Carl Wernicke of the Wernicke Area: A Historical Review.

The Wernicke area, also known as Brodmann area 22, is located in the posterior segment of the superior temporal gyrus in the dominant hemisphere. Carl Wernicke, a German neurologist, described this area in 1874. The life story of Carl Wernicke, a 19th-century medical genius, remains an inspiration for all neuroscientists even a hundred years later. We outline Wernicke's life story and academic achievements in neurosurgery, neurology, and psychiatry. We explore his remarkable ability to turn his many setbacks into steps forward, his controversial foray into psychiatry, and his wide-ranging set

Nonverbal auditory agnosia with lesion to Wernicke's area.

We report the case of patient M, who suffered unilateral left posterior temporal and parietal damage, brain regions typically associated with language processing. Language function largely recovered since the infarct, with no measurable speech comprehension impairments. However, the patient exhibited a severe impairment in nonverbal auditory comprehension. We carried out extensive audiological and behavioral testing in order to characterize M's unusual neuropsychological profile. We also examined the patient's and controls' neural responses to verbal and nonverbal auditory stimuli using functi

How Extended Is Wernicke's Area? Meta-Analytic Connectivity Study of BA20 and Integrative Proposal.

Understanding the functions of different brain areas has represented a major endeavor of contemporary neurosciences. The purpose of this paper was to pinpoint the connectivity of Brodmann area 20 (BA20) (inferior temporal gyrus, fusiform gyrus) in language tasks. A meta-analysis was conducted to assess the language network in which BA20 is involved. The DataBase of Brainmap was used; 11 papers corresponding to 12 experimental conditions with a total of 207 subjects were included in this analysis. Our results demonstrated seven clusters of activation including other temporal lobe areas (BA3, BA

tDCS of right-hemispheric Wernicke's area homologue affects contextual learning of novel lexicon.

Numerous studies have shown robust evidence of the right hemisphere's involvement in the language function, for instance in the processing of intonation, grammar, word meanings, metaphors, etc. However, its role in lexicon acquisition remains obscure. We applied transcranial direct current stimulation (tDCS) over the right-hemispheric homologue of Wernicke's area to assess its putative involvement in the processing of different types of novel semantics. After receiving 15 min of anodal, cathodal, or sham (placebo) tDCS, three groups of healthy participants learnt novel concrete and abstract wo

Progressive multifocal leukoencephalopathy selectively affecting Broca's and Wernicke's areas in an immunocompetent patient: A case report.

Progressive multifocal leukoencephalopathy (PML) is a rare, often fatal, demyelinating disease of the central nervous system. The disease almost exclusively presents in immunosuppressed patients, such as those with acquired immunodeficiency syndrome, a hematopoietic malignancy, or a transplanted organ; it is extremely rare in patients without immunosuppression. We present a case of a 74-year-old female with radiographic and histopathological findings consistent with PML that possibly arose in the setting of Sjögren's-related vasculitis but no immunosuppression.

Spectrum of MR imaging findings in Wernicke encephalopathy: are atypical areas of involvement only present in nonalcoholic patients?

BACKGROUND AND PURPOSE: Although MR imaging is considered the most effective method to confirm a diagnosis of WE, MR imaging studies designed to distinguish WE between NA and AL patients have yielded controversial results. The purpose of this study was to determine potential differences in MR imaging features between AL and NA patients with WE and to compare neurologic symptoms with MR imaging findings. MATERIALS AND METHODS: This retrospective study included 24 consecutive patients (male/female, 15:9; mean age, 54 years) diagnosed with WE in a university hospital (AL = 13, NA = 11). Clinical

Transcranial Direct Current Stimulation (tDCS) of Wernicke's and Broca's Areas in Studies of Language Learning and Word Acquisition.

Language is a highly important yet poorly understood function of the human brain. While studies of brain activation patterns during language comprehension are abundant, what is often critically missing is causal evidence of brain areas' involvement in a particular linguistic function, not least due to the unique human nature of this ability and a shortage of neurophysiological tools to study causal relationships in the human brain noninvasively. Recent years have seen a rapid rise in the use of transcranial direct current stimulation (tDCS) of the human brain, an easy, inexpensive and safe non

Sex differences of language abilities of preschool children with autism spectrum disorder and their anatomical correlation with Broca and Wernicke areas.

OBJECTIVE: People with autism spectrum disorder (ASD) often have language difficulties. This study focuses on whether there are sex differences in language ability in children with ASD and aims to analyze whether such differences may arise from developmental imbalances in the anatomical structures of Broca and Wernicke areas. METHODS: The language development quotient (DQ) scores of Gesell Developmental Scale (GDS) and the scores of language communication of Childhood Autism Rating Scale (CARS) were used to judge the language ability, and the FREESURFER software extracted the anatomical struc

Surgical resection of Wernicke's area brain arteriovenous malformation via posterior Sylvian fissure: Two-dimensional video.

The brain arteriovenous malformation (BAVM) within language-eloquent area poses a significant surgical challenge, demanding meticulous planning to ensure both preservation of language function and curative resection. This report details the successful microsurgical resection of a Spetzler-Martin grade II BAVM located in Wernicke's area in a 51-year-old male, ruptured three weeks ago and characterized by mild anomia. Following thorough discussion, the patient elected for microsurgery, providing informed consent, and the procedure was approved by the ethics committee. Given concerns regarding pa

Anodal tDCS over Wernicke's area improves verbal memory and prevents the interference effect during words learning.

