Two clinical isolates of Vibrio alginolyticus from New Jersey are reported, one from a mixed stump infection and the other grown in pure culture from the conjunctival discharge of a man with conjunctivitis. The biochemical characteristics and antibiotic susceptibilities of these two isolates are presented. Human infections caused by V. alginolyticus are reviewed.

Infections caused by Vibrio alginolyticus is generally disregarded because of the fact that it is an unusual pathogen for humans. In this report, a 57 years old male patient with otitis media has been presented. Intense and pure isolation of V. alginolyticus was achieved from the ear discharge sample. According to the history, it was detected that he was on vacation at Mediterranean coast three weeks ago, thus the transmission of the bacteria is possibly via seawater. The seawater bacterium Vibrio alginolyticus was detected in 5 of 20 water samples from seawater aquaria (from 3 of 5 units) and also from the surface of diseased stony corals. A total of 45 isolates were differentiated biochemically, of which 13 isolates (29%) proved to be V. alginolyticus. Vibrio alginolyticus is a microorganism of marine environment which occasionally may cause infections on man. We described four patients with external otitis who had prolonged contact with sea water presented itch and seropurulent fluid. V. alginolyticus was isolated by pure culture from ear fluid of three of them and associated to V. fluvialis in another. The administration of ciprofloxacin in three cases, and ceftazidime in the other succeeded in the remission of the symptoms.

Type III secretion systems (T3SSs) contribute to microbial pathogenesis of Vibrio species, but the regulatory mechanisms are complex. We determined if the classic ExsACDE protein-protein regulatory model from Pseudomonas aeruginosa applies to Vibrio alginolyticus. Deletion mutants in V. alginolyticus demonstrated that, as expected, the T3SS is positively regulated by ExsA and ExsC and negatively regulated by ExsD and ExsE. Vibrio alginolyticus was originally classified as biotype 2 of Vibrio parahaemolyticus. Most clinical isolates are recovered from superficial wounds or the external ear infections. V. alginolyticus is acknowledged to be nearly nonpathogenic in humans. The reason for presence of V. alginolyticus's virulence is uncertain. We describe a chronic myringitis case in a 47-year-old female due to V. alginolyticus. According to her anamnesis, it was detected that she had sea bathing history in Mugla Coast in Turkey. Pure isolation of V. alginolyticus was obtained from external auditory canal's culture.
