Water insecurity in a syndemic context: Understanding the psycho-emotional stress of water insecurity in Lesotho, Africa.

Syndemics occur when populations experience synergistic and multiplicative effects of co-occurring epidemics. Proponents of syndemic theory highlight the importance of understanding the social context in which diseases spread and cogently argue that there are biocultural effects of external stresses such as food insecurity and water insecurity. Thus, a holistic understanding of disease or social vulnerability must incorporate an examination of the emotional and social effects of these phenomena. This paper is a response to the call for a renewed focus on measuring the psycho-emotional and psyc

Exploring the invisible water insecurity of water utility shutoffs in Detroit, Michigan.

OBJECTIVES: Anti-TNFα agents are indicated in selected patients with rheumatoid arthritis (RA) who respond inadequately to methotrexate and particularly when glucocorticoids are mandatory. We evaluated whether a glucocorticoid-sparing effect occurred during the first year of anti-TNF-α therapy. METHODS: Between 2007 and 2009, the French multicentre, longitudinal, prospective, observational, population-based CORPUS cohort included biologic-naive patients with inflammatory joint disease. Patients with active RA treated with glucocorticoids were included. Patients who received at least one anti-

Food and Water Insecurity and Functional Disability in Adults.

IMPORTANCE: Evidence from diverse global populations suggests that household food insecurity (HFI) is associated with adult disability, but the association between household water insecurity (HWI) and disability remains understudied. Examinations of the joint association of dual HFI and HWI with disability are lacking. OBJECTIVE: To explore independent and joint associations of HFI and HWI with functional disability in Ecuadorian adults. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used data from the 2018 Ecuadorian National Health and Nutrition Survey, a nationally represen

Implementation Lessons of a Water Insecurity Intervention During a Drought in Mexico.

Background: We are early in our understanding of how to effectively implement interventions to mitigate the harm of extreme climate events on human health. We study the actions of the state government in Nuevo Leon, Mexico, which, in 2022, declared a state of emergency due to water shortages resulting from climatic and infrastructural issues. Objective: To document the facilitators and challenges to the rollout of the government's strategy to mitigate water insecurity, using the EquIR Implementation Science framework. Our analysis focused on the activities of the Ministry of Social Policy, whi

Water insecurity: An agenda for research and call to action for human biology.

Water insecurity-the lack of adequate and safe water for a healthy and productive life-is one of the greatest threats facing humans in the coming century. By 2030, half of the world is expected to be living in water-stressed conditions, given current climate change scenarios. A key goal of the UN Water Action Decade and Sustainable Development Goal 6 is to improve water security for the three billion people globally affected, but the future looks grim. For many communities, from Cape Town, South Africa to Flint, United States, the imagined dystopian future of severe water shortages has already

Development of water insecurity scale for rural households in Cameroon- Central Africa.

Background: Water represents the core of food-energy nexus and is vital for human survival. In developing countries, contaminated water and lack of basic water services undermine efforts to improve nutritional status and related health issues. In the rural areas of Central Africa, a majority of the population lacks access to improved water sources and has to devote considerable efforts to obtain water. Objectives: Using the following definition of water insecurity, i.e. it exists when access to adequate amount of safe and clean water does not occur all the times for the entirety of household me

Water Insecurity, Social Perspectives, and Health Impacts in Private Drinking Water Sources in Pennsylvania: Two Systematic Literature Reviews.

Over 43 million people in the United States rely on private drinking water supplies for domestic purposes, yet 20% of water wells are potentially contaminated in some form. In Pennsylvania, 2-3 million rural residents use private water supplies for domestic purposes, yet Pennsylvania has no state-wide well drilling standards or water quality testing requirements for private water sources. The lack of implemented standards and monitoring hides potential water supply and quality issues and may affect water trust, health, and overall water insecurity. Therefore, we conducted two interlinked syste

Water, Sanitation, and Hygiene (WaSH) insecurity in unhoused communities of Los Angeles, California.

