Critical care problems of the newborn: insensible water loss in small premature infants.

During the control period in this study, small premature infants, 1500 g in birth weight, had insensible water loss (IWL) of 2.28 g/kg/hr (equivlaent to 54 ml/kg/day), which is considerably higher than in full term infants. Under phototherapy, there was a significant increase in IWL (3.14 g/kg/hr) associated with increase of skin and rectal temperature. The heart rate and respiratory rates were comparable. Full term infants in stress situations such as phototherapy can compensate for water losses by increasing oral intake. In small premature infants, reliance must be placed upon increased amou

Insensible water loss in the critically III neonate. Combined effect of radiant-warmer power and phototherapy.

To quantitate radiant power and insensible water loss under phototherapy, 12 neonates were studied under radiant warmers for one hour each with and without phototherapy. Warmer power was measured by wattmeter and thermopile. Power density received from phototherapy was 4.4 mW/sq cm. Addition of phototherapy to the servocontrolled warmer caused a disease in power density received from the warmer (17.1 to 12.9 mW/sq cm). However, the total radiant-power density received with phototherapy and the warmer combined (17.3 mW/sg cm) did not differ from net power density received without phototherapy (

Insensible water loss in extremely preterm infants cared for in incubators with 70% and 90% relative humidity.

OBJECTIVE: To determine the insensible water losses (IWL) in preterm infants cared for in incubators with 70% or 90% relative humidity (RH). STUDY DESIGN: This was a single center retrospective study of extremely preterm infants between 22- and 28-weeks gestational ages nursed in 70% and 90% RH. IWL was calculated using a formula that utilized infants' fluid intake, urine output and body weight changes. IWL between the two groups were compared using independent t-test. Multivariate linear regression was used to assess the effects of humidity on IWL after adjusting for confounding variables.

[Electrolyte balance in major abdominal surgery. III. On insensible water losses from the peritoneal cavity (author's transl)].

Two groups of 16 patients each were studied during abdomino-surgical procedures. Patients of one group received an isotonic glucose solution to cover insensible water losses from the peritoneal cavity whilst patients of the other group were not treated with glucose. In these we found a significant increase in plasma osmolality and in mean corpuscular hemoglobin concentration of the red cells during anaesthesia and operation. The balance of osmotic free water was calculated from changes of plasma osmolality during the observation period. Calculating output as difference between known input and

Nurses' Knowledge of Insensible Water Loss and the Evolution of a Digital Decision-Support Tool for Personalized Fluid Balance Assessment: A Cross-Sectional Study.

Background/Objectives: Fluid balance management is a core nursing competency, particularly in high-acuity settings. However, the assessment of insensible water loss remains challenging due to the lack of standardized methods, which may increase variability in clinical reasoning. In this context, digital tools may support a more structured and personalized approach. This study aimed to assess nurses' knowledge of fluid balance and insensible water loss [perspiratio insensibilis (PI)] and to evaluate the usability and perceived quality of a pilot digital clinical decision-support tool (CareBalan

Safety and efficacy of double plastic wrap in reducing insensible water loss in preterm infants in first week of life - a pilot randomized controlled trial from a low-to-middle-income country.

BACKGROUND: Preterm infants experience excessive insensible water loss in postnatal period. Established practices like cling film might not be sufficient alone in reducing this loss. Being expensive, humidified incubators might not be affordable in developing countries. Hence, we tried to explore double plastic wrap (cling film attached to the bassinet wall and occlusive plastic wrap covering the baby) as a low-cost solution of increased insensible water loss in postnatal period. METHODS: In this pilot trial 63 inborn infants of less than 32 weeks of gestation or birth weight less than 1200 g

Alterations in Body Fluid Balance During Fin Swimming in 29 °C Water in a Population of Special Forces Divers.

Highly trained "combat swimmers" encounter physiological difficulties when performing missions in warm water. The aim of this study was to assess the respective roles of immersion and physical activity in perturbing fluid balance of military divers on duty in warm water. 12 trained divers performed 2 dives each (2 h, 3 m depth) in fresh water at 29 °C. Divers either remained Static or swam continuously (Fin) during the dive. In the Fin condition, oxygen consumption and heart rate were 2-fold greater than during the Static dive. Core and skin temperatures were also higher (Fin: 38.5±0.4 °C and

Comparison of conventional phototherapy and fiberoptic phototherapy in the very-low-body-weight infants.

Phototherapy is effectively employed for the treatment of neonatal hyperbilirubinemia, but it may influence the physiological hemodynamics of the infants, such as skin blood flow, insensible water loss and the redistribution of cardiac output. This is a retrospective chart review study involved totally 42 very low birth weight prematures(birth body weight less than 1500 gm, VLBW) who received conventional or fiberoptic phototherapy. Infants with congenital malformation, severe cardiorespiratory instability and the requirement of diuretics, inotropics or blood transfusion were excluded. The bod

Nursing Management of Humidification Levels for the Extremely Low Birth Weight Infant: A Process Improvement Protocol to Reduce Hypernatremia in the First 2 Weeks of Life.

