Association of psychiatric diagnoses and military sexual trauma type with denied posttraumatic stress disorder service connection.

BACKGROUND: Annually, thousands of Veterans file Department of Veterans Affairs' (VA) disability claims ("service connection") for posttraumatic stress disorder (PTSD) related to military sexual trauma (MST). Service connection provides free covered healthcare and compensation. To qualify, Veterans must meet PTSD diagnostic criteria, have sufficient evidence of the MST, and establish a link between their PTSD symptoms and MST. METHODS: Among VA-enrolled Veterans who filed for MST-related PTSD service connection between 2017 and 2022 (N = 19,509), we modeled the probability of denied PTSD serv

Examining bias in the award of Veterans Affairs (VA) disability benefits for posttraumatic stress disorder in women veterans: Analysis of evaluation reports and VA decisions.

Studies have raised concerns about possible inequities in the U.S. Department of Veterans Affairs (VA)'s awards of disability for posttraumatic stress disorder (PTSD) to women. However, the diagnoses and opinions made by disability examiners have not been studied. A sample of 270 initial PTSD examination reports and corresponding VA decisions were studied. Compared to men, women veterans were as likely to be diagnosed with a service-related mental disorder, χ2(1, N = 270) = 2.31, p = .129, odds ratio (OR) = 1.79, 95% CI [0.84, 3.80], and be granted service-connection, χ2(1, N = 270) = 0.49, p

Role of homelessness and incarceration in time-to-decision on veterans affairs disability benefits for a mental health condition.

Homelessness and incarceration are major psychosocial concerns in the United States that may be mitigated by access to disability benefits. Data from the Veterans Benefits Administration (VBA) provides a unique opportunity to study how the process of obtaining disability benefits and time-to-decision might be affected by homelessness and/or incarceration among veterans in the Veterans Health Administration (VHA). The purpose of this study was to examine how homelessness and/or incarceration status among VHA-enrolled U.S. veterans might affect outcomes of VBA disability benefits claims for a me

Poor validation of medical record ICD-9 diagnoses of gout in a veterans affairs database.

OBJECTIVE: Diagnostic codes based on medical records or claims data have been used to identify patient populations with gout for important epidemiologic and clinical studies. We evaluated whether we can document the accuracy of such diagnoses by review of medical records and then on direct interviews with a subset of patients. METHODS: Electronic medical records of 289 patients with 2 visits with ICD-9 codes for gout were extensively reviewed to search for documentation of features that would classify patients as having gout by 3 sets of proposed criteria, the American College of Rheumatology

Standard claims and appeals forms. Final rule.

The Department of Veterans Affairs (VA) amends its adjudication regulations and the appeals regulations and rules of practice of the Board of Veterans' Appeals (Board) to require that all claims governed by VA's adjudication regulations be filed on standard forms prescribed by the Secretary, regardless of the type of claim or posture in which the claim arises. This rulemaking also eliminates the constructive receipt of VA reports of hospitalization or examination and other medical records as informal claims for increase or to reopen while retaining the retroactive effective date assignment for

Proposed Changes to the Department of Veterans Affairs Disability Rating Rubric for Mental Disorders.

OBJECTIVE: The U.S. Department of Veterans Affairs (VA) provides disability benefits for general medical and mental health conditions related to military service. Despite advances in conceptualization, assessment, and diagnosis of mental disorders, the current rating rubric used to determine the award amounts received by veterans with posttraumatic stress disorder and other mental disorders has not been substantively revised since 1996. The VA recently proposed sweeping changes to the rating rubric for mental disorders, shifting the focus from a symptom-based algorithm to one based on function

Evolution of disability in late 19th century America: Civil War pensions for Union Army veterans with musculoskeletal conditions.

