OBJECTIVE: To investigate the position of the internal aperture of vestibular aqueduct and its relationship to hearing level in patients with enlarged vestibular aqueduct (EVA).
METHODS: The size of the common crus and the position of the internal aperture of vestibular aqueduct were compared among control subjects, EVA patients with and without other inner ear malformations. Auditory steady-state response thresholds were compared between EVA patients with different positions of internal apertures.
RESULTS: The common crus in EVA patients was shorter than in control subjects (p < 0.05). The

OBJECTIVES/HYPOTHESIS: Patients with enlarged vestibular aqueduct (EVA) often demonstrate an air-bone gap (ABG) at low frequencies on audiometric testing. The mechanism for this has not been well elucidated. We investigated vestibular aqueduct (VA) size and ABG in patients with EVA, and assessed the relationship between VA diameter and ABG.
STUDY DESIGN: Retrospective review.
METHODS: We assessed 98 ears from 49 individuals diagnosed with Pendred syndrome. The VA diameters of ears in groups with (n = 60) and without (n = 38) ABG were measured using axial computed tomography images. Three dif

Enlarged vestibular aqueduct, a recently identified anomaly, is typified by an enlarged vestibule, dilation of the ampullated portions of the horizontal and superior semicircular canals, an abnormal cochlea, and hearing loss. In the case described a 16-year-old boy had congenital hearing loss, episodic vertigo, and abnormal vestibular function testing. Tomograms and CT scans confirmed the diagnosis of bilaterally enlarged vestibular aqueducts. The vertiginous episodes decreased in frequency and severity with a no-salt-added diet. The authors conclude that the enlarged vestibular aqueduct is as

OBJECTIVE: Enlarged vestibular aqueduct (EVA) is the most common inner ear malformation. While a strong correlative relationship between EVA and hearing loss is well established, its association with vestibular dysfunction is less well understood. In this study, we examine the effects of EVA on the vestibular system in patients with EVA.
STUDY DESIGN: Prospective, cross-sectional study of a cohort ascertained between 1999 and 2013.
SETTING: National Institutes of Health Clinical Center, a federal biomedical research facility.

The large vestibular aqueduct syndrome (LVAS) is a distinct clinical entity characterized by stepwise progressive sensorineural hearing loss associated with isolated enlargement of the vestibular aqueduct. A correlative clinical, audiologic, vestibular, cytogenetic, and radiographic analysis of a family with inherited LVAS was performed. The male proband and his affected brother are offspring of unaffected parents, and have no other abnormalities. Pedigree analysis suggests autosomal recessive or X-linked inheritance with variable expressivity of LVAS in this family. This study is the first de

IMPORTANCE: Enlarged vestibular aqueduct (EVA), the most prevalent inner ear malformation causing hearing loss (HL) in various populations, is predominantly genetically mediated. Despite advancements in genetic diagnostics, the comprehensive phenotypic and genotypic spectrum of EVA remains insufficiently characterized.
OBJECTIVES: To characterize the natural history, clinical outcomes, phenotype, and genotype of EVA.
DESIGN, SETTING, AND PARTICIPANTS: This single-center, longitudinal, retrospective cohort study was conducted from March 2003 to October 2022, with follow-up until July 1, 2024.

Improved radiological examinations with newly developed 3D models may increase understanding of Meniere's disease (MD). The morphology and course of the vestibular aqueduct (VA) in the temporal bone might be related to the severity of MD. The presented study explored, if the VA of MD and non-MD patients can be grouped relative to its angle to the semicircular canals (SCC) and length using a 3D model. Scans of temporal bone specimens (TBS) were performed using micro-CT and micro flat panel volume computed tomography (mfpVCT). Furthermore, scans were carried out in patients and TBS by computed t

Four patients (7 ears) with a large vestibular aqueduct (VA) were examined for history of vertigo and vestibular function. Vertigo was observed in all the 4 patients. The caloric responses were significantly poorer in ears with a large VA than in the controls. On a patient with a large VA who had several attacks of sudden hearing loss and vertigo following minor head trauma, long-time exposure to sunshine, common cold, and exercise, vestibular function tests were performed several times; the caloric responses were found to fluctuate and direction changing apogeotropic positional nystagmus was

Multidirectional tomography (MDT) can be useful in determining the caliber, shape, and course of the vestibular aqueduct (VA) and cochlear aqueduct (CA). Clinical decisions have been based on the findings from MDT. Unfortunately, the clinical utility of these observations has been confusing and controversial because similar MDT techniques were not used. This study will address some of the difficult questions and clinical controversies derived from MDT observations. This new perspective has evolved with the use of high resolution computed tomography (HRCT). An analysis of 750 petrous bones for

BACKGROUND AND PURPOSE: The size of vestibular aqueducts (VAs) seen on CT studies varies. The current practice of calling a VA enlarged when it exceeds a certain threshold (eg, 1.5 mm at the midpoint) is arbitrary. Our hypothesis was that statistical analysis of the range of VA widths in a normal-hearing population would lead to a mathematic definition of the upper-limit-of-normal VA width.
MATERIALS AND METHODS: The VA midpoint and opercular widths were measured in 73 children with normal hearing. Statistical analysis yielded values of the 99 th, 97.5th, 95th, 90th, 75th, and 50th percentile

The vestibular aquaeduct was evaluated blindly in tomograms from patients with Meniere's disease or chronic otitis and from patients without ear disorders. The descending part of the aquaeduct was clearly visible in 95% of the patients without ear disease. In the two other groups, it was visible only in about 50%. It is suggested that a narrow or invisible vestibular aquaeduct is an unspecific radiological sign in a variety of ear diseases.

The gross morphology of the vestibular aqueduct and its clinical significance are better understood against the background of the endolymphatic duct and sac, which is still a matter of hypotheses. They are concerned with functions of the endolymphatic sac like secretion and absorption of endolymph, desintegration of cells and other debris from the inner ear and phagocytosis. The endolymphatic sac in the vestibular aqueduct is also thought to have a pressure regulation function for the labyrinth and also to deal with the metabolism of the inner ear and the desintegration of otoconia. OBJECTIVES: To review the controversial aspects of the enlarged vestibular aqueduct syndrome.
STUDY DESIGN: Contemporary review.
METHODS: A literature search using the terms "enlarged vestibular aqueduct and large vestibular aqueduct" were used to generate the articles for review in this article.
RESULTS: The enlarged vestibular aqueduct is a condition causing variable auditory and vestibular dysfunction. Although it has been 32 years since Valvasorri and Clemis recognized the clinical importance of the enlarged vestibular aqueduct, many controversial aspects of the diagnosis remain. The to

This report describes the case history of two male siblings with sensorineural hearing loss and an enlarged vestibular aqueduct (EVA). Sibling 1 presented with a history of intermittent self-limiting ataxia and hearing loss at the age of 25 months and sibling 2 presented with a similar history at the age of 18 months. MRI showed an enlarged endolymphatic duct and sac bilaterally in both children. Perchlorate discharge tests were positive in both infants leading to a diagnosis of Pendred syndrome. A number of conditions associated with EVA are discussed with a view to devising management strate

An enlarged vestibular aqueduct (EVA) is one of the most commonly identified inner ear bony malformations in children with sensorineural hearing loss of unknown cause. Most previous reports have focused on hearing loss, but individuals with EVA may also experience paroxysmal vertigo lasting minutes to hours. We report the clinical vestibular features and vestibular function testing of two children and one adult with EVA who had a history of sensorineural hearing loss and presented to our Neurotology Clinic for the evaluation of episodic vertigo. All the patients had an antecedent history of pr