The use of a humidified environment has long been established as a method of maintaining a thermoneutral environment and minimizing evaporative insensible water losses in the extremely low birth weight (ELBW) population. The ELBW infant is at high risk for insensible water losses leading to dehydration, excessive weight loss, temperature instability, and electrolyte abnormalities, thereby increasing the ELBW infant's risks of morbidity and mortality. Literature supports the use of a humidified environment; however, a lack of consensus has led to widespread variation in the implementation of hu

Role of atrial natriuretic peptide in disturbed water and electrolyte metabolism of guinea pigs infected with Pichinde virus.

Daily intake and output of water and electrolytes (Na+, K+, and Cl-) were determined for 14 days in control and Pichinde virus-infected strain-13 guinea pigs. Although water intake began to decrease 7 days after virus inoculation, total daily water output (insensible water loss, urine excretion, plus fecal water loss) had little change. However, insensible water loss alone increased markedly in the virus-infected animals. Both intake and excretions of urinary and fecal electrolytes decreased at the middle (days 7 to 10) and late (days 11 to 14) stages of viral infection. Differences between in

In vivo measurement of pediatric extracorporeal oxygenator insensible losses; a single center pilot study.

INTRODUCTION: Fluid overload on Extracorporeal Membrane Oxygenation (ECMO) is associated with worse outcomes. Previous in vitro studies have attempted to quantify oxygenator-related insensible losses, as failure to account for this fluid loss may lead to inaccurate fluid balance assessment and potentially harmful clinical management, such as unnecessary exposure to diuretics, slow continuous ultrafiltration (SCUF), or continuous kidney replacement therapy (CKRT). We performed a novel in vivo study to measure insensible fluid losses in pediatric ECMO patients. METHODS: Pediatric ECMO patients

Value of GPSkin for the measurement of skin barrier impairment and for monitoring of rosacea treatment in daily practice.

BACKGROUND: Stratum corneum hydration (SCH) and transepidermal water loss (TEWL) provide useful information about skin barrier function. This study aimed to determine the value of GPSkin Pro, a new handheld device determining both SCH and TEWL, to measure skin barrier impairment and to monitor barrier function in rosacea in daily practice. MATERIALS AND METHODS: Two pilots were performed. Pilot 1: in 27 healthy participants, GPSkin SCH and TEWL were compared to Aquaflux® and Epsilon® values at the forearm before and after skin barrier perturbation via tapestripping. Moreover, GPSkin values we

Can a handheld device accurately measure barrier function in ichthyoses?

BACKGROUND: Transepidermal water loss (TEWL) is a surrogate measure of skin barrier dysfunction. Historically, devices that measure TEWL are expensive, complex, and require connection to a computer and energy source. Consequently, measurement of skin's TEWL has been limited to the research setting. OBJECTIVES: Evaluate the accuracy of the handheld device gpskin Barrier Light® in comparison with a standardly used device, AquaFlux AF200® , for measuring TEWL. METHODS: Transepidermal water loss measurements by gpskin Barrier Light® and AquaFlux AF200® in ichthyotic and healthy skin were compare

Hydration of the low birth-weight infant.

Factors relating to water balance in the perinatal and neonatal periods are reviewed. The major areas considered are changes in body water compartments, insensible water loss, and renal function. A most striking feature in all these areas is the wide variation in 'normal' values, both within the same infant with advancing postnatal age and also between infants of the same postnatal age but of different gestational ages. A second feature is the fluctuation caused by changes in environmental conditions, e.g., temperature, humidity, type of warming device, use of phototherapy. Much of the literat

Transepidermal water loss in the orphan forms of ichthyosis.

As a surrogate measure of skin barrier dysfunction, we sought to determine differences in transepidermal water loss (TEWL) among ichthyosis subtypes and correlate TEWL with clinical severity. Subjects with Netherton syndrome had the highest TEWL values (increased water loss), while TEWL values were lowest in subjects with epidermolytic ichthyosis. TEWL correlated with severity only in lamellar ichthyosis and age was inversely correlated with TEWL (rs  = -.213, P = .02). TEWL is an objective measure that complements disease severity in ichthyosis and may be used as an adjuvant to monitor treatm

Water balance in the fetus and neonate.

Fetal water balance is dependent prenatally on the placental transfer of water from maternal to fetal circulation. Adequate amniotic fluid volume is one indicator of stable fetal status and development. Excessive or less than expected amniotic fluid volume may be a precursor to postnatal morbidity and mortality. Postnatal transition is marked by predictable changes in body water including contraction of extracellular volume and insensible fluid loss, primarily across the skin barrier. The degree to which these occur is determined by gestational and postnatal age. Neonatal complications and cli