This article examines the evolution of musculoskeletal (MSK) disability and its impact on mortality and work patterns in the late 19th century in America, in the context of the Civil War disability policy scheme. The study was conducted on 17,702 Union Army (UA) Civil War veterans. Of these, 10,789 were examined and diagnosed with major MSK conditions, rheumatism, sciatica, and spinal curvature, between 1862 and 1907. Analyses examine MSK (i) prevalence rates by birth cohort and age group; (ii) fatality rates as compared with other disabilities; (iii) risk rates by occupation type; and (iv) li

Racial/Ethnic and Geographic Differences in Traumatic Brain Injury (TBI) Service-Connected Disability Determination and Monthly Compensation Among Post-9/11 US Veterans.

PURPOSE: Veterans who incur a traumatic brain injury (TBI) during active duty service may be eligible to receive service-connected disability (SCD) compensation. This study examined potential racial/ethnic and geographic disparities in TBI SCD determination and overall monthly SCD compensation. Federal Tribal Land (FTL) was incorporated into existing VA geographic designations of urban, rural, highly rural, and US Territories (UST). METHODS: The association of race/ethnicity and geographic status with TBI SCD was estimated using logit models. Generalized linear models were used to assess the

Compensation and Pension Exams for Military Sexual Trauma-Related Posttraumatic Stress Disorder: Examiner Perspectives, Clinical Impacts on Veterans, and Strategies.

BACKGROUND: It is estimated that in one in three women veterans experience military sexual trauma (MST), which is strongly associated with posttraumatic stress disorder (PTSD). A 2018 report indicated the Veterans Benefits Administration (VBA) processed approximately 12,000 disability claims annually for PTSD related to MST, most of which are filed by women. Part of the VBA adjudication process involves reviewing information from a Compensation and Pension (C&P) exam, a forensic diagnostic evaluation that helps determine the relationship among military service, diagnoses, and current psychosoc

Changes in Post-traumatic Stress Disorder Service Connection Among Veterans Under Age 55: An 18-Year Ecological Cohort Study.

INTRODUCTION: Mandatory, age-based re-evaluations for post-traumatic stress disorder (PTSD) service connection contribute substantially to the Veterans Benefits Administration's work load, accounting for almost 43% of the 168,013 assessments for PTSD disability done in Fiscal Year 2017 alone. The impact of these re-evaluations on Veterans' disability benefits has not been described. MATERIALS AND METHODS: The study is an 18-year, ecological, ambispective cohort of 620 men and 970 women receiving Department of Veterans Affairs PTSD disability benefits. Veterans were representatively sampled wi

A sailor's pain: Veterans' musculoskeletal disorders, chronic pain, and disability.

A few years after leaving the navy, a 50-year-old Veteran* presents to a new family physician with chronic knee and back pain. He is seeking a new physician for opioid and benzodiazepine refills, referrals for ongoing acupuncture and massage therapy, and completion of Veteran Affairs Canada (VAC) disability claim forms for his back. He was medically released at the rank of Petty Officer owing to knee impairment secondary to a fracture sustained aboard ship. He twice strained his back on deployments, but did not develop chronic low back pain until after leaving the Canadian Forces (CF). On rele

Disability among US Army Veterans vaccinated against anthrax.

CONTEXT: To protect troops against the use of anthrax as a biological weapon, the US Department of Defense began an anthrax vaccination program in 1998. 14 years after the inception of the vaccination program, there is no evidence suggesting vaccination against anthrax carries long-term health risks for Active Duty Soldiers. OBJECTIVE: To investigate the association between Anthrax Vaccine Adsorbed (AVA) received while on Active Duty and subsequent disability determined by the Veterans Benefits Administration. Long-term ocular consequences of sulfur mustard in seriously eye-injured war veterans.

INTRODUCTION: Sulfur mustard (SM) has been used as a dangerous chemical warfare agent since the early 20th century. Although many descriptive studies about SM-induced ocular injuries are present in the medical literature, few of them have been conducted over a large group with serious ocular involvement. MATERIALS AND METHODS: This descriptive study was conducted on 149 severe SM-intoxicated war veterans. Ocular history, anterior and posterior segment findings using a slit lamp, and direct and indirect ophthalmoscopic findings were recorded. Severity of the disease was also recorded based on
