The aim of this study was to evaluate whether sustained hypertensives with high clinic blood pressure, despite multiple drug treatment, show a true resistant hypertension or a "white-coat effect," and whether the pretreatment white-coat effect is maintained despite pharmacological therapy. The occurrence of resistant hypertension was determined in 250 consecutive essential hypertensives who had had an ambulatory blood pressure monitoring before treatment assignment. Twenty-seven of 250 hypertensives with persistently high clinic blood pressure despite 3 months of adequate pharmacological thera

White coat hypertension is a clinical phenomenon characterized by elevated arterial blood pressure during a visit to the physician. In this study, we will review the key published studies related to this concept as they evaluate and compare morphological and behavioural characteristics between normotensive individuals, essential hypertension, and white coat patients. A definition of white coat hypertension, as well as information about its prevalence and factors contributing to it are also provided. We will comment on the implications of this type of hypertension in nursing practice, since rea

"White coat" or "office" hypertension is often defined as the transient rise or elevation of blood pressure that occurs in the medical setting. Patients with these elevated readings have normal ambulatory pressures as determined by individual home blood pressure measurements or continuous 24-hour blood pressure monitoring. This clinical finding is present in 20 to 40 percent of patients with hypertension. Although continuous blood pressure monitoring has several research implications, its use adds little to the diagnosis, prognosis and treatment options. Treatment decisions should be based on

DEFINITION AND PREVALENCE OF WHITE-COAT HYPERTENSION: White-coat hypertension describes the phenomenon of an abnormal blood pressure reading when taken by a physician and a normal reading with ambulatory or home monitoring. The results of studies suggest that the true prevalence of white-coat hypertension may be 20%. The magnitude of the effect using either the oscillometric or the auscultatory method of measurement appears to be comparable. PROGNOSTIC SIGNIFICANCE: The prognostic significance of the white-coat effect is not established, and prospective trials are needed to assess the risk. At

Home blood pressure readings by self-monitoring (14 readings in 7 days) have been compared to readings taken in the clinic in 937 participants of the Tecumseh Blood Pressure Study. In the absence of firm criteria "hypertension at home" was defined as having home readings in the upper decile of the whole population. If a clinic reading exceeded 140 and/or 90 mmHg a subject was categorized as having clinic "hypertension". Two hypertensive groups emerged; one with both clinic and home hypertension ("sustained" N = 47) and one with high clinic but normal home blood pressure ("white coat" N = 50).

BACKGROUND: The risk associated with white-coat hypertension is controversial. We evaluated mortality risks in white-coat hypertension subtypes defined according to the circadian pattern of blood pressure (BP) elevation over 24 h.
METHODS: In 44 119 patients with elevated office BP, white-coat hypertension subtypes were defined as normal BP in all circadian periods (day, night, and 24-h, 12 192 patients), normal 24-h, with nocturnal BP elevation (4368), and normal daytime, with 24-h BP elevation (3525). Associations of each subtype with all-cause and cardiovascular mortality were estimated by

White coat hypertension (WCH) is most likely a disorder associated with metabolic syndrome. The study was performed at the Internal Medicine Polyclinic of Dumlupinar University on routine check-up patients. WCH cases who were overweight or obese and desiring weight loss were divided into two subgroups according to whether they preferred to achieve weight loss by medication or diet therapy. The study included 324 cases (204 females) with WCH, 45 of whom were in normal weight range. Therefore, 86.1% (279) of cases with WCH were either overweight or obese, and 41.3% (134) of all WCH cases had dys

The objective of this cross-sectional study was to investigate blood pressure variability and pulse pressure in white coat hypertensives in comparison to established hypertensives and normal subjects. Four hundred and twenty patients were referred consecutively from general practice with newly diagnosed untreated essential hypertension. One hundred and forty-six control subjects were drawn at random from the Danish national register. Seventy-six patients from the former group were considered white coat hypertensive using a normalcy cutoff level of 135/90 mm Hg. Mean, standard deviation, and co

BACKGROUND: The 2025 American Heart Association/American College of Cardiology blood pressure guideline (2025-AHA/ACC-BP guideline) recommends out-of-office blood pressure (BP) assessment to exclude white-coat hypertension (WCH) in untreated and white-coat effect (WCE) in treated individuals among adults with office BP (OBP) ranging between 130/80-<160/100 mmHg, but its real-world diagnostic performance remains incompletely characterized.
METHODS: We evaluated the performance of 2025-AHA/ACC-BP guideline-recommended OBP thresholds for screening WCH/WCE among 147,404 Brazilian adults with OBP

AIM: This mini review explored the prevalence of white-coat hypertension (WCH), which is very common in children. It results in elevated office blood pressure (BP) but normal ambulatory BP monitoring (ABPM) readings.
METHODS: WCH can only be identified by analysing and comparing office BP readings and ABPM, which periodically records BP every 20-30 minutes over 24-hour period. This study provides initially the background for WCH in adults, together with a comprehensive overview of the most relevant paediatric data on WCH.
RESULTS: Accurate measurements of BP are very important for the diagno

