescribing HMV in the United States can be complex.

ObjectiveTo evaluate the technical performance and patient-ventilator synchronization of different invasive mechanical ventilators.MethodsSix ventilators were tested using the ASL5000 active lung simulator. Ventilators were categorized by air supply type: central (Dräger V600, Maquet Servo-u, Mindray SV800) and turbine-based (Savina 300, SV300, Resvent RS300). The measured parameters included tidal volume (VT), plateau pressure (Pplat), positive end-expiratory pressure (PEEP), peak pressure (Ppeak), time to reach 90% of inspiratory pressure (T (90%)), triggering delay (DT), and leak compensati

Hypoxia-related cardiovascular complications and unnecessary monitor and ventilator alarms are well-known problems during tracheobronchial suctioning. Preoxygenation together with temporary muting of acoustic alarms for tracheobronchial suctioning are provided by a single operational control of the ventilator "Evita", software release 9 (Drägerwerk AG). This integrated function was evaluated in medical and ergonomic respects. METHODS. Thirty tracheobronchial suctioning procedures each, with or without preoxygenation, were evaluated. The subjects were patients in our anesthesiological intensive

Clinicians who care for patients with chronic respiratory failure are faced with complex medical decisions when initiating and managing home mechanical ventilation. The technological advances and complexities of home mechanical ventilators (HMVs) have outpaced the education and resources provided to clinicians who manage this unique patient population. Many clinicians are familiar with 1 brand and depend on medical equipment suppliers when prescribing and managing home ventilators. A recent national safety recall in the United States of 1 of the most commonly used HMVs has led to an increase i

OBJECTIVE: In a contaminated environment, respiratory protection for ventilator dependent patients can be achieved by attaching a chemical, biological, radiological, or nuclear (CBRN) filter to the air intake port of a portable ventilator. We evaluated the effect of the filter on battery performance of four portable ventilators in a laboratory setting.
METHODS: Each ventilator was attached to a test lung. Ventilator settings were: assist control (AC) mode, respiratory rate 35 bpm, tidal volume 450 ml, positive end-expiratory pressure (PEEP) 10 cm H(2)O, inspiratory time 0.8 s, and FIO(2) 0.21

Mechanical ventilation is a useful treatment option for respiratory insufficiency following thoracic and cardiovascular surgery. Ventilation mode is classified as volume-controlled-ventilation(VCV) and pressure-controlled ventilation(PCV). Non-invasive ventilation(NIV) without tracheal intubation has been recently developed and is effective in patients with chronic obstructive pulmonary disease (COPD) exacerbation. Several pulmonary complications by mechanical ventilation such as ventilator-induced lung injury(VILI) or ventilator-associated pneumonia(VAP) could be avoided with NIV. According t

The central goal in the management of a patient with acute encephalopathy and encephalitis is the prevention of hypoxemic of hypoxic secondary insults. It is recommended that partial pressure of arterial oxygen should be more than 80 mmHg with mechanical ventilation and supplemental oxygen. About carbon dioxide tension, it is standard practice to ventilate to normocapnia instead of routine setting of hypocapnia. Hyperventilation therapy is limited to specialized conditions with intracranial hypertension which is refractory to other therapy and induces neurological deteriorations. And it is pre

The development of the computer technology brought reform in the field of medical equipment. Originally the mechanical ventilator was an instrument only as for running by pressure and the tool that let you breathe. However, it has a function to assist a measurement (tidal volume, peek pressure, etc.) and to wean from a ventilator. There is a case to use a mechanical ventilator for after a chest surgical operation. After the operation without the complication, it seems that there is not the special administration. However, special respiratory management is necessary in case of chronic respirato

BACKGROUND: Ventilator-associated pneumonia (VAP) is one of the common complications in patients in the intensive care unit. Abdominal massage may prevent the development of VAP by reducing residual gastric volume in enterally fed patients.
AIM: The purpose of this study is to review the literature on randomized controlled and quasi-experimental studies evaluating the effectiveness of abdominal massage in preventing VAP.
METHODS: The PRISMA-P (Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols) criteria were taken as the basis for creating the protocol of the systema

Mechanical ventilation can produce multiple complications. The most important acute complications are mechanical problems (respirator failure, problems with the connections and circuit, incorrect parameters or alarms), problems in the airway (disconnection, extubation, mal-positioning of the endotracheal tube, leaks, nose erosions, obstruction of the endotracheal tube due to secretions or kinking, mainstem bronchus intubation, bronchospasm, postextubation croup), pulmonary complications (ventilator-induced lung injury with barotrauma, volutrauma and biotrauma), hemodynamic complications, nosoc

Patients with chronic airflow obstruction and difficult or prolonged weaning are at increased risk for prolonged invasive mechanical ventilation (IMV). Several randomized controlled trials mainly conducted in patients who had pre-existing lung disease have shown that the use of noninvasive ventilation (NIV) to advance extubation in patients with difficult and prolonged weaning can result in reduced periods of endotracheal intubation, complication rates, and improved survival. Patients in these studies were hemodynamically stable, with a normal level of consciousness, no fever, and a preserved

Noninvasive ventilation (NIV) successfully treats primary respiratory failure in chronic obstructive pulmonary disease (COPD), acute pulmonary edema, and, in some patients, hypoxemic respiratory failure. Increasingly clinicians have applied NIV in an effort to shorten the duration of mechanical ventilation by facilitating weaning and preventing or treating post-extubation respiratory failure. Randomized controlled trials (RCTs) indicate that NIV may be an effective weaning tool in a subset of patients with acute-on-chronic respiratory failure from COPD, and that applying immediate NIV to extub

Although only a small proportion of full term and late preterm infants require invasive respiratory support, they are not immune from ventilator-associated lung injury. The process of lung damage from mechanical ventilation is multifactorial and cannot be linked to any single variable. Atelectrauma and volutrauma have been identified as the most important and potentially preventable elements of lung injury. Respiratory support strategies for full term and late preterm infants have not been as thoroughly studied as those for preterm infants; consequently, a strong evidence base on which to make

Even if non invasive respiratory support is widespread used in the management of respiratory failure, nevertheless mechanical ventilation and surfactant replacement are standard care for many preterm infants with respiratory distress syndrome (RDS). The interaction between exogenous surfactant and different modalities of ventilatory support are very important and can influence respiratory outcome. The optimization of surfactant replacement during respiratory support is crucial for the successful of this therapy. In course of mechanical ventilation lung recruitment manoeuvres before and after t

In last years opioids have been increasingly utilized to sedate patients during mechanical ventilation. First, in Hypnotic Based Sedation (HBS), they were added to hypnotics because of their analgesic properties. Successively, in Analgesic Based Sedation (ABS), both sedative and analgesic properties were utilized and opioids were given alone; hypnotics were added only if adequate sedation was not achieved at maximum dosage. Apart from their analgesic and sedative properties, opioid effects on respiratory function are of particular value in many mechanically-ventilated patients. Dose-dependent

Critically ill patients have severe sleep disruption and typically encounter loss of circadian sleep pattern, steep fragmentation, increasing proportions of transitional stages of sleep, and loss of slow wave and rapid eye movement sleep. Mechanical ventilation is associated with these same sleep abnormalities, but what is attributable to the intensive care unit environment versus mechanical ventilation itself may be difficult to discern. Recent studies have shown that the ventilator mode and inappropriate settings can contribute to sleep fragmentation, and it is important to avoid overventila
