Vernix caseosa peritonitis.

Vernix caseosa peritonitis (VCP) is a rare and serious complication of caesarean section. It is thought to occur as a result of spillage of amniotic fluid and or meconium into the maternal peritoneal cavity at caesarean section. It manifests as an acute abdomen days to weeks after a seemingly uncomplicated caesarean section. Only 18 cases have been reported in the literature and all but one are from the USA. The pathophysiology is incompletely understood. In the past, the management of VCP included a laparotomy and removal of suspected abdominal organs which were invariably found to be normal

Biology of the vernix caseosa: A review.

The vernix caseosa is a complex membranous structure comprising 80% water, 10% protein, and 10% lipids including barrier lipids such as ceramides, free fatty acids, phospholipids and cholesterol, synthesized partly by fetal sebaceous glands during the last trimester of pregnancy in an antero-posterior and dorsoventral manner. Because of its lipid content, vernix is hydrophobic and protects the skin from excessive water exposure during the development of the stratum corneum. The vernix caseosa has various functions during fetal transition from an intrauterine to an extrauterine environment, inc

Vernix Caseosa Peritonitis: A Scoping Review.

Background and Objectives: Vernix caseosa peritonitis (VCP) is rare. Nonspecific symptoms of acute abdomen during early puerperium make preoperative diagnosis of VCP challenging. We aimed to identify risk factors, early diagnosis and treatment options, and the association between the timing and severity of VCP diagnosis and maternal outcomes. Materials and Methods: We searched PubMed, PubMed Central, and Google Scholar. Articles were analyzed according to the PRISMA guidelines. The search items included: 'vernix caseosa peritonitis, 'vernix caseosa granuloma, 'maternal meconium peritonitis', '

Computed Tomography Findings in Vernix Caseosa Peritonitis.

INTRODUCTION: Vernix caseosa peritonitis (VCP) is a rare peripartum complication secondary to the introduction of fetal vernix into the maternal peritoneal cavity. Vernix caseosa peritonitis typically manifests a few hours to days after a cesarian section and is often initially misdiagnosed as a more common disease process resulting in delayed diagnosis. We report the computed tomography (CT) findings in 2 patients with VCP and reviewed the previously reported CT findings of VCP. CASES: Two patients, aged 17 and 24 years, presented with signs and symptoms of peritonitis within days of undergo

Vernix caseosa peritonitis: report of two cases.

Vernix caseosa peritonitis is a rare complication caused by inflammatory response to amniotic fluid spilled into the maternal peritoneal cavity. Most cases occur after cesarean section. We discuss herein two patients, aged 33 and 29 years, who presented with vernix caseosa peritonitis seven to nine days after a cesarean delivery. Laparotomy was performed and it revealed neither uterine rupture nor other surgical emergencies, but cheesy exudates on the serosal surface of all viscera. Appendicectomy was performed. Histopathologic study revealed acute fibrinous serositis and a mixed cellular infi

Fetal cocaine exposure: analysis of vernix caseosa.

Preliminary data regarding the use of vernix caseosa (VC) as an alternative to other biological specimens for the determination of fetal cocaine exposure are presented. Advantages of VC analysis include its presence on all newborn babies, historical record of drug exposure, and ease of collection and storage. Fifteen samples of vernix caseosa-five from babies known to be cocaine-exposed because of a positive benzoylecgonine result from the urine and umbilical cord blood and ten from nonexposed neonates-were analyzed for the presence of cocaine and metabolites. VC samples from three of the five

Neonatal aspiration syndrome due to vernix caseosa.

Fetal aspiration of meconium in amniotic fluid is a well-known cause of respiratory distress in newborn infants. It causes an irregular, coarse, nodular pattern on chest radiographs. Less known is that aspiration of vernix caseosa causes a similar syndrome. We present a post-mature infant in whom aspiration of vernix caseosa caused respiratory distress, ventilatory difficulty, and radiographic changes essentially the same as in aspiration of meconium.

Vernix caseosa peritonitis presenting as a soft tissue mass.