BACKGROUND: Access to water and sanitation is a basic human right; however, in many parts of the world, communities experience water, sanitation, and hygiene (WaSH) insecurity. While WaSH insecurity is prevalent in many low and middle-income countries, it is also a problem in high-income countries, like the United States, as is evident in vulnerable populations, including people experiencing homelessness. Limited knowledge exists about the coping strategies unhoused people use to access WaSH services. This study, therefore, examines WaSH access among unhoused communities in Los Angeles, Califo

Coping strategies for household water insecurity in rural Gambia, mediating factors in the relationship between weather, water and health.

BACKGROUND: Rural communities in low- and middle-income countries, such as The Gambia, often experience water insecurity periodically due to climate drivers such as heavy rainfall and reduced rainfall, as well as non-climate drivers such as infrastructural issues and seasonal workloads. When facing these challenges households use a variety of coping mechanisms that could pose a risk to health. We aimed to understand the drivers of water insecurity (climate and non-climate), the behavioural responses to water insecurity and the risks these responses pose to the health of communities in rural Ga

The Individual Water Insecurity Experiences (IWISE) Scale: reliability, equivalence and validity of an individual-level measure of water security.

OBJECTIVE: The lack of a validated and cross-culturally equivalent scale for measuring individual-level water insecurity has prevented identification of those most vulnerable to it. Therefore, we developed the 12-item Individual Water InSecurity Experiences (IWISE) Scale to comparably measure individual experiences with access, use, and stability (reliability) of water. Here, we examine the reliability, cross-country equivalence, and cross-country and within-country validity of the scale in a cross-sectional sample. METHODS: IWISE items were implemented by the Gallup World Poll among national

Water Insecurity, Sociopolitical Instability, and Resurgence of Cholera in Haiti, 2022: An Outbreak Investigation.

We investigated a resurgence of cholera in Haiti in 2022, occurring after 3 years without cases. We analyzed data from the National Cholera Surveillance System for the first reported cases in 2022 and interviewed field epidemiology teams. We used logistic regression to identify risk factors associated with confirmed cholera. Few suspected cases reported ever receiving oral cholera vaccine (14%) or previous hospitalization for cholera (7%). Recently changing water sources were associated with culture-confirmed cholera (odds ratio 5.55, 95% CI 2.13-15.12). Spatial analysis of cholera cases revea

Water insecurity and urban poverty in the Global South: Implications for health and human biology.

OBJECTIVES: Over half of the world's population (4 billion people) experience severe water scarcity at least one month per year, while half a billion people experience severe water scarcity throughout the year. Despite progress from national and global interventions, a staggering proportion of the Global South remains water insecure. Rapid urban growth and associated demographic changes, climate change, and governance failure have also fostered the growth and expansion of urban informal settlements and slums where widespread poverty and environmental hazards exacerbate the impact of water inse

Why is water insecurity so distressing? Evidence from Iran that interpersonal conflict is key.

Chronic household water insecurity is consistently linked to distress and poor mental health, yet the social pathways remain under-tested. We examine multiple theorized socially situated mechanisms that may explain how household water insecurity translates into depressive symptoms, using population-representative 2024 survey-interview data (N = 685) from Sistan and Baluchestan, Iran. Household water insecurity (HWISE-12), depressive symptoms (PHQ-9), and five mechanisms-interpersonal conflict, distributive injustice, procedural injustice, institutional distrust, and water-labor burden-were mea

Household water insecurity, depression and quality of life among postnatal women living in urban Nepal.

OBJECTIVES: This observational study aimed to assess distress related to the household water supply and to examine the relationship between household water insecurity and psychological health among Nepali postnatal women. METHODS: In total, 300 women consented to participate in the study, of which 267 women were able to participate in a follow-up 1 month after discharge. We developed a household water insecurity scale (HWIS; total score range 0-24 points) by adapting the household food insecurity access scale. RESULTS: The Cronbach's coefficient alpha for the HWIS was moderately acceptable.

The impact of a water promotion and access intervention on elementary school students in the presence of food insecurity.

OBJECTIVE: School-based interventions encouraging children to replace sugar-sweetened beverages with water show promise for reducing child overweight. However, students with child food insecurity (CFI) may not respond to nutrition interventions like children who are food-secure. DESIGN: The Water First cluster-randomised trial found that school water access and promotion prevented child overweight and increased water intake. This secondary analysis used mixed-effects regression to evaluate the interaction between the Water First intervention and food insecurity, measured using the Child Food