OBJECTIVES: The role of ambulatory blood pressure monitoring (ABPM) in the elderly is unclear. This study has examined differences between clinic and ambulatory blood pressure (BP) in a large cohort of older hypertensives, with particular respect to the factors influencing the direction and magnitude of this difference.
DESIGN: The Second Australian National Blood Pressure Study (ANBP2) is a general practice-based randomized-outcome trial in 6083 older hypertensives treated with an angiotensin-converting enzyme (ACE) inhibitor or diuretic-based regimen. Before starting treatment a subset of 7

OBJECTIVES: The difference between clinic and daytime ambulatory blood pressure is referred to as the white-coat effect. In this study, we investigated (i) the magnitude of the white-coat effect in subjects with different daytime ambulatory blood pressure levels, and (ii) the association of the white-coat effect with left ventricular mass.
METHODS: A total of 1581 subjects underwent clinic blood pressure readings, 24-h ambulatory blood pressure monitoring and left ventricular echocardiographic assessment. Their mean daytime systolic blood pressure varied from 88.0 to 208.9 mmHg and their mean

PURPOSE: To investigate the association between white-coat hypertension (WCH) and acute retinal vein occlusion (RVO).
METHODS: In this retrospective case-control study, patients aged 40 years or older diagnosed with acute-phase RVO were included. Patients with other pathologies served as non-RVO controls. Blood pressure (BP) was measured in the office during their initial visit, and information about home BP and hypertension (HTN) medication was obtained through interviews. After 1:2 age and sex-matching between the RVO and non-RVO groups, the proportions of HTN cases were compared. A similar

Although white coat hypertension (WCH) is believed to have an effect on health, there is no term defining WCH in metabolic syndrome. Consecutive patients 20 years old or older who underwent a check-up were included. The study included 1068 cases. The prevalences of hyperbetalipoproteinemia, hypertriglyceridemia, dyslipidemia, impaired glucose tolerance (IGT), and WCH were similar to excess weight in that they increased significantly until the seventh decade of life and decreased thereafter significantly (P < 0.05 in most steps). On the other hand, the prevalences of hypertension (HT), diabetes

BACKGROUND: The classical 'white coat response' to blood pressure measurement has been studied thoroughly. However, little is known about patients showing a reverse pattern, i.e. who have lower blood pressure readings at the clinic than outside healthcare facilities.
AIM: To estimate the proportion of patients whose blood pressure levels as determined by self-measurements at home are higher than those taken at the clinic and to explore possible associations with demographic, clinical, and psychological variables.
METHOD: Patients consecutively attending (n = 214) an academic family medicine

This study aimed to explore the relationship of thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), and free thyroxine (FT4) levels with hypertension subtypes. 1056 euthyroid adults were included as research samples. They underwent measurement of clinic blood pressure and 24-hours ambulatory blood pressure monitoring. Then, they were divided into normotension (NT), white coat hypertension (WCH), masked hypertension (MHT), and sustained hypertension (SHT) groups. The 24-hours dynamic electrocardiogram was performed to analyze the heart rate variability (HRV), so as to reflect the ca

OBJECTIVES: BpTRU (VSM MedTech Ltd, Vancouver, Canada) is an automated oscillometric device that provides serial blood pressure measurements in an office setting in the absence of a healthcare professional. We sought to determine whether the white-coat effect is reduced by a blood pressure measurement protocol using BpTRU compared with casual office measurements. Secondarily, we also sought to determine whether a blood pressure measurement protocol using BpTRU reduced white-coat hypertension compared with the casual office measurements, and reduced white-coat effect and white-coat hypertension

BACKGROUND: Different definitions of white-coat hypertension (WCH) may explain its variable outcome across studies.
METHODS: In an Italian study started in 1986, we followed 3,153 people with (office blood pressure (BP) >=140/90 mmHg) and 457 without office hypertension for a mean of 10.4 years. None had previous cardiovascular disease. All underwent 24-h ambulatory BP (ABP) monitoring. We defined white-coat hypertension (WCH) as an average 24-h ABP < 130/80 mmHg or <125/75 mmHg. The primary outcome was a composite of major adverse cardiovascular events (MACE) and all-cause mortality.

"White-coat" hypertension, i.e. high readings in the clinic but normal readings at home, has been demonstrated in 21-58% of hypertensive subjects without end-organ injury. The condition can be diagnosed by means of home blood pressure monitoring, which should therefore be used as a supplement to monitoring by the physician. Owing to the low blood pressure readings at home, these patients should not be treated with drugs. In the event of anti-hypertensive treatment, home blood pressure monitoring is useful in helping to decide the minimum required dose, thus reducing risk of side effects, and i

Patients with hypertension in the clinic but not during daily activities ("white coat" hypertension) may be at lower risk of hypertensive morbidity and mortality than patients with hypertension in both settings ("persistent" hypertension). We hypothesized that the white coat phenomenon was due to greater blood pressure reactivity to the stress of a clinic visit and that, as a consequence, white coat hypertensive patients would display greater blood pressure reactivity to exercise and mental stress, as well as increased emotional reactivity and higher levels of anger, anxiety, or depression. We