Vernix caseosa peritonitis (VCP) is a rare condition characterised by non-necrotising granulomatous inflammation within the peritoneal cavity following childbirth. Its presentation is typically with acute abdominal pain and fever following caesarean section (CS), and management involves broad-spectrum antibiotics and surgical intervention. Conservative management is rare.We report two cases of VCP, which presented with abdominal masses 4 weeks following elective CS. CT scan demonstrated a soft tissue mass with diffuse peritoneal thickening, suspicious for an atypical infection or malignancy. B

Radiology of vernix caseosa peritonitis: case report and discussion.

Vernix caseosa peritonitis is a rare complication of pregnancy that can occur following Caesarean section. If fetal vernix enters the peritoneal cavity, it may incite an acute inflammatory response leading to peritonitis. This diagnosis should be considered when patients present with an acute abdomen pain following Caesarean section. Radiological imaging may identify lesions in the abdominal wall wound or peritoneal cavity, and the diagnosis can be made by image-guided biopsy.

Vernix caseosa peritonitis: report of two cases with antenatal onset.

Maternal peritonitis secondary to fetal vernix caseosa is considered an infrequent complication of cesarean section in which commonly spilled amniotic fluid is incompletely lavaged. Nine of the 10 reported cases have been diagnosed in the postpartum period after an uneventful cesarean section. Characteristically, vernix elicits granulomatous inflammation, occasionally with a mass lesion simulating bowel perforation and leading to colectomy. One case of antenatal leakage of amniotic fluid has been reported, also with granulomatous inflammation. We report two additional cases of antenatal leakag

Vernix caseosa: an unusual cause of post-cesarean section peritonitis.

Surgeons should be aware of a very unusual cause of post-cesarean section peritonitis. This rare form of peritonitis is caused by the spillage of vernix caseosa material into the peritoneal cavity. The exact mechanism of the development of peritonitis in these few cases is unknown. Clinically, vernix caseosa peritonitis should be considered in the post-cesarean section acute abdomen after excluding the more common causes of peritonitis such as appendicitis and perforated viscus. Intraoperatively, the presence of cheese-like yellow patches limited to the serosal layer of visceral organs is the

Vernix caseosa peritonitis - no longer rare or innocent: a case series.

INTRODUCTION: Vernix Caseosa peritonitis is a rare post caesarean section complication with only 19 case reports in the literature to date. Vernix caseosa spilt at the time of caesarean section is thought to incite an inflammatory reaction, causing symptoms resembling an acute abdomen. CASE PRESENTATION: We discuss three Caucasian patients (aged 32 to 43 years) who presented in our health sector in Sydney with vernix caseosa peritonitis. Each had a protracted course with significant comorbidities requiring surgical and medical intervention. This contrasts with other reports suggesting that a

Temperature-induced changes in structural and physicochemical properties of vernix caseosa.

The skin of the third trimester fetus and early newborn exhibits a complex, multifunctional, highly hydrated but viscous skin-surface biofilm called vernix caseosa (VC). During birth, VC undergoes a substantial change from an aqueous and warm surrounding into a gaseous and colder environment postnatally. The aim of this study was to investigate the structural and physicochemical changes in VC, which accompany physiologically relevant variations in environment parameters, such as temperature and humidity. A remarkable difference was observed in water release and uptake properties: dehydration a

The role of medical imaging in the management of vernix caseosa peritonitis: A case report.

Vernix caseosa peritonitis is a rare postpartum cause of acute abdomen. It is thought to occur due to a granulomatous reaction from intraperitoneal amniotic fluid or meconium. We present a case of a 34-year-old woman with vernix caseosa peritonitis and associated radiographic findings on computed tomography and magnetic resonance imaging. Improved awareness and understanding of imaging findings in vernix caseosa peritonitis is important to ensure appropriate early management and to prevent removal of healthy organs.

Fatal Vernix Caseosa Aspiration Associated With Persistent Pulmonary Hypertension of the Newborn.

Vernix caseosa aspiration is an extremely rare condition resulting in high mortality if complicated by persistent hypertension of the newborn (PPHN). Herein we offer the first case report of PPHN due to massive vernix caseosa aspiration documented by histopathological examination. This case report is presented to provide a synopsis of the pathoetiology of PPHN related to vernix caseosa aspiration syndrome as likely to be encountered by neonatologists and general pediatricians involved with neonatal care.