BACKGROUND: Wernicke's area is a key component of the cortical language network, and it is functionally related to the comprehension of oral and written language. In addition to its main role in the perception of language, some other functions related to verbal learning also seem to involve the activity of this cortical region. It is unknown whether different degrees of neuromodulation on this area determine its effect on word learning. OBJECTIVE: We aimed to analyze the influence of the application of anodal transcranial DC stimulation (tDCS) over Wernicke's area at two different intensities

Brain functional connectivity alterations of Wernicke's area in individuals with autism spectrum conditions in multi-frequency bands: A mega-analysis.

Characterized by severe deficits in communication, most individuals with autism spectrum conditions (ASC) experience significant language dysfunctions, thereby impacting their overall quality of life. Wernicke's area, a classical and traditional brain region associated with language processing, plays a substantial role in the manifestation of language impairments. The current study carried out a mega-analysis to attain a comprehensive understanding of the neural mechanisms underpinning ASC, particularly in the context of language processing. The study employed the Autism Brain Image Data Excha

Speech perception following transcranial direct current stimulation (tDCS) over left superior temporal gyrus (STG) (including Wernicke's area) versus inferior frontal gyrus (IFG) (including Broca's area).

Imaging and neurocognitive studies have searched for the brain areas involved in speech perception, specifically when speech is accompanied by noise, attempting to identify the underlying neural mechanism(s). Transcranial direct current stimulation (tDCS), a noninvasive, painless cortical neuromodulation technique, has been used to either excite or inhibit brain activity in order to better understand the neural mechanism underlying speech perception in noise. In the present study, anodal (excitatory) and cathodal (inhibitory) stimulations were performed on 48 participants, either over the left

A Case of Bilateral Sudden Deafness Caused by Wernicke Encephalopathy.

Wernicke encephalopathy, also known as thiamine deficiency, is characterized by a triad of symptoms: ophthalmoplegia, ataxia, and confusion. However, additional symptoms may manifest depending on the area affected by the lesion. Although multiple cranial neuropathies are possible, sudden onset bilateral hearing loss has been described in rare situations as the first manifestation of Wernicke encephalopathy. Here, we present a case report detailing the clinical experience of a patient diagnosed with Wernicke encephalopathy, whose initial presentation included sudden bilateral deafness. The pati

Wernicke encephalopathy following splenectomy in a patient with liver cirrhosis: a case report and review of the literature.

OBJECTIVE: To report a case of Wernicke encephalopathy in the early stage after surgery. METHODS: A nonalcoholic female patient with hepatitis B-related cirrhosis and hypersplenism underwent splenectomy in a local hospital. No surgical complications occurred and the patient recovered well. However, on the eighth postoperative day she developed psychiatric and neurological disturbance without an obvious cause. She was then admitted to our hospital. Brain magnetic resonance imaging (MRI) with FLAIR T2 showed symmetric high-signal intensities in the periaqueductal area of the midbrain, which wer

Anodal tDCS over Broca's area improves fast mapping and explicit encoding of novel vocabulary.

An accumulating body of evidence suggests that transcranial direct current stimulation (tDCS) can be used to affect language processing, including word acquisition. There has been, however, no comprehensive study of effects of tDCS of the core language areas in relation to the main word-learning mechanisms. Two principal strategies have been posited as important for natural word acquisition: explicit encoding (EE) which relies on direct instructions and repetition of material, and fast mapping (FM) which operates implicitly, via context-based inference or deduction. We used anodal and cathodal

Resting functional connectivity of language networks: characterization and reproducibility.

The neural basis of language comprehension and production has been associated with superior temporal (Wernicke's) and inferior frontal (Broca's) cortical areas, respectively. However, recent resting-state functional connectivity (RSFC) and lesion studies have implicated a more extended network in language processing. Using a large RSFC data set from 970 healthy subjects and seed regions in Broca's and Wernicke's, we recapitulate this extended network that includes not only adjoining prefrontal, temporal and parietal regions but also bilateral caudate and left putamen/globus pallidus and subtha

Development of information sharing in language neocortex in childhood-onset drug-resistant epilepsy.

OBJECTIVE: We studied age-related dynamics of information sharing among cortical language regions with electrocorticographic high-gamma modulation during picture-naming and story-listening tasks. METHODS: Seventeen epilepsy patients aged 4-19 years, undergoing extraoperative monitoring with left-hemispheric subdural electrodes, were included. Mutual information (MI), a nondirectional measure of shared information, between 16 pairs of cortical regions of interest, was computed from trial-averaged 70-150 Hz power modulations during language tasks. The Wernicke conundrum and the anatomy of language comprehension in primary progressive aphasia.

Wernicke's aphasia is characterized by severe word and sentence comprehension impairments. The location of the underlying lesion site, known as Wernicke's area, remains controversial. Questions related to this controversy were addressed in 72 patients with primary progressive aphasia who collectively displayed a wide spectrum of cortical atrophy sites and language impairment patterns. Clinico-anatomical correlations were explored at the individual and group levels. These analyses showed that neuronal loss in temporoparietal areas, traditionally included within Wernicke's area, leave single wor
